목 적 : Comfilcon A 실리콘 하이드로겔렌즈의 착용시간에 따른 시력의 질과 눈의 자각적 증상을 비교하 였다. 방 법 : 최대 교정시력을 제공하는 도수의 실리콘 하이드로겔렌즈를 건강한 성인 27명 (평균24.30±2.96 세, 여: 14명, 남: 13명)의 대상자에게 착용시키고 밝은 조명(photopic)과 어두운(mesopic) 조명 상태에서 고대비도(100%)와 저대비도(10%) 시력표를 사용한 시력검사와 4종류의 공간주파수에서 대비감도검사를 시행하였다. 또한, 수차분석기를 이용하여 4 mm와 6 mm 동공크기에서 각막의 고위수차를 각각 측정하고, 렌즈착용 대상자는 콘택트렌즈와 관련된 자각적 증상에 대한 설문지를 작성하도록 하였다. 모든 측정과 설 문지 작성은 하루 동안 실리콘 하이드로겔렌즈를 착용하면서 2 시간과 9시간 착용 후 각각 수행되었다. 결 과 : 밝은 조명과 어두운 조명 상태에서 검사한 고대비시력과 저대비시력 및 대비감도는 렌즈착용 2시 간과 9시간 후 유의한 차이가 없었다. 3차 고위수차, 4차 고위수차, 코마 및 구면수차를 포함하는 각막의 고 위수차량 또한 렌즈착용 2시간과 9시간 후 대상자의 동공의 크기에 상관없이 차이가 없었다. 그러나 콘택트 렌즈 착용과 관련된 자각적 증상의 상대적 발생빈도 항목 중, 불편함(discomfort), 건조감(dryness) 및 눈 감음(closing eyes)의 증상은 렌즈착용 2시간과 9시간 후 비교에서 유의한 차이가 있었다(p=0.031, p=0.001, p=0.031). 결 론 : Comfilcon A 실리콘 하이드로겔렌즈를 9시간 착용한 결과 시력과 대비감도 및 각막의 고위수차 는 착용시간에 따라 변화가 없었으나 건조감과 불편한 자각적 증상은 착용시간이 길어지면서 증가하므로 이 러한 증상을 감소시킬 관리법의 지도가 필요할 것으로 생각된다.

Purpose: To compare the visual quality and subjective ocular symptoms with Comfilcon A silicone hydrogel contact lens wearing times. Methods: Twenty seven subjects (24.30 ± 2.96 years, 14 females, 13 males) took part in this study. Subjects were fitted with silicone hydrogel contact lenses providing best-corrected visual acuity. Best-corrected visual acuity (VA) at two contrast levels (100% and 10%) and best-corrected contrast sensitivity (CS) for four spatial frequencies (3, 6, 12 and 18 cycles/degree) were measured under photopic and mesopic conditions. Corneal higher-order aberrations (HOAs) were measured with 4-mm and 6-mm pupils using Wavefront Analyzer. Subjects were asked to complete the questionnaire for subjective ocular symptoms related contact lens. All measurements and questionnaires were performed after wearing silicone hydrogel lenses for 2 hours and 9 hours in a day. Results: Regarding visual quality under photopic and mesopic conditions, there was no statistically significant difference between 2 hours and 9 hours wear in high contrast, low contrast VA and CS. We also did not find out any significant changes in corneal HOAs, including third-order, fourth-order, coma, and spherical aberration for both 4-mm and 6-mm pupils between the two wearing times. Among the relative frequency items of subjective ocular symptoms related contact lens, there were statistically differences between 2 hours and 9 hours wear in discomfort, dryness, and closing eyes (p=0.031, p=0.001, and p=0.031, respectively). Conclusion: It can be identified that Comfilcon A silicone hydrogel contact lenses showed no change in the optical quality and visual quality of the eye for more than 9 hours wear. However, uncomfortable ocular symptoms and dryness increased as much as wearing time of the silicone hydrogel contact lens was longer. Thus, guidance of contact lens care to reduce those symptoms is considered necessary.
