Objectives : To develop a basic ADL instrument for persons with stroke, reflecting our cultural values. Method : The ADL instruments that have been developed previously were gathered, excluding the ADL items, which lowered the validity of the instrument. We surveyed occupational therapists on the values of basic ADL with a total of thirty-four item cards. The value of ADL was measured by Q sorting technique. And ninety-eight patients with stroke were evaluated to gain final items selection and weighted items. Results : Ten items for inpatients were selected. For inpatients, functional ambulation at indoor was weighted to four times; the excluded items using proper utensils, and cleaning mouth were weighted to two times. Conclusion : We developed a basic ADL instrument for inpatients and the range of score of each instrument was 0∼80. The internal consistency for inpatients was high (Cronbach α=0.94). However, further study requires the concurrent validity, construct validity and the test-retest reliability of the present ADL items.

The purpose of this study was to investigate the changes of the activities of daily living (ADL) by the motor learning program for upper extremity in stroke patients. The subjects were sixty-two members who were treated at the department of occupational therapy. The ADL were measured with Functional Independence Measure (FIM). The result's were as fol1ows; 1. The ability to perform daily activities after treatment was significantly increased independent of age, gender, cause, affected side and speech disability (p<.01). The differences between the pre-treatment and the post-treatment was no significant correlation to sex, cause, affected side and duration of treatment, but the speech disability was significant. Therefore the pre-treatment scores and post-treatment scores were significantly different. 2. The variation of the pre-treatment and the post-treatment in the detail item to the performance of daily life of the stroke patients through the upper extremity motor learning program was significantly increased in self-care, sphincter control, locomotion, mobility, communication and social cognition (p<.01) Consequently, The motor learning program for upper extremity significantly improved the performance level of all ADL areas in stroke patients.

Objectives: The objectives of this study were to demonstrate daily occupations and their self-perceived quality in persons with disability. Methods: Twenty five participants responded to a questionnaire, The Occupation Schedule (modified from an Activity Configuration Log). The subjects were asked to categorize their daily activities such as sleep, self-maintenance, work, rest, leisure and to rate their self-perceived quality. Results: The subjects had a high rate of unemployment (64%) and 12 subjects did not classify any occupation as work. The same occupation was classified into several different categories. The quality of occupation was rated at the satisfaction, importance, stress, and time adequacy. Conclusions: The results suggest that the subjects’ classification of occupations and their meanings are important sources of information for the science of occupation and the occupational therapy practice. Occupational therapists need to explore the alternative satisfying occupations to replace the category of work.

This longitudinal study of CVA patients has two parts: the first with 50 subjects and the second with 24 subjects. The first part of study shows that in making a prognosis of the early stage of CVA, the factor of visual neglect is most critical. The second part suggested that I-ADL is a better criteria than ADL for long-term CVA patients. 1) In the first study, the 50 subjects, hospitalized in the Department of Rehabilitation Medicine, Korea University Hospital from January 1998 to December 1999, were divided by the result of their MVPT (Motor Free Visual Perception Test) into two groups: one with visual neglect and the other with no visual neglect. In terms of ADL (Activities of Daily Living), the group with visual neglect significantly scored less that the group with no visual neglect. This result strongly supports the hypothesis that the factor of visual neglect is closely related with the CVA patients’ progress of ADL. 2) After two years, the follow-up study on remaining 24 clients (4 deceased, 22 unreachable) has been conducted in an attempt to identify any significant differences of the progress of ADL and I-ADL (Instrumental Activities of Daily Living) between two groups: neglect and neglect-free group. At this time, the ADL of both groups have remarkably improved with no significant difference between groups. However, both groups still scored significantly low in I-ADL. 3) These results lead us to the conclusion that only in the early rehabilitation period, the factor of visual neglect is very useful to predict a prognosis of the CVA patients. In addition, it is reasonable to suppose that rehabilitation for the chronic CVA patients should be guided by I-ADL rather than ADL.

Activities of daily living (ADL) and instrumental activities of daily living (I-ADL) are tasks of self-care, functional mobility, functional communication, home management, and community living that enable an individual to achieve personal independence. The purpose of this study were to investigate the effect of factors on ADL in stroke patients and to identify the relationship between the function of upper extremities and ability of ADL. Thirty-one stroke patients participated in this study. The results of this study were as follows: 1) There were significant functional gains between admission and discharge on the MFT score, dexterity, and the motor FIM(p<0.01). 2) There were moderate correlation between MFT score of the non-affected side and motor FIM score at discharge(r=0.499, p<0.01) and high negative correlation between dexterity of non-affected hand and motor FIM score at discharge(r=-0.704, p<0.01). 3) There was a significant difference on motor FIM score among academic background groups (p<0.01). 4) There was no significant difference on motor FIM score among caregiver groups (p<0.01). 5) Four factors were found. one factor comprised dexterity of non-affected hand at admission and discharge and duration of treatment. Two factor comprised MFT of non-affected side at admission and discharge and academic background. Three factor comprised MFT of affected side at admission and discharge and caregiver. Four factor comprised sex and affected side.

The purpose of this study were to investigate the independent ADL (Activities of Daily Living) level of elderly people using a questionnaire made up of a mobility scale, two basic ADL measurement tools, and an instrumental ADL measurement tool, and to differentiate between the levels according to various performance environments. The subjects were elderly people (over 60 years) who live in Wonju City (pop.240,000). A survey tool made up of 110 items was used for this study and the quiestionnaire was completed by 7 investigators who interviewed the respondents at gatherings for the elderly. The results were analyzed according to gender, 3 age groups (60-69, 70-75, and over 76), comfortable position preference (floor sitting or chair sitting), and housing type (detached house or apartment) by non parametric statistics, Kruskal-Wallis and Mann-Whitney U tests. The results were as follows: 1. According to age group, basic ADL dependence level was not significantly different in ‘cleaning house’, ‘travelling’, ‘street crossing’, ‘hospital visiting’, and ‘walking around’. Women were more significantly dependent than men in instrumental ADL: ‘going to the post office’, ‘driving’, and ‘street crossing’, but without significant difference of dependence level in basic ADL. 2. According to position pattern, dependence level was not significantly different for ‘face washing’, ‘hair washing’ or ‘foot washing’. At rest, people sitting on the floor were more dependent in instrumental ADL items only for ‘shopping’ and ‘attending religious services’. People living in detached houses reported significantly more difficulty using the toilet than those in apratments. 3. The most sensitive items for evaluating functional independence of the elderly were: ‘using the telephone’, ‘face washing’, ‘hair washing’, ‘foot washing’, and ‘bottle opening’ in basic ADL, and ‘hospital visiting’, ‘travelling’, and ‘street crossing’ in instrumental ADL. The Cronbach α value of mobility, basic ADL, instrumental ADL were 0.87, 0.86, and 0.91, respectively. To evaluate the ADL of old age, we have to use evaluation tools suitable to the Korean culture. The most useful items were ‘telephone use’, ‘face washing’, ‘hair washing’, ‘foot washing’, ‘bottle opening’ in basic ADL, and ‘hospital visiting’, ‘travelling’, and ‘street crossing’ in instrumental ADL. Especially, instrumental ADL is more valuable than basic ADL in evaluating old age. It is more important in evaluating the practical aspects of life in old age to consider housing and living patterns because there are some ADL items which are affected by comfortable sitting preference and housing style.

Although the availability of home care rehabilitation services have been greatly increased since community-based rehabilitation was introduced to Korea, there is still a dearth of studies investigating the performance level of ADL (activities of daily living) for the physically disabled in the rural areas. The purposes of this study were to investigate the ADL performance level of disabled persons living at home in the rural areas of Wonju city, Korea, using FIM (functional independence measure) and to identify the specific areas of the ADL to be trained or evaluated by physical therapists or occupational therapists. 298 disabled people were interviewed by 10 physical therapy students. Analysis of the interviews indicated: 1) Forty seven point seven percent of the respondents were elderly-disabled persons whose ages were 61 years old or above, and 69.5% of respondents has the history of chronic disability period of 5 years or more. 2) FIM score of bathing, and stairs climbing showed severe dependent trend 3) FIM scores of self-feeding, urination, defecation, and comprehension ability were mildly dependent. These results revealed that functional evaluation/treatment for discharging from hospitals to the rural areas should be emphasized on the specific ADL performance areas such as bathing, and stairs climbing.

The purpose of this paper is to evaluate the fact that rehabilitation therapy helps stroke patients in the area of activities of daily living(ADL). The ADL evaluation tool used in this study is the modified Barthel Index. The subjects were 71 stroke patients who admitted rehabilitation department of Samsung Medical Center from January to September 1997. All subjects received rehabilitation therapy. The sbuject's ADL were evaluated on admission and again at the time of discharge. The results follows : 1. Using the modified Barthel Index, revealed on admission 38.26 points at discharge 68.73 points. 2. Severe disability(Barthe1 score 60 or less) were reduced from 84.5% on admission to 29.6% at discharge. Very severe disability(Barthe1 score 40 or less) were also reduced from 66.2% on admission to 12.7% at discharge. 3. The degree of change in each area of the modified Barthel Index were measured. There was an overall positive change during the admission period. The greatest change occurred in the category of dressing and transferring. The least change in donning & doffing a brace and wheelchair mobility. The modified Barthel Index used to assess the change in ADL function of stroke patients woulds seem to be very useful method. And there should be a focus on the recovery of ADL function in stroke patients receiving rehabilitation therapy.

The purpose of this study was to evaluate the cognitive perceptual performance in relation to activities of daily living in stroke patients who were treated at department of rehabilitation medicine of the Samsung Medical Center from December 5. 1996 to April 10, 1997. The cognitive perceptual performance was measured by the Lowenstein Occupational Therapy Cognitive Assessment (LOTCA) and by the Modified Barthel Index (MBI) to evaluate activities of daily living(ADL). Sixty-seven stroke patients were the subjects in this study. Collected data analysis were completed by using t-test. ANOVA, correlation analysis, and multiple regression analysis. The following were as follow: 1. Among the subtest of the LOTCA, orientation to time, plain block design, reproduction of puzzle showed significant difference depending on patient's affected side. Orientation to time score was higher with left hemiplegic patients. Plain block design and reproduction puzzle score were higher with right hemiplegic patients. 2. Among the subtests of the MBI, dressing, ambulation, and self-care were significantly correlated with the subtests of the LOTCA in order of thinking operation, visuomotor organization, perception and orientation. 3. The result of the cognitive perceptual function affecting the MBI performance using multiple regression analysis, thinking operation was the most influential factor. The cognitive perceptual perfomance have significant correlation with activities of daily living skills. Also, it is an important factor to functional recovery after stroke. Therefore, accurate evaluation of cognitive perceptual performance after stroke with treatment of each patient's appropriate level should be done simultaneously with rehabilitation treatment to improve patient's activities of daily living skills.

A study was carried out to compare the daily foods which were eaten during 14~19 century among Korea (Chosun dynasty), China (Ming, thing dynasty), and Japan (Muromachi, Edo era). Specially, in Ming, Ching dynasty, Noodles and dumpling were cooked very often and in Muromachi, Edo era, many kinds of beans were used so much, and there were many kinds of steamed rice to compare with Korea and China. Also, in Japan at these era Woodong, Morisoba, and Somyen were used as favorite foods harbitually. 1. The crops were used as staple foods in three countries such as steamed rice, gruel, noodle, dumpling, rice cake etc. commonly. 2. In Chosun, a roe deer, deer, antelope, and wild boar were more favoite dishes than cow, chicken, and dog, generally people enjoyed to eat pork and chicken. In Ming Ching, pork, chicken, goose, dove, lamb, ass, and duck were used as food materials, also wild animals such as deer, rabbit, and monkey etc. were used. In Muromach era, wild animal as racoon dog, otter, deer, antelope, bear, were used as food materials, in early Edo era (Yayoi and Momoyama), beef and pork were used as daily foods but they were eaten decreasingly in Edo era. So in Japan at those era, the dishes prepared of fishes were more than those of meats. For example, Sushi was made from that time, people all over the world are enjoying to have Sushi at present time. 3. Also in these era, many kinds of vegetables and fruits were used in three countries, and new kinds of vegetables and fruits were introduced from foreign countries, for example, red pepper, pumpkin, sweet potato, potato in Korea, corn, sweet potato, red pepper in China, and water melon, pumpkin, corn, red pepper, marmellow, sweet potato, sugar beet in Japan.
