        병렬영상기법인 SENSE 기법은 슬관절 자기공명영상의 검사 시간을 획기적으로 단축할 수 있다. 그러나 기법 적용 시 SENSE factor를 증가시키면 영상에 인공물의 발생이 증가하는 문제점이 있어 개선을 위해 본 연구에서는 최소의 시간이 소요되면서 인공물이 발생하지 않는 최적의 SENSE factor를 제시하고자 하였다. 연구 방법은 SENSE factor 1.0을 기준 으로 0.5 간격씩 5.0까지 변화시켜 팬텀 실험과 임상실험을 시행하였다. 3.0T 초전도 자기공명영상장치와 dS Knee 코일 을 사용하여 T1, T2 강조영상을 획득하였으며, 영상의 비교평가는 임상 경력 10년 이상의 방사선사 10명이 5점 척도로 평가한 후, 일원배치 분산분석과 사후분석을 통해 유의한 차이가 있는지 판단하였다. 연구 결과 팬텀 실험은 T1, T2 강조 영상 모두 SENSE factor를 1.5 이하로 하였을 때 기준 영상과 차이가 없었으며, 임상실험은 SENSE factor를 2.0 이하로 하였을 때 기준 영상과 차이가 없었다. 결론적으로 슬관절 자기공명영상 시 검사 시간을 단축하면서 인공물이 발생하지 않는 최적의 SENSE factor는 팬텀 실험의 경우 1.5, 임상실험의 경우 2.0이 적정하리라 사료된다.

        The purpose of this study was to assess knee joint loading in the target knee during a golf swing compared to loading rates of high impact activities such as cutting and drop landings. Nine healthy competitive golfers completed golf swings with the target foot both straight and externally rotated 30 degrees, as well as drop landings and cutting maneuvers. Motion capture data was collected at 240 Hz and ground reaction force data was collected at 2400 Hz. The frontal and transverse knee moments were examined using repeated measures ANOVA through SPSS. The abduction moments were higher in golf swings as compared to the other high impact activities (p=.010), while the external rotation moments were lower (p=.003). There were no significant differences between externally rotated and neutral golf swings. These results suggest moments applied to the knee during a golf swing are similar to those applied during a high impact activity.

        목 적 : 후방십자인대는 무릎 관절의 안정화를 위한 중요한 인대로 인체의 균형을 유지하는데 매우 중요한 역할을 한다. 본 연구에서는 후방십자인대의 해부학적 구조에 따라 사방향 관상면 영상과 이중 사방향 시상면 영상을 획득하여 그 유용성을 평가하고자 한다. 대상 및 방법 : 2016년 12월부터 2017년 1월까지 환자 20명을 대상으로 검사하였고 SIEMENS사의 Skyra 3.0T에서 knee coil을 사용하였다. 검사 방법은 T2 COR, PD SAG을 획득하고 원위부 후방십자인대의 해부학적 구조를 고려한 PD OBL COR과 전체적인 구조를 고려한 PD Dual OBL SAG을 획득하였다. PCL의 관상면 비교를 위하여 밑단점의 길이와 폭을 측정하여 비교하였고, 시상면 평가를 위하여 기시점, 중간지점, 밑단점을 측정하여 비교하였다. 측정결과를 Mann-Whitney 방법을 사용하여 통계적 유의성을 검정하였다. 정성적 방법은 기존의 관상면, 시상면 영상과 사방향 관상면, 이중 사방향 시상면 영상에서 후방십자인대를 비교하여 미흡, 대등, 우월 세 단계로 평가하였다. 결 과 : 검사를 진행한 결과 기존 관상면 영상보다 사방향 관상면 영상에서 원위부 후방십자인대의 길이는4 7.9%, 폭 14.9% 증가하였고, 기존 시상면 영상보다 이중 사방향 시상면 영상에서 기시점 19.8%, 중간지점 18.3%, 끝단점 33%의 증가된 것 으로 측정되었다. 결 론 : 사방향 관상면 영상과 이중 사방향 시상면 영상 모두 기존 영상보다 우월한 영상 검사방법으로 평가되었다. 이에 슬 관절 후방십자인대의 해부학적 관찰에는 기존 검사 방법에 사방향 관상면 영상 검사와 이중 사방향 시상면 영상 검사를 병 행하는 것이 유용할 것으로 사료된다.

        Background: Total knee arthroplasty (TKA) recovers the alignment of the knee joint, but fails to automatically restore the alignment and function of the hip and ankle joints. It may affect the alignment and stability of the knee joint, therefore therapeutic intervention in hip and ankle joint is necessary for the rehabilitation process after TKA. Objects: The aim of this study was to comparison of the effects of the two exercise methods on the coronal plane alignment after TKA. This study conducted an experiment by dividing subjects into a lower extremity isometric co-contraction group (LEIC) and a quadriceps isolated isometric contraction (QIIC) group. Methods: A total of 37 subjects were randomly assigned to the LEIC (n1=19) or the QIIC (n2=18). Exercise was applied to five times per week for three weeks, starting on the eighth day after surgery. Range of motion exercises were performed as a common intervention and then each group performed quadriceps isometric contraction exercises with 10 sets of 5 repetitions. Radiological imaging was performed prior to surgery, one month and six months after surgery. In addition, the hip-knee-ankle angle (HKA) and tibiotalar angle (TTA) were measured. Results: The HKA was close to neutral in the LEIC rather than the QIIC (p<.05). The LEIC showed varus and the QIIC exhibited valgus TTA (p<.05). In a comparison of HKA and TTA over time, there was no significant change in either group (p>.05). According to the comparison of the TTA before surgery, the LEIC showed significant changes in the varus direction (p<.05), while there was no significant change in the QIIC (p>.05). Conclusion: The LEIC method triggered changes in the TTA and brought the HKA close to the neutral. Thus, LEIC is more effective than QIIC in creating stability in the coronal plane alignment of the knee and ankle joints after TKA.

        The purpose of this study was to identify the effects of mechanical traction applied to the knee joint on pain, knee range of motion (ROM), timed up and go (TUG) and Western Ontario and MacMaster Universities Osteoarthritis (WOMAC) in patients with knee osteoarthritis (KOA) of Kellgren-Lawrence radiological rating scale Ⅱ or Ⅲ. Twenty three patients participated in the experiment for a period of four weeks. After baseline assessment, the patients with KOA were randomized into two groups: the traction group (n1=12), which received traction with general physical therapy; and the control group (n2=11), which received general physical therapy only on unilateral knee joints. Patients received interventions once a day, three times a week, for four weeks. Wilcoxon signed-rank test was used to analyze the change of dependent variances within the group during pre and post intervention. Mann-Whitney U test was used to analyze the change of dependent variances as TUG and passive ROM between the two groups. Analysis of covariance was used to analyze the change of dependent variances as numeric pain rating scale (NPRS) and WOMAC score between the two groups. In Wilcoxon signed-rank test, the traction group improved significantly with regard to NPRS (p<.01), passive knee flexion ROM (p<.01), passive knee extension (p<.05), TUG (p<.01) and WOMAC scores (p<.01) after intervention for four weeks, but not for the control group. In the Mann-Whitney U test and analysis of covariance, no significant difference was seen among all the dependent variances after intervention for four weeks between the two groups. These outcomes suggest that further studies should be carried out to determine the effects of mechanical traction prior to using it for the treatment of patients with knee osteoarthritis.

        The purpose of this study was to determine the effect of sciatic nerve mobilization technique on perceived pain, straight leg raise test (SLR), and strength of knee extensor, location of symptoms (LOS) in patients with chronic low back pain. 22 patients with chronic low back pain were recruited for this study. The subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with 11 patients in each group. All patients received a routine physical therapy (hot pack and transcutaneous electrical nerve stimulation). The mobilization technique of the sciatic nerve was performed for 10 min in the case of the EG subjects. Outcome measurements included the level of the perceived pain, SLR, and strength of the knee extensor, LOS. The measurements were recorded 3 times: before the intervention, after the intervention, and at 1 hour of follow up. The two groups did not significantly differ with regard to the level of perceived pain, SLR, and strength of the knee extensor, LOS before the test (p>.05). In the case of the EG subjects, all the variables measured after the intervention significantly differed from those measured before the intervention (p<.05). However, in the case of the CG subjects, a significant difference was noted only with regard to the level of perceived pain (p<.05). The findings indicate that sciatic nerve mobilization technique exerts a positive effect on the control of subjective symptoms and knee strength in patients with chronic low back pain. Further studies are required to generalize the result of this study.

        To develop effective training methods for strengthening a weakened quadriceps femoris muscle in hemiplegic patients, we examined the effects of maximal isometric contraction of the nonparalyzed knee joint on the electromyographic activities of the paralytic muscle. An electromyogram (EMG) was used to record the electromyographic activities of the paralytic quadriceps femoris muscle in 27 hemiplegic patients. The maximal isometric contraction was measured for each subject to normalize the electromyographic activities. The maximal isometric extension and flexion exercises were randomly conducted when the knee joint angles of the nonparalyzed knees were 0˚, 45˚, and 90˚. The patients were encouraged to maintain maximal isometric contractions in both knee joints during each measurement, and three measurements were taken. A one-minute rest interval was given between each measurement to minimize the effects of muscle fatigue. An average from the three values was taken as being the root mean square of the EMG and was recorded as being the maximal isometric contraction. The electromyographic activity obtained for each measurement was expressed as a percentage of the reference voluntary contraction, which was determined using the values obtained during the maximal isometric contraction. The results of this study are summarized as follows: First, when the knee joint angle of the nonparalyzed knee was 0˚, the electromyographic activities of the paralytic medial aspect of rectus femoris were related to measurement by a maximal isometric flexion exercise than by an extension exercise (p<.05). Second, when the knee joint angle of the nonparalyzed knee was 90˚, the electromyographic activities of the paralytic lateral aspect of rectus femoris were related to measurement by a maximal isometric flexion exercise than by an extension exercise (p<.05). The results show that myoelectrical activities of paralytic quardriceps were not related to measurement angles and exercise directions of the nonparalized knee joint. Studies on various indirect intervention to improve muscular strength of patients with nervous system disorders of the weakened muscle should be constantly conducted.

        The purpose of this study was to examine the validity and reliability of the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC)-VA3.0 in patients with hip and knee osteoarthritis (OA). The sample consisted of 301 patients who had received treatments at the physical therapy units of 5 medical institutions in Andong City in june 2006. Questionnaires on the WOMAC were recruited by 12 physical therapists. The internal structure and reliability of the scales were evaluated by means of item-internal consistency (Cronbach's alpha coefficient: ), item-discriminant validity, and Pearson's relation coefficient. To explore construct validity, we conducted a principal component factor analysis with varimax rotation analysis. The criterion for factor extraction was an eigenvalue >1.0. The average age of the patients was 62.1 years. All WOMAC subscales (pain, stiffness, and physical function) were internally consistent with Cronbach's coefficients of .81, .91, and .80, respectively. The internal consistency reliability of item-each scale were also internally consistent with Cronbach's coefficient of .89 (Pearson's correlation coefficient: .71~.84), .93 (.89~.91), and .96 (.67~.91), respectively. However, high correlation was found among 3 items (.66~.83, .66~.67, and .67~.83), so the item-discriminant validity was low ( coefficient: .81, .91, .80, respectively). The construct validity by factor analysis was low because it was not consistent With WOMAC-VA3.0. In conclusion, the results reported here confirm the reliability of the WOMAC in patients with OA of the hip and knee. The collection of information on the hip and knee osteoarthritis using this instrument was acceptable to patients. A further prospective multi-center study will be necessary to prove the construct validity.

        The purpose of this study was to determine the influence of high-heeled shoes on walking of obese women as it was already proven an extrinsic factor of knee osteoarthritis in women with normal weight. In this study the aimed therefore in particular was to utilize high-heeled shoes in proving it's causal influence on knee osteoarthritis by measuring the angle and torque of the knee joint. Fifteen obese women (BMI>25 ) were measured in their twenties. Each angle and torque of their knee joints during walking on 6.5 cm high-heeled shoes and with a bare feet, were compared with each other and analyzed with a 3D motion analysis system. There was no significant difference in walking speed, cadence and stride length between the two conditions. However, there was a significant increase in a double limb support time and the stance phase when walking on high-heeled shoes as when walking with bare feet. The peak knee flexion angle and peak knee varus torque was higher when walking on high-heeled shoes than with bare feet. On the contrary, the peak knee flexion angle in the swing phase was not statistically different. The prolongation of peak knee varus torque was also proven. There was a significant increase in peak knee varus torque in the initial and last stance phases during walking on high-heeled shoes as compared to walking on bare feet. Through the above results, it was proven that when obese women walked on high-heeled shoes, rather than with bare feet, peak knee flexor and varus torque increased along with the changes of the in knee joint angle. Therefore, the influence of high-heeled shoes might be a significant intrinsic factor in knee osteoarthritis of obese women.

        The purpose of this study was to examine the effects of static stretching and Evjenth-Hamberg stretching the on range of motion (ROM) of the knee joint. The subjects were composed of twenty healthy males without weight-training experience. The ROM of the knee joint was measured by using an En-Knee. Three tests (performed on the 1st week, 4th week, and 8th week) were conducted to examine the change of each variable. Data were analyzed with a analysis of variance (grouptest) for repeated measures on last factor through SPSS 10.0. The data analysis revealed the change in the ROM was dependent on the stretching method. The results were as follows: The ROM was improved in both methods by each time, but the E-HS was more improved than the SS. In conclusion, this study indicated that the E-HS is more efficient than the SS on the ROM of the knee joint.

        The quadriceps-angle (Q-angle) and the ratio of hamstring/quadriceps (H/Q) are important for the stability of the knee and for protection from excessive stress. The aim of this study was to examine the association between Q-angle and H/Q ratio with and without knee osteoarthritis. We compared knee osteoarthritis patients with symptom-free women. The mean age of the patients in the arthritis group (25 women, osteoarthritis) was 59.7 years. The non-arthritis group consisted of 25 women with a mean age of 55.2 years. Of the 25 women with osteoarthritis, 5 had the condition in their left knee, 5 had it in their right knee, and 15 had it on both sides. There was no significant difference in the knee Q-angle of the left and right knees of the arthritis group and the non-arthritis-group (p>.05). The strength of all the muscles around the involved right knee in the arthritis group was significantly weaker than that of the non-arthritis group (p<.05). However, in the left knee, only the strength of the knee extensors and internal rotators was significantly weaker than that of the non-arthritis group (p<.05). The Q-angle was not associated with the H/Q ratio and internal rotators/external rotators ratio of the involved knee in the arthritis group (p>.05). Neither was the Q-angle associated with the pain level of an involved knee in the arthritis group (p>.05). The knee pain was not associated with the H/Q ratio of the involved knee in the arthritis group (p>.05). The Q-angle was not associated with the ratio of H/Q and pain level of the involved knee in the osteoarthritis women.

        The purpose of this study was to investigate the effects of visual information and different elevations of medially wedged insoles on the proprioceptive sense of the knee joint. The subjects of this study were 16 able-bodied men who were not athletic. An electrogoniometer was used to determine the error value between calculated 50% of full flexion (target position) and performed 50% of full flexion in a standing position with the upper extremities crossed. Tests were randomly performed in conditions. Visual variations included open eyes vs. closed eyes, while the elevation was adjusted through the use (or lack thereof) of medially wedged insoles of 10 mm, 14 mm, and 18 mm. The average error value in each condition was statistically analyzed. The findings of this study revealed as follows: 1) The average error value was significantly higher with the subjects' eyes open than with their eyes closed (p<.05). 2) The averaged error value was also significantly higher when the subjects were elevated 18 mm than with no elevation at all (p<.05). The findings of this study should be considered in lower extremity rehabilitation programs when medially wedged insoles used.

        The purpose of this study was to identify the influence of wedged insole and foot progression angle (FPG) on lateral thrust of knee in healthy subjects. Fifteen healthy male subjects were recruited from Suncheon First College, in Suncheon. The subjects randomly walked at the comfortable velocity under five conditions: bare footed, medio-lateral wedged insoles, toe-in and toe-out gait. The lateral thrust was measured by a accelerometer with telemeter during walking. Data was collected while each subject walked for about 10 gait cycle on a flat, level walkway at their normal speed. The middle three gait cycle were used for averaged peak value of lateral acceleration. The three averaged peak value of lateral acceleration were collected under each condition at heel strike. The results showed that averaged peak value of lateral acceleration increased significantly in medial wedged insole and toe-in gait and decreased significantly in lateral wedged insole and toe-out gait as compared with bare footed (p<.05). These results suggest that wedged insole as well as walking strategy, such as foot progression angle, may prevent progression of degenerative knee osteoarthritis.
