        Cellular myxoma is an uncommon type of myxoid benign tumor, predominantly occurring in adult female patients aged >40 years. This report aims to document a case of cellular myxoma that occurred in the buccal mucosa. Compared to intramuscular myxomas, cellular myxoma demonstrates hypercellularity and vascularity. Its manifestation in the soft tissue of the head and neck area is exceptionally rare. Generally, cellular myxoma manifests as a firm and immovable mass covered with normal oral mucosa, with no associated clinical symptoms. Homogenous low signal intensity on T1-weighted scans and high-signal intensity on T2-weighted magnetic resonance scans reveal cellular myxoma, as most lesions show well-defined margins and heterogeneous contrast enhancement. The significant histologic features include a focal or diffuse increase in cellularity with fibroblast-like cells and vascularity with an abundant collagenous matrix. Our presented case reflects these facts examinations, based on which a final diagnosis of cellular myxoma was made. Immunohistochemistry revealed locally and diffusely positive SMA and CD34. The clinical tendency of cellular myxoma with hypercellularity may affect the production of myxoid and collagenous substrates, and if complete resection is not performed, the possibility of local recurrence in the primarily affected region remains. Hence, complete surgical excision was performed under general anesthesia, and follow-up until a year after treatment revealed no observed recurrence. To achieve precise diagnosis and complete treatment without local recurrence, several diagnostic examinations should be considered.

        Background : Mucoepidermoid carcinoma (MEC) is the most common primary epithelial malignant salivary gland tumor in both adults and children. Histological grading of MEC is subjective, but plays an important role in predicting patient prognosis. Immunohistochemistry can accurately diagnose diseases and help with treatment and prognosis. The review of this paper was intended to be helpful in the differential diagnosis of mucinous epidermoid carcinoma. Methods : A PubMed search was carried out. Well-known biomarkers for mucoepidermoid carcinoma were searched in PubMed, and their differences with oral squamous cell carcinoma were compared. Results : When PubMed searched “oral mucoepidermoid carcinoma, biomarker”, a total of 241 papers were found, among which cytokeratin(22), Muc1(membrane-bound mucin1, 9), Muc4( membrane-bound mucin4, 6), Muc5ac (membrane-bound mucin5ac, 4), Muc5b (membrane-bound mucin5b, 3), p63 (15), PCNA (15), p53 (20), EGFR (Epidermal growth factor receptor, 21), c-erbB2 (HER2, 14), and pAKT (2) were searched and investigated. The biomarkers retrieved above were compared with those expressed in squamous cell carcinoma. Conclusion : Due to the above biomarkers, it is possible to classify mucoepidermoid carcinoma and differentiate it from other salivary gland tumors or oral squamous cell carcinoma.

        Differential diagnosis of the malignant lesion and the benign lesion is critically important for the precise treatment. A clinician should diagnose in a comprehensive manner considering clinical, radiological, and histopathological perspectives. The lesion in the oral cavity in this study was clinically and radiologically malignant. However, the lesion was histopathologically benign. Surgical intervention was not performed except biopsy. The lesion improved after about one month of supportive medication after the biopsy. The importance of decision making process was emphasized in this report.

        Enterococcus faecalis is a major causative agent of endodontic treatment failure. The purpose of this study was to investigate bactericidal effects of ethanol extract of Garcinia mangostana L. (mangosteen extract) on five strains of E. faecalis that were isolated from human oral cavities. The bactericidal effects of mangosteen extract were assessed by measurement of minimum bactericidal concentration (MBC) value. The cytotoxicity of mangosteen extract on immortalized human gingival fibroblasts, hTERT-hNOF, was determined based on cell counting method. The data revealed the MBC value of mangosteen extract against the E. faecalis strains was 4 ㎍/ml. Additionally, the cell viability of mangosteen extract on hTERT-hNOF was 83.7-89.1% at the 1 to 16 ㎍/ml. These findings indicated that mangosteen extract could be used as a root canal cleaner during management of endodontic treatment failure caused by E. faecalis.

        Kaposi’s sarcoma is one of the first recognized in HIV infection even patient can develop KS in any disease stage. Moreover, the oral cavity may be the first site of Kaposi’s sarcoma and oral mucosa involvements of KS in HIV positive are very critical because it had a higher death rate than those who having exclusively cutaneous manifestations of the disease. Early detection of KS oral involvement is very important to prevent disease burden. This case is about a 48 years old male who was first discovered Oral Kaposi’s Sarcoma as HIV infection sign. He didn’t notice any other discomfort other than oral mucosa swelling.

        This study was performed to investigate the correlation between the primary squamous cell carcinoma in oral cavity (POSCC) and paired metastatic oral squamous cell carcinoma in cervical lymph node (MOSCC) via immunohistochemical staining with Ki-67 and p53. The subjects included ten patients (20 specimens) who were diagnosed with OSCC with metastatic lymph nodes from 2010 to 2015 and surgically treated involving neck dissection in Kyungpook National University Hospital. Twenty specimens were stained immunohistochemically with Ki-67 and p53. The degrees of immunostaining by Ki-67 and p53 was evaluated as 0 (no positive cells), weak (1~25% positive cells), moderate (26-50% positive cells) and strong (>50% positive cells). Despite the strong tendency, there was no statistically significant result between expressions of Ki-67 and p53 in POSCC or MOSCC. We found that high expression of Ki-67 was significantly correlated with poor degree of differentiation. Our results suggest that expression of Ki-67 may be a predictable factor for degree of differentiation of POSCC and MOSCC.

        S. aureus is reported as a major cause of nosocomial infections after dental care and involved in endocarditis, bacteremia, osteomyelitis, peritonitis, and soft tissues etc. It is very important to identify the distribution and the diversity of toxin gene associated with the S. aureus expression in dental care patients with periodontitis directly for an effective prevention and treatment of dental diseases. Fifty four strains of S. aureus were isolated from the saliva of 129 patients who were diagnosed with periodontitis at dental clinics and hospitals located in Seoul. The distribution of the virulence gene and the genetic diversity of the strains were studied using the polymerase chain reaction with isolated strains. The enterotoxin test showed Seb was the most frequent gene with 88.9%. The hemolysin gene of Hla, Hib and Hld were the most frequently gene with 98.1% (53 strains), leukocidins gene of lukM showed 90.7% (49 strains), and laminin binding protein gene of Eno showed 100% (54 strains), respectively. The diversity of the enterotoxin gen was held as Seb-Seg-Sei gene of 35.2% (19 strains), the diversity of hemolysin gene of Hla-Hlb-Hld gene was 98.1% (53 strains) and the diversity of leukocidins gene of LukD-LukM were 88.9% (48 strains), respectively. Patients with dental disease showed somehow high toxin gene expression so that S. aureus in dental care area is judged to show very highly pathogen with a high and infection rate. In the future, additional studies for these toxin genes seem to be required.

        Neurotized melanocytic nevus (NMN) is categorized into intradermal/intramucosal type of acquired melanocytic nevus. In contrast to typical intramucosal nevus which has relatively distinct histological features, the diagnosis of NMN requires more attention due to its mimicry of benign neural tumors such as neurofibroma. The majority of lesional cells, NMN cells, showed a spindle cell morphology and abundant, eosinophilic cytoplasm which were positive for S-100, vimentin, and collagen type IV. Positive reaction for MART-1 was detected in the NMN cells as well as in the epithelioid nevus cells beneath the epithelium. Neurofibroma exhibited diffuse positivity for S-100, vimentin, CD34 and collagen type IV, but never expressed MART-1. Toluidine blue stained the numerous mast cells scattered in the lesion of neurofibroma, compared to the relatively minor detection of mast cells in NMN. Therefore, MART-1 is a useful marker in differentiating NMN from neurofibroma.

        The incidence of multidrug-resistant strains of Staphylococcus aureus continues to increase in hospitals and the community all over the world. As the importance of miner surgery in dentistry gets greater recently, the frequency in use of antibiotics has been increasing. Although several cases of acquired infection in oral cavity in dental hospital has been reported, the research on isolation of S. aureus and evaluation of its antibiotic resistance from outpatients in the dental clinic has not been found yet. The aim of this study was to evaluate the antimicrobial resistance of S. aureus isolates. The isolation rate of S. aureus were 41.7% in dental hospital personnel, 60.4% in outpatients and 62% in dental hygiene students. S. aureus which showed resistance to penicillin were 88.6%, ampicillin 88.6%, erythromycin 8.6%, tetracycline 4.3%, oxacillin 4.3%, cefoteten 4.3%. Six strains of S. aureus showed the susceptible to all antibiotics tested. Sixty-four(91.4%) strains were multiple antibiotic resistance. Fifty-two strains(72.8%) resisted to AM-P. and Seven strains(10.0%) resisted to AM-P-E, AM-P-GM, AM-P-TE.

        Adenosquamous carcinoma (ASC) is a rare malignant epithelial tumor in the oral cavity. It is regarded as a variant of squamous cell carcinoma (SCC) because ASC originates from the surface epithelium, has similar clinical features, and includes SCC portion which is histologically identical to the traditional form. This case report deals with two cases of ASC occurring in the oral mucosa of old male patients. Both case has tubular structures lined by cuboidal, basaloid, or small round cells under the conventional SCC component, which was reminiscent of the glandular differentiation of adenocarcinoma. On immunohistochemical studies, the glandular component of ASC showed positivity to CK-pan, CK7 and HMWCK, while the dysplastic squamous cells exhibited negativity to CK7. Accordingly, CK7 as well as morphological findings can lead towards differentiating ASC from other variants with gland-like structures, such as basaloid squamous cell carcinoma and adenoid squamous cell carcinoma.

        Metastatic tumors in oral cavity are rare, where their prognoses are considered to be extremely poor. Unless recognizing its primary origin, pathologic diagnoses for metastatic cancer have been troublesome for oral pathologists. This retrograde analysis was aimed at providing practical suggestion for the diagnoses of metastatic cancers to oral and maxillofacial region. We reviewed 20 patients diagnosed as metastatic cancers to oral cavity from 1991 to 2007. The patients were classified according to their clinical and histologic findings. We also reviewed 19 patients of mucoepidermoid carcinoma and 16 patients of adenoid cystic carcinoma to compare with those of metastatic cancers. Immunohistochemical staining for CK 5/6, CK 17, TTF-1, CEA was performed for differential diagnosis. Histologically, 20 cases compromised 11 cases of adenocarcinoma, 5 cases of undifferentiated carcinoma, 3 cases of squamous cell carcinoma, and one papillary carcinoma. The lung was the most common site for primary site (5/20), followed by the breast (2/20). In metastatic adenocarcinoma, TTF-1 positive cases were one lung cancer and a rectal cancer, and carcinomas from breast and rectum showed CK5/6 positive reaction. CEA was expressed in gastric and rectal carcinomas. In 19 cases of mucoepidermoid carcinoma, 13 cases (68.4%) are CK5/6 (+). In 16 cases of adenoid cystic carcinoma, 11 cases (68.8%) showed the positive reaction for CK5/6. TTF-1 is an antibody to show high sensitivity and specificity for lung adenocarcinoma, therefore, TTF-1 is helpful to make a diagnosis of metastatic adenocarcinomas from lung. Adenocarcinomas originated from salivary glands show high CK5/6 expression, but metastatic adenocarcinomas, except of those from breast and rectum, show no CK5/6 expression, lending support that CK5/6 may be useful to differentiate metastatic adenocarcinomas from carcinomas of salivary gland origin.

        Matrix metalloproteinases(MMPs) are involved in the degradation of extracellular matrix, which is re lated to infiltrative growth and metastasis of tumor and are regulated by tisslle inhibitor of metalloproteinases(TIMPs) or cell adhesion molecllles such as E-cadherin and epidermal growth factor receptor (EGFR). The aim of this study is to evaluate the relationship between MMP-2, MMP-9 expressions and clinico-pathologic factors, 끼MP-1 , TIMP-2, EGFR and E-cadherin expressions. lmmunohistochemical stains were perfom1ed on 55 cases of squamous cell carcinoma of the ordl cavity and the results were as follows. MMP-2 and MMP-9 expressions were noted in 30(54.5%) and 22(40.0%) of 55 cases, TIMP-1 and πMP-2 in 21(38.2%) and 33(60.0%), and E-cadherin and EGFR expressions in 35(63.6%) and 26(47.3%) of 55 cases, respectively. MMP-2 expression rdte was slightly higher 띠 cases without recurrence, and 끼MP- 2 expression rate was slightly higher in cases showing more inftltrative growth pattem. 까1e expression rate of EGFR was higher in cases with well differentiation(p=0.OO47), but no posi디ve relationship between the expression rate of Ecadherin and histologic grade was found. Cases with positive reaction for MMP-9 showed an increasing tendency of nega디ve reaction for TIMP-1. π1e expression rate of MMP-2 was higher in cases with positive reaction for E-cadherin and EGFR with no statistical significance. 까1e expression rate of MMP-9 was significantly higher in cases with positive reaction for E-cadherin(p=0.022l). These results suggest that MMP-2, MMP-9, TIMP-1 and TIMP-2 expressions are involved in the development of oral squamous cell carcinomas, but MMP-2, MMP-9, 끼MP-1 , and 끼MP-2 expressions might not seem to be a useful prognostic factors because there were no significant relationship between clinicopathologic parameters.
