여주대학 안경광학성공적인 수면작용 근시조절 각막교정술( Orthokeratology )은 콘택트렌즈를 이용하 얘 각막형상올 변화시켜 근시를 효과적으로 조절하는 것이다. 본 연구의 목적은 시험 렌즈를 이용하지 않고 직접주문 방법으로 피탱된 Ovemight Orthokeratology 콘택트 렌즈의 피탱성공흉올 얻기 위한 것이며， 이를 위하여 굴절이상도， 시력 및 각막형상 악 변화 정도률 조사하였다. 둥가구면 굴절이상도가 1-5D 사이인 18명의 끈시자를 대상으로 DreimLens를 착 용시키고 3 깨훨 동안 추적 관찰하였다. 렌즈 착용 전， 렌즈 착용 후 1 일째， 7 일째， 14 일째， 30일째 및 90 일째 각각 자각식 굴절검사， 자동 굴절 및 각막곡률 검사， Orbscan Topography /Pachymetry System 및 세극 둥올 이용한 전안부 검사를 실샤하였다. 나안시 력의 변화는 착용 l 일째부터 나타났고 7 일째까지 유의 한 감소를 보였다. 착 용 90 일째 89%에서 일상생활을 하는데 지장이 없는 정도인 0.8 이상으로 개선되고 64%는 1.0 이상을 유자하였다. 착용 90일째 둥가구면 굴절이상도는 평균 2.85D 정도 감소되었고， 각막중심부 굴절력은 1.13D 정도 감소되었다.90 일 동안 세극둥 검사로 부터 헨즈착용올 중단할 정도의 심한 각막부종이나 상피손상은 발견하지 못하였다. 본 연구 결과 나안시력의 개선， 끈시도의 감소 및 각막중심부 꿀절력의 감소 둥이 선 행 연구결과와 유사하였고 직접주문 펴탱법악 성공률이 매우 높았다.과 · 성균관대학교 교육대학원 서울 공안과와원 • 서물보건대학 안경광학과
The purpose of this study is to offer data for a successful overnight orthokeratology treatrnent by direct order fitting not by trial lens fitting. A successful ovemight orthokeratology is defined as an effective means of controlling myopia through temporarily reducing myopia. In order to achieve this purpose, we investigated the 없nount of refractive error change and its persistence. We also assessed visual, refractive and topographic changes in subjects undergoing overnight orthokeratology. Eighteen myopic subjects between 1.00 D and 5.00 D were recruited for three months trial of ovemight orthokeratology using DreimLens manufactured by Dreim Intemational Co. After commencing lens wear, subjects were examined on days 1, 7, 14, 30 and 90. Subjective refraction, autokeratometry, Orbscan topography! pachymetry and slit lamp examinations were performed at each session. After 1 day of lens wear, significant change of uncorrected visual acuity was noted and improved apparently by day 7. Uncorrected visual acuity of 89% of subjects at day 90 was over 0.8 and that of 64% of subjects was maintained over 1.0. The mean subjective refraction from baseline at day 90 was reduced 2.85 D. The mean change in central corneal f]attening was 1.13 D from baseline at day 90. Comeal edema and epithelium damage after wearing overnight orthokeratology lens did not show significant difference comparing to before wearing lens by a slit lamp examination. The results of this study were in accordance with the preceding works in terms of the improvement of uncorrected visual acuity, reduction of myopia. The success rate of direct order fitting of orthokeratology showed high. In order to success orthokeratology treatment, a careful patient selecting by clinical criteria and its consistent monitoring would be essential.
