Purpose : This study aimed to compare near contrast visual acuity between the near trial frame correction and multifocal contact lens (MFCL) wear in a clinical setting, and to analyze changes in higher-order aberrations (HOAs) between the naked eye (NE) condition and MFCL wear, thereby evaluating differences in visual performance according to age and pupil diameter. Methods : This study aimed to compare near contrast visual acuity between the near trial frame correction and multifocal contact lens (MFCL) wear in a clinical setting, and to analyze changes in higher-order aberrations (HOAs) between the naked eye (NE) condition and MFCL wear, thereby evaluating differences in visual performance according to age and pupil diameter. Results : Near contrast visual acuity was better under photopic conditions than under mesopic conditions, as indicated by lower log MAR values. Under all illumination and contrast conditions, MFCL wear resulted in a significant reduction in near contrast visual acuity of approximately 0.09 - 0.12 log MAR compared with the near trial frame correction (p<0.001). Differences in contrast visual acuity and some HOA components were also observed among the age groups. The HO RMS significantly increased with MFCL wear compared with the NE condition, and the Z(3,-1) vertical coma coefficient shifted significantly in the negative direction at both 3-mm and 5-mm pupil diameters (p<0.050). Conclusion : Near contrast visual acuity was better under photopic conditions than under mesopic conditions and was significantly reduced with MFCL wear compared with the near trial frame correction.
