While magnetic resonance imaging (MRI) provides high soft-tissue contrast and diverse quantitative information, inefficiencies in clinical workflows have been consistently cited as a problem due to long examination times, complex protocols, motion artifacts, and the burden of repeat imaging. Recently, artificial intelligence (AI) has been applied throughout the entire MRI workflow—from pre-examination protocol determination and quality control during acquisition to undersampled data reconstruction, image interpretation assistance, quantification, and result integration—contributing to reduced examination times and improved operational efficiency. In particular, deep learning-based reconstruction has established itself as a core technology for accelerated MRI, and recent studies have reported that acquisition time can be reduced while maintaining diagnostic performance. Furthermore, because the application of AI varies across different organs, such as prostate MRI, pituitary MRI, lumbar spine MRI, head and neck MRI, temporomandibular joint MRI, and brain tumor MRI, clinical implementation must consider both organ-specific characteristics and examination objectives. In addition, the level of evidence differs substantially across workflow stages; in particular, reading triage and operations management are supported by considerably less MRI-specific clinical validation than the reconstruction stage. However, issues regarding generalization performance, external validation, justification of AI decisions, standardization, regulation, and liability still remain in actual clinical adoption. This paper reviews the latest AI technologies for MRI workflow optimization step by step and comprehensively discusses key issues and future directions for clinical implementation.
