약제별 발생률은 Gadobutrol이 0.338%로 가장 높았으며, Gadoterate meglumine(Dotarem® 0.272%, Clariscan® 0.204%), Gadoxetate disodium(0.178%) 순이었다.
This retrospective study analyzed 193 cases of gadolinium-based contrast agent (GBCA) adverse reactions at a single institution from January 2021 to December 2025 to determine incidence, severity, and risk factors. Out of 66,233 administration records, the five-year average incidence was 0.292%, showing a significant annual increase (Z=2.762, p=0.006). The agent-specific reaction rates were highest for Gadobutrol (0.338%), followed by Gadoterate meglumine (Dotarem® 0.272%, Clariscan® 0.204%) and Gadoxetate disodium (0.178%). Gadobutrol showed a significantly higher reaction rate compared to Clariscan® (RR=1.662, p=0.009) and Primovist® (RR=1.896, p=0.031), peaking at 0.517% in 2025. A history of adverse reactions was a significant risk factor for moderateto- severe reactions (OR=3.82, p=0.031). In conclusion, radiological technologists specializing in MRI must recognize the higher incidence and increasing trend of Gadobutrol-related reactions. It is essential to update premedication protocols to incorporate a history of adverse reactions to iodinated contrast media and to strengthen targeted safety monitoring systems for high-risk patients.
