This study aims to review the literature on social disconnection among older adults and the prevention of solitary death to identify directions and areas for improvement in community support. It distinguishes living alone, social isolation, and loneliness and examines factors associated with social disconnection from the perspectives of social networks and social support. Academic literature and official policy documents will be analyzed, focusing on outreach, relationship building, service coordination, and continuity of support. Evidence of reduced social isolation or loneliness will be distinguished from direct evidence of effectiveness in preventing solitary death. This study seeks to inform community-based social work practice that extends beyond occasional welfare checks to provide sustained support for older adults’ daily lives and social relationships.
