목적 : 근시안을 대상으로 비구면 멀티포컬 콘택트렌즈 착용 후 안구의 고위수차의 변화를 분석하였다. 방법 : 각막굴절교정수술의 경험이 없고 안질환이 없는 건강한 성인 33명(평균 연령: 23.03±2.8세, 평균 등 가구면굴절력: -3.28±1.67 D, 66안)을 대상으로 하였다. 단초점 콘택트렌즈와 Low 가입도 및 High 가입도의 멀티포컬 콘택트렌즈(Biotrue, Bausch + Lomb, USA)를 착용시킨 후 Wavefront Analyzer(KR-1W, Topcon, Japan)를 사용하여 암소시(scotopic, 0 lx)와 명소시(photopic, 20 lx) 상태에서 전체 고위수차, 코마수차, 구면수차를 측정하였다. 통계는 SPSS(version 21.0) 프로그램을 이용하여 분석하였다.
결과 : 전체 고위수차는 멀티포컬 콘택트렌즈(Low 및 High 가입도) 착용 시 나안 상태보다 증가하였고 (p=0.012, p<0.001), 가입도가 높으면 더 증가하였다. 코마수차도 단초점과 멀티포컬 콘택트렌즈(Low 및 High 가입도) 착용 후 나안 상태보다 증가하였다(p<0.001, p<0.001). 구면수차는 멀티포컬 콘택트렌즈(Low 및 High 가입도) 착용 시에는 나안 상태보다 더 음의 방향으로 증가하였고 가입도가 높을수록 증가하였다.
결론 : 비구면 멀티포컬 콘택트렌즈 착용 후 전체 고위수차, 코마수차는 증가하였으며, 구면수차는 음(-)의 방향으로 증가하였다. 멀티포컬 콘택트렌즈의 경우 착용 시 구면수차가 음(-)의 방향으로 증가하면 멀티포컬 콘택트 렌즈의 가입도 효과가 줄어들 수 있어, 착용자의 눈의 구면수차 방향에 따라 근거리 교정 효과가 감소 또는 증가 할 수 있고, 코마수차의 증가로 시력의 질이 저하될 수 있을 것으로 사료된다.

Purpose : To analyze changes in high-order aberrations (HOA) after wearing aspheric multifocal contact lenses (MFCL) for myopic eyes.
Methods : Thirty-three healthy adults (mean age: 3.03±2.89 years, mean equivalent spheric power: -3.28±1.67 D, 66 eyes) with no history of ocular surgery, and systemic disease were enrolled. After wearing single vision contact lens (SVCL) and low and high add MFCL (Biotrue, Bausch + Lomb, USA) HOAs, coma, and spherical aberrations (SA) were measured in a scotopic (0 lx) and photopic (20 lx) condition using Wavefront Analyzer (KR-1W, Topcon, Japan). Statistical analysis was performed using SPSS (version 21.0).
Results : Total HOAs increased in the eyes wearing MFCL (Low and High addition) compared to the naked and SVCL wearing eyes (p=0.012, p<0.001), and it increased more with high add MFCL. Coma aberration increased in the eyes wearing SVCL and MFCL compared to the naked eye (p<0.001, p<0.001). The SA in the eyes wearing MFCL changed to the negative direction and much more with the high add MFCL.
Conclusion : After wearing aspherical MFCL, the total HOA and coma aberration increased, and the SA changed to the negative direction. According to individual aberrations of the presbyopia wearing aspheric MFCL, the negative SA of MFCL could be compensated or enforced that MFCL addition effect can be decreased and visual quality may be decreased due to coma aberration.
