        The purposes of this study was to evaluate the effect of low power GaAsAl laser on tissue contraction in a linear incision wound on rat skin. The linear incision wound was made on the midline of the backside in the experimental animals. Low power laser applications with different intensities such as 3, 6, or 10 mW were applied to the experimental animals twice a day for 10 days. On either the seventh or tenth postoperative day, the quantitative analysis of the inflammatory reaction surrounding the linear incision wounds on the rats were performed using enzymatical analysis of myeloperoxidase (MPO) activity. The number of neutrophil was from a normal blood sample that was obtained from the normal experimental animals. Each concentration of neutrophil showed .04-.62 unit activity of MPO. Therefore, the 6 unit activity of MPO per neutrophil was unit. On the 7th and 10th post operative day, non treated tissues demonstrated increased MPO activity as compared to that of normal tissue. These data indicated that the inflammatory reaction of tissue was induced after wound induction and the MPO activity were increased in the inflammed tissues. While both 3 mW or 6 mW intensity of laser treatments did not affect the tissue MPO activity, 10 mW intensity of laser treatment significantly decreased the tissue MPO activity on the 7th and 10th post operative day. These data demonstrated that only 10 mW intensity of laser treatment successfully suppressed tissue inflammatory reaction after wound induction. In conclusion, these findings suggested that 10 mW of GaAIAs laser treatments effectively suppressed the inflammatory reaction of tissue that was induced during the wound healing process.

        The purpose of this study was to compare the experimental pain threshold when used in luminous lamp radiation and nonluminous lamp radiation with healthy person. Thirty normal subjects were randomly assigned two groups: a luminous lamp radiation group, and a nonluminous lamp radiation group. The infrared lamps were applied on L3 for thirty minutes. Each group was measured for experimental pain threshold and local temperature before, 15 and 30 minute radiation. For statistical differences in change of the experimental pain threshold and local temperature due to differences in lamp ray was compared using the independent t-test. And, General linear model for profile plots test was used. The results were as: 1. Local temperature was significantly increased in the nonluminous lamp group (p<.01). 2. Experimental pain thershold was significantly increased in the luminous lamp group (p<.05),(p<.01). This study indicate that luminous lamp radiation was more effects of increase experimental pain thershold than nonluminous lamp radiation. Further study is needed to compare the effects of after period radiation.

        The purpose of this study was to determine the effect of intramuscular stimulation (IMS) therapy in older persons with musculoskeletal pain. The subjects were 181 older persons (54 males, 127 females) with musculoskeletal pain. Intramuscular stimulation unit with needles (size mm) was applied for the treatment. The analgesic effects were measured by visual analog scale (VAS). Results showed that the post-treatment VAS score was significantly decreased after IMS therapy for fifteen minutes compared to pre-treatment score. In addition, the post-treatment VAS score was significantly decreased in patients with chronic pain (pain duration of one year after onset) compared to the post-treatment VAS score in patients with subacute pain (pain duration less than three months after onset). There was no significant difference in analgesic effects according to gender and age groups. It is determined from this study that IMS therapy can be beneficial for patients with chronic musculoskeletal pain in clinical setting. Further study is needed to identify whether the IMS therapy can change the pain threshold in patients with neurologic pain.

        The purpose of this study was to compare the body alignment during standing on level and wedge board. Twenty healthy college students (8 females, 12 males) were evaluated in this study. Diagnostic contourline potographic imaging system (Model JTC-1, Jodang Trading Co.) was used to measure body alignment. Sagittal and frontal plane images were used to analyze the body alignment. The result showed that the cervical and lumbar lordotic curve significantly decreased during standing on wedge board when compared with standing on level. On the other hand, thoracic kyphosis significantly increased during standing on wedge board. There was no significant difference in body alignment according to gender, weight, and height. Clinically, patients with low back pain and severe lordosis may be affected by heel wedge. Further study is needed to identify whether the standing on wedge board can change the body alignment in patients with low back pain and spinal deformity.

        The purpose of this study was to identify vital capacity (VC) in relation to the changing position of 19 patients with spinal cord injury (SCI) and 20 normal controls. Among the 19 SCI patients, there were 9 quadriplegics and 10 paraplegics. The vital capacity was measured in each subject during sitting, standing, lying, and head down position of 30 degrees. The data were analysed by the Kruskal-Wallis test, Mann-Whitney test, and Wilcoxon signed rank test. For the SCI, significant difference of VC accoring to the 4 positions between quadriplegics and paraplegics. In the control group, significant difference of VC according to the 4 positions. In 4 positions the VC of men were significantly larger than that of women between two groups. No statistical significant difference was shown in VC by the postural change between quadriplegics and paraplegics.

        The purpose of this study is to introduce prosthetic discipline and ambulation training after hemipelvectomy due to osteosarcoma. Over the past years, when the malignant bone tumors occurs in the extremities amputation is not enough to prevent a part recurrence and distal transformation resulting in fatal prognosis. On the other hand, these procedures could bring about a difficulty in rehabilitation in curing patient who have had hemipelvectomy. However the recent development of chemotherapy and diagnostic facility have permitted the orthopedic surgeons to many try amputations for the treatment of the malignant bone tumors. Unfortunately, there has not been many researches on hemipelvectomy. Since there is no studies found on hemipelvectomy either. Therefore, we introduce successful procedures for rehabilitation through the ambulation training for patients who have had amputation. One of our patients, who is an eighteen years old male, has had hemipelvectomy on the eighteenth of June in 1997 after his anticancer treatment over 12 times. He has had physical therapy of prosthetic ambulation training at the department of rehabilitation medicine Yonsei University Medical Center from the fifteenth of October to December '2nd in 1997.

        The objective of this study was to identify the effects of pelvic tilting exercise on gait patterns of hemiplegic patients. The subjects of this study were 31 hemiplegic in- and out-patients of the Rehabilitation Hospital, Yonsei University Medical Center, from September 24, 1997 through November 5, 1997. Pre- and post-treatment change in gait patterns were measured using a ink foot-print. The data were analyzed by the paired t-test, one-way ANOVA, and independent t-test. The findings were as follows: The difference in gait patterns between pre- and post-treatment was statistically significant, with an increase in gait velocity to 7.98 cm/sec post-treatment; an increase in cadence to 7.29 steps/min; a narrowing of the base of support to 1.33 cm; an increase in step length of 3.92 cm on the less affected side and 3.73 cm on the more affected side; an increase in stride length of 5.82 cm on the less affected side and 5.92 cm on the more affected side(statistically not significant in foot angle). In relation to sex, age, cause of stroke, and laterality of paralysis, the difference in gait patterns between pre- and post-treatment was not statistically significant. Where there was no significant difference of the effects of pelvic exercise regarding the degree of spasticity, the presence of a decrease in proprioception, and the duration of treatment. In conclusion, hemiplegic pelvic tilting exercise was found to have transmitting positive effect in improving gait patterns.

        The ability to maintain an upright position during quiet standing is a useful motor skill. The Sensory Organization Test (SOT) is a timed balance test that evaluates somatosensory, visual, and vestibular function for maintenance of upright posture. The Fugl-Meyer Sensorimotor Assessment (FMSA) balance subscale is the functional status assessment that indicates amount of assistance needed during various balance tasks. Functional Independence Measure (FIM) is the functional status assessment tool and FIM can be used clinically as an outcome measure. The purpose of this study was to see if the SOT can be used as a evaluation tool to measure hemiplegic patients' balance ability. Thirty-six patients with hemiplegia participated in this study. SOT scores were significantly correlated with FMSA balance scores and FIM scores(p<0.05). However correlation coefficients were not so high (r=0.60, and r=0.51, respectively). Therefore, further study is needed to verify the SOT's usefulness when physical therapists are evaluating for hemiplegic patients' balance ability.

        The purpose of this investigation was to examine the influence of head and neck(HN) position in the transverse plane on the static production of elbow extension force in the involved(paretic) upper extremity of patients with hemiparesis. On this study, thirty patients who had experienced a cerebrovascular accident were matched with neurologically intact subjects. Force of static elbow extension was tested with a hand-held dynamometer, twice with the HN rotated toward the paretic side and twice with the HN rotated toward the non-paretic side. Elbow extension force differed significantly with the HN in the two position in patients with hemiparesis but not in normal persons(=0.05). Results of this study support the conclusion that HN position in the transverse plane influences the production of static elbow extension force on the paretic side in patients with hemiparesis.

        The purpose of this study was to introduce rotation-plasty procedure and prosthetic ambulation training. The recent development of chemotherapy and diagnostic facility have permitted the orthopaedic surgeons to try limb saving procedures rather than amputations for the treatment of the malignant bone tumors. If the tumors around the knee joint were treated by mid-thigh amputation or hip disarticulation, it would impose the client with a great handicap for rehabilitation. Rotation-plasty procedure was first done by Borggreve, in 1930 for the congenital short femur. Recently this procedure was used a malignant bone tumor at the distal femur by Kotz and Salzer in 1982. In spite of its cosmetic problem of the distal stump, this procedure has the great functional advantage of converting the above-knee amputation to the below-knee amputation. The inverted foot was also good to control the prosthesis as a below-knee stump and heel functioned as a patella to support the body weight. This 15 years old girl case was had rotation-plasty due to osteosarcoma of the distal femur with 3rd postoperative chemotherapy, and admitted to Yonsei rehabilitation hospital for prosthetic ambulation training. Then, the case had excellent functional results of prosthetic ambulation training with rotaion-plasty after 3 months.

        The purpose of this case study was to introduce a myoelectric hand prosthesis for upper extremity amputee and prosthetic training program. Limb loss can result from disease, injury, or congenital causes. Trauma has been increasingly important role as the cause of amputaion in young, vigorous, and otherwise healthy individuals. The higher the level of amputation the greater the functional loss of the part, and the more the amputee must depend on the prostheis for fuction and cosmesis. Myoelectrical control of prostheses is a recent development and has been steadily gaining in clinical use over the past 20 years. Such a prosthesis uses signals from muscle contraction within the stump to activate a battery driven moter that operates specific component fuctions of the prosthesis. This twenty years old male case was operated a right above-elbow amputation due to tracffic accident and admitted to Yonsei Rehabilitaion hospital for the preprosthetic and prosthetic training. The case was able to successfully complete his myoelectric hand prosthesis training in the February of 1995.

        The purpose of this paper is to review the literature of AIDS/HIV (acquired immune deficiency syndrome/human immunodeficiency virus) and to introduce physical therapy of the AIDS client. It was first reported that five young homosexual men, in 1981 in Los Angeles, died of pneumonia caused by pneumocystis carinii - called "pest of twentieth century". AIDS was implied a fatal breakdown of the functioning of the normal human immune system. The major routes of transmission has occurred via l)sexual contact between men, 2)heterosexual intercourse, 3)contaminated blood and blood products, and 4)intrauterine or pre-natal infection from mothers. AIDS clients who may require physical therapy for a wide range of problems due to respiratory, neurological, musculoskeletal and painful syndromes as well as general decline in fitness and function. AIDS is growing rapidly in our contury as well as all over the world. Therefore, physical therapist has to correct his understandings of AIDS in order to protect himself and to give proper physical therapy to the AIDS clients.

        The purpose of this paper is to provide an overview of the clinical physical therapy program used at the University of Yonsei Rehabilitation Hospital, for the practicing university trained physical therapists who may be unfamiliar with patients who have suffered a myocardial infarction. The four primary phases of the cardiac physical therapy graded exercise program are: 1) coronary care unit program (phase I), 2) general ward program (phase II), 3) convalescence program (phase III), 4) maintenance program (phase IV). The exercise prescription defines the exercise intensity, duration, frequency, and mode of exercise a after pre- discharge low level graded exercise test(LL-GXT) or symptom limited maximum graded exercise test. A typical exercise routine consists of preparation warm-up exercise, therapeutic exercise, cool-down exercise. Physical therapy is involved in the acute care and rehabilitation of the patient after a myocardial infarction. Therefore, the physical therapist must throughly comprehened the cardiac anatomy, cycle, performance, conduction system, pathogenesis, risk factors, and exercise benefits.
