        Objective : The purpose of this study was to investigate attitudes of Korean occupational therapists toward introducing an accreditation system of occupational therapy education programs. Method : The self-administering written questionnaire was employed for this survey research. The questionnaire was developed and reviewed by the Accreditation Board for the Education of Korean Occupational Therapists and was composed of 18 items which included both closed-ended and open-ended questions. The questionnaire was distributed to Korean occu-pational therapists who attended at the 10th annual conference of Korean Association of Occupational Therapists in October 2002.Results : Among 106 respondents, 80.2% expressed their concern about the sudden increase of new occupational therapy education programs and they believed that the curricula of new in-stitutions and the quality of the institutes’ graduates would be different. On the question of ne-cessity of introducing the accreditation system of occupational therapy education programs, 89.7% responded that it was necessary. All respondents agreed that clinical fieldwork experience was necessary and the preferred length was 16, 24, and 12 weeks in order. The areas to be in-cluded in the accreditation were the number of qualified professors, fieldwork hours, and the contents of curricula in order. 77.4% of the sample responded that they would trust the result and process of the accreditation. Conclusion : Based on the result of the survey, it could be inferred that the majority of oc-cupational therapists were concerned about improving quality of the profession rather than its quantitative expansion. Therefore, it is expected that the accreditation system of occupational therapy education programs would ensure the qualitative growth of occupational therapy pro-fession in Korea.

        Objective : The purpose of this study was to compare the sensory-based behaviors of young children with and without cerebral palsy as reported by their parents on the Short Sensory Profile.Method : Short Sensory Profile questionnaire was completed by parents of 59 children with cerebral palsy 1 through 9 years of age and parents of 129 children without cerebral palsy 1 through 6 years of age.Results : The performance of children with cerebral palsy was significantly different form that of children without cerebral palsy on 4 of 7 factors. Factors were difference found included Movement Sensitivity, Auditory Filtering, Low Energy/Weak, Visual/Auditory Sensitivity. Conclusion : Finding from the study suggest that young children with cerebral palsy have probable deficit in a variety of sensory processing abilities as measured by the Short Sensory Profile. The Short Sensory Profile can provide information about the sensory processing skills of children with cerebral palsy to assist occupational therapist in assessing and planning inter-vention for these children.

        Purpose : This study investigated the consumer satisfaction of the rehabilitation counseling via internet for parents of children with developmental disabilities aged under 6. Methods : The web site for internet counseling (ddchild.com) consisted of 1,500-page information for educating children at risk, with mental retardation, autism, learning disabilities, and cerebral palsy. Internet counseling was conducted in the form of Q/A, real-time chatting, monthly interview, and search engine. The consumer survey form was dispatched to a total of 1,459 registered members and 91 (7%) of them replied to the survey. The survey included general information of the respondents, the types of disabilities they used mostly, their satisfaction level, criticisms and suggestions to this internet counseling. Results : The general characteristics of the respondents were: (1) The users were distributed across the nation from Seoul and its vicinity (61%) to Cheju (1%); (2) The respondents’ 68% was middle or lower middle class; (3) The types of disabilities were children at risk including learning disabilities and ADHD (37%), autism (21%), mental retardation (21%), cerebral palsy(18.5%). The 68% of the respondents were satisfied with the internet counseling, and 65% actively used the data base consisted of rehabilitation information for educating children with developmental disabilities. Regarding the usage of the data base, 49% replied that this program was very helpful, because the instructions were described with concrete examples, 47.7% “helpful in understanding how to teach these children,” and 27.7% “in better understanding the behaviors of these children.” As to the suggestions, the respondents demanded two things. One is that the government build more schools and centers for educating these children along with more economic support, and the other requests are related to the operation of this internet counseling: enforcing of speech therapy program, opening of demonstration program with regard to the usage of this system, and setting up a psychological counseling for the parents. Conclusion : Operation of the internet counseling system can be a viable approach for providing early intervention programs for parents with developmental disabilities with low-cost effectiveness in the country where early intervention has not been systematically established.

        Objectives : To develop a basic ADL instrument for persons with stroke, reflecting our cultural values. Method : The ADL instruments that have been developed previously were gathered, excluding the ADL items, which lowered the validity of the instrument. We surveyed occupational therapists on the values of basic ADL with a total of thirty-four item cards. The value of ADL was measured by Q sorting technique. And ninety-eight patients with stroke were evaluated to gain final items selection and weighted items. Results : Ten items for inpatients were selected. For inpatients, functional ambulation at indoor was weighted to four times; the excluded items using proper utensils, and cleaning mouth were weighted to two times. Conclusion : We developed a basic ADL instrument for inpatients and the range of score of each instrument was 0∼80. The internal consistency for inpatients was high (Cronbach α=0.94). However, further study requires the concurrent validity, construct validity and the test-retest reliability of the present ADL items.

        Objective: The purpose of this study was to research the current state of occupational assessment tools and their use by Korean occupational therapist. Method: Data was obtained from October 13, 2001 to December 30, 2001 by means of a survey questionnaire. The participants were 96 occupational therapists (general hospitals, university-related hospitals, rehabilitation center, etc.). Analysis of the data was done by using descriptive statistics. Results: The length of time for initial evaluation was 42 minutes per one patient and reevaluation periods were every 3 months or as functional changes were detected. Among the assessment tools, the most frequently used tools w ere: Purdue Pegboard Test (47.9%) for assessing motor area, Motor-free Visual Perception Test (56.3%) and Mini-Mental Status Examination (54.2%) for cognitive perceptual assessments, Functional Independence Measure (56.3%) for daily activity assessment, Denver Developmental Screening Test (51%) for development assessment, Sensory Profile (14.6%) for sensory integration assessment, and Canadian Occupational Performance Measure (16.7%) for occupational performance assessment. Conclusion: The results of this study supported that the choice of evaluation tools may be depended on the administration length of time and simple procedures. Based on the results, provisions of more extended educational opportunities would promote the use of a greater variety of standardized assessment tools by Korean occupational therapists.

        Objective: This study was performed to measure the level of stress of cerebral palsy workers, and to investigate the relationship between the stress dimensions and stress inducing factors of cerebral palsy workers in Korean manufacturing industry. Method: The sample used in this study consists of 66 employees with cerebral palsy wokers. They are employees of the protective employment companies located in Seoul, Inchon, and Gyung-gi province. The Psycho-Social Well-being Index (PWI), which was designed through modification of Goldberg’s 60-items of General Health Questionnaire (GHQ), was used to measure the stress level. The following variables, independent and moderate variables, are designed for the same purpose. The independent variables are general and disability characteristics, workplace-related characteristics of the respondents. The moderate variables include social support, type A behavior pattern, self-esteem and coping strategy. The collected data were analyzed using SAS version 6.12 program. Results: The results of this study were summarized as follows; 1. For the moderate variables, the low self esteem, and A type behavior were more significant to measure the stress level. 2. The multiple regression analysis showed that the significant variables on the level of stress of cerebral palsy workers were low self-esteem and A type behavior pattern. Conclusion: The results of this study suggest that appropriate health promotion programs, should be established and provided for reducing the stress of cerebral palsy workers in manufacturing industry.

        Objectives: To quantify the range of motion of upper limb while eating with and without a universal cuff (UC). Method: The feeding cycle was divided into pre-feeding and feeding phases. The pre-feeding phase was operationally defined as scooping water with a spoon. The feeding phase was operationally defined as bringing the spoon to the mouth. A three-dimensional CMS70P analysis system was used to record upper limb motion during eating activities. Ten male and eight female healthy volunteers participated in this study. Results: (1) in pre-feeding phase: when a spoon without a UC was used, wrist was flexed and shoulder was adducted. However, when a UC with a spoon, the wrist was extended and shoulder was abducted. Also, the range of motions (ROMs) of shoulder flexion and internal rotation were significantly increased (p<0.05). (2) In feeding phase: when a spoon without a UC was used, wrist was flexed and radially deviated. However, when a spoon was used in a UC with a spoon, wrist was extended and ulnarly deviated. The ROMs of forearm supination, shoulder abduction, and external rotation were significantly increased(p<0.05). Conclusion: The ROMs of upper extremities with UC were significantly different from without UC. Therefore, when applying a universal cuff on patients, appropriate ROMs for a universal cuff should be considered prior to OT treatment.

        This study examined the correlations among the Berg Balance Scale, which is a clinical tool used to evaluate balance ability, spatiotemporal parameters of gait, and falling; determined the parameters most closely related to falling; and identified a discriminatory parameter and its predictability. Thirty-four subjects aged 72 to 92 years participated in this study. Following a questionnaire survey about falling, the Berg Balance Scale and spatiotemporal parameters of gait were measured. The results revealed that the incidence of falls increased with aging and an accompanying reduction in the flexion range of motion of the hip joint. The gait characteristics of elderly people who fell easily included a slower walking speed, shorter stride, and longer stance time than other elderly. When the cutoff score was set at 45, the Berg Balance Scale was able to identify correctly those individuals who truly have experience of falling than when the cutoff score was set at 39. But when the cutoff score was set at 39, the scale's specificity identifying correctly those individuals who truly have not experience of falling was higher than at the cutoff score of 45. Therefore, the Berg Balance Scale is an appropriate screening method in a clinical setting for the early detection of elderly people at risk of falling. In conclusion, elderly people with a Berg Balance Scale score. below 45 are the most likely to fall owing to their decreased balance ability.

        Objective: To explore the effect of a eye patching technique in order to decrease patients’ unilateral neglect. For this study, eye patching is easy to get and a simple method that most patients find acceptable. Method: This study used the ABA design. The subject was a male patient at the age of 66 who had the left hemiplegia and left unilateral neglect caused by right cerebral vascular accident. He was required to wear the eye patching on the right eye during all the period of ADL because unilateral neglect causes many problems in the majority of ADL. The assessments for the unilateral neglect had been performed by the Line bisection and the Albert test once a session (day). Results: Regarding the Means of the errors to the Line bisection and the Albert test, the Means of the errors was decreased during the period of the intervention (B) and was decreased after the withdrawal (A'). Conclusions: Right monocular patching was effective to release the unilateral neglect. Further study is necessary to increase the sample size and the duration of experiments.

        The purpose of this study was to investigate the effect of proprioceptive neuromuscular facilitation (PNF) approach on the spasticity of affected side in patients with hemiplegia. Three subjects with hemiplegia participated in this study. In single-subject research design (a multiple baseline across individuals) was employed in this study. The intervention program including PNF to the unaffected side was introduced for 30 minutes each day during each intervention phase. Muscle tone of affected side was measured with Tone Assessment Scale, and active hip abduction distance of affected side was taken with the subject supine. And then the muscle tone and the hip abduction distance were measured again 30 minutes later following the intervention. The results showed that the PNF application had some beneficial effects on both muscle tone and active hip abduction. This result suggest that PNF application to non-paretic limb can be effective in reducing muscle tone and improving hip abduction range on paretic limb in persons with hemiplegia. However, further research is needed to prove the effect of PNF application on functional improvement.

        The purpose of this article was to explore the potential for the use of mental practice for the rehabilitation in persons with stroke. Clinical application of mental practice in physical medicine and rehabilitation have not flourished to the same extent as application of mental practice to sports psychology. Studies from the disciplines of kinesiology, neurophysioloy and psychology support the use of mental practice in the post-stroke population. A model is included to demonstrate how mental practice can work through motivation, cognition and neural reorganization to facilitate motor relearning and functional independence. Recent evidence using modern brain-imaging techniques suggest that motor planning during both imagined and actual movements use very similar brain centers. Influencing factors such as imaging ability, task, and method of instructions are discussed. The advantage and disadvantage of using mental practice are included. The potentials of mental practice as a method of promoting and acquiring the exercise functions of stroke patients are being recognized, and is expected to have high effectiveness as a method of improving the functions of the patients in the field of occupational therapy.

        The purpose of this study is to provide developmental data of the performance of children between the age of 6 to 12 to verify the hierarchical order of acquiring the cognitive competencies and to identify whether differences in the performance between male and female tested in revised Loewenstein Occupational Therapy Cognitive Assessment (LOTCA). The test consists of six subtests designed to be representative of various cognitive abilities. The subjects were 60 normal children from age 6 to 12 were assigned to a group 1 from 6.0∼7.11, a group 2 from 8.0∼9.11 and a group 3 age 10.0∼12.0. We measured their cognitive-perceptual ability with LOTCA. The results were a progress with from 6 to 12 years in both and speed of performance. Male group showed better score than females in Shape Identification of Visual Perception, Logic Question of Thinking Operation, Attention And Concentration. The Females are better than males in Motor Imitation of Motor Praxis, Pegboard Construction and the performance duration, Colored Block-Design and the performance duration, Plain Block-Design and the performance duration, and The length of time taken. Several subtests except Visual Perception, Direction on Cs’ Body, Spatial Relation of Spatial perception, Motor Praxis, Two-Dimensional Model of Visuomotor Organization, ROC Unstructured, ROC Structured, Pictorial Sequence A, and Pictorial Sequence B of Thinking Operation, which are showed significant differences between age groups. With the above results, we conclude that LOTCA is an appropriate tool for occupational therapists to use in assessing cognitive-perceptual abilities.

        The purpose of this study was to obtain normative data and comparing cognitive abilities of patients with stroke and normal population by administering Lowenstein Occupational Therapy Cognitive Assessment (LOTCA). The subjects were 31 patients with stroke and 19 adults in a good health. The results were as follows: 1. The experimental group received statistically lower scores than the control group in all items except for object identification, shape identification and copying geometric forms. 2. There was no significant difference in analysis of items based on affected side. 3. There was a significant difference in the relationship between educational background and visuomotor organization and thinking operation. 4. There was a significant difference in the relationship between treatment period and visuomotor organization. 5. There was no significant difference in the relationship between LOTCA scores and age and onset of the disease. The result shows that there was a significant difference in LOTCA scores between the experimental group and control group. These data are expected to be used as a baseline.

        The purpose of this study was to compare and evaluate various hand functions in three hip flexion postures(70°, 90°, 110°)in persons with spastic diplegic cerebral palsy using by Jebsen Hand Function Test. 14 children with mild or moderate spastic diplegia were chosen in the Rehabilitation Hospital, Yonsei University Medical Center. Both hands were tested 3 times by ten intervals in three hip flexion postures (70°, 90°, 110°). Data Collected were analysed using nonparame-tric test, Friedman's Test. Results were as follows: 1. The right hand mean score of Jensen Hand Function Test showed no significant difference in any of the three hip flexion postures. 2. The left hand mean score of Jensen Hand Function Test showed no significant di- fference in any of the three hip flexion postures. The results showed that the hand function of children with spastic diplegia was not affected in three hip flexion postures, but it is necessary to maintain well supported pelvic posture for good hand function.

        The purpose of this study was to evaluate the cognitive perceptual performance in relation to activities of daily living in stroke patients who were treated at department of rehabilitation medicine of the Samsung Medical Center from December 5. 1996 to April 10, 1997. The cognitive perceptual performance was measured by the Lowenstein Occupational Therapy Cognitive Assessment (LOTCA) and by the Modified Barthel Index (MBI) to evaluate activities of daily living(ADL). Sixty-seven stroke patients were the subjects in this study. Collected data analysis were completed by using t-test. ANOVA, correlation analysis, and multiple regression analysis. The following were as follow: 1. Among the subtest of the LOTCA, orientation to time, plain block design, reproduction of puzzle showed significant difference depending on patient's affected side. Orientation to time score was higher with left hemiplegic patients. Plain block design and reproduction puzzle score were higher with right hemiplegic patients. 2. Among the subtests of the MBI, dressing, ambulation, and self-care were significantly correlated with the subtests of the LOTCA in order of thinking operation, visuomotor organization, perception and orientation. 3. The result of the cognitive perceptual function affecting the MBI performance using multiple regression analysis, thinking operation was the most influential factor. The cognitive perceptual perfomance have significant correlation with activities of daily living skills. Also, it is an important factor to functional recovery after stroke. Therefore, accurate evaluation of cognitive perceptual performance after stroke with treatment of each patient's appropriate level should be done simultaneously with rehabilitation treatment to improve patient's activities of daily living skills.
