Purpose: This study was conducted to assess the post-discharge experiences of caregiving mothers of pediatric patients with intestinal failure who were receiving home TPN treatment. Methods: This was a qualitative study utilizing Colaizzi’s phenomenological research method. The eight participants were mothers of pediatric outpatients from the short-gut syndrome clinic at a tertiary hospital in Seoul who were continuing home TPN treatment through a CVC. Data were collected from January to May 2022 through individual in-depth interviews, and analyzed. Results: Analysis of 127 meaningful statements from the mothers identified 12 themes and 36 sub-themes, organized into five categories: “Mixed emotions regarding hospital discharge”, “Problems after discharge”, “Stress in everyday life”, “Support from nurses and family members”, and “Looking to the future”. Conclusion: The study results provided insights into the meaning and value of the post-discharge experiences of mothers of pediatric patients with intestinal failure. These findings will be valuable in the development of interventions to provide education and other support measures for primary caregivers of children with intestinal failure.

목적 : 본 연구에서는 문헌 고찰을 통해 가족 간병인을 대상으로 역량강화 프로그램의 특성에 대하여 분석 하였다. 연구방법 : Pubmed, Excerpta Medica Database(Embase)를 사용하여 2013년 1월부터 2023년 5월까지 출판된 논문을 검색하였다. 주요 검색어는 “empowerment program”, “caregiver”, “caregivers”, “family caregiver”를 사용하였고 총 420편 검색 중 최종 5편의 국외 논문을 선정했다. Hierarchy of levels of evidence for evidence-based practice, PEDro Scale을 통해 연구의 질을 평가하였다. 연구의 분석 내용을 제시하기 위해 Patient, Intervention, Comparison, Outcome(PICO)를 사용하였다. 결과 : 선정된 논문의 대상자는 치매, 경도인지장애, 뇌졸중, 다발성 경화증 환자의 간병인이었다. 중재 기간 4일에서 12주 동안 가족 간병인에게 교육, 상담, 토론 등을 진행하였고 우울, 삶의 질, 간병 부담, 간병 태 도, 자기효능감 등의 영역에서 효과적임을 확인하였다. 결론 : 본 연구에서는 성인, 노인 만성 질환자의 가족 간병인을 대상으로 한 역량강화 프로그램 중재가 효과 적임을 확인하였다. 이는 가족 간병인에 대한 중재 프로그램 임상 활용 및 추후 연구를 위한 정보 제공 및 근거를 마련하였다.

Purpose: The purpose of this study was to describe the perception of biomedical ethics in nurses and nurse’s aide of comprehensive nursing care service. Methods: The subjects were 287 registered nurses and 81 nurse’s aides who were working in comprehensive nursing care service. The data were collected from December 2 to 15, 2019 using a 4-point Likert scale questions. The data were analyzed by descriptive statistics, t-test, ANOVA, Scheffé test and Dunnett T3 test, using the SPSS/WIN 23 program. Results: The average score of perception of biomedical ethics in nurses were 2.95±0.25 and nurse’s aides were 3.08±0.25 points. The perception of biomedical ethics by general characteristics related to age and marital status(p=.001), education(p=.007), a total career length and a career length of comprehensive nursing care service(p<.001), job satisfaction(p=.004) of the nurse biomedical ethics score was high and statistically significant. However, the higher the age of nurse’s aide, the higher the score was statistically significant(p=.007). The perception of biomedical ethics by characteristics related to biomedical ethics was statistically higher among nurses saying that the values of biomedical ethics were very firm (p =.002), those who have experience of having issues biomedical ethics (p =.001), those who believed that rules and procedures for biomedical ethics in a hospital were well organized (p =.003), those who believed that biomedical ethics problems would become more complex and increase in the future (p =.017), and those who experienced ethical dilemmas (p =.019). Conclusion: In the future, biomedical ethics education should be provided for nursing service teams.

노인인구 증가 및 만성퇴행성 질환의 증가 추세 속에서 장기요양보호 대상에 대한 간병서비스의 필요성은 증가하는데 반해 가족에 의한 노인간병과 보호의 기능은 점점 어려워지고 있는 현실을 감안할 때, 앞으로 간병인에 대한 사회적 요구는 지속적으로 증가할 것으로 예상된다. 이러한 시점에서 본 연구는 노인전문병원에서 건강상의 문제를 지닌 노인환자를 가족들을 대신해 보수를 받고 돌봐주는 간병인을 대상으로 노인에 대한 태도와 업무만족을 알아보고자 하였다. 따라서 본 연구의 목적은 간병인들의 노인에 대한 태도와 업무만족 수준을 알아보고, 실제로 노인의 태도가 업무만족에 어떠한 영향을 미치고 있는지의 관계규명을 통해 앞으로 노인에 대한 바람직한 이해 및 긍정적인 인식과 태도 전환을 통해 서비스의 질은 물론 업무만족을 높이는 방안을 모색해 보고자 하였다.
