목적: 임상의의 대부분이 폭주부족을 진단하는데 폭주근점(NPC)을 적용하고 있으나 고등학생의 폭주근점이 제시되지 않고 있어 측정값을 알아보고자 한다. 방법: 안질환과 사시, 억제, 조절장애가 없고 원/근 나안 또는 교정시력이 1.0 이상인 고등학생 137명을 원/근 사위도와 3가지 시표를 사용하여 폭주근점을 측정하여 원거리 보다 근거리에서 외사위도가 더 증가하고 3가지 시표 중 2가지 이상에서 폭주근점이 10 cm 이상을 폭주부족군, 10 cm 미만을 정상군으로 나누어 푸쉬업 방법으로 측정하였다. 결과: 정상군에서 파괴값의 평균은 AT 6.06±1.89 cm, PL 5.77±1.93 cm, PLRG 6.68±3.19 cm였으며, 전체 평균은 6.17±2.43 cm였다. 회복값의 평균은 AT 10.40±2.43 cm, PL 10.11±2.55 cm, PLRG 11.37±5.08 cm였으며, 전체 평균은 10.62±3.58 cm였으며, 폭주부족군에서의 파괴값의 평균은 AT 12.26±4.42 cm, PL 13.83±5.70 cm, PLRG 14.35±5.63 cm였으며 전체 평균은 13.40±5.19 cm였다. 회복값의 평균은 AT 16.95±5.00 cm, PL 18.51±6.37 cm, PLRG 20.93±8.66 cm 였으며 전체 평균은 18.65±6.73 cm였다. 결론: 정상군에서 파괴값과 회복값의 전체평균은 6.17±2.43 cm, 10.62±3.58 cm였고 폭주부족군에서의 파괴값과 회복값 전체평균은 13.40±5.19 cm, 18.65±6.73 cm로 측정되었다. 고등학생의 폭주근점의 임상적 진단값으로 파괴값은 8 cm, 회복값은 14 cm를 제안한다.

Purpose: In spite of the fact that near point of convergence(NPC) is an important factor in diagnosing the convergence insufficiency(CI), NPC is measured by various methods without the standardized method. so we measure the NPC break values and the recovery values of high school students. Methods: The subjects were 137 high school students who didn`t have any eye diseases, suppression, strabismus, and accommodative disorder. And their distance and near visual acuity with correct or without correct were above 1.0. We measured NPC by three methods, and the distance and near phoria were measured, and then classified them into the normal subjects and convergence insufficiency(CI) subjects-their NPC were above 10 cm and esophoria is greater at distance than at near and analyzed the results. Results: The NPC break values and the recovery values for the normal subjects were AT 6.06±1.89 cm, PL 5.77±1.93 cm, PLRG 6.68±3.19 cm, total 6.17±2.43 cm and the recovery values were AT 10.40±2.43 cm, PL 10.11±2.55 cm, PLRG 11.37±5.08 cm, total 10.62±3.58 cm respectively, and the NPC break values and the recovery values for the convergence insufficiency(CI) subjects were AT 12.26±4.42 cm, PL 13.83±5.70 cm, PLRG 14.35±5.63 cm, total 13.40±5.19 cm, and the recovery values were AT 16.95±5.00 cm, PL 18.51±6.37 cm, PLRG 20.93±8.66 cm, total 18.65±6.73 cm respectively. Conclusion: The NPC break and recovery values for the normal subjects were 6.17±2.43 cm, 10.62±3.58 cm, insufficiency(CI) subjects were 13.40±5.19 cm, 18.65±6.73 cm respectively. I suggest a clinical cutoff value of 8 cm for NPC break and 14 cm for the MPC recovery.
