목적 : 젊은 성인의 안구건조 선별검사를 위해 OSDI 설문지를 이용하여 SPEED 설문지의 적합성을 알아보고자 하였다.
방법 : 본 연구는 19세에서 31세 사이의 대학생 134명을 대상으로 안구건조 평가와 관련하여 OSDI와 SPEED 설문지를 비교하였다. 또한, 눈물막파괴시간(TBUT)을 측정하였고, Keratograph 5M을 이용하여 비침입적 눈물 막파괴시간(NITBUT)과 눈물띠 높이(TMH)를 측정하였다.
결과 : SPEED와 OSDI 설문지의 Cronbach's α는 각각 0.861과 0.864였고, SPEED의 민감도와 특이도는 78.0%와 95.2%, OSDI의 민감도와 특이도는 86.1%와 100.0%로 나타났다. OSDI와 SPEED 사이에는 강한 상관 관계가 나타났다(r=0.819, p<0.001). OSDI와 SPEED를 통한 안구건조 진단은 각각 59.7%(n=80)와 64.2%(n=86) 였다. OSDI 점수에 영향을 미치는 요인은 콘택트렌즈 착용 (p=0.001), 성별(p=0.017), 스마트폰 사용 시간(p=0.031), 수면 시간(p=0.001)이었다. 또한, SPEED 점수에 영향을 미치는 요인은 성별(p<0.001), 스마트폰 사용 시간(p=0.001), 콘택트렌즈 착용(p=0.016)으로 나타났다.
결론 : 본 연구에서는 OSDI를 이용하여 안구건조 평가를 위한 SPEED 설문지의 적합성을 조사하였다. 연구결과는 SPEED 설문지가 젊은 성인의 안구건조증 평가를 위한 선별검사로 유용하게 사용될 수 있음을 시사한다.

Purpose : To investigate the suitability of the standard patient evaluation of eye dryness (SPEED) questionnaire using the ocular surface disease index (OSDI) to evaluate for the screening test of dry eye in young adults.
Methods : This study was performed a comparison of the SPEED with the established OSDI questionnaire regarding the evaluation of dry eye from 134 undergraduate students aged 19 to 31 years. In addition, tear film breakup time (TBUT) was measured, and non-invasive tear break-up time (NITBUT) and tear meniscus height (TMH) were also measured using Keratograph 5M. Results : The Cronbach’s α of the SPEED and OSDI questionnaires was 0.861 and 0.864, respectively. The sensitivity and specificity of SPEED were 78.0% and 95.2%, and those of OSDI were 86.1% and 100.0% respectively. There was a highly significant correlation between OSDI and SPEED values with a correlation coefficient of 0.819 (p<0.001). Diagnosis of dry eye using OSDI and SPEED was 59.7% (n=80) and 64.2% (n=86), respectively. The factors influencing the OSDI score were contact lens wear (p=0.001), sex (p=0.017), hours of using smartphone (p=0.031), and sleeping hours (p=0.001). Also, the factors influencing the SPEED score were sex (p<0.001), hours of using smartphone (p=0.001), and contact lens wear (p=0.016).
Conclusion : This study investigated the suitability of the SPEED questionnaire using the OSDI for the evaluation of dry eye. The results indicate that the SPEED questionnaire would be usefully used as a screening test for the evaluation of dry eye in young adults.
