본 연구는 기초연금 수급 취약노인을 대상으로 가족갈등이 우울에 미치는 영향과 공적 돌봄서비스 이용의 매개효과를 분석하는 데 목적이 있다. 이를 위해 한국복지패널(KOWEPS) 18차년도(2023년) 자료를 활용하 여 결측치가 없는 기초연금 수급자 955명을 대상으로 분석하였다. 가족갈등을 독립변수, 돌봄서비스 이용 을 매개변수, 우울을 종속변수로 설정하였으며, 헤이즈(Hayes)의 PROCESS Macro Model 4를 활용한 매개분석을 실시하였다. 분석 결과, 가족갈등은 우울에 유의한 정(+)의 영향을 미치는 것으로 나타나 노년 기 정신건강에 있어 관계적 요인의 중요성이 확인되었다. 또한 가족갈등 수준이 높을수록 돌봄서비스 이용이 증가하는 것으로 나타났으며, 돌봄서비스 이용은 우울과 정(+)의 관계를 보였다. 부트스트래핑 분석 결과, 돌봄서비스 이용은 가족갈등과 우울 간 관계를 부분 매개하는 것으로 확인되었다. 이는 돌봄서 비스 이용이 건강 악화, 기능 저하, 사회적 고립 등 누적된 취약성을 반영하는 지표로 작용할 가능성을 보여주며, 이른바 ‘돌봄의 역설(paradox of care)’을 지지하는 결과로 해석되었다. 이 연구결과를 바탕으로 본 논문은 취약노인의 정신건강 증진을 위해 기능 중심 돌봄을 넘어 심리·정서적 지원과 사회적 관계 강화를 포함한 통합적·관계 중심 돌봄체계 구축이 필요함을 주장한다.
This study aims to analyze the impact of family conflict on depression among vulnerable elderly recipients of basic pension and the mediating effect of using public care services. To this end, the study chose the 18th wave (2023) of the Korea Welfare Panel Survey (KOWEPS) and analyzed the data of 955 basic pension recipients whose data had no missing values. The study set family conflict as the independent variable, use of care services as the mediating variable, and depression as the dependent variable, and conducted a mediation analysis using Hayes’ PROCESS Macro Model 4. The analysis showed that family conflict had a significant positive effect on depression, confirming the importance of relational factors in mental health during old age. Furthermore, the study found that higher levels of family conflict were associated with increased use of care services, and care service use showed a positive relationship with depression. Bootstrapping analysis confirmed that care service use partially mediates the relationship between family conflict and depression. This suggests that the use of care services may serve as an indicator of accumulated vulnerabilities such as deteriorating health, functional decline, and social isolation. It can be also interpreted as a result that supports what is called ‘paradox of care.’ Therefore, this suggests that to promote the mental health of vulnerable elderly people, it is necessary to establish an integrated and relationship-centered care system that goes beyond function-centered care, leading to one that includes psychological and emotional support and strengthens social relationships.
