A relational psychoanalytic reinterpretation of the negative therapeutic reactions of narcissistic clients
본 연구는 두 편의 질적 사례를 재분석하여, 자기애적 내담자와 치료 자 사이에서 나타나는 부정적 치료반응이 내담자의 자기애적 욕구와 방 어, 그리고 치료자의 역전이가 상호작용하며 관계적으로 공동 구성되는 과정을 탐색하였다. 분석 결과, 자기애적 욕구가 충분히 수용되지 못하고 좌절될 때 내담자의 부정적 치료반응이 두드러졌으며, 이때 치료자는 발 달적 자기대상으로서 공감적으로 반응해야 한다는 요구와 병리적 자기애 를 유지하는 방어를 해석적으로 다루어야 한다는 요구 사이에서 긴장과 이중성을 경험하였다. 이러한 과정에서 나타나는 역전이는 치료자의 주 관성과 자기애적 취약성을 드러내는 동시에, 관계 장면이 어떻게 구성되 고 있는지를 보여주는 중요한 자료로 기능하였다. 부정적 치료반응은 공 감, 현실 직면, 한계 설정 사이에서 두 사람의 갈등이 높아질 때 나타났 으나, 내담자가 유지했던 기존의 관계 패턴이 드러나면서 새로운 치료 국면으로 이행할 수 있는 계기가 되기도 했다. 이에 본 연구는 자기애적 내담자의 부정적 치료반응이 내담자의 자기애적 욕구와 방어, 치료자의 주관성과 역전이가 상호작용하며 공동으로 구성된 현상임을 확인하였다. 본 연구의 재해석 과정을 통해, 발달적 자기대상 역할과 해석적 개입 사 이의 균형을 조절하고 치료자의 역전이를 의미화하는 작업이 자기애적 내담자와의 임상에서 핵심적인 과제가 될 수 있음을 보여주었다.
This study reanalyzed two qualitative cases to explore how negative therapeutic reactions in narcissistic patients are relationally co-constructed through the interaction of patients’ narcissistic needs and defenses and therapists’ countertransference. Findings show that such reactions intensify when narcissistic needs are insufficiently met, while therapists experience tension between empathic selfobject responsiveness and interpretive interventions addressing defensive processes. Countertransference both reflects the therapist’s subjectivity and provides insight into the evolving relational field. Although negative reactions emerge amid conflicts around empathy, confrontation, and limit-setting, they also create opportunities for therapeutic progression by revealing entrenched relational patterns. The study underscores the importance of balancing empathic attunement and interpretation, and of meaning-making of countertransference, in working with narcissistic patients.
