Effects of Driver Rehabilitation on Safe Driving in Older Drivers with Cognitive Impairment: A Systematic Review
목적 : 본 연구는 인지 저하 노인 운전자를 대상으로 운전 재활 중재가 운전 수행 및 안전성 지표에 미치는 효과를 체계적 으로 고찰하고자 하였다. 연구방법 : PubMed, Embase, Scopus, Web of Science를 활용하여 2016년 1월부터 2026년 5월까지 출판된 논문을 검색하였다. PRISMA 2020 흐름도에 따라 논문을 선정하였으며, 최종 5편의 논문이 포함되었다. 결과 : 포함 연구의 중재는 시뮬레이터 기반 훈련, 비디오 피드백 중재, 다요소 운전 기술 훈련으로 분류되었다. 중재 결과, 다양한 운전 수행 및 안전성 지표의 향상에 긍정적인 효과가 있음을 확인할 수 있었으며, 특히 실제 도로훈련이 포함된 다요소 운전 기술 훈련은 안전 운전 수행 능력의 향상과 더불어 장기 추적 시점에서의 효과 유지 가능성을 보여주었다. 결론 : 운전 재활 중재는 인지 저하 노인 운전자의 안전 운전 수행 능력을 향상시킬 가능성을 보여준다. 그러나 포함 연구 수가 적고 연구 설계의 제한이 있어 결과 해석에는 신중함이 요구된다. 따라서 향후 충분한 표본 수와 장기 추적 설계를 갖춘 연구를 통해 실제 교통사고 예방 효과를 검증할 필요가 있다.
Objective : This study aimed to systematically review the effects of driving rehabilitation interventions on driving performance and safety outcomes in older drivers with cognitive impairment. Methods : A literature search was conducted using PubMed, Embase, Scopus, and Web of Science for articles published from January 2016 to May 2026. Articles were selected according to the PRISMA 2020 flow diagram, and five articles were finally included in the analysis. Results : The interventions were classified into simulator based training, video feedback intervention, and multicomponent driving skills training. Overall, the findings suggested potential improvements in driving performance and safety outcomes. In particular, multicomponent training with on-road practice improved safe driving performance and suggested possible long term maintenance of effects. Conclusions : Driver rehabilitation interventions may improve safe driving performance in older drivers with cognitive impairment. However, caution is needed due to the limited number of studies and methodological limitations. Therefore, larger long term follow up studies are needed to verify effects on actual crash prevention.
