Verifying the Effectiveness of an Autonomic Nervous System Regulation-Based Bottom-Up Drama Therapy Program for Drug Offenders in Correctional Facilities: Focusing on the Etude Wheel Model
This study verified the clinical effectiveness of a bottom-up embodied intervention program to aid the recovery of drug offenders in correctional facilities and prevent recidivism, addressing the limitations of existing top-down, insight-oriented approaches (e.g., CBT). By integrating the NIDA PhAB model and Polyvagal Theory, a 21-session drama therapy program based on the ‘Etude Wheel Model’ was designed, focusing on stabilizing the autonomic nervous system. The study involved 24 drug offenders (12 in the experimental group, 12 in the waitlist-control group) at a designated drug offender rehabilitation correctional facility. Analysis of six objectively validated scales revealed that the experimental group, compared to the control group, showed statistically significant improvements in practical behavioral indicators, including drug abstinence self-efficacy and the stage of change for recovery. Furthermore, physiological hyperarousal was effectively calmed, evidenced by adaptively improved interoceptive sensitivity and dramatically decreased emotion dysregulation. While executive functioning deficits in the frontal lobe were immediately alleviated, metacognition—a higher-order cognitive system— did not reach significant changes through short-term intervention alone. In conclusion, this study demonstrates the clinical validity of bottom-up embodied interventions that directly restore the impaired parasympathetic nervous system, proving that a drama therapy-based micro-society can foster successful resocialization for drug offenders.
