폭주부족을 진단하는데 중요한 요소인 폭주끈점은 표준화된 측정방볍이 제시되지 않고 여러 방법이 사용되고 있는 실정이므로, 시표의 접근속도를 1cm 단계로 조정 할 수 있는 전동식 폭주근접 측정장치플 개발하여 각 측정방법에 따른 폭주근점을 비교 분석하였다. 안질환파 사시, 억제, 조절장애가 없고 원/근 나안 또는 교정시력이 l.0 이상인 산과 창원소재 고등학생과 대학생 135 명을 대상으로 원/근 사위도 벚 폭주끈점을 정하여 정상군과 폭주부족 예상군으로 나누었다. 정상군에서 폭주근점 파괴값은 6.8 :t 2.7 cm, 회복값은 10.9 :t 3.6 cm, 폭주부족 예상 군에서 파괴값은 13.4 :t 4.8 cm, 회복값은 18.8 :t 5.9 cm 로, 폭주부족 예상군이 정상군 보다 파괴값은 6.6cm, 회복값은 7.9cm 더 별었다. 정상군의 폭주큰점은 조절타게트 5.2 :t 2.1 cm 펜라이뜨 5.7 土2.8cm 펜라이트 + 적녹안경 6 .4 :t 3.0 cm 순으로 크게 측정 되었다. 또한 시표의 이동속도가 빨라지면 회복값이 증가하는 경향을 보였으며, AC/A 비가 낮을수록 폭주근점이 별어지는 경향을 보였다. 동일한 대상에서 측정방볍별 10 회 반복측정결과 정상꾼과 폭주부족 예상군 모두 측정횟수가 증가할수록 폭주근점이 멸어지는 경향을 보였으며, 정상군에서 1-5회와 6-10 회 측정값의 평균 비교, 1-3회와 8-10 회 측정값의 평균 비교에서 모든 측정방 법의 파괴값만 유의한 차이를 보였고 (p 二0.000) , 폭주부족 예상군은 펜라이트+ 적녹 안경 의 파괴 값만 유의 하였다 (p = 0.(01). 이상의 결과로부터 본 연구에서 개받한 전동식 폭주근점 측정장치를 이용하여 타 게트의 종류 및 이닫속도가 다른 조건에서 폭주근점을 측정하였을 때 비슷한 결과플 얻었으며, 이 장치는 임상에서 유용하게 활용될 수 있을 것으로 생각된다
In spite of the fact that near point of convergence(NPC) is an important factor in diagnosing the convergence insufficiency(CI), NPC is measured by various methods without the standardized method. So, we developed the electric measuring device which can be controled the approaching speed of target in unit of 1 cm, compared our results with various methods. The subjects were 135 high school and college students in Changwon and Masan city who didn't have any eye diseases, suppression, strabismus, and accommodative disorder. And their distance and near visual acuity with correct or without correct were above l.0. We measured NPC by different methods, and the distance and near phoria were measured by Howell Card, and then classified them into the norrnal subjects and the expected CI(convergence insufficiency) subjects - their NPC were above 10 cm and their Calculated AC/ A ratio were below 3 : 1, and analyzed the results. The NPC break values and the recovery values for the norrnal subjects were 6.8 :t 2.7 cm and 1O.9 :t 3.6 cm respectively, and the NPC break values and the recovery values for the expected convergence insufficiency(CI) subjects were 13.4 :t 4.8 cm and 18.8 :t 5.9 cm respectively. The results showed that the NPC break values and the recovery values for the expected CI(convergence insufficiency) subjects had increased greatly(6.6 cm & 7.9 cm) compared with the norrnal subjects'. The NPC of norrnal subjects were 5.2 :t 2.1 cm with Accommodative target(AT), 5.7 :t 2.8 cm with Penlight target, and 6.4:t 3.0 cm with Penlight target with red!green glass. And recovery values(of NPC) tended to increase according as the target moved quickly, and the NPC tended to recede in the distance according as the AC/ A ratio was low. As a result of repeating NPC test 10 times to the same person with various methods, the NPC tended to recede in the distance according to repeating the test. And Only the NPC break values of the various methods for the normal subjects had meaningful difference(P" 0.000) and only the NPC break values of the Penlight target with recl/green glass for the expected CI subjects had meaningful difference(P = 0.001) when we compared 1 ~ 5 times with 6 ~ 10 times, and 1 ~3 times with 8~10 times of the test respectively. As mentioned above, we attained the similar results when NPC was measured in different conditions - speed and targets of various kinds with our developed electric measuring device. So, this device will be very useful in measuring NPC.
