본 연구의 목적은 만성 뇌졸중 환자의 상지운동 기능회복을 위해 신경근 전기자극기법과 양수협응운동기법을 병행한 양측성 복합훈련 프로토콜의 효과를 검증하고 또한 원위운동기능 훈련이 근위운동기능향상으로 전이될수 있는 가를 검증하고자 하였다. 만성 뇌졸중 환자집단을 양측성훈련집단(11명)과 단측성훈련집단(10명), 그리고 통제집단(5명)으로 분류하고, 조준운동과제를 이용 사전검사실시한 후 양측성훈련 프로토콜과 단측성훈련 프로토콜, 그리고 무훈련프로토콜을 각 각의 집단에 적용한 후 다시 사후검사를 실시하였다. 양측성복합훈련집단은 운동시간이 감소하였고, 최대속도가 증가하였으며, 최대속도의 가변오차가 감소하였으며, 감속구간경과시간이 감소하여, 손상측 상지의 운동기능이 크게 향상된 것으로 나타났다. 또한 양측성복합훈련 프로토콜을 적용을 통한 상지 운동기능향상의 전이효과를 확인하고 그 효용성을 뒷받침하고 확장하는 결과를 얻었다.

This study investigated the effectiveness of coupled bilateral training protocol and the possibility of within upper limb transfer effect from distal joint movement to proximal joint movement for motor functional recovery of chronic stroke patients. Twenty-one stroke patients were randomly assigned into coupled bilateral training group(11-individuals) and unilateral training group(10-individuals). Age-matched five normal individuals were assinged into control group(no training group). During the pretest and posttest, each subject performed transverse plane target aiming movements from damaged left or right hand side alone and with normal side simultaneously. The results showed that the coupled bilateral group resulted positive intralimb transfer effect at posttest when both arms used simultaneously in aiming task. The coupled bilateral group resulted faster movement time, higher peak velocity, smaller variable error of peak velocity, and faster duration time in deceleration phase than pretest. Further, these results confirmed that intralimb transfer effect from distal joint training to proximal joint motor capacity improvement is possible benefit effect from coupled bilateral training protocol for functional motor recovery of chronic stroke patients.
