A 63-year-old woman with a history of nephrolithiasis and long-term hemodialysis (HD) presented with abdominal pain. Abdominal computed tomography (CT) showed urinoma induced by renal forniceal rupture, and three causative calculi. No active stone growth or new stones were observed when compared with previous CT, however, fragmentation and migration of an existing stone was observed. After ureteroscopic lithotripsy and stenting with a double-J catheter, the patient made a full recovery. To the best of our knowledge, this is the first case of spontaneous renal forniceal rupture in an anuric HD patient caused by calculi. Renal forniceal rupture can occur, even in anuric HD patients.
Treatment of spontaneous retroperitoneal hemorrhage (SRH) is mainly conservative. However, if intra-abdominal hypertension (IAH) or abdominal compartment syndrome (ACS) develops, the treatment strategy should be more aggressive. Surgical decompression has been the gold standard; however, it is very invasive and highly morbid. Thus, percutaneous catheter
drainage (PCD) has been introduced as an alternative therapeutic option. We report on a case of successful PCD for prevention of progression of IAH to ACS in a patient with SRH after coronary stent implantation. This case showed that PCD can be an efficacious and safe method for treatment of IAH with impending ACS.
Hemarthrosis is known to occur with various etiologic factors, including trauma, proliferative synovial tissue entrapment, hypervascularization of joint components, and coagulation disorders such as hemophilias. Hemarthrosis is reported in up to 1.6% of patients who have undergone total knee replacement. Recurrent hemarthrosis is primarily associated with coagulation defects in children; however, in adults it is most frequently associated with ligament rupture, if not related to any trauma. Spontaneous hemarthrosis has been rarely reported with findings of lateral meniscal degeneration and central bone marrow lesions in osteoarthritis patients. Idiopathic spontaneous intraperitoneal hemorrhage is a rare, often fatal condition; however, the patients present with nonspecific symptoms. Even after a comprehensive radiological examination and surgical exploration, the origin of hemorrhage often remains unclear. A 74-year-old woman with sudden abdominal pain, nausea, and vomiting was admitted to our hospital. She had no history of trauma. Computed tomography showed a huge hematoma along the small intestinal mesentery without evidence of visceral organ abnormality causing bleeding. The hematoma was completely removed by surgery and no origin of hemorrhage was detected. Herein, we report on a case of idiopathic spontaneous intraperitoneal hemorrhage.
Spontaneous intracranial hypotension (SIH) causes headache in the absence of tissue injury such as trauma, spinal cord injury, surgery, or epidural anesthesia. Epidural blood patch in the epidural space where CSF leakage occurs is effective for treatment of SIH. However, when the leakage site is unknown, administration of autologous blood into the lumbar epidural space could be effective. Here we report on patients who suffered from headache by SIH and could not confirm the leakage site, however, treatment by lumbar epidural blood patch was administered successfully.
Spontaneous resolution of aortic dissection is extremely rare and only a few cases have reported. We report on the case of an elderly man who presented with DeBakey type III aortic dissection, which was resolved spontaneously over two years after antihypertensive treatment. We also discuss the possible mechanism and the role of conservative management.
Spontaneous perirenal hematoma is an unusual but serious complication of tumors, vascular disease, ruptured cyst, or infection. The authors report the case of a diabetic patient who presented with spontaneous perirenal hematoma and who was subsequently diagnosed to have emphysematous pyelonephritis.
