하루용 소프트 콘택트렌즈로 개발된 Focus Dailies@ AquaRelease™(FDAR) 와 Focus Dailies@(FD) (Ciba Vision Co.) 의 피팅 성공률 착용감 및 선호도에 관한 임상성능을 비교 분석하였다. 본 연구의 취지에 동의하며 콘택트렌즈 착용으로 부작용이 없는 18세 이상 성인 50 명 (2 1. 5 :t 2.7 세)(남성 14 명， 여성 36 명)을 대상으로， 한쪽 눈에는 FDAR, 다른쪽 눈에 는 FD를 하루 착용 주기로 5회 착용시키고， 착용 1 일째와 착용 5 일째 전반적인 피팅 상태， 세극등검사， 렌즈 전면의 NIBUT 및 설문조사를 실시하였다. 대상자의 54% 가 디스포저블 소프트렌즈를 착용하고 있었으며， 68%가 1 년 이상 착 용경력을 갖고， 98%가 하루 10시간 이상 착용하고 있었다. 소프트렌즈를 착용하는 동안 대상자의 82% 가 불편을 느끼며 그 이유는 건조감이 44%로 가장 높았다. FDAR과 FD를 착용하기 전과 5회 착용후 세극등으로 검사한 윤부충혈， 구결막충혈， 검결막충혈， 각막상피 스테이닝， 결막 스테이닝 및 침윤은 모두 CCLRU 기준을 적용했 을 때 2등급 이하로 별도의 조치가 필요한 사례는 나타나지 않았다. 처음 피팅한 렌즈 의 전반적인 피팅상태와 중심안정상태 역시 허용될 수 없는 경우는 발견되지 않았다. 렌즈를 착용한 상태에서 Tearscope로 측정한 렌즈 전면 비침습성 눈물막파괴시간 은 착용 1 일 및 5 일째 FDAR은 5.2초와 5 .1초， FD는 5.2초와 4.9초였으며， FDAR과 FD 사이에서 1 일째에는 유의한 차이가 없었으나， 5 일째에는 FDAR 이 0.2초 정도 더 긴 것 으로 나타났다 (p < 0.05). 착용 5 일 째 자각적 증상에 대 한 설문조사 (10등급)에 서 FDAR 이 FD보다 초기 착용 감 (p = 0.035), 렌즈를 제거하기 전 충혈 (p = 0.073), 저녁무렵 건조감 (p 二0.094) 및 전 반적인 착용감 (p = 0.096) 에서 더 양호하였다 (p < 0.10). 또한 최종적인 선호도에서 FDAR 이 FD보다 두 배 이 상 높은 것으로 나타났다. 이상으로부터 렌즈를 착용하는 동안 습윤성분(폴리비닐알콜)이 렌즈표면에 퍼지도 록 개발된 Focus Dailies@ AquaRelease™는 처음 피팅렌즈의 성공률이 100% 였으며， 기존의 Focus Dailies@보다 NIBUT, 자각적인 착용감 및 선호도가 더 높은 것으로 나타났다.
The purpose of this study is to evaluate the cl!nical performapçe for lens fit, comfort and subjective preference for Focus Dailies"" AquaReleaseun versus Focus DailieslKJ(Ciba Vision Co.) one• day disposable contact lenses. Informed consent to take part in the study was received from a11 subjects. 50 subjects(21.5::!:2.7 years)(14 males and 36 females) enrolled in the study did not have any complications related to contact lens wear. One eye randomly selected from a11 subjects was fitted with FDAR contact lenses and the other eye was fitted with FD contact lenses for a total of 5 days daily waer. The principal measures were non-invasive break-up time for anterior surface of contact lens on eye, overall lens fit, slit lamp examination and subjective comfort which were assessed at the day 1 and day 5 of daily wear. The 54% of subjects used disposable contact lenses, 68% of subjects did wear contact lenses during 1 year or longer and 98% of subjects were 10 hours or above of daily wear. The 82% of subjects had discomfort during contact lenses wear, and the highest reason of discomfort was dryness(44%). The CCLRU scale grades for limbal hyperamia, bulbar conjunctival hyperamia, palpebral conjunctival hyperamia, epithelial staining and conjunctival staining examined by slit lamp were grade 2 or below at day 1 and day 5 for both FDAR and FD lenses. There was not required to any treatment for a11 subjects during period of study. Also, there was not found in the case of non-acceptable condition for centration and overall lens fit at initial fitting of the prescribed contact lens The NIBUT of anterior surface of contact lens on the eye measured by Tearscope were 5.2 s of day 1, 5.ls of day 5 in FDAR lens and 5.2 s of day 1, 4.9 s of day 5 in FD lens. There was no significant difference at day 1 between FDAR and FD lenses, but FDAR lens was longer than FD lens at day 5(p < 0.05). On 5 days of daily wear schedule, the initial comfort was better for FDAR compared to FD lenses(p 二0.035), and end of day redness(p 二0.073), end of day dryness(p = 0.094) and overall comfort(p 二0.096) were better for FDAR compared to FD lenses at 10% of significant level. The subjective preference was two times or more higher for FDAR compared to FD lenses In conclusion, _Release of additional non-functionalised PV A from the nelfilcon A(Focu_s Dailies(ß) AquaReleaseT1\1) lenses showed better than nelfilcon A(Focus Dailies,RJ) lenses for NIBUT, subjective comfort and subjective preference
