목적: 강원도 지역의 원시안 초등학생들을 대상으로 현성굴절검사와 조절마비굴절검사 결과를 비교, 분석하여 원시안의 굴절이상 정도의 기초자료로 활용하고자 한다.
방법: 원시안인 저학년(1.2.3학년) 30안, 고학년(4.5.6학년) 20안, 총 50안을 대상으로 하였으며, 원시정 도에 따른 구면굴절력의 분류는 경도원시 +0.25D ~ +2.00D, 중등도원시 +2.25D ~ +5.00D 으로 분류하였다. 검사는 조절마비제 점안 전과 후에 자동굴절검사기와 검영기로 굴절검사를 시행하였다.
결과: 현성굴절검사(MR)와 조절마비굴절검사(CR)의 구면 굴절력 평균값은 +1.00±0.52D, +1.59±1.23D 로 조절마비굴절검사의 원시도가 0.59±0.59D 높게 측정되었다(p<0.05). 학년에 따른 MR과 CR의 구면 굴절력 평균값은 저학년에서는 +0.91±0.52D, +1.44±1.25D로 조절마비굴절검사의 원시도가 0.53±0.59D 높 게 측정되었으며, 고학년에서는 +1.14±0.51D, +1.80±1.18D로 조절마비굴절검사의 원시도가 0.66±0.58D 높게 측정되었다 (p<0.05). 원시정도에 따른 MR과 CR의 차이비교에서는 경도원시, 중등도원시의 구면 굴절력이 각각 0.48D, 1.75D 차이가 나타났다.
결론: 초등학생 원시안을 대상으로 현성굴절검사와 조절마비굴절검사 굴절력을 비교해본 결과 전체학생 및 저학년과 고학년의 구면 굴절력 비교에서 유의한 차이가 있었다. 그리고 원시정도에 따른 비교에서는 경도원시, 중등도원시의 구면굴절력이 증가할수록 차이가 증가하는 것을 확인할 수 있었다.

Purpose : The purpose of this study was to apply as the basic data on hyperopic refractive error through comparison and analysis of manifest refreaction (MR) and cycloplegic refraction (CR) in elementary school students with hyperopia in gangwon-do region.
Methods : Total of 50 eyes; 30 eyes of lower grades (grade 1.2.3) 20 eyes of the upper grades (Grades 4.5.6) participated , and were divided into 2 groups such as the mild hyperopia (+0.25D ~ +2.00D), moderate hyperopia (+2.25D ~+5.00D). MR and CR were performed with auto-refractometer and retinoscope.
Results : The mean values of spherical power of MR and CR were +1.00±0.52D and +1.59±1.23D respectively, and that of CR power was 0.59±0.59D (p<0.05). In the lower grades, there were +0.91±0.52D and +1.44±1.25D, respectively, showing that CR was also higher than manifest refraction by 0.53±0.59D. In the upper grades, as the lower grade groupthere were +1.14±0.51D and +1.80±1.18D, respectively, showing that CR was also higher than MR by 0.66±0.58D (p<0.05). The differences between MR and CR according to the degree of hyperopia were 0.48D and 1.75D, respectively in mild and moderate hyperopic groups.
conclusions : It was found a significant difference in the spherical power between the whole students and the lower and upper grades in the hyperopic elementary school students. It was verified that the difference of spherical power between mild and moderate hyperopia were also increased as the refractive powers increased in comparison of the degree of hyperopia.
