목적 : 본 연구는 가정환경기반의 작업치료의 중재방법과 내용을 분석하여 임상적용을 위한 근거를 제시하고자 한다.
연구 방법 : 2009년부터 2018년까지 전자데이터 베이스인 ProQuest, ScienceDirect, Goole scholar를 통하여 논문을 검색하였다. 검색용어는 “Home care program” OR “Home health service” OR “Home based rehabilitation” OR “Visiting home” AND “Occupational therapy”를 사용하였다. 배제기준을 통 하여 최종적으로 19개의 연구를 분석대상으로 선정하였다.
결과 : 총 19편의 연구는 근거수준이 Level Ⅰ로 전체비율을 차지하였다. 포함된 연구에 대한 방법론적 질적수준(PEDro 점수)는 평균 8.0점으로 6~8점인 ‘좋음’ 수준에 속하였다. 가정방문 작업치료 중재는 낙상위험요인이 있거나 낙상경험이 있는 노인을 대상으로 한 가정환경수정 및 일상생활수행훈련이 많았다. 측정한 종속변인은 일상생활수행능력이 30.00%, 상지기능 24.80%를 차지하였다.
결론 : 본 연구를 통해 작업치료사들에게 가정방문 작업치료중재에 대한 임상적 근거를 제공하고자 하였다. 추후에는 노인성 질환 및 정신건강장애를 갖는 환자들을 대상으로 다양한 중재적용이 이루어져야 할 것이며 다각도의 측면에서 중재결과를 도출해야 할 것이다.

Objective: The aim of this study is to analyze the home-based occupational therapy interventions and to provide evidence based practice regarding home based occupational therapy.
Methods: For the main keywords of a database search, “Home care program” OR “Home health service” OR “Home based rehabilitation” OR “Visiting home” AND “Occupational therapy” were used. Using Science Direct, ProQuest and Google Scholar, we systematically examined papers published in journals from 2008 to 2018. Finally, 19 studies met the inclusion criteria.
Results: Evidence level of total 19 studies were level Ⅰ. PEDro score in all the articles reviewed was from 6 to 8 points in the ‘Good’ range. The Most frequency of visiting home occupational therapy intervention were home modification or activities of daily living training applied to fall risk older adults or experienced fall. Dependent variables included 30.00% Activities of daily living training, 24.80% upper extremity function.
Conclusion: Through this systematic review, we were able to provide clinical evidence on visiting home occupational therapy to the occupational therapist. In the future, based on this study, research on intervention multiintervention applied to older adults with geriatric illness or mental health.
