목적 : 뇌졸중 환자들의 재활의료 서비스 이용현황을 분석하여 미국과 일본의 재활의료 서비스와의 비교를 통해 우리나라 재활서비스 수가체계의 개정 필요성과 정책 개선 방안을 제안 하고자 한다.
연구방법 : 재활의료 이용현황 분석을 위해 심평원 DB를 활용하였다. 2016년 1월 1일부터 12월 31일까 지 주 상병코드가 I60, I61, I62, I63, I64인 입원 환자 중 18세 이상 100세 이하인 환자 78,719명을 대상으로 전문재활 청구율, 항목별 전문재활 청구율, 입원기간 중 의료기관 이동 횟수, 이동한 의료기관에서의 전문재활 청구율을 분석하였다. 또한 회복기 의료기관의 지역사회 복귀에 대한 효과를 알아보기 위해 2018년 1월부터 12월 까지 재입원율을 분석하였다. 우리나라 재활의료 이용현황을 통해 문제점을 미국과 일본의 재활의료 체계와 비교하여 재활의료 서비스 개선방안을 제시 하였다.
결과 : 급성기 치료가 주로 이루어져야하는 상급종합병원에서부터 만성기 환자 기능 유지를 담당해야하는 요양병원에 이르기까지 종별 구분 없이 모든 재활치료가 이루어지고 있었다. 또한, 회복기 의료기관의 평균 재입원율이 61.9%로 초기 집중치료를 위한 비용대비 지역사회 복귀율은 낮은 것으로 나타났다. 결론 : 선진국의 재활의료 체계를 조사, 비교하여 우리나라 재활서비스 전달체계 확립, 재활의료 서비스 제공 범위확대와 수가 비용 현실화, 퇴원 후 지역사회 연계수가 확립 등을 제시하였다.
The purposes of this study are developing a stress index for each individual major trail in the national parks and contribute to establishing a park management policy based on scientific analyses using the index. We collected data from 144 sections of trails in national parks, conducted the Delphi survey and factor analysis, and generated the stress index by standardizing scores of factors contributing to the stress for each trail. As aresult, total eight factors contributing to the stress were considered and four of them, i.e. the amount of visits, the effect of visits, the degree of trail destruction, and species diversity, were selected to generate the stress index. How to weigh factors was determined by their importance obtained in the second Delphi survey and the average from a 5 point Likert scale. Then, the cumulative probabilities from the standardized scores were graded into five groups. In conclusion, there were twenty trails from the six national parks including Jirisan, Bukhansan, Deogyusan, Naejangsan, Seoraksan in the first grade which gained over 80 as the stress index.
To evaluate the effect of Jingansikpungtang water extract on cultured mouse spinal sensory neuron which was inhibited by glucose oxidase(GO)-induced cytotoxicity, NR assay and TBARS assay for lipid peroxidation were carried out after the cultured mouse spinal sensory neuron were pre-incubated with various concentrations of Jingansikpungtang water extract for 3 hours prior to exposure of GO. GO, a oxygen radical, decreased the survival rate of the cultured mouse spinal sensory neuron cells on NR assay. Jingansikpungtang water extract have efficacy of decreasing lipid peroxidation increasing by GO in cultured mouse spinal sensory neuron. From above the results, it is concluded that Jingansikpungtang has marked efficacy as a treatment for the damages caused in the GO-mediated oxidative process..
This study provides basic data for the cost operating strategy of forest welfare infra facilities. We use the Price-Sensitivity Measurement technique (PSM) for a forest welfare infra facility (healing forest, wood camping, forest leisure sport, etc.). In addition, we conduct a survey in the four target locations (Kyung-gi, Kyung-sang, Chung-cheong, and Jeolla) and receive answer from 390. The results show that higher the price of present infra facilities, higher the price sensibility. The forest welfare infra facilities we include are healing forest 1,894 won, wood camping 5,625 won, mtb park 9,530 won, mountain horseback riding 14,556 won, paragliding 20,252 won, mountain ski 3,075won. However, we investigate zip-lines at lower than current price levels.
This study examined the effectiveness of and satisfaction of visitors with the Environmental Improvement Project on national parks’ trail entry areas. A survey was conducted to improve the proposal framework based on the data. Data for the study were obtained from 6400 respondents who used 28 national park offices at 32 national park trail entry areas. The field research was collected in 3200 surveys, divided between pre- and post-implementation of the Environmental Improvement Project. The study variables consisted of socio-demographics, visitors’ perceptions on the environmental improvement of trail entries, effects of the environmental improvement, and visitor satisfaction. Several important results were found. First, the environment of trail entries has a significant impact on the formation of images and user satisfaction of national parks. Second, the influence of the trail entry improvement showed that 71.8% of visitors rated the trail entry as environmentally friendly after the Environmental Improvement Project, compared to 48.5% of visitors rated the trail entry as environmentally friendly before the Environmental Improvement Project. The findings indicated that a top priority of environmental improvement of the trail entries needs to be the installation of convenience facilities such as rest areas, restrooms, and direction boards (pre: 34.5%, post: 32.6%). Third, functionality and the other nine variables were shown to enhance the effect of the environmental improvement of the trail entries at the 1% statistical significance level. Finally, results indicated that the overall satisfaction of the visitors with the park was significantly higher after the environmental improvement (t-value=25.741, p=.000), showing the satisfaction score was higher after the environmental improvement (pre: 3.51, post: 4.12), with a statistically significant difference at the 1 % level. This study helps to understand trail entries, which have influenced the image of the national park and satisfaction. These findings can be utilized to manage and design a national park as a public space.
