        목적 : 본 연구는 상호작용식 메트로놈(Interactive Metronome; IM)을 이용한 작업치료가 편마비 뇌성마비 아동의 신체 양측 협응과 균형 능력, 상지기능에 미치는 효과를 검증하기 위한 개별 실험 연구이다. 연구방법 : 선천성 뇌성마비로 진단 받은 편마비 아동 1인을 대상으로, 개별실험 연구방법(single-subject experimental research design) 중 ABA 설계를 사용하였다. 총 20회기로 매주 2회기씩 10주간 진행하였다. 기초선 4회기는 중재 없이 양측 협응을 평가하였고 중재기 12회기는 상호작용식 메트로놈을 이용한 중재를 1일 1회 40~45분씩 실시한 후 양측 협응을 평가하였다. 재기초선 4회기는 중재 없이 양측 협응을 평가하였다. 중재전과 후의 균형검사를 실시하였고, 중재 전, 중, 후에는 상지기능 검사를 실시하여 변화를 알아보았다. 결과 : 상호작용식 메트로놈 훈련을 통한 중재 결과, 양측 협응과 균형능력이 유의하게 향상되었고, 양측 협응의 향상에 따른 중재 전, 중, 후의 상지기능은 향상을 보였으나 유의하지 않았다. 결론 : 상호작용식 메트로놈 훈련이 편마비 뇌성마비 아동의 양측 협응과 균형능력 향상에 효과가 있다는 것을 확인하였고 편마비 뇌성마비 아동의 양측 협응과 균형 훈련의 중재방법으로 사용될 임상적 기초 자료로 활용될 수 있을 것이다.

        The purpose of this study was to investigate the effects of different objects and target location of dominant hand on the non-dominant hand movement kinematics in a bimanual reaching task. Fifteen right-handed volunteers were asked to reach from same starting point to the different target point of right and left hand with grasping the objects of different size. Independent variables were 1) three different object types (small mug cup, name pen, and PET bottle), and 2) three different target locations (shorter distance, same distance, and longer distance than the non-dominant hand) of the dominant hand. Dependent variables were movement time (MT), movement distance (MD), movement mean velocity (MVmean), and movement peak velocity (MVmean) of the non-dominant hand. Repeated measures two-way analysis of variance (ANOVA) was used to test for differences in the non-dominant hand movement kinematics during bimanual reaching. The results of this study were as follows: 1) MT of the non-dominant hand was increased significantly when traveling with grasping the mug cup and reaching the far target location, and was decreased significantly when traveling with grasping the PET bottle and reaching the near target location of the dominant hand. 2) MD of the non-dominant hand was significantly increased during reaching the far target location, and significantly decreased during reaching the near target location with dominant hand. 3) MVmean of the non-dominant hand was increased significantly when traveling with grasping the PET bottle, and was decreased significantly when traveling with grasping the mug cup of the dominant hand. Therefore, it can be concluded that the changes of the ipsilateral hand movement have influence on coupling of the contralateral hand movement in bimanual reaching.

        Objective: To explore the effectiveness of sensory integrative procedures in promoting postural control, bilateral coordination and motor planning outcomes in child with developmental delay. Design: A single-subject ABA research design. Setting: Early Intervention Center of University. Participant: A 7-year-old boy with developmental delay. Interventions: Sensory integrative procedure. The subject was tested four times before, four times during, and three times after intervention. Main Outcome Measure: Part of subtests of DeGangi-Berl Test of Sensory Integration, Bruininks-Oseretsky Motor Proficiency and Sensory Integration and Praxis Test. Result: Postural control and motor planning are effected on sensory integrative procedure. Bilateral coordination did not change by this study. The quality of performance was increased by the therapy.

        Purpose: Functional recovery of the paretic upper extremity post stroke continues to be one the greatest challenges faced by rehabilitation professionals. Although many tasks require the coordinated participation of both hands, rehabilitation strategies for the most part have focused on the paretic limb. This study investigated whether coupled bilateral coordinated movements and repetitive high frequency TMS enhanced upper limb corticomotor excitability and motor function poststroke. Methods: For this study, we compared the neurophysiological and behavioral changes associated with two such rehabilitation protocols: coupled bilateral training and coupled unilateral training. Twenty chronic stroke patients were randomly assigned to the two training group over 8-week period. For this experiment, transcranial magnetic stumulation, EMG, Ag/AgCI suface electrode, rehabilitation training equipment, jebsen-talyer hand function kit, box & block, and pinch & power grip test kit were used. Results: First, in the box & block and JTT task, coupled bimanual group showed a reduction in movement time in comparision to receiving coupled unilateral group. Second, in the cortical excitability within affected hemisphere, using TMS, an increased in map volume, and map number following coupled bilateral group. Overall, these finding suggest that a coupled bilateral movement and high frequency repetitive TMS protocol effective in recovering upper limb motor function and cortical excitability in chronic stroke patients.

        Four age group (6.5, 10.4 years, college; 20.9years, elderly; 74.3 years) completed four unimanual aiming and four bimanual aiming movement tasks. Unimanual tasks included movements of left and right hands independently across a short (7.5 cm) and long (15 cm) distance. Bimanual coordination tasks included simultaneous movements of both hands to targets located at equal distance. While child perform the tasks, EMG activity was recorded using Silver-silver Chloride Cup electrodes placed over the bellies of the long head of triceps (antagonist) and the medial head of biceps muscles (agonist) on both left and right arms. Results indicated the coupling of the two hands in bimanual aiming movements appears to be present across the life span. However, bimanual control strategies for groups were different in both symmetrical and asymmetrical movements. Results of neuromuscular aspects of bimanual movement control indicated that (1) modification of the agonist and antagonist onset latency may underlie bilateral motor coordination in symmetrical movements, and (2) modification of the antagonist duration for college students and antagonist duration or time to peak of antagonist muscle contraction for 6 years and elderly may underlie bilateral motor coordination in asymmetrical movements. Results were discussed.
