Background: Despite fall prevention strategies suggested by researchers, falls are still a major health concern in older adults. Understanding factors that differentiate successful versus unsuccessful balance recovery may help improve the prevention strategies. Objects: The purpose of this review was to identify biomechanical factors that differentiate successful versus unsuccessful balance recovery in the event of a fall. Methods: The literature was searched through Google Scholar and PubMed. The following keywords were used: ‘falls,‘ ‘protective response,‘ ‘protective strategy,’ ‘automated postural response,’ ‘slips,’ ‘trips,’ ‘stepping strategy,‘ ‘muscle activity,’ ‘balance recovery,‘ ‘successful balance recovery,‘ and ‘failed balance recovery.’ Results: A total of 64 articles were found and reviewed. Most of studies included in this review suggested that kinematics during a fall was important to recover balance successfully. To be successful, appropriate movements were required, which governed by several things depending on the direction and characteristics of the fall. Studies also suggested that lower limb muscle activity and joint moments were important for successful balance recovery. Other factors associated with successful balance recovery included fall direction, age, appropriate protective strategy, overall health, comorbidity, gait speed, sex and anticipation of the fall. Conclusion: This review discusses biomechanical factors related to successful versus unsuccessful balance recovery to help understand falls. Our review should help guide future research, or improve prevention strategies in the area of fall and injuries in older adults.

Background: Total hip replacement (THR) is performed in patients with femur fractures and osteoarthritis. THR patients have balance problems even after surgery. There is a lack of research on vibration stimulation interventions for balance in THR patients. Objectives: To investigated the effect of vibration stimulation intervention on the balance and gait of THR patients. Design: Randomized controlled trials. Methods: 44 subjects were randomly assigned to a vibration stimulation group (VSG) and a non-vibration stimulation group (N-VSG). Seven study subjects dropped out, and 37 completed the study. Timed up and go (TUG) and Berg balance scale (BBS) were used for balance assessment, and 10-meter walk (10MW) was used for gait assessment. The intervention program was conducted three times a week for 4 weeks. Results: Significant differences within the groups in balance (BBS, TUG) and gait (10MW) between the VSG and the N-VSG. There was no difference between the two groups in any variable. Conclusion: The VSG improved the balance and gait of THR patients without any difference from the N-VSG. However, VSG showed a higher effect size than N-VSG.

Background: The range of motion (ROM) and balance ability of the ankle joint affect the stability of the ankle and prevent injuries or hurts from falling. In the clinical tests conducted recently, the floss band is widely used to enhance the range of joint motion and exercise performance, and there are many studies that have applied it to ankle joint increasing dorsi flexion (DF) angle. Objects: This study compared the effects on the range of ankle motion and static/dynamic balance ability of the ankle through three conditions (before floss band intervention, after floss band intervention, and after active exercise intervention) for adults. Methods: One intervention between floss band and active exercise was applied randomly and another intervention was applied the next day. After each intervention, the ROM of the ankle joints and the static balance was checked by measuring conducting one leg test. And the dynamic balance was checked by conducting a Y-balance test. Results: In the case of DF, the range of joint motion showed a significant increase after floss band intervention compared to before floss band intervention (p < 0.05). Static balance ability showed a significant increase after the intervention of floss band and active exercise compared to before the intervention of floss band (p < 0.05). The dynamic balance ability showed a significant increase after the intervention of the floss band compared to before intervention of the floss band and after active exercise intervention (p < 0.05). Conclusion: Based on these results, it was confirmed that the application of floss band to the ankle joint increases DF and improves the static and dynamic balance ability. Based on this fact, we propose the application of a floss band as an intervention method to improve the ROM of the ankle joint and improve the stability of the ankle in clinical field.

Background: Neuromuscular electrical stimulation (NMES) is used for muscle strengthening. While voluntary muscle contraction follows Henneman et al.’s size principle, the NMES-induced muscle training disrespects the neurophysiology, which may lead to unwanted changes (i.e., declined balance ability). Objects: We examined how the balance was affected by abdominal muscle training with the NMES. Methods: Fifteen young adults (10 males and 5 females) aged between 21 and 30 received abdominal muscle strengthening with NMES for 23 minutes. Before and after the training, participants’ balance was measured through one leg standing on a force plate with eyes open or closed. Outcome variables included mean distance (MDIST), root mean square distance (RDIST), total excursion (TOTEX), mean velocity (MVELO), and 95% confidence circle area (AREA) of center of pressure data. Two-way repeated measures analysis of variance was used to test if these outcome variables were associated with time (pre and post) and vision. Results: All outcome variables were not associated with time (p > 0.05). However, all outcome variables were associated with vision (p = 0.0001), and MVELO and TOTEX were 52.4% (45.5 mm/s versus 95.6 mm/s) and 52.4% (364.1 mm versus 764.5 mm) smaller, respectively, in eyes open than eyes closed (F = 55.8, p = 0.0005; F = 55.8, p = 0.0005). Furthermore, there was no interaction between time and vision (F = 0.024, p = 0.877). Conclusion: Despite the different neurophysiology of muscle contraction, abdominal muscle strengthening with NMES did not affect balance.

Material balance evaluation is an important measure to determine whether or not nuclear material is diverted. A prototype code to evaluate material balance has been developed for uranium fuel fabrication facility. However, it is difficult to analyze the code’s functionality and performance because the utilization of real facility data related to material balance evaluation is very limited. It is also restricted to deliberately implement various abnormal situations based on real facility data, such as nuclear diversion condition. In this study, process flow simulator of uranium fuel fabrication facility has been developed to produce various process data required for material balance evaluation. The process flow simulator was developed on the basis of the Simulink-SimEvents framework of the MathWorks. This framework is suitable for batch-based process modeling like uranium fuel fabrication facility. It dynamically simulates the movement of nuclear material according to the time function and provides process data such as nuclear material amount at inputs, outputs, and inventories required for Material Unaccounted For (MUF) and MUF uncertainty calculation. The process flow simulator code provides these data to the material balance evaluation code. And then the material balance evaluation code calculates MUF and MUF uncertainty to evaluate whether or not nuclear material is diverted. The process flow simulator code can simulate the movement of nuclear material for any abnormal situation which is difficult to implement with real process data. This code is expected to contribute to checking and improving the functionality and performance of the prototype code of material balance evaluation by simulating process data for various operation scenarios.

The Republic of Korea is implementing safeguards for domestic nuclear facilities through cooperation with the IAEA. But it is not to evaluate the material balance for the material unaccounted for, MUF in the bulk handling facility. Although the development of a material balance evaluation program is underway, there are no related regulations. The State Regulatory Authority, SRA is performing material balance evaluation, MBE on the facility based on the design information and material balance results of the facility. However, it is not possible to directly derive measurement uncertainty for the facility’s measurement equipment, which is an important variable of MBE. To solve this problem, it is trying to derive a method suitable for the domestic environment by investigating the some measurement uncertainty estimation methods and analyzing characteristics of them. In this study, the traditional measurement uncertainty estimation method, GUM method and GUM-S1 method were studied and the advantages and disadvantages were analyzed. Due to the problems mentioned above, the uncertainty quantification technique currently being used cannot be applied to the evaluation of the domestic material balance. Therefore, we are tying to apply them to the evaluation the domestic material balance through the above three methods or a combination of them appropriately. Through this continuing study, it is expected that it will be possible to present a plan to derive measurement uncertainty optimized for the domestic MBE environment.

Background: Changes in foot sole shape can result in plantar muscle weakness, lack of proprioception, increased ankle instability, and an inability to balance. Objectives: To investigated whether different floor surfaces influenced the effect that short-foot exercises have on the foot core system of healthy individuals. Design: Pretest-posttest control group experimental design. Methods: This exercise shortens the length of the foot by raising the medial longitudinal arch, strengthening the intrinsic muscle of the sole, and maintaining the shape of the medial longitudinal arch. Results: There was a significant difference in the angle of the medial longitudinal arch in the mat exercise group (MEG) and vibration exercise group (VEG) before and after the intervention (P<.05). As the arch angle decreased, the arch height increased. Post hoc analysis showed that there was no difference between the floor exercise group (FEG) and MEG or between the MEG and VEG (P>.05). In terms of the weight distribution index (WDI, there was a significant difference in the MEG and VEG (P<.05) but not in the FEG before and after the intervention (P>.05). The post hoc analysis results showed that there was no difference between the FEG and MEG (P>.05). Conclusion: Various methods to improve the strength of intrinsic muscles of the sole are more effective than no stimulation, and that short-foot exercise combined with sensory stimulation can be recommended for maintaining and increasing the medial longitudinal arch.

When we think about an idea of United Nations Tolerance Program UNTP, it is important for us to share memory with world citizens that we had 2314 veterans from 11 countries in the Busan U.N. Forces Cemetery since January of 1951. Recently, we had 13 veterans, who got buried coming from 4 Americans, 3 Hollanders, 2 Frenchmen, 2 Germans, 2 Englishmen, 1 Canadian and 1 headband after they died in their countries. (Joongang daily newspaper June 19, 2022). "I believe that creating a fifth office of the UNTP in Republic of Korea (Seoul, Busan, Kyunggi or Jeju Island), as a twin organization of the UNEP in Nairobi, Kenya, would provide a geopolitical balance to the international organization and a useful base for peace island studies in the conflict laden region of East Asia. Establishing this office in S. Korea(Seoul, Busan, Kyunggi or Jeju Island), with its unique geographical location as a hub for peace talks and environmental negotiations between Russia, North Korea, Japan, China, and Taiwan, would contribute to peace and development in the region." Background: Older adults use different ankle muscle activation patterns during difficult static balance conditions. It has been suggested that this is related to a decline in proprioception with age, resulting in reduced postural balance. However, the association between proprioception and ankle muscle activity during quiet standing has not been directly assessed. Objects: This study aimed to investigate the effects of age and sensory condition on ankle muscle activity and the association between ankle proprioception and ankle muscle activity. Methods: We recruited 10 young women and 9 older women. Ankle proprioception was evaluated using joint position sense (JPS) and force sense (FS) divided by dorsiflexion and plantarflexion. The electromyographic activity of the tibialis anterior (TA) and gastrocnemius (GCM) muscles was collected during quiet standing. Results: Older women activated GCM muscle more than young during quiet standing and when performing difficult tasks. Older women had more errors in JPS dorsiflexion and FS plantarflexion than did young. The GCM muscle activity is related to JPS dorsiflexion and FS plantarflexion. Conclusion: Lower proprioception of the GCM with age leads to increased muscle activity, resulting in reduced postural balance. There was no difference in TA proprioception or muscle activity among older women with frequent physical activity.

Background: Neuromuscular electrical stimulation (NMES) is a physical modality used to activate skeletal muscles for strengthening. While voluntary muscle contraction (VMC) follows the progressive recruitment of motor units in order of size from small to large, NMES-induced muscle contraction occurs in a nonselective and synchronous pattern. Therefore, the outcome of muscle strengthening training using NMES-induced versus voluntary contraction might be different, which might affect balance performance. Objects: We examined how the NMES training affected balance and proprioception. Methods: Forty-four young adults were randomly assigned to NMES and VMC group. All participants performed one-leg standing on a force plate and sat on the Biodex (Biodex R Corp.) to measure balance and ankle proprioception, respectively. All measures were conducted before and after a training session. In NMES group, electric pads were placed on the tibialis anterior, gastrocnemius, and soleus muscles for 20 minutes. In VMC group, co-contraction of the three muscles was conducted. Outcome variables included mean distance, root mean square distance, total excursion, mean velocity, 95% confidence circle area acquired from the center of pressure data, and absolute error of dorsi/plantarflexion. Results: None of outcome variables were associated with group (p > 0.35). However, all but plantarflexion error was associated with time (p < 0.02), and the area and mean velocity were 37.0% and 18.6% lower in post than pre in NMES group, respectively, and 48.9% and 16.7% lower in post than pre in VMC group, respectively. Conclusion: Despite different physiology underlying the NMES-induced versus VMC, both training methods improved balance and ankle joint proprioception.

Background: The purpose of this study was to investigate whether the function of stroke patients could be improved by simultaneously performing bridging and masticating exercises. And, this was tested by examining whether the effects of arousal and cognitive improvement due to mastication muscle activity could affect exercise ability. Objectives: To compare the effects, the movement of the center of pressure and the stability limit change were measured using a balance analysis platform (BT4). Design: Randomized controlled trial. Methods: In this study, 36 chronic stroke patients were randomly assigned to either single training group of the bridging exercise or combined training group that performed both the bridging exercise and the mastication exercise. The exercise was performed for thirty minutes a day, three times a week, for eight weeks. Results: In both groups, the static balance ability improved after exercise, but the dual-task training group had a better improving effect on the distance and area of the center of pressure while the eyes was open. The dynamic balance ability also improved after exercise, and the dual-task training was more effective in reducing forward, left, and right movements, but not in backward movement. Conclusion: The importance of mastication was recognized in the rehabilitation of chronic stroke patients, and it can be expected to use mastication to improve balance in stroke patients in the future.

Background: Scoliosis can be caused by postural changes. The effects of Schroth exercises and Kinesio taping for scoliosis have been reported independently, but there are few studies on the effects of combining both methods. Objectives: To investigated the effects of Schroth exercises and kinesio taping on the Cobb angle and dynamic balance ability in patients with scoliosis. Design: Quaxi-experimental study. Methods: Participants were divided into the taping (n=3) and non-taping (n=3) groups. Pre-tests for the Cobb angle and dynamic balance were performed prior to the first intervention, and post-tests were performed after the intervention was completed. The non-taping group performed Schroth exercises for a total of 12 35-minute sessions thrice a week for 6 weeks. Meanwhile, in the taping group, Kinesio tape was attached before performing the same exercises and was maintained for 24 hours after the exercises were finished. Results: In the taping group, there was no significant change in the Cobb angle and dynamic balance ability pre- and post-intervention (P>.05). Similarly, there was no significant change in the Cobb angle and dynamic balance ability pre- and post-intervention (P>.05) in the non-taping group. Furthermore, there was no significant difference upon comparing the two groups (P>.05). Conclusion: Kinesio taping combined with Schroth exercises did not give additional benefits in terms of the Cobb angle and dynamic balance ability than that in the application of Schorth exercises alone.

Background: During postural control, older adults are more dependent on proprioception than are young adults. Ankle proprioception, which plays an important role in maintaining postural balance, decreases with age. Published studies are insufficient to establish a significant age difference in postural sway resulting from the known age-related decrease in ankle proprioception and do not examine various detailed test conditions. Objects: The present study aimed to compare ankle proprioception between older and younger groups along dimensions of position vs. force proprioception and dorsiflexion vs. plantarflexion. The present study also aimed to compare postural sway between young and older women during quiet standing under two sensory conditions. Methods: We recruited seven young women aged 21–24 and seven older women aged 60–63. Ankle proprioception was assessed as the accuracy of the joint position sense (JPS) and the force sense (FS). Postural sway was assessed using center-of-pressure measurements recorded during quiet standing under two sensory positions: eyes open and eyes closed with head tilted back. Results: Older women had lower JPS in dorsiflexion and lower FS in plantarflexion than did younger women. We found no significant age differences in JPS in plantarflexion or in FS in dorsiflexion. We observed a main effect of group on postural sway in two sway parameters out of three. We observed significant differences in JPS with dorsiflexion, and in FS with plantarflexion. Conclusion: Proprioception for ankle plantar flexor decreased more significantly with aging than did that for ankle dorsiflexor, accounting for the impaired postural balance observed in older women.
