        Three different cats who had chronic kidney disease (CKD) were treated for more than one month with fluid therapy in an animal clinic. Although this long-term treatment and hospitalization, there was no clinical improvement in clinical signs as well as serum biochemical indexes including blood urea nitrogen (BUN), creatinine (CREA), and phosphate (PHOS). All cases were then injected three times with allogeneic stem cells through an intravenous route for treatment on Day 0, 7, and 14 or 30. On the same day, clinical observation and blood tests for serum biochemistry were conducted together. Upon administrating stem cells to the CKD cats, clinical conditions and the indexes of BUN and CREA were clinically improved within normal ranges. Additionally, one of the cats who had the renal cysts presented clinical improvement with showing decreased cysts size than before.

        Angiotensin receptor blockers, such as telmisartan, are considered effective in the treatment of hypertension and proteinuria due to chronic kidney disease (CKD) in cats. It selectively blocks the AT1 receptor and does not affect the AT2 receptor, thus effectively blocking the activity of the renin angiotensin aldosterone system. This study aims to compare over time the changes in various indicators, including systemic hypertension and proteinuria, before and after the administration of telmisartan in cats with CKD. Decrease in blood pressure (BP) (p<0.001) and urine protein to creatinine (UP/C) ratio (p<0.001) were found to be statistically significant over time after the administration of telmisartan. BP and the UP/C ratio were 160 ± 22.2 and 0.50 ± 0.647 before telmisartan administration (Day 0), 150 ± 21.0 and 0.27 ± 0.487 on the 30th day (Day 30), 150 ± 17.0 and 0.25 ± 0.376 on the 60th day (Day 60), and 140 ± 17.8 and 0.15 ± 0.233 on the 90th day (Day 90) after administration, respectively. BP and UP/C were statistically significantly lower in cats with CKD over time at each time point from Day 0 to Day 90 at 30 day intervals. Especially after 90 days of telmisartan administration, the improvement of BP and UP/C were estimated to be about 20 mmHg and 0.35, respectively. In conclusion, the oral administration of telmisartan to cats with CKD is effective in improving BP and proteinuria, which has a positive effect on long-term survival in cats with CKD.

        채소내의 칼륨은 만성 신부전 환자에게 악영향을 주는 것으로 알려져 있다. 하지만 채소는많은 다른 영양 물질 또한 함유하고 있으므로, 이러한 환자들에게 채소의 섭 취는 불가피하다. 본 연구는 완전제어형 식물공장에서 신장환자들을 위해 적합한 저칼륨 상추 개발을 위해 수행되었다. 본 연구에 사용된 상추 품종은 ‘찰스’ 품종이 었다. 완전제어형 식물공장에서 정식 후 28일간 수행되었으며, 광 환경은 LED (W:R, 9:1), 일장 16시간, 200μmol·m-2·s-1 이었고 재배 온도, 상대습도와 이산화탄 소 농도는 각각 15-21oC, 65%와 600-650mg·L-1 이었다. 1차 실험에서 배양액은 수확 전 1주전과 2주전에 칼륨농도를 10%와 5%로 낮게 처리하였고, 이를 토대로 2차실 험의 처리는 수확 전 2주전 5%, 1% 낮춘 처리와 원수로 재배한 처리였다. 식물체 내의 다량원소 분석을 위해 수확 후 ICP 분석을 하였으며 생체중과 엽수를 측정하였다. 실험 결과 식물체 내 칼륨함량은 대조구에 비해 2주전 처리에서 통계적으로 유의하게 낮았다. 생체중과 엽수는 대조구와 처리구에서 유의적인 차이를 보이지 않았으나 처리구간의 유의적인 차이는 있었다. 신장환자들 의 채소섭취량을 고려했을 때, 저칼륨 상추 생산을 위한 적당한 칼륨농도 조성은 수확 2주전 1%와 5%로 낮춘 처리였다. 본 연구를 통해 양액 내 칼륨함량과 처리시리를 조절하여 신장환자를 위한 저칼륨 상추를 개발할 수 있었다.

        A 7-year-old spayed female English Cocker Spaniel dog presented with polyuria (PU), polydipsia (PD), intermittent vomiting, and weight loss. Physical examination revealed pale, tacky mucous membranes and severe emaciation. Hematological and biochemical examinations revealed moderate normocytic normochromic non-regenerative anemia and moderate azotemia. Abdominal ultrasonography demonstrated bilaterally small lumpy-bumpy kidneys with hyperechoic parenchyma as well as loss of renal corticomedullary junction. Based on clinical history and examinations, the dog was diagnosed with chronic kidney disease (CKD). The dog was treated with supportive care including fluid therapy, phosphate-binding agent, and histamine H2-receptor antagonist. Darbepoetin Alfa was administered to control renal secondary non-regenerative anemia. Prescribed diet with low-protein and low-phosphorus was fed to alleviate CKD signs. Further, dietary probiotics were supplemented. This case demonstrates that oral probiotic supplementation helped reduce blood urea-nitrogen (BUN) levels. This case indicates that dietary probiotics can be a potential alternative therapeutic agent for management of renal failure

        Chronic kidney disease can be treated if it is detected early, but as the disease progresses, it becomes impossible to recover. Finally, renal replacement therapy such as transplantation or dialysis should be used. Ultrasonography is used to diagnose kidney cancer, inflammatory disease, nodular disease, and chronic kidney disease. It is used to identify information about degree of inflammation using information such as kidney size, internal echo characteristics. Currently, the degree of disease in the clinic uses the value of glomerular filtration rate. However, even in ultrasound, changes in the degree of inflammation and disease can be observed. In this study, we used ultrasound images to quantify the changes in brightness, size, cortex, and subclinical changes of the kidney with progression of the disease, and compared them with the glomerular filtration rate used in clinical practice . In 105 cases, we performed 35 cases of normal kidney, 35 cases of early kidney disease, and 35 cases of terminal kidney. The brightness of the cortex of the image was obtained and the difference in brightness between the cortex and the proximal portion was obtained by the slope. The graph of the portion which was not smooth due to the ultrasonic characteristics was used as the function regrass. The size reduction was obtained from the original data. The results were as follows: It was proportional to the glomerular filtration rate. It is considered that the algorithm can be applied to the disease if the algorithm study continues.

        This is the case report of a 58-year-old man who developed acute severe hypoglycemia after a spinal block. He had a history of injecting insulin combined with 50% dextrose for hyperkalemia control. After the spinal block, he presented with hypo-glycemic symptoms and went into convulsions. Following injection of midazolam and 10 ml of 50% dextrose, his mental state returned to alert. We recommend careful monitoring of blood sugar level of chronic kidney disease patients who under-go neuraxial block.

        A 72-year-old woman with diabetic chronic kidney disease visited the authors’ hospital with fever, dysuria, and left flank pain. She did not complain of typical angina upon her admission, however, given the presence of a new-onset left-bundle branch block, elevated cardiac enzymes, and documented E. coli septicemia, coronary angiography and percutaneous coro-nary interventions were performed for the mid-left anterior descending artery and the mid-to distal-right coronary artery. We should keep in mind that urosepsis in patients with diabetic chronic kidney disease, who are at high risk of cardiovascular disease, can be associated with painless acute myocardial Infarction.
