        Bacterial meningitis is an uncommon complication of pituitary macroadenoma. Bacterial meningitis can occur in patients with pituitary macroadenoma who have received sphenoidal surgery or had cerebrospinal fluid (CSF) rhinorrhea. A 62-year-old male visited our hospital for headache and fever. Brain magnetic resonance imaging showed a pituitary macroadenoma. CSF study revealed acute bacterial meningitis. Intravenous antibiotics and hydrocortisone replacement therapy were started, and lead to good clinical outcome. Bacterial meningitis should be considered in patients with a pituitary macroadenoma who present with meningitis symptoms, even though in the absence of rhinorrhea or surgical history.

        This study was evaluated compressive strength of fiber reinforcement mortar using basalt fiber as part of a basic study for development to fiber reinforcement mortar using basalt fiber. Results of assessment, influence of the compressive strength according to used superplasticizer (SP) has shown that without, in addition, compressive strength according to used viscosity agent (VA) is determined that the compression strength decreased due to the bubbling.

        A total of 57 patients with basilar apex aneurysms were treated at our hospital from January 1990 to May 2011. The 57 patients were comprised of 18 male and 39 female patients aged from 31 to 81 years old (Mean age: 58.4 years old). Twenty nine cases were treated surgically and 28 cases underwent endovascular coiling. Medical and surgical records, postoperative angiographies, and follow up angiographies were reviewed retrospectively. Basilar apex aneurysm remains the ultimate challenge to neurovascular surgeons. Mortality and morbidity of endovascular coiling cases was relatively lower than that of surgical cases.

        Cervical epidural injection is a common treatment for management of neck, shoulder, and upper extremity pain. Although several complications are possible, cardiopulmonary arrest and intraventricular pneumocephalus are rare serious complications, mainly occurring when the dura has been punctured. We report on a case of cardiopulmonary arrest and intraventricular pneumocephalus after cervical epidural injection with loss of resistance technique using air. After performance of cardiopulmonary resuscitation for 30 minutes, a pulse was regained; however, the patient’s mental status was coma. Despite receiving conservative treatment, the patient ultimately died without recovery of consciousness.

        After percutaneous endoscopic lumbar discectomy (PELD), postoperative radiating leg pain could persist and become a common problem. This symptom provides important information for use in deciding on performance of reoperation for remnant disc or recurrent disc herniation. Of 225 patients who underwent PELD, 51 patients had persistent radicular pain. Steroid injection to epidural space resulted in pain reduction of at least 50 % in 15 (30.3 %) of these patients. Thirty five cases showed a bad outcome after steroid injection, indicating high sensitivity for detection of a remnant or disc herniation in follow up Magnetic Resonance images (25 cases, 71.4%). Twenty one patients were treated by reoperation. Steroid injection has a diagnostic value with a high sensitivity for mechanical compression of the nerve root as remaining or recurrent disc herniation in MRI for the necessity of a re-operation.

        The principle of treatment of type 2, 3 odontoid process fracture is controversial. The debate is in regard to which is effective between conservative treatment and surgery. We enrolled 41 patients with type 2, 3 odontoid process fracture from January 2007 to December 2011 at Chosun University Hospital. Patients in Group I (GI) received a halo vest and those in Group II (GII) underwent surgery by ant. screw fixation. The bone fusion was classified according to stable bone union, fibrous union, and unstable non-union. Patients in GI received halo vest for a mean period of 4.9 months and those in GII received a neck collar for a mean period of 3.8 months. Tweleve patients in GI and 17 patients in GII had stable bone union. Three patients in GI and two patients in GII had fibrous union. Five patients in GI and two patients in GII had non-union. The halo vest was a good conservative treatment for type 2, 3 odontoid process fracture : however, low bone fusion rate is a problem in particularly old age. However, ant. screw fixation is a better treatment in regard to short surgical time, short fusion time, and high bone fusion rate.
