        Background: Shoulder impingement syndrome, a major cause of shoulder pain, involves weakness of the scapular retractor muscles. The major scapular retractor muscles are the middle trapezius and rhomboid major muscles; however, the latter is excluded in most studies. Objects: We aimed to measure the thickness of the middle trapezius and rhomboid major muscles using an ultrasonic diagnostic imaging system while performing four different shoulder retraction exercises and comparing the thicknesses and ratio of the thicknesses of these muscles. Methods: The thickness of the middle trapezius and rhomboid major muscles was measured in 24 healthy adults using ultrasound. Muscle thickness was measured three times in the Reference posture and four times while performing four different exercises that involved scapular retraction. The averages and standard deviations of the measured muscle thicknesses were obtained and compared. The ratio of muscle thickness and rate of changes in muscle thickness between the reference posture and the four exercises were compared. Results: For both, male (n = 10) and female (n = 14), there was a significant difference in the thickness of the middle trapezius muscle between the reference posture and the four exercises (p < 0.05) and in the thickness of the middle trapezius and rhomboid major muscles between male and female (p < 0.05); however, there was no significant difference in the ratio of the thicknesses of these muscles. Although a significant difference in the rate of change in muscle thickness during the four exercises was noted, there was no significant difference in the ratio of change in muscle thickness. Conclusion: This study demonstrates the ratio of the thicknesses of the middle trapezius and rhomboid major muscles and the rate of change in their thickness during exercises involving scapular retraction in healthy people in their 20s–30s.

        Background: Shoulder external rotation exercises are commonly used to improve the stabilizing ability of the infraspinatus. However, during exercise, excessive activation of the posterior deltoid compared to the infraspinatus causes the humeral head to move anteriorly in an abnormal position. Many researchers have emphasized selective activation of the infraspinatus during shoulder external rotation exercise. Objects: This study aims to delineate the optimal exercise method for selective activation of infraspinatus by investigating the muscle activities of the infraspinatus and posterior deltoid according to the four shoulder exercise methods and two forearm positions. Methods: Thirty healthy individuals participated in this study. The participants were instructed to perform shoulder external rotation exercises following four exercise methods: sitting external rotation (SIER); standing external rotation at 90° abduction (STER); prone external rotation at 90° abduction (PRER); side-lying external rotation (SLER), and two forearm positions (neutral, supinated). The electromyography (EMG) signal amplitude was measured during each exercise. Surface EMG signals were recorded from the posterior deltoid, infraspinatus, and biceps brachii. Results: EMG results of the infraspinatus and posterior deltoid in PRER, were significantly higher than that of the other exercises (p < 0.01). The EMG ratio (infraspinatus/posterior deltoid) in SIER was significantly higher than that of the other exercises. EMG activation of the posterior deltoid in SIER, PRER, and SLER was significantly higher in neutral than in supinated (p < 0.01). Furthermore, the EMG of the infraspinatus in SIER was significantly higher in neutral than in supinated (p < 0.01). The EMG ratio (infraspinatus/ posterior deltoid) in SIER was significantly higher in neutral than in supinated (p < 0.05.) Contrarily EMG ratios in PRER and SLER were significantly higher in supinated than in neutral (p < 0.05). Conclusion: The results show that clinicians should consider these exercise methods and forearm positions when planning shoulder external rotation exercises for optimal shoulder rehabilitation.

        Background: Lower back pain (LBP) is a major cause of disability and a common musculoskeletal disorder encountered at some point in life. Dysfunction of the lumbar vertebrae has been associated with decreased flexibility of the hamstrings, which exhibited a strong positive correlation with LBP. Hamstring tension affects lumbar pelvic rhythm. We aimed to activate pelvic stability with compression by Active Therapeutic Movement (ATM), muscle energy technique (MET) was applied to increase the flexibility of the hamstring. Objects: In this study, we aimed to investigate the effects of MET with ATM and general MET were applied to the hamstring of adults, who were in their twenties with nor without LBP, on their pelvic inclination and the length of their hamstring. Methods: A total of 32 subjects were briefed about the purpose of this study and agreed to participate voluntarily. Before the experiment, all subjects were pre-examined, and they were divided into an LBP group and a no lower back pain group accordingly. Thereafter, all subjects participated in both in a crossover manner. After at least one week, they switched to another group and participated in the same experiment. Results: The study results revealed that both groups demonstrated significant results in the modified active knee extension test (p < 0.01) and the sit and reach test (p < 0.01) performed to assess the hamstring flexibility; an interaction (p < 0.05) was noted. Moreover, a more significant difference was observed between the MET with ATM and the general MET. Although significant results were obtained for the pelvic inclination (p < 0.01), interaction was not noted. Conclusion: Conclusively, in this study, when the MET with ATM was applied to the two groups, there was a significant difference compared to the general MET for hamstring flexibility, but it was confirmed that there was no significant difference for the pelvic inclination.

        Background: Backward walking exercise may offer some unique and potentially beneficial differences compared with forward walking exercise. There is still a lack of research on backward walking exercises and forward head posture. Objects: The purpose of this study was to determine the effect of backward walking exercise on college students forward head posture in their 20s. Methods: Twenty-one subjects participated in the experiment. The subjects were those with a craniovertebral angle (CVA) of 55 degrees or less who understood the purpose and method of this study and voluntarily agreed. A camera capable of taking pictures of the lateral plane was installed at a distance of 1.5 meters before exercising. Pictures were taken before walking backward, and after walking for 15 minutes on the treadmill, the images were taken in the same way. The composition of the backward walking exercise was walking at a rate of 1.0 for 5 minutes, and the remaining 10 minutes were walking at a rate of 1.5. Wilcoxon signed rank test was used to compare CVA and craniorotational angle (CRA) before and after exercise. Results: As a result of this study, there was a significant difference in CVA before and after exercise (p < 0.05). There was a significant difference in CRA before and after exercise (p < 0.05). Conclusion: The backward walking exercise and verbal command seems to have positively influenced the changes in CVA and CRA among college students in their 20s. It seems that studies to confirm balance or muscle activity as well as changes in forward head posture through the long-term intervention of the backward walking exercise should be conducted.

        Background:Limitation of hamstring extensibility is often associated with various musculoskeletal problems such as alterations in posture and walking patterns. Thus, certain appropriate strategies need to be established for its management.Objects:The aim of this study was to compare the effects of the neural mobilization technique and static stretching exercises on popliteal angle and hamstring compliance in young women with short hamstring syndrome (SHS).Methods:Thirty-three women with SHS were randomly assigned to either group-1 (n1=17) that underwent the neural mobilization technique or group-2 (n2=16) that underwent the static stretching exercises. Outcome measures included the active popliteal angle (APA) and a hamstring’s electromyographic (EMG) activity at a maximum popliteal angle of the baseline. Intervention for each group was performed for a total time of 3-min (6 sets of a 30-sec application).Results:There were significant interactions between time and group in the APA [group-1 (pre-test to post-test): 69.70±8.14° to 74.14±8.07° and group-2: 68.66±7.42° to 70.52±7.92°] (F1,31=6.678, p=.015) and the EMG activity of the hamstring (group-1: 1.12±.30μV to .69±.31μV and group-2: 1.19±.49μV to 1.13±.47 μV)(F1,31=6.678, p=.015). Between-group comparison revealed that the EMG activity of the hamstring was significantly different at post-test between the groups (p<.05). Furthermore, in within-group comparison, group-1 appeared to be significantly different for both variables between pre- and post-test (p<.05); however, group-2 showed significant difference in only the APA between pre- and post-test (p<.05).Conclusion:These findings suggest that the neural mobilization technique and static stretching exercises may be advantageous to improve hamstring compliance in young women with SHS, resulting in a more favorable outcome in the neural mobilization technique.

        차(􃹍) 위원회 대 장 뤼크 뒤송 사건이란, 프랑스인 장 뤼크 뒤송이‘다르질링’이라는 문구와 찻주전자 그림이 조합된 상표를 등록해 도서, 잡지, 상담업 등에 사용하자, 인도 통상산업부 산하로 다르질링 지역에서 생산되는 차를 관리하는 인도 차위원회가 뒤송을 상대로 해당 상표의 취소와 손해배상 등을 구하며 파리지방법원에 소제기한 사건이다. 준거법이 무엇인지, 등록을 하지 않은 상태에서도 ‘다르질링’이 지리적 표시로서 보호되는지, 차위원회에게 원고적격이 있는지, 차와는 분야가 다른 도서, 상담업 등에 상표를 사용하는 것(사용관련성이 없는 이른바 교차 유형)도 지리적 표시 침해인지 등이 쟁점이 되었다. 이에 제1심 법원은 원고 패소 판결을 선고하였으나, 항소심 법원인 파리 항소법원은 항소를 인용하고 제1심 판결을 취소하는 원고 승소 판결(대상판결)을 선고하였다. 대상판결은 결과적으로는 지리적 표시의 침해를 인정했지만, 내용 면에서는 민법 등 일반 법리를 원용한 것이 특색이다. 침해 당시 이 사건 지리적 표시등록이 없었던 점, 침해 상표와 피침해 지리적 표시의 사용관련성이 부정되는 점을 고려한 것으로 본다. 한국법상으로는 지리적 표시 등록을 해야 비로소 지리적 표시권이 발생하므로, 국내 사안에도 비슷한 논리를 적용할 수 있을 것이다.

        강원도 고성 뒤배재 현무암은 산출상태, 구성광물, 주성분원소 및 미량원소의 특징에 의해 하부 현무암과 상부 현무암으로 구분된다. 하부 현무암은 집괴암의 특징을 보이고, 맨틀 및 지각기원의 포획암과 감람석, 휘석, 사장석 포획광물이 있다. 상부 현무암은 주상절리로 산출되며, 맨틀 포획암이 상대적으로 더 많고, 포획광물로는 감람석이 우세하다. 현무암의 주성분원소 및 미량원소 분석 결과 상부 현무암은 하부 현무암에 비하여 초생 마그마에 가까운 조성을 보인다. 원시맨틀로 표준화한 미량원소 및 희토류원소의 부화 및 결핍 패턴은 두 현무암이 매우 유사하나, 하부 현무암이 상부 현무암 보다 LREE의 부화 정도가 더 크다. 뒤배재 화산체의 하부 현무암은 약 0.8-1.2%, 상부 현무암은 약 3.7-4.0%의 동일한 맨틀 물질인 석류석 페리도타이트의 배취 용융으로 형성되었다. 그리고 하부 현무암내의 포획된 화강암, 포획광물의 반응연 및 사장석, 석영 등의 포획광물은 하부 현무암을 형성한 마그마와 지각물질과의 동화 가능성을 지시한다.

        The aim of the current study was to assess the effectiveness of backward gait training on the treadmill in patients with spastic diplegic cerebral palsy (CP). Twelve patients with spastic diplegic CP participated in the study. An 8-week course of backward gait training was administered to the subjects for 3 days per week. Pre-intervention and post-intervention assessments of temporal-spatial gait parameters, the symmetry of the bilateral lower extremity weight bearing, and gross motor function were analyzed using motion analysis system, force plate, and Gross Motor Function Measurement (GMFM). There were significant improvements (p<.05) in the measures of both step length and right stance phase time. Joint kinematics showed increase in right hip abduction in initial contact and terminal swing, right hip external rotation and knee flexion in mid-swing, left ankle dorsiflexion in initial contact and terminal swing (p<.05). The symmetry of the bilateral lower extremity weight bearing and GMFM also significantly increased (p<.05). These findings indicate that backward gait training using a treadmill is beneficial for patients with spastic diplegic CP.

        It is important to find the effective position for cough and sputum clearance in respiratory physical therapy. The purpose of this study was to compare the changes in peak expiratory flow (PEF), forced expiratory volume in 1 second (FEV1), and peak cough flow (PCF) related to functional level and measurement position in patients with Duchenne muscular dystrophy. Twenty one subjects were classified into three functional levels, and measurements was undertaken in three different measurement positions (upright sitting, 45˚ reclining and supine). Vitalograph PEF/FEV DIARY was used to measure PEF and FEV1, and Ferraris Pocket Peak was used to measure PCF. Mixed two-way analysis of variance and Bonferroni post-hoc test were used for statistical analysis. The results of the study were as follows: 1) Significant main effects for measurement position were found. 2) PEF was the highest in upright sitting, followed by 45˚ reclining, and supine in order. 3)FEV1 in upright sitting and 45˚ reclining were significantly greater compared with that in supine. 4) PCF in upright sitting and 45˚ reclining were significantly greater compared with that in supine. 5) No significant main effects for functional level were found in PEF, FEV1, and PCF. 6) No significant functional level by measurement position interactions were found in PEF, FEV1, and PCF. Therefore, it is concluded that upright sitting and 45˚ degree reclining positions are recommended for effective cough and sputum clearance.
