        The purpose of this study was to assess the effects of High-Voltage Pulsed Current (HVPC) and ultrasound on adjuvant-induced arthritis in rats. Adjuvant arthritis was induced in female Sprauge-Dawley rats by the subcutaneous injection of a single dose of 1 ㎖ of Complete Freund's Adjuvant (CFA) (1 mg of Mycobacterium Butyricum suspended in 1 ㎖ paraffin oil) into the right hind paw. A randomized, parallel-groups design of 24 subjects was used. All rats were randomly assigned to control (n=8), ultrasound (n=8), and HVPC (n=8) were compared with those of injured rats. The rats in the pulsed ultrasound group were treated at 1 MHz frequency with 5 W/㎠ intensity in 1:4 mode for 5 minutes per day. The rats in the HVPC group were treated at 120 pulses per second and 50 ㎲ phase duration, 20 mA intensity for 30 min per day. Treatment was done in the left and right hind limb for 2 weeks. We evaluated clinical, radiographic, hematologic and histopathologic findings before and after treatment and obtained the following results. 1. Edema of the right hind paw was more significantly reduced in the ultrasound and HVPC groups than the control group on days 9, 12, and 14 (p<.05). Edema of the left hind paw was more significantly reduced in ultrasound and HVPC groups than the control group on days 12, 14 (p<.05). 2. WBC counts of the ultrasound and HVPC groups as compared with the control group were becoming remarkably decreased after the treatment. 3. In radiologic findings, arthritis formation was seen according to the score of arthritis, which was the highest in the control group, upon the observation of radiographs of the left and right hind paws. However, no statistically significant difference was present in the score within three groups. 4. In the histopathologic findings, ultrasound and HVPC groups had effectively suppressed erosions of articular cartilage and inflammatory cell infiltrations. Therefore, the results of the study show that rats that were treated with the ultrasound and HVPC effectively suppressed adjuvant arthritis. However, no statistically significant difference was present between the ultrasound group and the HVPC group.

        The purpose of this study was to assess the effects of ultrasound on adjuvant-induced arthritis in rats. Adjuvant arthritis was induced in 26 female Sprauge-Dawley rats by the subcutaneous injection of a single dose of .1 mL of Complete Freund's Adjuvant (CFA) (1 mg of Mycobacterium Butyricum suspended in .1 paraffin oil) into the right hind paw. After confirming inflammatory edema and arthritis in the paw 2 weeks later, the arthritic rats were divided into 3 groups, i.e., a control group, a pulsed ultrasound group (Group A), and a continuous ultrasound group (Group B) with 8 rats placed in each group. The rats in Group A were treated with pulsed ultrasound at 1 MHz frequency with .5 intensity in 1 : 4 mode for 3 minutes. The rats in Group B were treated with continuous ultrasound at 1 MHz frequency with 2 intensity in the continuous mode for 3 minutes. The ultrasound treatment was done in the left and right ankles for 2 weeks. Clinical, radiographic and histopathologic findings were then evaluated before and after treatment and yielded the following results. 1. No significant difference was present in body weight between the control group and the treated groups. 2. A statistically significant decrease in the edema of the paw was seen in the rats in Group A that was treated with pulsed ultrasound by 26~29 days after the treatment started (p<.05). 3. According to radiological examination, Group A showed the lowest score in arthritis scale which means it showed a tendency to suppress arthritic inflammation of the left and right hind paws. However, no statistically significant difference was present in the score between the control group, Group A and Group B. 4. According to histopathologic findings, the degree of infiltration by inflammatory cells and hypertrophy of the synovium were less in Group A compared with the control group and Group B. The results of the study show that rats that were treated with the pulsed ultrasound effectively suppressed adjuvant arthritis. However, more effort is needed to objectively prove the effectiveness of ultrasound by developing more sensitive testing methods that could quantitatively evaluate the treatment effects of acute rheumatoid arthritis and by trying out different ultrasound treatment methods.

        Arthritis may well be the diagnosis most commonly encountered by physicla therapists. Until the past few year, physical therapy treatment programs for arthritis consisting of heat, ROM, isometric exercise and rest were the rule. But now physical therapy treatment activities have changee. New philosophies, about exercise in particular, have emerge, as well as an improved understanding and application of physical agents. Therefore a review of the literature regarding physical therapy for rheumatoid arthritis is presented.

        Background: Taraxacum platycarpum has been used in traditional medicine in Korea to treat intoxication and edema and as a diuretic. According to previous reports, it has anti-cancer, anti-gastritis, and anti-inflammation effects. However, the improvement effect of T. platycarpum on rheumatoid arthritis has not been investigated. The anti-oxidative and anti-inflammation effects of the aerial parts of T. platycarpum are different from those of its subterranean parts. Thus, we evaluated the effect of the water extracts of Taraxaci radix (WTR) on type II collagen-induced rheumatoid arthritis (CIA) in animal models. Methods and Results: Rheumatoid arthritis was induced by type II collagen. WTR (100㎎/㎏ and 500㎎/㎏) was administered to the animal models. Methotrexate was used as the positive control. The levels of interleukin-6, TNF-alpha, and type II collagen IgG in the animals were measured by using enzyme-linked immunosorbent assay. Treatment with 500㎎/㎏ WTR decreased the serum levels of interleukin-6, TNF-alpha, and collagen IgG in the CIA models. Moreover, treatment with WTR diminished the arthritisinduced swelling of the hind legs and monocyte infiltration in the bloodvessels of the animal models. Conclusions: These results indicate that WTR has the potential to improve rheumatoid arthritis by reducing the levels of inflammatory cytokines such as interleukin-6 and TNF-alpha. However, further experiments are required to elucidate the influence of WTR on signal transduction in vitro and in vivo.

        본 연구는 엉치엉덩관절의 골절, 탈구, 엉치엉덩 퇴행성관절염 진단을 위한 가장 좋은 영상을 얻기 위해 골반 팬톰과 엑스선관을 각도 변화를 주어 엉치엉덩관절의 Joint space 가장 잘 나타내는 영상을 얻어 방사선사에게 주관적 평가를 받아 보았다. 평가 결과 결론으로 엉치엉덩관절염과 퇴행성 관절염 발견을 위한 단순촬영에서는 엎드린상태에서 촬영은 검사반대쪽 엉덩이를 25°~30° 들어올리고 엑스선관을 앞엉덩뼈가시에서 2.5 cm 안쪽을 지나는 시상면에 대해 수직촬영 하고 누운자세에서는 검사쪽 엉덩이를 25°~30° 들어올리고 위앞엉덩뼈가시가 중앙에서 위앞엉덩뼈가시가 중앙에 대해 엑스선관 각도를 발쪽으로 5° 촬영 하며 반드시 양쪽 엉치엉덩관절후전사방향(RAO,LAO) 및 전후사방향(RPO, LPO) 모두 촬영을 하게 되면 관절염 진단을 내리는데 있어 많은 도움이 되리라 본다.

        Background : Osteoarthritis is an inflammatory disorder related with oxidative stress and apoptosis leading to cartilage damage. Recently, Cirsium japonicum var. maackii (CJM) was reported to play a protective role in various inflammatory response. However, the role of CJM in cartilage degeneration and osteoarthritis progression is still unknown. Therefore, this study aims to investigate the protective effect of CJM in a animal model of osteoarthritis and cartilage degradation. Methods and Results : First, in order to determine active ingredient contents of CJM, we were carried out total polyphenolic content and total flavonoid content analyses. As a result, dried aerial parts of CJM were found to contain 149.2 ± 24.1 ㎎·GAE/g dry weight and 27.9 ± 2.0 ㎎·NE/g dry weight in boiling water extraction. Also, the HPLC analysis of CJM showed major compounds identified as cirsimarin and cirsimaritin. In addition, CJM protected against osteoarthritic cartilage destruction in an osteoarthritis mouse model induced by destabilization of the medial meniscus, as demonstrated by histopathological analysis. Conclusion : The results of this study demonstrate that CJM may protect against the osteoarthritis and cartilage destruction. Further study is needed to identify the mechanism for their improvement effects of osteoarthritis and cartilage destruction.

        Due to the thick cortical structure, subtrochanteric fractures of femur is often caused by high energy trauma and there are only a few report of it from low energy trauma. We have experienced a case of bilateral subtrochanteric fracture which oc-curred after a simple slip down. The fracture was healed with applying intramedullary nail. We believe the fracture may have occurred from prolonged use of steroid which was used to treat underlying rheumatoid arthritis.

        Salmonellae are gram-negative bacilli of the family Enterobacteriaceae. Gastroenteritis was the most frequent clinical sign and osteoarticular infection like septic arthritis were extremely scarce in a patient with Salmonella infection. An immunocompromised state are known risk factors for development of septic arthritis due to Salmonella. The treatment of rheumatoid arthritis (RA) has recently seen a paradigm shift with the introduction of biological therapy. Tumor necrosis factor-alpha antagonist (anti TNF-α) have especially been able to play major roles in the treatment of RA. But there is concern that this will result in an increased incidence of infection. There have been few reports of Salmonella infection following anti TNF-α in a RA patient. Here, we report a rare case of septic arthritis of the left knee by Group B non-typhoidal Salmonella in a RA patient who was receiving etanercept therapy. The patients was treated with arthroscopy guided irrigation and debridement, and administration of intravenous antibiotics (ciprofloxacin) for 4 weeks. Since then, he achieved a full recovery from the infection.

        Complex of mulberry extract (CME) is composed of extracts of mulberry (Morus alba L.) fruit, mulberryleaves and black beans (Glycine max (L.) Merr.). In this study, we investigated prevention effects of CME on degenerativearthritis. The SC50 value of DPPH radical scavenging by CME was 158.49±11.35㎍/mL. We found that CME significantlyreduced the production of nitric oxide (NO) and protein expression of cyclooxygenase-2 (COX-2) in RAW 264.7 cells whichwere activated by LPS. Experiments using animal model of degenerative arthritis showed that CME (400㎎/㎏ bodyweight) inhibited the production of TNF-α (77.5%) and IL-1β (95.0%). Furthermore, it was observed that CME reduced to85.9% of paw edema induced by carrageenan. These results suggest that CME could improve degenerative arthritis.

        Achyrantes japonica (AJ) has been used to treat edema and arthritis in the traditional Korean medicine. Toelucidate the anti-inflammatory and anti-nociceptive effects of ethanol extract of AJ, the carrageenan-induced pawedema using a plethysmometer and thermal hypersensitivity using the plantar test were measured. Ibuprofen was used asa control drug. Treatment with AJ (200㎎/kg p.o.) significantly reduced paw edema, compared to the carrageenan -treated rats. In the plantar test, the thermal withdrawal latency in AJ - treated group was significantly increased than thecarrageenan - treated group. The results indicate that AJ could have be the anti-inflammatory and anti-nociceptive prop-erties.

        CT는 단층촬영을 이용한 의학적인 영상 진단 기법이다. CT는 3차원적인 방사선학적 영상 기법으로 염증의 평가에는 적합하지 않으나, 석회화된 조직을 직접적인 3차원 영상으로 보여주므로 뼈 손상의 평가에는 유용하 다. 본 연구는 실험적으로 유도된 랫드의 보조관절염에서 관절의 병리학적 변화와 뼈 파괴의 정량적 분석을 3 차원 CT 영상을 통하여 평가하고자 실시되었다. 그 결과 랫드의 보조관절염에서 병변의 파괴성 진행이 3차원 CT 영상을 통해 정량화될 수 있었고, 따라서 관절염 질환의 상태 및 실험적인 치료 약제의 효능 평가에 3차원 CT 영상 기법이 효과적일 것으로 생각된다.

        류마티스성 관절염의 동물 모델인 CIA(collagen induced arthritisd) 생쥐에 CRN을 복강 투여하여 CIA의 발병률 및 관절염지수 등에 대한 효과를 조사한 결과는 다음과 같다. RF(rhumatoid factor) 인자인 IgG와 IgM 생산량, 그리고 염증 사이토카인인 IL-6와 TNF-α의 생산량이 감소하였다. 사이토카인 분석결과, 비장세포에서 Th1과 Th2 세포에서 분비되는 IFN-γ와 IL-4를 측정하여 CRN 투여로 Th1 세포에서 Th2 세포로 면역반응이 전환됨을 시사하는 것은 CRN의 치료효과로 사료된다. 혈청 중에서 RA 인자인 IgG와 IgM, 염증 사이토카인인 TNF-α와 IL-6 및 type II bovine collagen antibody 생산량 이 대조군에 비하여 CRN 투여군이 감소하여 CRN이 관절염의염증 사이토카인을 억제하는 것으로 확인된다.

        본 연구에서는 택칠에서 분리된 CRN(corilagin)이 류마티스성 관절염 치료제로 사용될 수 있는가능성을 조사하기 위하여 류마티스성 관절염의 동물 모델인 CIA(collagen induced arthritis) 생쥐에 CRN을 복강 투여하여 CIA의 발병률 및 관절염 지수 등에 대한 효과를 관찰하여 다음과 같은 결과를 얻었다. 택칠에서 분리한 corilagin을 관절염 유발 생쥐에게 투여한 결과, 관절염의 발병진행이 억제되고 관절염 지수가 감소되는 것을 확인하였다. 세포 조직학적 관찰을 통하여 염증성 세포들이 현저하게 줄어들고, 관절강이 잘 확보되었으며, pannus의 형성이 보이지않고 연골 또한 잘 보호되어져 있음을 확인하였다. 관절염 유발 생쥐의 형광유세포 분석결과 각 조직에 침윤되는 염증세포가 감소하였다.
