        The purpose of this study was to investigate the effects of visual information and different elevations of medially wedged insoles on the proprioceptive sense of the knee joint. The subjects of this study were 16 able-bodied men who were not athletic. An electrogoniometer was used to determine the error value between calculated 50% of full flexion (target position) and performed 50% of full flexion in a standing position with the upper extremities crossed. Tests were randomly performed in conditions. Visual variations included open eyes vs. closed eyes, while the elevation was adjusted through the use (or lack thereof) of medially wedged insoles of 10 mm, 14 mm, and 18 mm. The average error value in each condition was statistically analyzed. The findings of this study revealed as follows: 1) The average error value was significantly higher with the subjects' eyes open than with their eyes closed (p<.05). 2) The averaged error value was also significantly higher when the subjects were elevated 18 mm than with no elevation at all (p<.05). The findings of this study should be considered in lower extremity rehabilitation programs when medially wedged insoles used.

        The purpose of this study was to examine the effect of the angle of a wedged insole on knee varus torque during walking. Fifteen healthy subjects were recruited. Knee varus torque was measured using three-dimensional motion analysis (Elite). Knee varus torque was normalized to gait cycle (0%: initial contact; 100%: ipsilateral initial contact) and stance phase (0%: initial contact; 100%: ipsilateral toe off). The average peaks of knee varus torque during the stance phase of the gait cycle according to the different insole angles (10 or 15 degrees) were compared using one-way ANOVA with repeated measures. The results showed that in the early stance phase, the average peak knee varus torque increased significantly for both the medial 10 and 15 degree wedged insole conditions and decreased significantly for both the lateral 10 and 15 degree wedged insole conditions as compared with no insole (p<.05). However, there were no significant differences between the 10 and 15 degree wedged insole conditions with either the medial or lateral wedged insole (p>.05). In the late stance phase, the average peak knee varus torque increased significantly for the medial 10 and 15 degree wedged insole conditions (p<.05), but not for the lateral 10 and 15 degree wedged insole conditions as compared with no insole (p>.05). We suggest that these results may be beneficial for manufacturing foot orthotic devices, such as wedged insoles, to control medial and lateral compartment forces in the knee varus-valgus deformity. Further studies of the effects of wedged insole angle on knee varus torque in patients with medial-lateral knee osteoarthritis are needed.

        The purpose of this study was to find the difference in muscle firing rate between each muscle according to the knee angle with the quadriceps femoris which is a representative action muscle of the lower extremity. Seven normal healthy subjects were recruited. The median frequency (MDF) of muscle contraction was recorded from vastus lateralis, vastus medialis, and rectus femoris muscles using the surface EMG, in 5 seconds, during maximal isometric knee extension. The data were analyzed by the two-way repeated ANOVA. The results of the study were as follows: 1) median frequency of muscle contraction was significantly higher at the vastus lateralis, vastus medialis, and rectus femoris in descending order. 2) median frequency of muscle contraction was significantly higher at the , , and in descending order. Consequently, muscle recruitment at the knee decreases the EMG activity of the lengthened muscle. This study suggests that the change in EMG activity at different muscle lengths resulted in affecting the muscle firing rate during the knee extension.

        The seismic performance of a three-story knee-braced moment-resisting frame (KBMRF), which was typically employed to support pipelines for oil or gas, was investigated. Nonlinear static pushover analyses were performed first to observe the force-displacement relationship under increasing seismic load. Then nonlinear dynamic time-history analyses were carried out usingeight earthquake ground motion time-histories scaled to fit the design spectrum of UBC-97. It turned out that the maximum inter-story drift was smaller than the drift limit of 1.5 % of the structure height, and that the columns remained elastic. Based on these analytical results, it can be concluded that the seismic performance of the structure satisfies all the requirements regulated in the seismic code.

        The purpose of this study was to identify the influence of wedged insole and foot progression angle (FPG) on lateral thrust of knee in healthy subjects. Fifteen healthy male subjects were recruited from Suncheon First College, in Suncheon. The subjects randomly walked at the comfortable velocity under five conditions: bare footed, medio-lateral wedged insoles, toe-in and toe-out gait. The lateral thrust was measured by a accelerometer with telemeter during walking. Data was collected while each subject walked for about 10 gait cycle on a flat, level walkway at their normal speed. The middle three gait cycle were used for averaged peak value of lateral acceleration. The three averaged peak value of lateral acceleration were collected under each condition at heel strike. The results showed that averaged peak value of lateral acceleration increased significantly in medial wedged insole and toe-in gait and decreased significantly in lateral wedged insole and toe-out gait as compared with bare footed (p<.05). These results suggest that wedged insole as well as walking strategy, such as foot progression angle, may prevent progression of degenerative knee osteoarthritis.

        Manual tracking is an experimental paradigm that can be used to study information processing in continuous movements involving accurate, ongoing control of motor performance. The purpose of this study was to identify the effects of knee tracking training, using the paretic side, on gait in stroke patients. Nine patients with hemiplegia participated in the study. The timed 10 m gait speed test and tracking test were administrated. The tracking test was composed with ranges of to and to . The tracking training consisted of five times every week for 4 weeks. The data were analyzed by non-parametric paired sign test of Wilcoxon. The flexion/extension error of the tracking test was significantly reduced on the paretic side, while the nonparetic side was not statistically significant. The transfer of the skill to the functional activity was shown in the significant improvement at timed 10 m gait speed test. This study shows that individuals with chronic who have impaired knee movement can be trained to improve their knee control through intensive practice at a knee movement tracking task and that the skill learned from such training is transfered to a more functional gait speed.

        The purpose of this study was to investigate the effect of knee exercises on the onset times of vastus medialis oblique muscle (VMO) and vastus lateralis muscle (VL) and in healthy subjects. Fifteen subjects (7 men, 8 women) in a mean age of 26.4 years participated in the study. Electromyographic (EMG) signals were recorded from the VMO and VL under four exercises. Knee exercises consisted of open kinematic terminal knee extension, straight leg raising, isometric hip adduction exercise, and closed kinematic terminal knee extension. No significant differences were found in the onset times of EMG activities of VMO and VL in the four exercises. There were also no significant differences among the exercises. These results coincided with previous studies that found no difference between onset of VMO and VL. However, it is difficult to say that there is no difference between onset of VMO and VL in healthy subjects. To confirm this results, further researches that follow same on set determination metod and exercises are needed. Not only is the study of onset time of muscle needed, but also the studies of the amount of activation and the rate of increase of muscle activation are needed.

        This study was designed to identify the effects of foot position on electromyographic (EMG) activity of the quadriceps femoris during maximum voluntary contraction (MVC) in standing. Twenty young adults who had not experienced any knee injuries were recruited. Their Q-angles were within a normal range. They were asked to stand in five different foot positions ( externally rotated, internally rotated, neutral, plantarflexed, and dorsiflexed foot position). The EMG activities of the vastus lateralis (VL), rectus femoris (RF), and vastus medialis oblique (VMO) were recorded in standing by surface electrodes and normalized by MVC EMG values derived from manual muscle test. The normalized EMG activity levels (%MVC EMG) of muscles in the five foot positions were compared using repeated measures ANOVA. The EMG activity levels of the VL, RF, and VMO were the highest when foot was externally rotated. The EMG activity levels of the VL and RF were significantly different among the foot positions (p<.05). However, EMG activity levels of the VL, RF, VMO, and VMO/VL ratio did not show significant differences in each foot position (p> .05). The results suggest that the quadriceps femoris may be effectively activated by performing MVC at an externally rotated foot position. Therefore, the externally rotated foot position can be considered as an effective foot position for quadriceps femoris strengthening exercise. Further studies are needed to identify whether there are differences in the effects of foot position on muscle strength after MVC exercise of quadriceps femoris in standing.

        This study addresses the effects of treadmill training on hyperextended knee and cadence in patients with hemiplegia. A single subject research design with multiple baselines across individuals was used for the study. Two patients with hemiplegia participated in the experiment. The experiment consisted of interventions where the patients were asked to ambulate for 15 minutes at a comfortable walking speed on the treadmill with 11% slope grade and were allowed to rest for 10 minutes. Patients, then, were asked to ambulated 20 meters at walkway. The number of occurrences of knee hyperextension and the total number of steps were recorded. The results showed that the occurrence of knee hyperextension decreased by approximately 30% after the first session of the treadmill training and continued to gradually decrease during the following sets of treadmill training. Meanwhile, there was a slight increase in the cadence to a negligible extent. These results suggest that the gait training on the sloped treadmill may be helpful for correcting the knee hyperextension in patients with hemiplegia.

        The purpose of this study was to determine the effect of electrical stimulation biofeedback on motor learning of quadriceps muscle isometric exercise in 3 patients who have undergone total knee replacement surgery. A multiple baseline design across subjects was used. The electrical stimulation biofeedback was provided with each patient during quadriceps isometric exercise, which last 10 to 14 sessions with 10 repetitions each sessions. After training patients received 4 retention tests. Maximum muscle activity was measured pre- and post- electrical stimulation biofeedback training and retention test to evaluate the effect of biofeedback training. Maximum isometric muscle activity of quadriceps was increased after electrical stimulation biofeedback training in all subjects. The results indicate that a electrical stimulation biofeedback training is a useful method to improve motor learning of quadriceps isometric exercise in total knee replacement.

        It was recently reported that exercise-induced fatigue is related to joint position sense although some controversy remains. The purposes of this study were to examine the effect on the accuracy of reproducing the knee angles after a fatiguing isokinetic quadriceps exercise at four different levels (10%, 30%, 50%, and 70% of maximal force) and to find the optimal exercise level without causing knee joint proprioception impairment. Forty healthy women, ages 19 to 27, were randomly assigned to four experimental groups. Before and after the exercise, accuracy of positioning with respect to auditory feedback for specific angles was estimated by calculating the mean errors between specific angles and reproduction angles. Fatigue was measured by EMG signals displayed by a frequency spectrum analysis during the quadriceps exercise. Results showed that there was no significant difference in accuracy of the knee joint positioning sense following the exercises in group 1, group 2, and group 3 (10%, 30%, and 50% of maximal force, respectively); the exception being group 4 (70%). Fatigue level was significantly increased in group 4 but there were no significant increases of fatigue level in group 1, group 2, or group 3. The results concluded that the optimal exercise level to acquire the therapeutic exercise effectiveness without position sense impairment was at 50% of maximal force. Further studies using large sample size and patient groups with poor knee joint proprioception would be needed to confirm this conclusion and to clarify the possibility of clinical applications.

        Human balance is maintained through a complex process involving sensory detection of body motions, integration of sensorimotor information within the central nervous system, and execution of appropriate musculoskeletal responses. The basic task of balance is to position the body center of gravity (COG) over some portion of the support base. When the COG extends beyond the base of support, the person has exceeded the limits of stability (LOS). At this point, a step or stumble is required to prevent a fall. Automatic postural responses operate to keep the COG over the base of support. They are a set of functionally organized, long-loop responses that act to keep the body in a state of equilibrium. There are four commonly identified automatic postural responses, or strategies. These are ankle strategy, hip strategy, suspensory (knee) strategy, and stepping strategy. Thus, the purpose of this study was to evaluate the LOS using various knee strategies. Forty subjects participated in this study. The subjects were comprised of 20 males and 20 females who were without neurologic, orthopaedic or balance performance impairments. The LOS was measured with a Balance Performance Monitor (BPM) Dataprint Software Version 5.3. The results of this study were as follows: 1) Knee joint angle which is to increase stability of standing balance with using knee strategy was at mid-range. 2) There were statistically significant differences in anteroposterior LOSs according to the knee strategy. 3) There were no statistically significant differences in mediolateral LOSs according to the knee strategy. 4) There were statistically significant differences of anteroposterior LOSs with using knee strategy according to gender. 5) There were no statistically significant differences in mediolateral LOSs with using knee strategy according to gender.

        The purpose of this study was to determine whether the vastus medialis oblique muscle(VMO) had greater electrical activity than the vastus lateralis muscle(VL) when hip adduction and knee extension exercise were performed. Electrical activity of the VMO and VL was measured on 42 healthy subjects (28 men, 14 women) during maximal voluntary isometric contractions of hip adduction and knee extension by an EMG-BIOFEEDBACK. The results showed that the electromyographic activity of the VMO was significantly greater than that of the VL during the hip adduction exercise. Differences noted with knee extension by performing hip adduction exercises. Isometric hip adduction exercises, therefore, may be advisable in the treatment of patients with patellofemoral pain.
