목적 : 본 연구는 저항운동으로 6주간 적용된 혀-유지 메뉴버 프로그램이 Shaker 운동 프로그램과 비교하여 정상성인의 삼킴 근육 활성도에 미치는 영향을 알아보고자 하였다.
연구방법 : 2015년 10월부터 12월까지 10대에서 50대의 건강한 성인 26명을 대상으로 혀-유지 메뉴버 그룹과 Shaker 운동 그룹으로 무작위 할당하여 주 5일, 하루 1회, 30분씩 6주간 적용하였다. 표면근전도를 사용하여 설골상근 (suprahyoid muscle)과 설골하근(infrahyoid muscle)에 대한 근활성도를 수집하였다. 시각적 비율척도(Visual Analog Scale), 자각인지 척도(Borg-Rating of Perceived Exertion Scale) 척도를 사용하여 삼킴 시 노력과 피로를 측정하였다.
결과 : 혀-유지 메뉴버 그룹과 Shaker 그룹 모두 설골상근과 설골하근의 근활성도가 유의한 향상을 보였지만, 두 그룹 간의 유의한 차이는 없었다. 또한 두 그룹 모두 삼킴 시 노력 및 피로가 통계학적으로 유의한 차이를 보였으나, 두 그룹 간의 유의한 차이는 없었다.
결론 : 건강한 성인을 대상으로 6주간 혀-유지 메뉴버를 통해 삼킴 근육 근활성도에 대한 긍정적인 효과를 확인하였다. 향후에는 삼킴장애 환자를 대상으로 혀-유지 메뉴버에 대한 임상적 효용성을 검증하여 유용한 치료기법으로 사용되기를 기대한다.
Background: Lumbopelvic stability is highly important for exercise therapy for patients with low back pain and shoulder dysfunction. It can be attained using a pelvic compression belt. Previous studies showed that external pelvic compression (EPC) enhances form closure by reducing sacroiliac joint laxity and selectively strengthens force closure and motor control by reducing the compensatory activity of the stabilizer. In addition, when the pelvic compression belt was placed directly on the anterior superior iliac spine, the laxity of the sacroiliac cephalic joint could be significantly reduced.
Objects: This study aimed to compare the effects of EPC on lumbopelvic and shoulder muscle surface electromyography (EMG) activities during push-up plus (PUP) and deadlift (DL) exercise, trunk extensor strength during DL exercise.
Methods: Thirty-eight subjects (21 men and 17 women) volunteered to participate in this study. The subjects were instructed to perform PUP and DL with and without the EPC. EMG data were collect from serratus anterior (SA), pectoralis major (PM), erector spinae (ES), and multifidus (MF). Trunk extensor strength were tested in DL exercise. The data were collected during 3 repetitions of all exercise and the mean of root mean square was used for analysis.
Results: The EMG activities of the SA and PM were significantly increased in PUP with pelvic compression as compared with PUP without pelvic compression (p<.05). In DL exercise, a significant improvement in trunk extensor strength was observed during DL exercise with pelvic compression (p<.05).
Conclusion: The results of this study indicate that lumbopelvic stabilization reinforced with external pelvic compression may be propitious to strengthen PUP in more-active SA and PM muscles. Applying EPC can improve the trunk extensor strength during DL exercise. Our study shows that EPC was beneficial to improve the PUP and DL exercise efficiency.
목적 : 본 연구는 건강한 성인들을 대상으로 삼킴 시 음식물의 점도 및 양을 객관적으로 측정하고, 단계적으로 점도와 양을 증가시킬 때 나타나는 삼킴근육의 근활성 변화를 알아보고자 하였다. 연구방법 : D광역시 ○○대학교에 재학중인 건강한 성인 40명(남 20명, 여 20명)을 대상으로 surfaceElectromyographic(sEMG)를 사용하여 음식물의 점도와 양에 따른 삼킴 시의 목뿔위근육과 목뿔아래근육의 활성크기를 측정하고, 음식물의 점도와 양의 변화 시 삼킴에 따른 노력의 정도를 객관화된 수치로 나타내고자Visual Analogue Scale (VAS)을 사용하였다. 결과 : 음식물의 점도가 높아질수록 목뿔위근육의 근활성도가 높아지는 것을 알 수 있었으며, 음식물의 양이 많아질수록 목뿔위근육과 목뿔아래근육의 근활성도가 높아지는 것으로 나타났다. 결론 : 본 연구를 통해 임상에서 구강 내 조절 및 인두기 삼킴 반사가 저하된 환자들을 대상으로 연하의 식이를 결정할 때 점도의 정도와 양을 고려한 식이 추천에 도움을 줄 수 있을 것이다. 또한 음식물의 삼킴 시 목뿔위근육과 목뿔아래근육에 요구되는 근활성도를 추측하는데 도움이 될 것이다.
The purpose of this study was to investigate intra-rater reliability and determine the validity of electromyography (EMG) measurements to represent muscle activity and ultrasonography (US) to represent muscle thickness during manual muscle testing (MMT) to external abdominal oblique (EO) and lumbar multifidus (MF). Twenty healthy subjects were recruited for this study and asked to perform MMT at differing levels. The subjects' muscle activity using EMG was measured by a ratio to maximum voluntary contraction (MVC) and root mean square (RMS) methods. The subjects' muscle thickness using US was measured by raw muscle thickness and change ratio of thickness to maximum (MVC) or resting condition. In three trials, measurements were performed on each subject by one examiner. The intra-rater reliability of measurements of EMG and US to EO and MF was calculated using intra-class coefficients. The intra-rater reliability of all measurements was excellent (ICC=.75~.98) in EMG and US. The conduct validity was calculated by one-way ANOVA with repeated measurements to compare whether the EMG and US measurements were different between MMT at different levels. There was only a significant difference between all grades at %MVC thickness measurement of US. These results suggest that a %MVC thickness measurement of US was a more sensitive and discriminate in all manual muscle testing grades. This information will be useful for the selection of US measurement and analysis methods in clinics.
Surface electromyogram (sEMG), which is a bio-electrical signal originated from action potentials of nerves and muscle fibers activated by motor neurons, has been widely used for recognizing motion intention of robotic prosthesis for amputees because it enables a device to be operated intuitively by users without any artificial and additional work. In this paper, we propose a training-free unsupervised sEMG pattern recognition algorithm. It is useful for the gesture recognition for the amputees from whom we cannot achieve motion labels for the previous supervised pattern recognition algorithms. Using the proposed algorithm, we can classify the sEMG signals for gesture recognition and the calculated threshold probability value can be used as a sensitivity parameter for pattern registration. The proposed algorithm was verified by a case study of a patient with partial-hand amputation.
