        본 연구에서는 만성요통을 가진 여성노인의 체형 및 요부안정화를 위해 12주간 필라테스 운동 프로그램을 적용한 후 체형에 미치는 영향을 알아보고, 통증의 변화를 규명함으로써 노화를 늦추고 신체활 동 영역을 높일 수 있는 노인에게 적합한 운동 프로그램 개발에 필요한 자료를 제공하고자 하였다. 이에 3~5cm 중증도 통증범위의 만성요통을 가진 16명여성노인을 대상으로 주관적 통증척도지를 통해 실험집단 8명, 통제집단 8명을 단순무선표집(Simple Random Sampling)으로 선정하였다. 요부안정화 필라테스 운동 프로그램은 주 2회씩 총 12주간 50~60분간 실시하였으며, 1세트 종료 시 1분 휴식을 주는 방식으로 실시 하였다. 그 결과 첫째, 체형 및 통증에서는 경추 기울기에서 요부안정화 운동그룹의 경우 운동 전에 비해 감소하였으나 통제집단은 증가하였다. 둘째, 요부 통증의 결과 운동집단의 경우 운동에 비해 감소하였으나 통제집단은 증가하였다. 결론적으로 요부안정화 필라테스 운동은 만성요통을 가진 여성노인에게 있어서 통 증을 감소시킬 수 있는 프로그램으로 적용될 수 있으며, 체형을 개선시키는데 도움을 줄 수 있는 것으로 확인되었다.

        Background: Previous studies have reported that improving the spinal stability could be more effective in the prevention and treatment of recurrence. Lumbar stabilization exercise is known to strengthen the lumbar extension muscles and enhance physical, psychological and social functions. Objectives: To investigated the effect of lumbar spiral stenosis on the kinetic link training and lumbar stabilization exercise. Design: A randomized controlled trial. Methods: Study was preformed by randomly allocated 28 LSS participants into a kinetic link training group (KLT, n=14) and a lumbar stabilization exercise group (LSE, n=14). Kinetic link training and lumbar stabilization exercise were performed to subjects in both groups 5 times a week for 6 weeks. To verify the effect of LSS, changes in VAS, ODI, and proprioception before and after intervention were observed. Results: In KLT, statistically significant changes were found in VAS, ODI, and Proprioception before and after intervention. In LSE, there were significant changes in VAS and ODI before and after intervention. KLT and LSE before and after intervention indicated significant differences in proprioception. Conclusion: KLT and LSE are applied to LSS, there are effects of pain decrease, lumbar recovery and proprioception improvement.

        본 연구의 목적은 해부학적 근거로 제작한 5가지의 EMS 요추 복압 벨트가 요추 안정화에 미치는 영향을 연구하는 것이다. 본 연구는 요방형근, 척추세움근, 내복사근, 외복사근, 대요근으로 총 5가지의 코어 근육을 선정하여 이에 맞는 근육 모양과 통증 유발점인 압통점을 고려해 전도성 원단으로 패턴을 설계하여 복압 벨트에 결합했다. 총 4가지 동작으로 실험을 진행하여 각각 다른 EMS복압 벨트가 요추 안정화에 미치는 영향을 알아보았다. 건강한 신체의 20대 남성 5명을 대상으로 진행하였고 선정 조건은 최근 3개월 동안 요통 과거력이 없고 사전 검사를 통해 제한되는 동작이 없고, 체간의 근력이 정상등급에 속한 대상이다. 실험 동작의 순서는 하지직거상 검사, 좌전굴, 체전굴, 배근력으로 다음 동작에 제한되지 않게 선정하였다. 동작 간의 휴식은 2분으로 진행하였고, EMS복압 벨트를 착용 후 실험을 진행할 때는 혈류량 증가와 근 활성화를 위해 전기자극을 10분씩 적용하고 진행하였다. 실험 결과의 통계는 비모수 검정으로 윌콕슨 검정과 프리드만 검정을 실시함으로 구체적인 차이를 분석하였다. 본 연구의 결과 5가지의 패턴 중 5,4,3,1,2 순으로 Type별 순위 결과를 확인할 수 있었으며 각 근육의 움직임과 연관성이 있는 실험 동작에 조금 더 유의미한 결과를 확인할 수 있었다. 본 연구의 결론은 해부학적 근거로 제작된 패턴으로 인해 각 근육에 각기 다른 모양으로 전기자극을 전달하였을 때 구분되는 효과를 확인할 수 있었으며 일반인 대상으로 일상생활이나 트레이닝에 있어 요추 안정화를 향상할 수 있을 것으로 기대된다.

        Background: Few comparative studies have been conducted on strengthening the anterior and posterior muscles of the trunk via lumbar stabilization exercises. Objectives: To compare the effects of forward leaning exercise and supine bridging exercise in stability exercise. Design: Randomized controlled clinical trial (single blind). Methods: Thirty subjects with spondylolisthesis were participated in this study. Fifteen subjects performed the bridging exercises and fifteen subjects performed the forward leaning exercises. Each exercise was held for ten seconds per repetition, and four repetitions were considered one sub-session. A total of four sub-sessions were performed in one full exercise session. The full exercise session required thirty minutes, including rest time. Trunk strength and range of motion and Oswestry disability index were measured. Results: Two weeks later, trunk flexion strength and trunk extension range of motion were significantly increased in the forward leaning exercise group than in the supine bridging group, trunk extension strength were significantly increased in the supine bridging exercise group than in the forward leaning group. After two weeks, the pain score was significantly lower in the forward leaning exercise group than in the supine bridging group. Conclusion: This study has shown that stabilization exercises are effective in increasing range of motion and strength in spondylolisthesis subjects. It was especially confirmed that the method of strengthening the anterior muscles of the trunk is more effective than the standard stabilization exercise method.

        Background: Walking is a complex activity. The main components of walking include balance, coordination, and symmetrical posture. The characteristics of walking patterns of stroke patients include slow walking, measured by gait cycle and walking speed. This is an important factor that reflects post-stroke quality of life and walking ability. Objective: This study aimed to examine the effect of deep lumbar muscle stabilization exercise on the spatiotemporal walking ability of stroke patients. Design: Quasi-experial study Methods: The experiment was conducted 5 times per week for 4 weeks, with 30 minutes per session, on 10 subjects in the experimental group who performed the deep lumbar muscle stabilization exercise and 10 subjects in the control group who performed a regular exercise. Variables that represent the spatiotemporal walking ability (step length, stride length, step rate, and walking speed) were measured using GAITRrite before and after the experiment and were analyzed. Results: There was a significant difference in the pre- and post-exercise spatiotemporal walking ability between the two groups (p<.05). Furthermore, there was a significant difference in the step rate and walking speed between the two groups (p<.05). Conclusions: Deep lumbar muscle stabilization exercise is effective in improving the walking ability of stroke patients. Therefore, its application will help improve the spatiotemporal walking ability of stroke patients.

        Background: Lumbar disc herniation (LDH) causes neurological symptoms by compression of the dura mater and nerve roots. Due to the changed in proprioception inputs that can result in abnormal postural pattern, delayed reaction time, and changed in deep tendon reflex. Objective: To investigate the effects of lumbar stabilization exercises on motor neuron excitability and neurological symptoms in patients with LDH. Design: Randomized Controlled Trial (single blind) Methods: Thirty patients with LDH were recruited; they were randomly divided into the balance center stabilization resistance exercise group (n=15) and the Nordic walking group (n=15). Each group underwent their corresponding 20- minute intervention once a day, four times a week, for four weeks. Participants’ motor neuron excitability and low back pain were assessed before and after the four-week intervention. Results: There were significant differences in all variables within each group (p<.05). There were significant differences between the experimental and control groups in the changes of upper motor neuron excitability and pain (p<.05), but not in the changes of lower motor neuron excitability and Korean Oswestry Disability Index. Conclusion: Lumbar stabilization exercises utilizing concurrent contraction of deep and superficial muscles improved low back function in patients with LDH by lowering upper motor neuron excitability than compared to exercises actively moving the limbs. Lumbar stabilization exercises without pain have a positive impact on improving motor neuron excitability.

        Background: We developed a novel integrative lumbar stabilization technique that combines lumbar extension (LE) exercise with abdominal drawing-in maneuver (ADIM) to ameliorate low back pain (LBP) associated with neuromuscular imbalance and instability, based on the collective evidence of contemporary spinal rehabilitation. Objects: The specific aim of the present study was to investigate the effects of LE exercise with and without ADIM on core muscle strength, lumbar spinal instability, and pain, as well as functional characteristics in individuals with LBP using advanced radiographic imaging techniques. Methods: patients with mechanical LBP (N = 40, 6 males; 35.1±7.6 years) were recruited and randomly assigned either to the combined LE and ADIM (experimental group) or the LE alone (control group). Outcome measures included the visual analog scale, the modified Oswestry Disability Index, muscle strength imbalance (MSI), and radiographic imaging. The lumbar intervertebral displacement (LID), intervertebral (IV) and total lumbar extension (TLE) angles were calculated to evaluate the lumbar segmental instability. Results: The experimental group showed significant differences in the L3-L4, L5-S1 LIDs, L4-L5 and L5-S1 IV angles, and TLE angle as compared to the controls (p<.05). Immediate pain reduction and muscle strength imbalance ratio were significantly different between the groups (p<.05). Conclusion: These results suggest that the addition of ADIM significantly increased lumbar spinal stabilization in individuals with LBP, thereby reducing pain associated with functional lumbar flexion during daily activities.

        Background: Lumbar stabilization (LS) improve the thickness of the quadratus lumborum (QL) muscle and muscle activity of the gluteus medius (GM) muscle during hip abduction in a side-lying position in patients with low back pain (LBP). Objects: The purpose of this study was to assess the effects of LS on muscle thickness of QL and muscle activity of GM during hip abduction in side-lying in patients with LBP. Methods: The study included 32 patients with LBP, who were randomly divided into the control group and experimental group, each with 16 patients. All subjects performed 35° preferred hip abduction (control group) and 35° hip abduction with LS (experimental group) during side-lying. An ultrasonography and a surface electromyography were used to measure the thickness of the QL muscle, and the muscle activities of the GM muscle respectively. Independent t-test was used to compare the muscle thickness of the QL and the muscle activity of the GM muscle, respectively. Results: Anterio-posterior diameter in the muscle thickness of QL muscle was decreased significantly in hip abduction with LS more than in preferred hip abduction (p<.001), but medio-lateral diameter in the muscle thickness of QL muscle was not significantly different between in preferred hip abduction and in hip abduction with LS (p=.06). The muscle activity of GM was increased significantly in hip abduction with LS more than in preferred hip abduction (p<.001). Conclusion: These findings suggest that hip abduction with LS could be recommended as a hip abduction for LS and a prevention unwanted compensatory pelvic lateral tilting movement.

        This study aimed to investigate the effect of the abdominal drawing-in maneuver (ADIM) and abdominal expansion maneuver (AEM) on trunk stabilization, as well as trunk muscle activities and differences in quadruple visual analogue scale, Korean Oswestry Disability Index, and Fear Avoidance Beliefs Questionnaire scores, in patients with chronic low back pain and lumbar spine instability. To increase intra-abdominal pressure during the trunk stabilization exercise, the technique of pushing the abdomen out using diaphragmatic abdominal breathing suggested by Pavel Koral was used, which we termed the AEM. Fifty patients who tested positive on more than three of the five lumbar spine instability tests were separated from 138 patients with chronic low back pain of these patients, 16 were placed in the control group (trunk stabilization exercise), 17 were placed in the ADIM group (trunk stabilization exercise with ADIM), and 17 were placed in the AEM group (trunk stabilization exercise with AEM). Each group participated in the study for 30 minutes three times weekly for 4 weeks. Surface electromyography was used to measure the trunk muscle activities during the kneeling forward and supine bridging positions, and one-way repeated analysis of variance was used to determine the statistical significance of the trunk muscle activities in the rectus abdominis, internal oblique (IO), erector spinae, and multifidus (MF) muscles. The ADIM and AEM groups showed relatively larger improvements in psychosocial and functional disability level than control group. There were significant changes among the three groups, those from the measured values of the AEM group was significantly higher than the other two groups in changes in IO and MF trunk muscle activities (p<05). This finding demonstrates that trunk stabilization exercises with AEM is more effective than ADIM for increasing trunk deep muscle activity of chronic low back pain patients with lumbar spine instability.

        The purpose of this study is to identify the effective lumbar area exercise program to prevent falls of the elderly by dividing 14 female elders over 65 living in Daegu into two groups such as resistance exercise group and resistance & stabilization exercise group, and applying a 60 minute exercise program three times a week for 12 weeks. The followings are the results. First, both of resistance exercise and resistance & stabilization exercise brought improvement of lumbar strength and there was no significant difference between two groups. Second, there was no significant difference in lower limb strength in resistance exercise group but resistance & stabilization exercise group showed significant increase in lower limb flexor strength. To make a summary of the above results, it can be said that 12 weeks’ resistance & stabilization exercise is effective to the lumbar strength and lower limb strength of the elderly. Accordingly, if combining resistance exercise and stabilization exercise appropriately, we can build a successful preventive program which even the elderly who cannot perform resistance exercise easily can follow.

        The lumbar multifidus muscle, which can be separated into deep fascicles (DM) and superficial fascicles (SM), is important for lumbar segmental stability. However, no previous studies have investigated the effect of lumbar stabilization exercises on the thickness of DM and SM. Thus, the purpose of this study was to assess DM thickness after three different lumbar segmental stabilization exercises. In total, 30 healthy male participants were recruited and randomly assigned to one of three exercise groups: hollowing in the quadruped position (H-Quad), contralateral arm and leg lift (CALL), and bilateral arm and leg lift (BALL). Each lumbar segmental stabilization exercise was conducted over 4 weeks. Ultrasonography was used to compare the DM and SM thickness before and after the 4 weeks of exercise. A mixed-model analysis of variance using Scheffe's post-hoc test was used for statistical analysis. The results showed a significant effect for the measurement time (before vs. after 4 weeks of exercise) in the DM (F=31.26, p<.05) and SM (F=4.56, p<.05). At the end of the 4 weeks, the DM thickness had increased significantly in the H-Quad exercise group, and the SM thickness had increased significantly in the CALL and BALL exercise groups. Also in the BALL exercise group, the SM thickness was greater compared with that in the H-Quad exercise group. These findings suggest that the thickness of the DM and SM were increased by different types of lumbar segmental stability exercise after 4 weeks.

        The objective of this research was to examine the effects of lumber stabilization exercise and a general physiotherapy program for caregivers with chronic low back pain. Sixteen people participated in this study and were randomly assigned to two groups for either lumbar stabilization exercise or for general physiotherapy, respectively. The experiment was performed for eight weeks. To examine the general as well as the medical characteristics of the participants, the following measurements were used: Visual Analogue Scale (VAS); Oswestry Disability Index (ODI); Back Performance Scale (BPS); Roland - Morris Disability Questionnaire (RMDQ); and Beck Depression Index (BDI). To compare the general and medical characteristics of the participants in the two groups, an independent t test were used. During the experiment, a paired t test was conducted to determine whether there was a significant difference in the values of VAS, ODI, BPS, RMDQ, and BDI before and after the experiment. To examine the difference in the VAS, ODI, BPS, RMDQ, and BDI values in the two groups, ANCOVA was used with pre test values as a covariate. According to the test results, in the lumbar stabilization exercise group, the VAS, ODI, BPS, RMDQ, and BDI values showed a statistically significant difference before and after the test (p<.05). In comparison, in the general physiotherapy program group, only the ODI and BPS values showed a statistically significant level of improvement. Regarding the degree of improvement, participants in the lumbar stabilization exercise group showed statistically significant progress compared to those in the general physiotherapy group. In summary, lumbar stabilization exercise is regarded as more effective than general physiotherapy for treating caregivers with chronic low back pain. In future studies, it will be useful to expand the research and to examine the long term effects of lumbar stabilization exercise on workers.

        The purpose of this study was to investigate the effects of meditation music with lumbar stabilization exercise on balance, concentration and muscle activation. Participants are divided into Group A and Group B. Group A listened to meditation music while doing 4 sets of lumbar stabilization exercise. Group B did not listen to meditation music while doing 4 sets of lumbar stabilization exercise. Surface EMG data was obtained rectus abdominis, external oblique, erector spinae, multifidus muscle during lumbar stabilization exercise. To examine the effect of before-after experiment for concentration and balance, paired t-test was adopted. Changes in EMG data of each muscles were analyzed by independence t-test. There were statistically significant increasing at concentration level and balance level in Group A. But significant difference was not shown in muscle activation. The clinical effect of the meditation music with lumbar stabilization exercise was investigated in this study with more excellent results in concentration and balance. In conclusion, these results suggest that meditation music and lumbar stabilization exercise may be useful in elderly people.

        The aim of this study was to investigate the effects of lumbar stabilization on both trunk and lower limb muscle activity and center of pressure (COP) in single leg standing. Surface electromyography (EMG) was used to collect muscle activity data, the mean velocity of COP was measured using a force plate, and a pressure biofeedback unit was used for lumbar stabilization training. The findings of this study are summarized as follows: 1) The EMG activity of the erector spinae decreased significantly and the activity of the rectus abdominis, internal oblique, external oblique, gluteus maximus, and gluteus medius increased significantly with lumbar stabilization single leg standing. 2) No differences in activity in the tibialis anterior, medial gastrocnemius, rectus femoris, and medial hamstrings were found with single leg standing. 3) The mean velocity of COP in the antero-posterior and medio-lateral directions in the lumbar stabilization single leg standing decreased significantly compared with the preferred single leg standing. The findings of this study therefore indicate that lumbar stabilization can facilitate the co-activation of deep stabilization and global muscles that improve postural control capability during single leg standing.

        To improve abdominal muscles strengthening, double straight leg lowering (DSLL) has been widely used in physical therapy, fitness program, and athletic program. The purpose of this study was to investigate the effects of the lumbar stabilization maneuver with a pressure biofeedback unit on the muscle activity of rectus abdominis (RA), external oblique (EO), and internal oblique (IO) during DSLL. Fourteen healthy young men were recruited from university population. The electromyography (EMG) activity was recorded from the RA, EO, and IO of both sides. The normalized EMG activity was compared using a paired t-test. The study showed that EMG activity in the RA, EO, and IO was significantly higher during DSLL with lumbar stabilization (DSLL-LS) compared to performed DSLL (p<.05). These results suggest that DSLL-LS is recommended as an effective method for strengthening exercise for the abdominal muscles.

        요통은 산업화된 현대사회에서 흔히 발생하는 질환이다. 요통의 다양한 원인 중에서도 척추의 정상 가동범위를 넘은 불안정성이 요통의 한 원인으로 보고되고 있다. 이 문헌고찰의 목적은 요추 부위의 안정성을 부여하는 운동 프로그램 후 근육 기능의 강화, 통증의 감소를 보고한 논문들을 비교, 분석하는 것이다. 그리하여 요통의 한 원인인 척추의 안정성을 증가시켜 요추 부위의 통증을 감소시키고 기능을 향상시키는 운동 프로그램의 효과를 입증하는 것이다.
