In Sri Lanka, the shoulder in asphalt pavements has been constructed using the materials transported from borrow pit in the iRoad Project due to the low quality of in-situ soils. After excavating 150~200mm thick and 500mm wide shoulder area, the borrow pit materials are placed and compacted according to specifications. The excavated in-situ soils are dumped in designated location. It is estimated that this process of shoulder construction is not economical due to high material transportation cost and can also induce the environmental issues by disposal of in-situ soils. It can also cause distresses such as surface rutting and edge drop-off in soft shoulder section due to bearing capacity failure and off-tracking of vehicle. The heavy rainfall in Sri Lanka can induce severe erosion problem when using the soft shoulder. To improve the strength and durability of pavement shoulders in the iRoad Project, the soil stabilization will be a good alternative to solve the above mentioned problems. The use of in-situ soils with addition of soil stabilizer enables to reduce the construction cost of shoulder section and mitigate the environment issues. The objective of this task is to review the application of soil stabilization method for soft shoulder construction in the iRoad Project. Firstly, the quantitative analysis of soil strength improvement due to soil stabilization was done for soil samples collected from iRoad construction sites. Two types of soils were selected from iRoad Project sites and prepared for soil stabilization testing by the Road Development Authority. Secondly, the appropriate stabilizer was selected at given soil type based on test results. Three different stabilizers, ST-1, ST-2, and ST-3, produced in Korea were used for estimating soil strength improvements. Finally, the optimum stabilizer content was determined for improving shoulder performance. The uniaxial compressive strength (UCS) test was conducted to evaluate the strength of stabilized soil samples in accordance with ASTM D 1633. The use of bottom ash as a stabilizer produced from power plant in Sri Lanka was also reviewed in this task.

Hard Shoulder Running is one of the ways to handle road traffic by installing LCS signals on the shoulder, making use of the shoulder as a straight road during rush hour, and thereby increasing the capacity of the road in the short term. Since it was first introduced on Youngdong Expressway in September 2007, it has been implemented in 32 highway sections and has improved the speed of traffic. The Seoul-Chuncheon Expressway, which is a private investment toll road, introduced Hard Shoulder Running only for some sections in 2014, but due to the geomorphic characteristics of the lanes with many tunnel sections, It did not perform the function of Hard Shoulder Running. Therefore, they are considering expanding the tunnel section of Hard Shoulder Running, and it is necessary to examine the safety of the tunnel part to implement this. The purpose of this study is to investigate the effect of the change of geometry, such as reduction of lane width in tunnels, and the installation of various guiding facilities, on the safety of tunnels by applying a driving simulator. Virtual reality data were constructed at 3.25m and 3.0m respectively in the tunnel section of the Seongjong Tunnel of Seoul-Chuncheon Expressway and Geumnam Tunnel to determine the feasibility of Hard Shoulder Running and appropriate speed and lane width. EEG was used for analysis. The test subjects were 15 young drivers / 15 old drivers. Analysis items were classified into four categories: driving speed, steering wheel operation, lateral placement of vehicle, and driver's psychological factors. As a result of analysis of the experimental results, it was confirmed that the allowance width of outer wall and m of tunnels was secured, and the lane width of 3.0 m was stable in terms of driver driving behavior, and operation speed was lower than 50 km/h.

Background: Sitting posture influences movements of scapulothoracic and glenohumeral joints and changes the shoulder muscle activities. The development and maintenance of correct sitting posture is important for the fundamental treatment of shoulder pain during rehabilitation. Objects: The purpose of this study was to investigate the effects of the sitting postures and the shoulder movements on shoulder muscle activities for both male and female. Methods: Twenty-eight subjects without shoulder-related diseases participated in this experiment. The subjects had randomly adopted three different sitting postures (upright posture, preferred posture, maximum slouched posture) and shoulder flexion angles in scapular plane (30˚, 90˚, 120˚). Surface electrodes were collected from upper trapezius (UT), anterior deltoid (AD), and posterior deltoid (PD) and the active shoulder range of motion was measured in each sitting posture and shoulder flexion angle. Results: The active range of motions of the shoulder external rotation and the flexion in the scapular plane decreased from the upright posture to the maximum slouched posture (p<.05, mixed-effect linear regression with random intercept, Tukey post-hoc analysis). All muscles showed the highest EMG activities at 120˚ shoulder flexion with the maximum slouched posture and did not show the gender differences. Conclusion: Increased shoulder muscle activities may become the potential risk factor for the shoulder impairment and pain if people continuously maintain the maximum slouched posture. Therefore, an upright position is necessary during shoulder exercises, as well as in activities of daily living, including motions involving lifting the arms.

The purpose of this study was to identify the effects of shoulder stabilization exercises and stretching on shoulder height and muscle activity in young adults with rounded shoulder posture. The subjects were young adults with rounded shoulder posture, and divided into two groups: shoulder stabilization exercises and stretching exercises. The shoulder stabilization exercise group performed Thera band exercises and stretching exercises. The stretching exercise group performed stretching exercises. The groups performed the exercises for 40min, three times a week for four weeks. Shoulder height was measure by Vanier calipers and muscle activity was measured by electromyography. Shoulder height was significantly decreased in both groups after intervention, however there was no difference between groups. There was a significant difference in muscle activity between the two groups after the intervention, however there was no difference between the groups. This study suggests that shoulder stabilization exercise and stretching affect shoulder height and muscle activity.

Background: Methods for exercising serratus anterior (SA) and upper trapezius (UT) muscles are important for the recovery of patients with various shoulder disorders, yet the efficacy of closed or open kinetic chain exercises have not yet been evaluated. Objects: The purpose of this study was to compare the activation of the SA and UT muscles during scapular protraction considering both closed and open kinetic chain exercises. Methods: Thirty subjects were randomly divided into experimental groups (closed kinetic chain exercise) and control groups (open kinetic chain exercise) in which scapular protraction was performed at 90° or 125° shoulder flexion. Electromyographic activity data were collected from the SA and UT muscles per position and exercise method. Results: Separate mixed 2-way analysis of variance showed significant differences in the activation of the SA (F1,28=6.447, p=.017) and the UT (F1,28=35.450, p=.001) muscles between the groups at 90° and 125° shoulder flexion. Also, the SA/UT ratio measures at 90° and 125° shoulder flexion significantly differed between the groups (F1,28=15.457, p=.001). That is, the closed chain exercise was more effective than open chain exercise for strengthening the SA muscle and controlling the UT muscle, 125° of shoulder joint was more effective than 90°. Conclusion: The findings suggest that scapular protraction with shoulder 125° flexion at the closed kinetic chain exercise may be more effective in increasing SA muscle activation and decreasing UT muscle activation as well as increasing the SA/UT ratio than open kinetic chain exercise.

The aim of the present study was to investigate the effects of unilateral shoulder bag weight and heel heights on trunk muscle activities. This study recruited 40 healthy women in their twenties. Electromyography activities of rectus abdominis muscle, external abdominal oblique muscle, internal abdominal oblique muscle and erector spinae muscle during standing position were measured using a surface electromyography system. The overall muscle activities of the trunk muscles were significantly increased in the contralateral trunk muscles (p<.05). Inaddition, the trunk muscles overall activities on the contralateral side without the shoulder bag was positive correlation with the heel height and unilateral shoulder bag weight (p<.05). Both high-heeled wear and unilateral shoulder bags are fashion items that cause asymmetry in the trunk muscles of women in their twenties. These findings suggest that the increase in the weight and heel height of the unilateral shoulder bag in women cause asymmetry of trunk muscle activities.

Background: Scapular posterior tilt (SPT) is important in the prevention of abnormal scapular movement and pain during elevation of the arm. However, previous studies have overlooked increased upper trapezius (UT) muscle activity interrupting the normal force couple of scapular motion and compensation of levator scapulae (LS) muscle activated simultaneously with UT during SPT exercise. Objects: The purpose of this study was to compare the effects of modified SPT with depression exercise versus SPT exercise on serratus anterior (SA), lower trapezius (LT), UT, and LS muscle activities and the clavicular tilt angle, in subjects with rounded shoulder posture (RSP) and myofascial pain in the UT muscle region. Methods: Eighteen subjects with RSP were recruited and randomly allocated to 2 groups; 9 in the SPT group and 9 in the SPT with depression group. All subjects met the specific RSP criteria and had myofascial pain of UT region. Depending on the allocated group, subjects performed the assigned SPT exercise and EMG data were recorded during the each exercise. Clavicular tilt angle was defined as the angle between the line joining the medial and lateral end of the clavicle and a horizontal line. Results: The SA muscle activity was significantly greater in SPT with depression than with SPT exercise (p<.05). The UT, LS muscle activity and the clavicular tilt angle was significantly lower in SPT with depression than with SPT exercise (p<.05). Conclusion: These findings were insightful because the potential risk of pain from overactivation of the UT and LS was considered, in contrast with SPT exercise. SPT with depression exercise can be implemented as an effective method to facilitate scapular muscle activity for stability and to prevent myofascial pain in the neck and shoulder.

Background:Rounded shoulder posture (RSP), a postural abnormality, might cause shoulder pain and pathologic conditions. Although most previous research has investigated RSP focusing on the proximal structures of the shoulder, such as the scapula and pectoralis muscles, the relationship between RSP and anterior distal structures of the upper extremity, such as the biceps brachii muscle and elbow joint, is not clearly understood.Objects:This study aimed to investigate the correlations between RSP and the biceps brachii length, elbow joint angle (EJA), pectoralis minor length, general pectoralis major length, humeral head anterior translation (HHAT), glenohumeral internal rotation (IR), external rotation (ER), and horizontal adduction (HAD).Methods:Twelve subjects with RSP (6 male, 6 female) were recruited. All subjects fulfilled the RSP criteria indicated by a distance ≥2.5 cm from the posterior aspect of the acromion to the table in the supine position. The examiner measured each of the following parameters twice: RSP, biceps brachii length, EJA, pectoralis minor length, pectoralis major length, HHAT, glenohumeral IR, ER, and HAD. Pearson’s correlation coefficient(r) was used to assess the correlation between RSP and all the variables. Results:There was a significant moderate positive correlation between RSP and biceps brachii length (r=.55, p=.032), moderate negative correlation between RSP and pectoralis minor length (r=-.62, p=.015), and moderate positive correlation between RSP and HHAT (r=.53, p=.038).Conclusion:The biceps brachii length, pectoralis minor length, and HHAT could be used to evaluate patients with RSP. Better understanding of the correlation between these factors and RSP could help in the development of effective methods to treat patients with this condition in clinical management.

Background:Individuals with spinal cord injury (SCI) rely on their upper limbs for body-lifting activity (BLA). While studies have examined the electromyography (EMG) and kinematics of the shoulder joints during BLA, no studies have considered foot position during BLA.Objects:This study compared the effects of different foot positions during BLA on the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion in individuals with SCI.Methods:The study enrolled 13 mens with motor-complete paraplegic SCI, ASIA (American Spinal Injury Association) A or B. All subjects performed BLA with the feet positioned on the wheelchair footrest and on the floor independently. Surface EMG was used to collect data from the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii. The peak plantar pressure was measured using pedar-X and the knee flexion angle with Image J. Borg’s rating perceived exertion scale was used to measure the physical activity intensity level. The paired t-test was used to compare the shoulder muscle activities, peak plantar pressure, knee flexion angle, and rating perceived exertion between the two feet positions during BLA.Results:The activity of the latissimus dorsi, pectoralis major, serratus anterior, and triceps brachii and rating perceived exertion decreased significantly and the peak plantar pressure and knee flexion angle increased significantly when performing BLA with the feet positioned on the wheelchair footrest compared with on the floor (p<.05).Conclusion:These findings suggest that individuals with SCI may perform BLA with the feet positioned on the wheelchair footrest for weight-relief lifting to decrease the shoulder muscle activities and the rating perceived exertion and to increase the peak plantar pressure and the knee flexion angle.

This study used both kinesiotaping and extracorporeal shock wave therapy on patients diagnosed with frozen shoulder - a common musculoskeletal disorder in adults - in order to observe the effects on the joint range of motion. 21 adult(male 12, female 9) were selected and distributed into randomized groups. One group received kinesiotaping (n=10) and the other group received kinesiotaping together with extracorporeal shockwave therapy (n=11). After a 6 week duration of receiving kinesiotaping and extracorporeal shockwave therapy, changes in the joint range of motion in the patients were observed. Post-treatment of frozen shoulder, the changes in abduction within the shoulder joint were as follows: in both groups there was a noticeable increase in the joint range of motion (p<.05). Post-treatment of frozen shoulder, the changes in external rotation within the shoulder joint were as follows: both groups showed a significant increase in the joint range of motion (p<.05). The result of suggest that, it can be inferred that both the extracorporeal shockwave therapy and kinesiotaping are effective in increasing the joint range of motion in patients with frozen shoulder.

Background: Patients after rotator cuff (RC) surgery experienced pain, weakness and limited of motion of the shoulder. Physical therapists have used heat therapy, electrotherapy, range of motion (ROM) exercise and other methods to treat patients after RC surgery. In addition, functional taping is also used to support joint movement and to increase shoulder joint stability. Objects: The purpose of this study was to determine the initial effects of functional taping using non-elastic tape on pain, strength and ROM of the shoulder following RC surgery. Methods: Forty-eight patients with who underwent RC surgery volunteered for this study. The subjects were randomly divided into an experimental group (EG, n1=25) and a control group (CG, n2=23). First, non-allergic tape was applied to the shoulder to prevent skin irritation. The EG applied functional taping using non-elastic tape and the CG applied sham taping using elastic tape. Assessment tools included the shoulder pain and disability index for functional activity score, visual analog scale for level of pain, shoulder muscle strength, hand grip strength and ROM testing. Results: Pain score in the both group significantly decreased (p<.05), and change in pain score of in the EG increased significantly than in the CG (p<.05). Shoulder strength and ROM in the both group significantly increased (p<.05). Especially external rotation and extension of the shoulder ROM in the EG increased significantly more than in the CG (p<.05), but the rate of change in the two groups showed no significant difference. Conclusion: These results suggest that functional taping using non-elastic tape was initially effective in decreasing pain score level in patients with RC surgery.

Background: The functioning of the serratus anterior (SA) muscle is essential to normal scapulohumeral rhythm during forward flexion (FF) of the shoulder. Also, SA weakness and overuse of the upper trapezius (UT) is observed in patients with shoulder dysfunction and trapezius myalgia. We designed a combination exercise involving FF and scapular protraction with resistance (CFFSP) to activate the SA muscle and to deactivate the UT muscle. Objects: The purpose of this study was to determine whether or not CFFSP would be more effective in activating the SA muscle than FF alone and FF with scapular protraction (FFP). Methods: Nineteen subjects (12 men and 7 women) participated in this study and performed FF, FFP, and CFFSP at 120°. Surface electromyography was applied to the SA, UT, and pectoralis major (PM) muscles, as was one-way analysis of variance (ANOVA) with repeated measures. Statistical significance was set at .05. Bonferroni adjustment was used to counteract the problem of multiple comparisons, with a statistical level of significance of .017 (.05/3). Results: A statistically significant difference was found in relation to the three positions for the SA muscle (p<.001) and the SA/UT ratio (p=.005) using ANOVA. Significantly different results, depending on the position, were also demonstrated using the Bonferroni post-hoc test for the SA muscle (FF=28.27±16.20, FFP=45.66±15.81, and CFFSP=62.4±27.21) and for the SA/UT ratio (FF=3.04±2.14, FFP=3.61±2.38, and CFFSP=5.95±3.01). Significant differences between the three positions was not found regarding the average amplitude of SA/PM muscle ratio (SA/PM: p=.060). Conclusion: We recommend the use of CFFSP to strengthen the SA muscle at 120°.

Background: Shoulder stabilization commonly involves two components: the glenohumeral stabilization exercise (GSE) and scapular stabilization exercise (SSE). Despite the fact that the shoulder stabilization has advantageous merit, to our knowledge, only a few studies have compared the superior of the GSE and the SSE. Objects: The purpose of this study was to assess the effects of GSE in patients with nonspecific shoulder pain. Methods: Thirty subjects with nonspecific shoulder pain were randomly divided into an experimental group and control group, each with 15 patients. The experimental group used an GSE, whereas the control group did SSE. All subjects were measured in shoulder stability, scapular symmetric alignment, pain, muscle power, and range of motion before and after the intervention. Results: GSE resulted in significantly better shoulder stability (p=.046, from 8.67±7.54 score to 13.93±9.40) in the experimental group compared with SSE in the control group. However, no significant effects were observed for scapular symmetric alignment including the angles of inferior scapular distance (p=.829) and inferior scapular height difference (p=.735), pain (p=.113), muscle power including shoulder flexion (p=.723) and abduction (p=.897) and range of motion including shoulder flexion (p=.853) and abduction (p=.472). Conclusion: These findings suggest that GSE may be more effective in increasing the shoulder stability than the SSE in patients with nonspecific shoulder pain, probably through a centralization effect on the shoulder mechanism.

In women’s clothing the shoulders are distorted and exaggerated. Consequently, the wearers’ bodies are transformed and interpreted according to modern standards. We therefore refer to this aspect of women’s clothing as the power shoulder. This study aims to analyze the differences in sensibility images on variations in the shoulder shape of power shoulder jackets. Nine samples were examined that involved combinations of three variations of the shoulder height and three variations of the shoulder width. The data was evaluated by 123 fashion design majors. Four factors were selected: attractiveness, personality, activity, and self-respect. Among these factors, attractiveness is the most important. The study results indicated that changes in shoulder height acted as a major effect that had an independent influence on all four factors. In contrast, changes in shoulder width acted as a major effect that had an independent influence on attractiveness, personality, and activity but not on self-respect. It also had an influence on the interaction effect of attractiveness, personality, activity, and self-respect. We anticipate that this study will help individual customers select clothing that is suitable for their preference and body shape because it is now possible for them to estimate images of power shoulder jackets.

PURPOSES : Visibility of lanes on the road improves as retro-reflectivity rises. This helps reduce traffic accidents at nighttime or in bad weather. This study aimed to evaluate the effects of improved visibility on reduction of traffic accidents when the color of the lane in the median of the highway was changed from yellow to white. METHODS: In order to evaluate the accident reduction effect of the white left shoulder line, Hauer (1997)'s Comparison-Group method was chosen as an analysis methodology. The JungBu-2 highway, which is equipped with the white left shoulder line and is in trial operation, was chosen as a target for analysis. Data of accidents for 10 months before and after installation was collected and analyzed. In addition to the number of accidents, the types of accidents were classified into nighttime accidents, accidents in bad weather, and median collisions. The accident reduction effect of improved visibility of lanes was analyzed. RESULTS : The analysis’finding showed that installation of the white left shoulder line decreased the number of accidents by 28%. Moreover, improvement in visibility was effective in reducing nighttime accidents by 67.63%, accidents in bad weather by 55.19%, and median collisions by 48.55%. CONCLUSIONS: Based on the analysis’results in this study, it was concluded that white left shoulder line on the freeway contributed to reduce traffic accidents, especially nighttime accidents, accidents in bad weather, and median collisions.

Background: Shoulder horizontal adduction (HA) is performed in many activities of daily living. The limited range of motion (LROM) of HA is affected by the tightness of the posterior deltoid, infraspinatus, teres major, and posterior capsule of glenohumeral joint. The LROM of shoulder HA contributes to excessive scapular abduction. Objects: The aim of this study is to compare the scapular abduction distance and three-dimensional displacement of the scapula during shoulder horizontal adduction between subjects with and without the LROM of shoulder HA. Methods: 24 subjects (12 people in LROM group and 12 people in normal ROM group) participated. Subjects with less than 115° of HA ROM were included in LROM group. Shoulder HA was performed 3 times for measuring scapular abduction distance and three-dimensional displacement of the scapula. Tape measure was used for measuring scapular abduction distance. Scapular abduction distance was normalized by dividing the scapular size. Polhemus Liberty was used for measuring the three-dimensional displacement of the scapula. Results: Normalized scapular abduction distance was significantly greater in LROM group than normal ROM group (p<.001). Three-dimensional displacement of the scapula during shoulder HA was greater in LROM group than normal ROM group (p<.05). Conclusion: LROM group had a greater scapular abduction and three-dimensional displacement of the scapula during shoulder HA compared to normal ROM group.

Background: Strengthening the supraspinatus is an important aspect of a rehabilitation program for subacromial impingement and tendinopathy. Many authors recommended empty-can (EC), full-can (FC), and prone full-can (PFC) exercises to strengthen the supraspinatus. However, no ultrasonography study has yet investigated supraspinatus muscle architecture (muscle thickness; MT, pennation angle; PA, fiber bundle length; FBL) in relation to supraspinatus strengthening exercises. Objects: The purpose of this study was to compare the architecture (MT, PA, and FBL) of the supraspinatus muscle during three different types of exercises (EC, FC, and PFC) using diagnostic ultrasound. Methods: Participants performed three different exercises: (A) EC; the arm was maintained at 60° abduction with full internal rotation in the sitting position, (B) FC; the arm was maintained at 60° abduction with full external rotation in the sitting position, and (C) PFC; the arm was maintained at 60° abduction with full external rotation in the prone position. Ultrasonography was used to measure the MT, PA and FBL of the supraspinatus. One-way repeated analysis of variance with Bonferroni’s post-hoc test was used to compare between the three exercises and the initial position of each exercise. Results: Compared with each initial position, the FC exercise showed the greatest mean difference in muscle architecture properties and the PFC exercise showed the least mean difference. Conclusion: The findings suggest that the FC exercise position may have an advantage in increasing the amount of contractile tissue or producing muscle power and the PFC exercise position may be useful in a rehabilitation program because it offers the advantage of maintaining the muscle architecture properties.

Background: For the treatment of forward head posture (FHP) and forward shoulder posture, methods for strengthening scapular retractors and deep cervical flexors and stretching pectoralis and upper cervical extensors are generally used. No study has yet assessed whether suboccipital release (SR) followed by cranio-cervical flexion exercise (CCFE) (SR-CCFE) will result in a positive change in the shoulders and neck, showing a “downstream” effect. Objects: The purpose of this study was to investigate the immediate effects of SR-CCFE on craniovertebral angle (CVA), shoulder abduction range of motion (ROM), shoulder pain, and muscle activities of upper trapezius (UT), lower trapezius (LT), and serratus anterior (SA) and LT/UT and SA/UT muscle activity ratios during maximal shoulder abduction in subjects with FHP. Methods: In total, 19 subjects (7 males, 12 females) with FHP were recruited. The subject performed the fifth phase of CCFE immediately after receiving SR. CVA, shoulder abduction ROM, shoulder pain, muscle activities of UT, LT, and SA, and LT/UT and SA/UT muscle activity ratios during maximal shoulder abduction were measured immediately after SR-CCFE. A paired t-test and Wilcoxon signed-rank test were used to determine the significance of differences in scores between pre- and post-intervention in the same group. Results: The CVA (p<.001) and shoulder abduction ROM (p<.001) were increased significantly postversus pre-intervention. Shoulder pain was decreased significantly (p<.001), and LT (p<.05) and SA (p<.05) muscle activities were increased significantly post- versus pre-intervention. The LT/UT muscle activity ratio was increased significantly post- versus pre-intervention (p<.05). However, there was no significant change in UT muscle activity and SA/UT muscle activity ratio between pre- and post-intervention (p˃.05).
