        The purpose of this study was to identify the effects of manual facilitation and a stick on lumbar and hip joint flexion angles in subject with lumbar flexion syndrome during forward bending from a sitting position. Fifteen subjects with lumbar flexion syndrome were recruited for this study. As a pretest, all subjects performed three repetitions of bending the trunk forward until the tips of their fingers touched the target bar. After this pretest, the subjects practiced the forward bending of the trunk 10 times, using either manual facilitation or a stick. Then, as a posttest, all subjects repeated the pretest procedure. The flexion angles of lumbar spine and hip joint during forward bending in a sitting position were measured using a three-dimensional motion analysis system. A paired t-test was used to determine the statistical differences between pre-test and post-test flexion angles and pre- and post-test flexion angle differences between forward bending with manual facilitation and forward bending with a stick. The level of statistical significance was set at p=.05. The results of the study showed that the angle of the lumbar flexion decreased significantly and the bilateral hip flexion angle increased significantly when performing forward bending with stick and manual facilitation. Furthermore, the angle of lumbar flexion decreased significantly and the angle of bilateral hip flexion increased significantly in forward bending with a stick compared to forward bending with manual facilitation. The findings of this study indicate that both forward bending with manual facilitation and sticks could be used to prevent excessive lumbar flexion and increase hip flexion, and that forward bending with a stick is more effective than forward bending with manual facilitation for inducing lumbar spine and hip joint angle changes.

        The purpose of this study was to investigate intra-rater reliability and determine the validity of electromyography (EMG) measurements to represent muscle activity and ultrasonography (US) to represent muscle thickness during manual muscle testing (MMT) to external abdominal oblique (EO) and lumbar multifidus (MF). Twenty healthy subjects were recruited for this study and asked to perform MMT at differing levels. The subjects' muscle activity using EMG was measured by a ratio to maximum voluntary contraction (MVC) and root mean square (RMS) methods. The subjects' muscle thickness using US was measured by raw muscle thickness and change ratio of thickness to maximum (MVC) or resting condition. In three trials, measurements were performed on each subject by one examiner. The intra-rater reliability of measurements of EMG and US to EO and MF was calculated using intra-class coefficients. The intra-rater reliability of all measurements was excellent (ICC=.75~.98) in EMG and US. The conduct validity was calculated by one-way ANOVA with repeated measurements to compare whether the EMG and US measurements were different between MMT at different levels. There was only a significant difference between all grades at %MVC thickness measurement of US. These results suggest that a %MVC thickness measurement of US was a more sensitive and discriminate in all manual muscle testing grades. This information will be useful for the selection of US measurement and analysis methods in clinics.

        The reliability of the thickness measurement of the lumbar multifidus (LMD using real-time ultrasonography (US) was determined in only the superficial fiber of the lumbar multifidus (SM). However, previous studies have not examined the reliability of the deep fiber of the LM (DM). The purpose of this study was to determine the intrarater and the interrater reliability of the thickness measurements of DM using US. Eleven heathy males participated in the study. The thickness of the DM was measured with an US in the prone position. Reliability was examined using intraclass correlation coefficients (ICC), standard error of the measurement (SEM), and the Bland and Altman plot. ICC(3,1) was used to calculate the interrater reliability of the thickness measurement of DM using the values from both the first and second test sessions. Additionally, ICC(3,1) was used to calculate the intrarater reliability of the measurements over two days using the measurements obtained in test session 1 and lest session 2. The results of this study were as follows: 1) the ICC(3,1) value for interrater reliability was .94 in the first test session, and .93 in the second test session. 2) the ICC(3,1) values for intrarater reliability of the measurements over two days was .90 in both the first examiner and the second examiner. The interrater reliability and interrater reliability of the DM measurements, obtained via the US protocol used in this research was excellent. Therefore, we conclude that the thickness measurement of the DM obtaioned from the US protocol used in this research would be useful for clinician assessment of the thickness of the DM.

        The aim of this study was to investigate the effects of lumbar stabilization on both trunk and lower limb muscle activity and center of pressure (COP) in single leg standing. Surface electromyography (EMG) was used to collect muscle activity data, the mean velocity of COP was measured using a force plate, and a pressure biofeedback unit was used for lumbar stabilization training. The findings of this study are summarized as follows: 1) The EMG activity of the erector spinae decreased significantly and the activity of the rectus abdominis, internal oblique, external oblique, gluteus maximus, and gluteus medius increased significantly with lumbar stabilization single leg standing. 2) No differences in activity in the tibialis anterior, medial gastrocnemius, rectus femoris, and medial hamstrings were found with single leg standing. 3) The mean velocity of COP in the antero-posterior and medio-lateral directions in the lumbar stabilization single leg standing decreased significantly compared with the preferred single leg standing. The findings of this study therefore indicate that lumbar stabilization can facilitate the co-activation of deep stabilization and global muscles that improve postural control capability during single leg standing.

        Low back pain is a common phenomenon among the golfers. In an attempt to understand low back pain, the kinematic changes and golf swing motion analysis has been performed to focus on lumbar spine in pro. golfers. According to the swing pattern, significant variations of the lumbar joint forces and loads has related with muscles activities so the motion analysis of lumbar spine were discussed. The purpose of this study was to analyze motion of lumbar spine and it was to compare joint force during golf swing in pro. golfers. The swing motion of the subjects was tracked using a 3D motion analysis system by Motion Analysis Ltd. and SIMM software. The angle changes of lumbar spine rapidly in vx direction during the top back swing and the finish and in vy direction during the follow through and in vz direction during the down swing and the impact(Subject A). The angle changes of lumbar spine rapidly in vx direction during the top back swing and in vy direction during the down swing, the impact and the follow through and in vz direction during the down swing(Subject B). In conclusion, subject A and B both show sudden angle changes between 1st-3rd lumbar spine and 4th-5th lumbar spine during the stage from address to top back swing which caused by over upper body twisting.

        In dancers, intact muscular coordination is a well balanced antagonist, which could be a decisive factor in protection against injury as dancers often have hypermobile joints and their ankle joints often bear their full body weight in extreme positions. The purposes of this study were to identify the isokinetic strength to the knee and ankle and the isometric strength of the trunk in female collegiate dancers and controls. Furthermore, the study aimed to investigate the peak torque ratio of knee extension to flexion, ankle plantarflexion (PF) to dorsiflexion (DF), and dominant legs to nondominant. Twenty-one female collegiate dancers (20.0 years of age) and twenty-one female collegiate students (19.3 years of age) performed isokinetic maximum efforts of the knee extensors and flexors at and , the ankle plantarflexors and dorsiflexors at and and isometric maximum efforts of the lumbar extensors at 0˚, 12˚, 24˚, 36˚, 48˚, 60˚, and 72˚. The results were as follows: The isokinetic peak torque of the knee extensors and the ratio of knee extensors to flexors of dancers were significantly higher than those of controls (p<.01). However, the isometric peak torque of the back extensors (p<.01) and isokinetic peak torque of the ankle plantarflexors and dorsiflexors (p<.05) of dancers were significantly lower than those of controls. Further studies are needed to identify the difference in proprioception of the joints between dancers and controls.

        An abdominal drawing-in maneuver (ADIM) with a pressure biofeedback unit can be used to prevent excessive lumbar lordosis during bridging exercise. Therefore, in this research, the effects of an ADIM on lumbar lordosis and lower extremity muscle activity during bridging exercise were investigated in thirty healthy adults. Surface electromyography (EMG) and VICON system were used to collect kinematic data and muscle activity, respectively. A paired t-test was used to determine a statistical significance. The results showed as follows: (1) When performing bridging exercise with an ADIM, the height of the anterior superior iliac spine and greater trochanter decreased significantly (p<.05). (2) When performing bridging exercise with an ADIM, the trunk extension angle and pelvic angle increased significantly (p<.05). (3) When performing bridging exercise with an ADIM, the EMG signal amplitude increased significantly in the rectus abdominis, internal oblique abdominis, external oblique abdominis, medial hamstring, and lateral hamstring (p<.05). (4) When performing bridging exercise with an ADIM, the EMG signal amplitude decreased significantly in the erector spinae (p<.05). From the result of this research, an ADIM trained with pressure biofeedback unit during bridging exercise is effective to prevent excessive contraction of erector spinae, to limit excessive motion of pelvis from sagittal plane and to increase muscle activity of abdominal muscles and hamstring muscle.

        To improve abdominal muscles strengthening, double straight leg lowering (DSLL) has been widely used in physical therapy, fitness program, and athletic program. The purpose of this study was to investigate the effects of the lumbar stabilization maneuver with a pressure biofeedback unit on the muscle activity of rectus abdominis (RA), external oblique (EO), and internal oblique (IO) during DSLL. Fourteen healthy young men were recruited from university population. The electromyography (EMG) activity was recorded from the RA, EO, and IO of both sides. The normalized EMG activity was compared using a paired t-test. The study showed that EMG activity in the RA, EO, and IO was significantly higher during DSLL with lumbar stabilization (DSLL-LS) compared to performed DSLL (p<.05). These results suggest that DSLL-LS is recommended as an effective method for strengthening exercise for the abdominal muscles.

        The purpose of this study was to compare the electromyography (EMG) activities of the lumbar extensor muscles during motion of trunk flexion-extension and compare range of motion (ROM) with a 3-dimensional motion analysis system of the lumbar region between subjects with chronic low back pain (CLBP) and healthy subjects during the trunk flexion-extension, trunk rotation and trunk lateral flexion cycle. Thirty CLBP subjects and thirty healthy subjects were included. We measured the root mean square (RMS) value of the lumbar extensor muscles from resting, standing, lumbar flexion and return position. The RMS ratio was normalized from maximal EMG activity of the lumbar extensor muscles during trunk motion. The results of this study showed that the RMS ratio of the lumbar extensor was significantly higher in CLBP subjects than healthy subjects during all of trunk motion (p<.05). The ratio of the highest RMS value during flexion and extension was higher in CLBP subjects than in healthy subjects (p<.05). The ROM of the lumbar region was significantly lower in CLBP subjects than healthy subjects during trunk flexion-extension, trunk rotation and lateral flexion cycle. The relationship between the RMS ratio for full lumbar flexion and the ROM of lumbar flexion was not correlated significantly. CLBP subjects have both decreased ROM of the lumbar region and higher muscle activities of the lumbar extensor muscle than healthy subjects.

        This study is aimed at investigating the influence of different quantitative knowledge of results on the measurement error during lumbar proprioceptive sensation training. Twenty-eight healthy adult men participated and subjects were randomly assigned into four different feedback groups(100% relative frequency with an angle feedback, 50% relative frequency with an angle feedback, 100% relative frequency with a length feedback, 50% relative frequency with a length feedback). An electrogoniometer was used to determine performance error in an angle, and the Schober test with measurement tape was used to determine performance error in a length. Each subject was asked to maintain an upright position with both eyes closed and both upper limbs stabilized on their pelvis. Lumbar vertebrae flexion was maintained at for three seconds. Different verbal knowledge of results was provided in four groups. After lumbar flexion was performed, knowledge of results was offered immediately. The resting period between the sessions per block was five seconds. Training consisted of 6 blocks, 10 sessions per one block, with a resting period of one minute. A resting period of five minutes was provided between 3 blocks and 4 blocks. A retention test was performed between 10 minutes and 24 hours later following the training block without providing knowledge of results. To determine the training effects, a two-way analysis of variance and a one-way analysis of variance were used with SPSS Ver. 10.0. A level of significance was set at .05. A significant block effect was shown for the acquisition phase (p<.05), and a significant feedback effect was shown in the immediate retention phase (p>.05). There was a significant feedback effect in the delayed retention phase (p<.05), and a significant block effect in the first acquisition phase and the last retention phase (p<.05). In conclusion, it is determined that a 50% relative frequency with a length feedback is the most efficient feedback among different feedback types.

        The purpose of this study was to compare the onset times of muscle activities and the order of muscle firing in hamstring gluteus maximus, and lumbar erector spinae muscle during active hip extension between subjects with low back pain (LBP) and healthy subjects. Thirty subjects, 15 with LBP and 15 healthy subjects, participated in this study. Electromyographic activity was recorded during active hip extension in prone and standing position. Relative onset times of these muscle activities were determined. Similar muscle firing order in hamstring, gluteus maximus, and lumbar erector spinae muscle showed in both groups and positions. However, the onset time of gluteus maximus was significantly later in prone and standing active hip extension in subjects with LBP than in healthy subjects. The onset time of lumber erector spinae muscle activity was significantly delayed in subjects with LBP in standing active hip extension, The delayed onset times of gluteus maximus and lumbar erector spinae muscles' activities were probably related to LBP. Further studies are needed to identify whether the delayed onset times of gluteus maximus and lumbar erector spinae muscle activities are the contributing factors to LBP.

        In children with cerebral palsy, bone density is decreased by disturbance of bone remodelling due to lack of normal weight bearing and muscle contraction through physical activity. Loss of bone density cause fracture, delays treatment with immobilization, and leads to functional limitation. The purpose of this study was to investigate bone mineral density of lumbar spine in children with spastic quadriplegia and diplegia. Six spastic quadriplegia and 14 spastic diplegia were evaluated in this study. QDR 4500 X-ray densitometer was used to measure bone density at lumbar spine (L1~L4). Children with cerebral palsy showed lower bone density than that of normal children. Bone density in children with spastic quadriplegia and diplegia was , , respectively. However, there was no significant differences in bone density between children with spastic quadriplegia and diplegia. There was no significant difference in bone density relation to motor development level, height, and weight. Further study is needed to find the appropriate interventions for preventing loss of bone density in children with cerebral palsy.

        In today's society, many women wear high-heeled shoes, but the effect of heel height on lumbar lordosis has not been clearly defined. The objective of this study was to identify the influence of heel height and general characteristics of subjects on lumbar lordosis. The subjects of this study were 40 healthy women who were students of the Department of Physical Therapy, College of Rehabilitation Science, Taegu University. Flexible ruler measurement was used to measure the lumbar lordosis at barefoot, 3 cm and 7 cm high-heeled standing positions. The results were as follows: 1) Significant statistical decrease in lumbar lordosis was observed as heel heights were increased from barefoot to 7 cm high heel. 2) There were no statistically significant differences between lumbar lordosis according to three different heel heights and weight, body mass index. 3) Lumbar lordosis measured at different heel heights was related to subject's height. With increasing subject's height, lumbar lordosis that measured from each heel height was significantly decreased. As heel heights were increased from barefoot to 7 cm high heel, significant statistical decrease in lumbar lordosis was observed in the subjects whose height were 151~160 cm. 4) Intrarater reliability on lumbar lordosis taken with a flexible ruler was good, with Cronbach values of 0.8971 for barefoot, 0.8107 for 3 cm and 0.9002 for 7 cm high-heeled standing positions.

        The purpose of this study was to assess the fatigue in lumbar and abdominal muscles in patients with chronic low back pain compared with normal subjects using spectral analysis with mean power frequency and median power frequency. The experimental group consisted of twenty subjects who had experienced chronic low back pain for over one year after the onset day. A control group consisted of twenty normal subjects with no history of low back pain. All subjects stood in an apparatus to perform sustained contraction in the lumbar and abdominal muscles for 30 seconds with 60% maximal voluntary isometric contraction (MVIC). The resulting electromyographic (EMG) recorded time serial data were transformed into frequency serial data by Fast Fourier Transformation (FFT). The results were as follows: 1) lumbar muscles measured, the frequency change ratio of both median power frequency and mean power frequency was significantly greater for experimental group compared with control group group (p<0.05). In measured two abdominal muscles (inferior rectus abdominis, obliquus externus abdominis) except superior rectus abdominis, the frequency change ratio of both median power frequency and mean power frequency was significantly greater for experimental group compared with control group (p<0.05). 2) In all three (longissimus thoracis, iliocostalis lumborum, multifidus) lumbar muscles measured, the initial frequency value of both median power frequency and mean power frequency was significantly lower for the experimental group compared with the control group (p<0.05). In the two (inferior rectus abdominis, obliquus externus abdominis) abdominal muscles measured (superior rectus abdominis not included), the initial frequency value of both median power frequency and mean power frequency was significantly lower for the experimental group compared with the control group (p<0.05). These results suggest that in patients with chronic low back pain there is a trend for more fatigue to occur in both lumbar and abdominal muscles than in the normal control group. This would seem to suggest that in treatment programs for patients with chronic low back pain, improvement of endurance in all trunk muscles should be considered.

        The purposes of this study were to investigate biomechanical variables of the lumbar spine for women who enjoy recreational exercises regularly, and to determine the factors that influence these variables. These variables were determined by the X-ray pictures of the lumbar area of 80 housewives who visited the department of rehabilitation at the Y Hospital from October 1997 to March 1998. The sacral inclination angle, the sacrohorizontal angle, the lumbosacral joint angle, and the lumbar lordotic angle were analysed. The t-test, correlation analysis, and multiple regression analysis were used to determine the significant differences and relationships among variables. The result were as follows: 1) There was a significant difference in the sacral inclination angle (p<0.01), the sacrohorizontal angle (p<0.05) and the lumbar lordotic angle (p<0.05) between the bilateral and the unilateral exercise group. 2) With the sacral inclination angle, the sacrohorizontal angle, the lumbosacral joint angle and the lumbar lordotic angle, correlation was found between the sacral inclination angle and the sacrohorizontal angle (p<0.01), the sacral inclination angle and the lumbosacral joint angle (p<0.05), the sacral inclination angle and the lumbar lordotic angle (p<0.05), and the sacrohorizontal angle and the lumbosacral joint angle (p<0.01). 3) In the bilateral exercise group, the sacral inclination angle correlated with age (p<0.01). The sacrohorizontal angle correlated with age (p<0.01) and exercise time (p<0.01). The lumbar lordotic angle correlated with age (p<0.05) and exercise duration (p<0.05). In the unilateral exercise group, the sacral inclination angle correlated with age (p<0.01), while the sacrohorizontal angle correlated with age (p<0.01) and exercise duration (p<0.05). The lumbar lordotic angle correlated with age (p<0.05).

        골다공증은 골절의 위험도가 증가하는 질환으로, 본 연구에서는 이중에너지 엑스선 흡수계측법(DXA)을 이용하여 허리뼈의 회전과 기울기에 따른 골밀도를 비교 분석하였다. 실험 결과 허리뼈의 회전에 따른 골밀도는 감소하지만 허리뼈의 기울기 변화에서는 그 결과는 예측할 수 없었다. 이러한 이유는 허리뼈의 기울기 변화가 허리뼈 1번과 4번사이 일부분에서 겹침으로 인하여 뼈의 전체 면적과 골밀도가 변화한 결과로 판단되어진다. 즉, 방사선사는 정확한 골밀도 결과 값을 얻기 위하여 허리뼈의 회전방지와 기울기에 따른 겹침을 방지하기 위한 노력이 필요할 것으로 판단되어진다.
