        Chronic obstructive pulmonary disease (COPD) is associated with multiple comorbidities, including depression, which carries a higher risk of exacerbation and hospitalization in patients with stable COPD. A newly developed questionnaire, the COPD Assessment Test (CAT), was developed as an alternative to other complex, time-consuming tools for quantifying the symptom burden of COPD in routine practice. It is possible that the correlation between the CAT and depression scales could be useful for early evaluation and management of depression in COPD patients. Thus, we investigated the relationship between the CAT and depression as measured by the Patient Health Questionnaires-9 (PHQ-9). We performed a retrospective observational COPD cohort study. A total of 97 patients were enrolled. The Korean versions of the CAT and PHQ-9 were completed for stable patients. A correlation analysis was performed between the PHQ-9 and CAT scores. Significant depression among the groups based on the 2011 GOLD guidelines occurred only in class Gold B and D patients (40% and 60%, respectively). The frequency of depression was significantly higher in the group with higher CAT scores (20~29 versus ≥30; odds ratio: 5.67 versus 22.66). Significant association was observed between the PHQ-9 and CAT scores (r=0.545 and P<0.001). As a result, the PHQ-9 score was significantly higher in COPD patients with a higher CAT score. The CAT is a simple and valuable predictor of depression in COPD patients, and it should be frequently used to detect COPD patients with depression in clinical practice.

        The purpose of this study was to compare the effects of underwater treadmill gait training (UTGT) and overground treadmill gait training (OTGT) on the gait, balance ability and pulmonary function of stroke patients. Twenty subjects were recruited for this study. The subjects were randomly assigned to two groups: UTGT (n1=10) and OTGT (n2=10). The 10 m walk test (10 MWT), Berg Balance Scale, Timed Up and Go (TUG) test, center of pressure, pulmonary function of forced vital capacity (FVC), forced expiratory volume after 1 sec (FEV1) and FEV1/FVC were measured before and after 4 weeks of training. Both groups undertook the gait training for 30 min a day, 3 times a week, for 4 weeks, and rating of perceived exertion of the groups were measured and compared. All the studied variables were significantly improved in both groups (p<.05) at the end of the study, except in the FEV1 of OTGT (p>.05). There was significant between-group difference in all of the variables, except in the 10 MWT (p>.05). These findings suggest that UTGT is more effective than OTGT in improving the balance and pulmonary functions of stroke patients.

        The purpose of this study was to parallel circuit training and circuit training with sonic systemic mechanism was to compare the differences in pulmonary function and chest expansion in adult men. This study was performed on 20 subjects. 20 subjects were divided into two groups; Circuit training group(n=10), Circuit training with sonic systemic mechanism(n=10). Both of the group performed the exercise 3 times a week for 5 weeks. The data was analyzed by the Repeated t-test for comparing before, during and after changes of factors in each group and the Independent t-test for comparing the between groups. The result are as follows. Circuit training group was statistically significant difference FVC, FEV1/FVC(p<.05), Circuit training with sonic systemic mechanism group was statistically significant difference PEF, VC in pulmonary function(p<.05). Circuit training group was statistically significant difference FEV1/FVC of between the two group in pulmonary function(p<.05). Circuit training group and circuit training with sonic systemic mechanism group was statistically significant difference in chest expansion(p<0.05) and there was no statistically significant difference of between the two group in chest expansion(p>.05).

        The purpose of this study compared the ability of feedback breathing training (FBT) and balloon blowing training to enhance the breathing of elderly people. The subjects were randomly and evenly divided into a feedback breathing training group (FBTG) and a balloon blowing training group (BBTG). Each group trained 3 times a week for 4 weeks, with the training suspended during the last 2 weeks. Pulmonary function measurements were obtained before the test and 2, 4 and 6 weeks after the test: forced vital capacity (FVC), forced expiratory volume at one second (FEV1), FEV1/FVC, peak expiratory flow (PEF) and vital capacity (VC). A repeatedmeasures ANOVA was conducted for the significance test. The FBT resulted in a significant increase in the FVC, FEV1/FVC, PEF, and VC of the elderly smokers after 4 weeks and a significant decrease in the FVC, FEV1/FVC, and PEF after 6 weeks. The BBT resulted in a significant increase in the FVC, FEV1, FEV1/FVC, PEF, and VC of the elderly smokers after 4 weeks and a significant decrease in the FVC, FEV1/FVC, and PEF after 6 weeks. In conclusion, An at home breathing rehabilitation program, in addition to balloon blowing, could increase the breathing performance of elderly people.

        This research was conducted to investigate the effect of continuous antagonistic muscle strengthening exercise and Evjenth-Hamberg stretching on the pulmonary function of university students with abnormal transformation of forward head posture(FHP). The results of study subject to the continuous antagonistic muscle strengthening(CAS) group(n=10) and Evjenth- Hamberg stretching(EHS) group(n=10) that was conducted 3 times a week for 6 weeks are as follow. FVC, IVC, and MVV all were shown to be significant in the pre·post comparison between the CAS group and EHS group(p<.05), and significant difference was shown for MW between the two groups(p<.05) in which the CAS group showed better effect. Based on the results above, it is considered that combining continuous antagonistic muscle strengthening exercise has better effect on pulmonary function compared to application of only Evjenth-Hamberg stretching.

        This study is to examine the effects of a feedback breathing device exercise and diaphragm breathing exercise on pulmonary functions of chronic strokes patients. The selection of 20 subjects was divided equally and placed into a experiment group and a control group and the intervention was applied four times per a week for five weeks. In each session, both groups received rehabilitative exercise treatment for 30 minutes, and a feedback breathing device exercise for 15 minutes. In addition, experimental group conducted a combination of diaphragm breathing exercise for 15 minutes. Prior to and after the experiment, patients’ pulmonary functions were measured using a spirometer. The pulmonary function tests included FVC, FEV1, FEV1/FVC, PEF, VC, TV, IC, ERV, IRV. With respect to changes in the pulmonary functions of both groups, the experimental group significantly differed in FVC, FEV1, TV, ERV but did not in PEF, FEV1/FVC, VC, IRV. The control group did not significantly differ in any of the tests. There were significant differences in FEV1, FEV1/FVC, TV, ERV between the two groups, but no significant differences in FVC, PEF, FEV1/FVC, VC, IRV between them after the experiment. The experimental group, which conducted a combination of a feedback breathing device exercise and diaphragm breathing exercise, saw their respiratory ability increase more significantly than the control group. The breathing exercise was found to improve pulmonary function in chronic stroke patients.

        A 40-year-old male was admitted with dry cough of two months’ duration. Radiologic examination revealed an endobronchial mass obstructing the right middle lobar bronchus and poststenotic pneumonia. Despite failure in bronchoscopic diagnosis, due to suspected malignancy and difficulty for bronchoscopic resection, we performed a right middle lobectomy. The histopathological diagnosis was a lipomatous hamartoma, which was exophytic and endobronchial. We report on a rare surgical case of endobronchial lipomatous hamartoma which had occlusive and exophytic growth across the bronchial wall.

        Pulmonary fibrosis (PF) is defined as a gradual, interstitial fibrous disease of the lung parenchyma. PF causes collapse of the lung lobe, which can lead to lung lobe torsion of the other side. A 14-year-old male Pekinese dog was referred to the Veterinary Medical Center, Chungbuk National University. The chief complaint was acute dyspnea. The dog had a history of chronic cough, which lasted for 18 months, and the cough had recently deteriorated. Tachypnea was observed on physical examination. As a result of thoracic radiographs, ultrasonography, and computed tomography, lung lobe torsion and collapse were diagnosed. A postmortem examination revealed lung lobe torsion of the left cranial lobe and carnification of the right cranial lobe. Histologically, severe and diffuse interstitial fibrosis with distortion of alveolar architecture and severe congestion and/or hemorrhage were observed in the right cranial lobe and left cranial lobe, respectively. Although the cause of pulmonary fibrosis was undetermined, this case showed a typical lung lobe torsion caused by pulmonary fibrosis.

        Inonotus obliquus is a traditional medicine mushroom that was developed from traditional medicine originating in ancient. It has been applied for cancer or immunotherapy,but its effect on pulmonary tuberculosis is not reported. Therefore, we measured the pulmonary tuberculosis therapeutic effect of methyl alcohol extract from MGIT 960 system with fluorescent indicator. Inonotus obliquus extract showed 14 day more inhibitory activity than the positive control. In addition, the anti-pulmonary tuberculosis activity of Inonotus obliquus was 50㎛. These results suggest that Inonotus obliquus methyl alcohol extracts could contribute to inhibition of pulmonary tuberculosis.

        The aim of this study was to investigate the effects of respiratory muscle training (RMT) with abdominal drawing-in maneuver (ADIM) on pulmonary function. Twenty-two subjects with restrictive breathing participated in this study. All the subjects were randomly assigned to three groups (7 subjects in RMT group, 7 subjects in RMT with ADIM group, 8 subjects in control group). The first group performed the RMT by using incentive respiratory spirometer (IRS). The second group performed the RMT by using IRS and the ADIM by using a Stabilizer. The exercises were conducted over four days. The pulmonary function was evaluated using the spirometer to measure the force exploratory volume in 1 second () and forced vital capacity (FVC). Measurements were conducted on the first day and the last day. A paired-t test was used for pre-post changes and the change rates in FVC and among each group were investigated by a one-way ANOVA. The findings of the the study were as follows: 1) There were significant differences of FVC and between pre and post in the two training groups (p<.05) 2) There was no significant difference of the change ratio the FVC and between the RMT group and RMT with ADIM group. Therefore, it is concluded that respiratory muscle and ADIM training, combined with two methods of treatment would suggest positive evidence for improving pulmonary function.

        The purpose of this study was an determine whether elastic band on treadmill training might effect the chest expansion and pulmonary function of the 20's men. 40 subjects with experimental group(male: 20) and control group(male: 20) was participated in experiment. During four weeks, each group participated thirty minutes for three times per week. Subjects were assessed using pre-value and postvalue measurement chest length(chest length for resting, chest expansion) and pulmonary function(forced vital capacity, forced expiratory volume at one second, FEV1/FVC, peak expiratory flow, vital capacity, tidal volume, expiratory reserve volume, inspiratory reserve volume) by the CardioTouch 3000S(BIONET, USA). These findings suggest that experimental group can be used to improve chest expansion, pulmonary function than control group. In comparison of both groups, post-test was more improved in experimental group. In conclusion, experimental group helped improving function of pulmonary volume and respiratory muscle, and thus it indicates that the functions will be more improved through the continued respiratory exercise program.

        This study aimed to compare 2 protocols recommended to patients with chronic cervical cord injury: each protocol included breathing exercises (inhalation-oriented or exhalation-oriented) and facilitation maneuver for the accessory respiratory muscles. Seventeen patients with chronic cervical cord injury volunteered to participate in this study, and we randomized these patients into 2 groups: the inhalation-oriented breathing exercise group (IOBEG) and exhalation-oriented breathing exercise group (EOBEG), consisting of 8 and 9 patients, respectively. Patients in the IOBEG performed inspiratory exercises using intermittent positive pressure breathing devices, while those in the EOBEG performed expiratory exercises using incentive spirometry. All exercises were performed by the subjects twice a day for 4 weeks, with each session lasting an average of 20 min. The outcomes were assessed on the basis of the pre- and post-treatment values of vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and FEV1/FVC. In the IOBEG, no significant differences were observed between the pre- and post-treatment values of any of the measured variables (p>.05); however, in the EOBEG, significant improvement was noted in the VC, FVC, FEV1 measured (p<.05) after the treatment. In addition, the rates of change in the values of VC, FVC, and FEV1 differed significantly between the 2 groups (p<.05). These findings suggest that the EOBEs can enhance respiratory function and are clinically feasible in patients with chronic cervical cord injury. Further studies will be undertaken to evaluate the clinical application of these findings.

        The aim of this study was to investigate the effect of abdominal drawing-in maneuver (ADIM) on peak exploratory flow (PEF), forced exploratory volume in 1 second (FEV1), and low back pain during forced expiration. Twenty-two subjects (14 subjects in experimental group, 8 subjects in control group) participated in this study. The stabilizer was used for ADIM training for five consecutive days. Vitalograph PEF/FEV1 DIARY and visual analogue scale (VAS) were used to determine forced expiratory pulmonary function and low back pain, respectively. Independent t-test and analysis of covariance were used for statistical analysis with a significance level of .05. The findings of this study were as follows: 1) There were no significant differences of ADIM effect on PEF and FEV1 between experimental group and control group. 2) There was a significant pain reduction in experimental group with ADIM. 3) PEF and FEV1 increased significantly in the fifth day compared with the first day pre-exercise baseline. Therefore, it is concluded that ADIM was effective in improving PEF and FEV1, and reducing VAS during forced expiration in patients with chronic low back pain.

        The purpose of this study was to identify the differences in pulmonary functioning after respiratory exercise with IPPB (Intermittent Positive Pressure Breather) in patients with progressive muscular dystrophy (PMD). The subjects were 46 patients with PMD who were admitted to the Rehabilitation Medicine Department of Youngdong Severance Hospital. The subjects were assigned into one of 2 groups. The control group received comprehensive treatments such as ROM exercise, deep breathing exercise, moist hot packs, and ultrasound twice a day while admitted at the Rehabilitation Medicine Department. Unlike the control group, the subjects at the experimental group received respiratory exercise treatment with IPPB. The subjects were admitted for 10~19 days, and the average length of hospital stay was 12.2 days. Pulmonary functioning was evaluated at admission and discharge by SENSOR MEDICS. The data were analyzed by a paired t-test and a independent t-test. The results were as follows: 1) The change of each parameter of pulmonary function tests were significantly improved in all groups after respiratory exercise treatment during admission (p<0.05). 2) By comparing the change of each parameter of pulmonary function tests between the experimental group and control group, the parameters of vital capacity (VC), forced vital capacity, forced vital capacity predicted (FVCP) and forced expiratory volume in 1 second (FEV1) were significantly improved in the experimental group which had received the pulmonary exercise treatment with IPPB (p<0.05). In conclusion, this study suggests that the pulmonary exercise treatments with IPPB facilitated improvement in the pulmonary functioning for the PMD patients during their hospital stay.

        The objective of this study was to identify pulmonary functional variations in relation to postural changes, lapse after changing position, and the use of abdominal band in the cervical cord injured. The subjects of this study were 19 quadriplegic patients who had been admitted to the department of the Rehabilitation Hospital, College of Medicine, Yousei University, from April, 1997 through May 3, 1997. A spiroanalyzer was used to measure pulmonary function in supine, standing, time after changing position, and recording to the position, application method, and tightness of the abdominal band. The data were analyzed by the repeated measure one-way ANOVA, and Wilcoxon signed rank test. The findings were as follows: 1. All phase of the patients' pulmonary function improved significantly in supine posture in contrast to standing (vital capacity by and expiratory reserve volume by ). 2. The longer the time lapsed from supine posture to standing, the patient's expiratory reserve volume, maximum ventilation volume, vital capacity, and forced expiratory volume increased. 3. When the patient lay in supine position, the maximum ventilation volume, vital capacity, and the forced vital capacity increased then the center line of the abdominal band was placed along iliac crest; on the other hand, when the patient was standing, placing the bottom line of the abdominal band along iliac crest increased the maximum ventilation volume, vital capacity, and forced expiratory volume. 4. In placing the abdominal band in the patients, leaving space between the top and bottom lines of the band helped increased in maximum ventilation volume, vital capacity, and forced vital capacity for patient in supine as well as in standing. 5. When placing the abdominal band to patients in supine posture, reducing the length of the band by 2.5% along the patient's waist line increased the patients' vital capacity, while reducing the length by 10% to patients in standing increased the maximum ventilation volume. The abdominal band should be placed in such a way that the bottom part of the band should be more tightly fastened while leaving enough room for a hand to be placed in between the body and the band for the top part of the hand. It should also be noted that in a supine position, the bottom line of the band should be placed along the iliac crest, while in standing, the center line should be placed along the iliac crest. The length of the band should also be reduced by 2.5% of the waist line in supine position, and in standing, the length should be reduced by 10%. It should also be noted that the pulmonary function of the patients should be measured at least 10 minutes after one position change.

        Acute upper airway obstruction can cause negative pressure pulmonary edema (NPPE). It is a well-known perianesthetic complication, but is uncommon for intubated patients. Radiation injury of lung can easily lead to pulmonary hemorrhage or NPPE caused by changes of transthoracic or transpulmonary pressure. It is due to injury of alveolar-capillary membrane by radiation. We report the case of a 71-year-old man with a history of radiation therapy for lung cancer after surgical resection who developed the abrupt onset of pulmonary hemorrhage and NPPE during general anesthesia for hydrocele and inguinal hernia operation.

        Background : Korean Red Ginseng has been reported to have various anti-virus, anti-tumor, anti-inflammation and immune-regulating activity. However, Korean Red Ginseng Crude Saponin (KRGCS) effect on Pulmonary inflammation has been insufficient research. We have investigated the effect of KRGCS on pulmonary inflammation in mouse model. Methods and Results : Pulmonary inflammation induced by cigarette smoke condensates (CSC) and lipopolysaccharide (LPS) in BALB/c mice. 100uL (ratio 1:1) of CSC (4mg/mL) and LPS (100ug/mL) solution instilled with intratracheal injection once per a week for 3 weeks in mice. KRGCS was also administered orally a dose of 10mg/kg and 25mg/kg for 3 weeks. KRGCS treatment groups decreased the number of inflammatory total cells in lung tissues and bronchoalveolar lavage fluid (BALF) and the number of neutrophils in BALF dropped more than negative control groups. Additionally, KRGCS reduced the absolute number of CD4+ and CD8+ cells in BALF, and the number of CD11b+ Gr-1+cells in the lung and BALF. Furthermore, we found that KRGCS declined significantly the level of the cytokine chemokine (C-X-C motif)-1 (CXCL-1), Macrophage inflammatory protein-2 (MIP-2), and tumor necrosis factor alpha (TNF-α) in BALF. Conclusion : These results that KRGCS has an inhibitory effects on pulmonary inflammation such as COPD, may be a candidate of effective herbal pharmaceuticals for the cure of lung diseases.

        Diffuse involvement of the right pulmonary artery (PA) associated with fistula between the PA and coronary artery is report-ed in a woman with Takayasu’s arteritis. Both the subclavian arteries were totally occluded and drained by the meanderinged artery arising from both common carotid arteries. Lung perfusion scan revealed perfusion defect of right lung. Two fistulas were identified. A large fistula was between the right PA and left circumflex artery. A small fistula was between the right PA and left anterior descending artery. This is a rare case of Takayasu’s arteritis presenting with a coronary – pulmonary artery fistula that is secondary to a diffuse unilateral involvement of PA.
