        Purpose: This study explored illness experiences and the demands for a health-related program tailored to individuals with chronic disease at one of the Korean Seon Centers. It also utilized this information as foundational data for developing a program tailored to their needs. Methods: The study utilized focus group interviews to collect data from members of one of the Korean Seon centers who have chronic disease, regarding their illness experiences and their expectations for disease management programs. The collected data was then analyzed using the qualitative content analysis method. Results: The analysis yielded five overarching themes and 14 subthemes. These themes included opposing responses to disease diagnosis, reflections on the causes of disease, efforts toward disease recovery, obstacles to disease recovery, and the need for a customized disease management program. Conclusion: The findings highlighted the challenges the participants faced in their disease recovery process. They expressed a strong desire for a comprehensive management program that incorporates in-depth information, experience sharing, and education. Such a program would empower individuals to take control of their physical and mental well-being.

        Background: Osteoarthritis is a common condition with an increasing prevalence and is a common cause of disability. Osteoarthritic pain decreases the quality of life, and simple gait training is used to alleviate it. Knee osteoarthritis limits joint motion in the sagittal and lateral directions. Although many recent studies have activated orthotic research to increase knee joint stabilization, no study has used patellar tendon straps to treat knee osteoarthritis. Objects: This study aimed to determine the effects of patellar tendon straps on kinematic, mechanical, and electromyographic activation in patients with knee osteoarthritis. Methods: Patients with knee osteoarthritis were selected. After creating the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), leg length difference, Q-angle, and thumb side flexion angle of the foot were measured. Kinematic, kinetic, and muscle activation data during walking before and after wearing the orthosis were viewed. Results: After wearing the patellar tendon straps, hip adduction from the terminal stance phase, knee flexion from the terminal swing phase, and ankle plantar flexion angle increased during the pre-swing and initial swing phases. The cadence of spatiotemporal parameters and velocity increased, and step time, stride time, and foot force duration decreased. Conclusion: Based on the results of this study, the increase in plantar flexion after strap wearing is inferred by an increase due to neurological mechanisms, and adduction at the hip joint is inferred by an increase in adduction due to increased velocity. The increase in cadence and velocity and the decrease in gait speed and foot pressure duration may be due to joint stabilization. It can be inferred that joint stabilization is increased by wearing knee straps. Thus, wearing a patellar tendon strap during gait in patients with knee osteoarthritis influences kinematic changes in the sagittal plane of the joint.

        Background: Stroke is one of the causes affecting gait and balance. Taping is considered an effective method for improving balance and gait in stroke patients. Numerous studies have confirmed the functional effects of taping in stroke patients. However, there is still no consensus regarding the use of taping to improve gait and balance. Objects: The purpose of this review was to investigate the effects of taping on the balance and gait of patients with stroke through meta-analysis of studies. Methods: PubMed, Medline, Embase, Web of Science, Cochrane Review, RISS, DBPia, and Science on were used to collect articles on Kinesio and non-elastic taping. The key terms were “Stroke”, “Hemiplegia”, “Taping”, “Tape”, “Balance”, and “Gait” with cut-off of October, 2022. Taping group was compared with control groups with sham, placebo, and no taping. The outcome measures included the Berg Balance Scale (BBS), Timed Up and Go (TUG) test, and gait speed (cm/s). Eighteen studies (524 patients) were selected for the meta-analysis. Results: Overall, taping improved balance and gait in stroke patients, and Kinesio and nonelastic taping had similar effect sizes. Taping improved the BBS and TUG, and was most effective on gait speed. Contrary to the expectation that a longer duration of taping would be more affective, taping was most effective when the total taping duration was shorter than 500 minutes. In addition, the effect size of taping was greater when it was simultaneously attached to multiple locations. Conclusion: This meta-analysis supports the use of taping to improve gait and balance in stroke patients, and provides guidelines for the location, duration, and type of tape to increase taping efficiency.

        Background: Studies using Smovey and Vibration foam rollers are mostly conducted with normal adults, breast cancer patients, and athletes. However, there are not many studies applied to stroke patients to date. Objectives: The purpose of this study is to investigate the effect of vibration exercise on the range of motion of the shoulder and knee joints in stroke patients. Design: A randomized controlled trial. Methods: A total of 36 stroke patients, with 12 in the Smovey and Vibration foam roller group (SVG), 12 in the Smovey and Non-Vibration foam roller group (SNVG), and 12 in the control group (CG) were randomly assigned to exercise three times a week for 6 weeks. The range of motion of the shoulder and knee joints was measured for each group before and after 6 weeks of exercise. For joint range of motion, shoulder flexion and extension and knee flexion and extension were measured using BPMpro. Results: In terms of the difference in time, the flexion and extension of the shoulder and the flexion and extension of the knee on the paretic side were significant in the SVG and SNVG. The CG was not significant. Shoulder flexion and extension and knee flexion and extension on the unaffected side were all insignificant. The differences between each group were significant between the SVG and the CG and the SNVG and the CG. SVG and SNVG were not significant. Conclusion: It was found that vibration exercise had a positive effect on the range of motion of the shoulder and knee joints. Therefore, it is thought that vibration exercise can be provided as a clinical intervention method for shoulder and knee range of motion.

        Background: Core muscle weakness occurs due to trunk asymmetry and spinal malalignment after stroke. Core exercise is being implemented to improve trunk control and sitting position in stroke patients. Objectives: To investigated the effects of core muscle electrical stimulation on trunk control and dynamic balance in stroke patients. Design: Quasi-experimental study. Methods: A total of 30 stroke patients were recruited and divided into two groups: experimental group (core muscle electrical stimulation group) and control group (sham core muscle electrical stimulation group). Trunk impairment scale (TIS) was used to measure trunk control. BioRescue was used to measure sitting dynamic balance. Results: In both groups, all TIS scores and sitting dynamic balance abilities were improved increased significantly after intervention (P<.05). Changes in TIS scores and sitting dynamic balance abilities were significantly greater in the experimental group than the control group (P<.05). Conclusion: It was found that core muscle electrical stimulation can be used as an effective method for trunk control and balance recovery in stroke patients.

        Background: Despite its high prevalence, interventions in plantar fasciitis remain inconclusive. To improve the daily life of plantar fasciitis patients, it is necessary to identify appropriate interventions to improve pain and foot pressure and balance. Improving the interventions for plantar fasciitis is one of the main goals for rehabilitation. Objectives: To investigate the changes in pain and static-dynamic foot pressure and balance through the evaluation of 42 plantar fasciitis patient before and after the foot stretching and joint mobilization interventions. Design: Randomized controlled trial. Methods: The 42 subjects of the study were divided into a stretching group and a joint mobilization group with 21 subjects being assigned to each group. The results before and after the experiment were analyzed by applying stretching and joint movement interventions for a total of 6 weeks. Results: Before and after the intervention, there were significant improvements in the pain index and static-dynamic foot pressure and static balance in both stretching and joint mobilization groups. However, the between-group comparison indicated no significant differences. Conclusion: The stretching and the joint mobilization can improve the pain and static-dynamic foot pressure and static balance in plantar fasciitis patients.

        목적 : 본 연구는 무릎관절 치환술 환자를 위한 작업치료 임상 가이드라인을 체계적으로 개발하고자 하였다. 전 세계적으로 무릎관절 치환술을 받은 환자들의 회복과 지역사회 복귀에 있어 작업치료의 역할이 매우 중요하다는 문헌이 보고되고 있으나, 우리나라에서는 관절 치환술 환자를 위한 작업치료가 거의 제공되지 않는다는 현실이 있다. 본 연구에서는 무릎관절 치환술 환자를 위한 근거기반 작업치료 중재를 확인하고 이것을 가이드라인을 통해 제안하고자 한다. 연구방법 : 가이드라인 개발 방법은 영국 작업치료사협회의 「Practice guideline development manual(4th edition)」에서 소개한 절차를 따랐다. Patient/Population, Intervention, Comparison and Outcomes(PICO) 임상 질문 설정은 이해관계자의 의견을 수렴하여 한국의 보건·의료제도를 고려하여 설정하고 이를 바탕으로 체계적 고찰을 진행하였다. 데이터베이스에서 총 7,241건의 논문을 추출하였으며 논문의 질 평가 및 비평적 읽기를 통해 최종 66편의 근거 논문을 선정하였다. 논문의 중재를 바탕으로 가이드라인 내용을 구성하고 권고 강도는 Grading of Recommendations, Assessment, Development, and Evaluations(GRADE) approach 체계를 활용하여 [권고]와 [제안]으로 구분하였다. 결과 : 무릎관절 치환술 환자를 위한 작업치료 임상 가이드라인 권고 내용은 ‘환자 중심’과 ‘서비스 제공 중심’으로 범주화하였다. ‘환자 중심’범주에는 ‘일상생활 활동기능 향상’, ‘정신건강의 안정(불안, 우울 등)’, ‘환자 만족도 증가’, ‘지역사회복귀 향상’을, ‘서비스 제공 중심’범주에는 ‘작업치료사를 포함한 다학제 재활팀 구성’과 ‘작업치료 를 통한 병원 입원 기간 단축’을 위한 내용을 포함하였다. 결론 : 무릎관절 치환술 환자를 위한 작업치료 임상 가이드라인은 수술환자들의 안정적인 지역사회 복귀와 입원 기간 단축에 기여할 것이다. 또한 정형외과 작업치료사 역량 강화를 위한 교육자료로 활용될 수 있으며 의료비 용 감소를 위한 정책 마련에 근거 자료가 될 것이다.

        Background: The weakness of the gluteus medius (GM) is associated with various musculoskeletal disorders. The increasing GM activity without synergistic dominance should be considered when prescribing pelvic drop exercise (PD). Isometric hip extension or flexion of the non-weight bearing leg using thera-band at the ankle during PD may influence hip abductor activities. Objects: To determine how isometric hip extension or flexion of the non-weight bearing leg using thera-band at the ankle during PD influences the activities of three subdivisions of GM (anterior, GMa; middle, GMm; posterior, GMp), tensor fasciae latae (TFL), contralateral quadratus lumborum (QL), and GMp/TFL, GMm/QL activity ratios in patients with GM weakness. Methods: Twenty-three patients with GM weakness were recruited. Three types of PD were performed: PD, PD with an isometric hip extension of the non-weight bearing leg (PDE), and PD with an isometric hip flexion of the non-weight bearing leg (PDF). Surface electromyography (SEMG) was used to measure hip abductor activities. One-way repeated-measures analysis of variance was used to assess the statistical significance of muscle activities and muscle activity ratios. Results: GMa, GMm, and GMp activities were significantly greater during PDF than during PD and PDE (p < 0.001, p = 0.001; p = 0.001, p = 0.005; p = 0.004, p = 0.004; respectively). TFL activity was significantly greater during PDE than during PD and PDF (p < 0.001, p < 0.001, respectively). QL activity was significantly greater during PDF than during PD (p = 0.003). GMp/TFL activity ratio was significantly lower during PDE than during PD and PDF (p = 0.001, p = 0.001, respectively). There were no significant differences in the GMm/QL activity ratio. Conclusion: PDF may be an effective exercise to increase the activities of all three GM subdivisions while minimizing the TFL activity in patients with GM weakness.

        Background: Scapular dyskinesis may cause not only rotator cuff (RC) tear but also weakness of the upper extremity, studies on scapular dyskinesis that may occur after RC repair is still lacking. Objects: To determine whether scapular dsykinesis was present in patients after arthroscopic RC repair and to investigate the influence of passive scapular stabilization on upper extremity strength. Methods: A total of 30 patients after RC repair participated in this study. To compare the scapula of the arthroscopic RC repair shoulder and the contralateral shoulder, the winged scapula (WS) was measured using a scapulometer and scapular dyskinesis was also classified by type. Fixed instruments for muscle strength measurements were used to measure upper extremity muscle strength differences depending on passive scapular stabilization position or natural scapular position. A chi-square test, an independent t-test and a 2-way mixed measures analysis of variance (ANOVA) was used as statistical analysis. In analyses, p < 0.05 was deemed to be statistically significant. Results: Postoperative shoulder had a significant association with scapular dyskinesis and the WS compared to the contralateral shoulder (F = 0.052, p < 0.01). Postoperative shoulder, muscle strength in the shoulder abduction (p < 0.01), elbow flexion (p < 0.01) and forearm supination (p < 0.05) were significantly greater in the scapular stabilization position than in the scapular natural position. Conclusion: Patients underwent arthroscopic RC repair had a significant association with scapular dyskinesis and muscle strength was improved by a passive scapular stabilization position, therefore scapular stabilization is important in rehabilitation program.

        Background: Like many other countries, falls and related injuries in older adults are great concerns in South Korea. In particular, falls are common in medical institutions, often causing the increase of the length of hospitalization. Objects: The purpose of this review was to help understand and address falls in hospitalized individuals in South Korea. Methods: The review was conducted on literature published in Korean from 2010 to 2022, searched in the Korea Citation Index and PubMed. Keywords used for the search were as follows: falls, fall risk, fall risk assessment, hospital, inpatient, intervention, Korea, and prevention. Results: A total of 54 articles were found and reviewed. The most common place of fall accidents was the inpatient room, where there were many cases of falls while walking. Loss of balance was the most common cause of falls, and many falls occurred in patients admitted to the internal medicine. Furthermore, a risk of falling increased with the type of medications taken. In terms of tools to assess patients’ fall risk, the Morse Fall Scale (MFS) was commonly used. Patient-specific fall prevention activities were common to address falls, and they decreased the frequency of falls and the fear of falling. Factors influencing the effectiveness of the fall prevention activities included attitudes toward falls, education, environmental factors, patient safety culture, and self-efficacy in preventing falls. Conclusion: Our results should help understand and address falls and injuries in medical institutions.

        Background: The application of exercise therapy and manual therapy to the thoracic spine is a widely used method of treating neck pain. Nevertheless, studies on the application of foam rollers and vibrating foam rollers to patients with neck pain are lacking. Objectives: To investigated the immediate effects of thoracic spine foam rolling and vibration foam rolling on pain and range of motion (ROM) in patients with chronic neck pain. Design: Randomized crossover trials. Methods: 24 patients with chronic neck pain participated in the study. The study subjects measured pain and ROM. Subjects were divided into vibration foam roller group, foam roller group, and control group. Results: Pain was not significantly different between and within groups (P<.05). There was a significant difference in flexion, extension, left rotation, and right rotation ROM in the vibration foam roller group after intervention (P<.05). There was a significant difference in flexion and extension ROM in the foam roller group after intervention (P<.05). There was a significantly greater increased extension ROM in vibration foam roller and foam roller groups compared with the control group (P<.0167). Conclusion: Thoracic spine foam rolling and vibration foam rolling improve ROM when treating patients with chronic neck pain. Therefore, it is recommended to use it in combination with other treatments

        Background: In order to improve the daily life of stroke patients, it is necessary to restore walking performance. Improving gait performance is one of the main goals for stroke rehabilitation. Objectives: To investigate the change in gait parameters through evaluation of one stroke patient before, during, and after wearing the weight vest. Design: A case study. Methods: This study is an ABA study design. In principle, one assessment per day (A1) was measured three times without wearing the weight vest. Intervention assessment (B) was performed 6 times while wearing the weight vest. The second baseline evaluation (A2) was measured three times with the weight vest removed again. Results: The gait speed was increased when the weight vest was removed than when the weight vest was worn. Also, gait symmetry increased when wearing the weight vest. Conclusion: Wearing the weight vest can reduce the gait asymmetry in stroke patients and increase the gait speed after wearing the weight vest.

        Background: Total hip replacement (THR) is performed in patients with femur fractures and osteoarthritis. THR patients have balance problems even after surgery. There is a lack of research on vibration stimulation interventions for balance in THR patients. Objectives: To investigated the effect of vibration stimulation intervention on the balance and gait of THR patients. Design: Randomized controlled trials. Methods: 44 subjects were randomly assigned to a vibration stimulation group (VSG) and a non-vibration stimulation group (N-VSG). Seven study subjects dropped out, and 37 completed the study. Timed up and go (TUG) and Berg balance scale (BBS) were used for balance assessment, and 10-meter walk (10MW) was used for gait assessment. The intervention program was conducted three times a week for 4 weeks. Results: Significant differences within the groups in balance (BBS, TUG) and gait (10MW) between the VSG and the N-VSG. There was no difference between the two groups in any variable. Conclusion: The VSG improved the balance and gait of THR patients without any difference from the N-VSG. However, VSG showed a higher effect size than N-VSG.

        Background: A weight-bearing jacket during treadmill gait training is being used for rehabilitation in stroke patients. However, the effect on various gait parameters has not been confirmed. Objectives: The purpose of this study was to investigate the comparison of exercise intensity through speed increase and weight-bearing jacket during treadmill gait training to restore gait ability in stroke patients. Design: Randomized controlled trials. Methods: This study recruited 24 stroke patients. The subjects were randomly assigned to the experimental group (wearing a weight-bearing jacket during treadmill gait training, n=12) and the control group (speed increase during treadmill gait training, n=12). Gait measurement device (G-walk) was used to measure the gait parameters of the subjects. Results: Both the experimental group and control group showed a significant increase in cadence, gait speed, stride length, and gait symmetry index post intervention (P<.05). In comparison between the two groups, the experimental group showed a significant increase in the gait symmetry index post the intervention than the control group (P<.05). Conclusion: Through this study, it was found that wearing a weight-bearing jacket was a more effective method for improving the gait symmetry index than increasing speed during treadmill gait training in stroke patients.

        Background: For the rehabilitation of stroke patients, it is necessary to confirm the differences in the mechanical properties of the paretic- and non-pareticside masseter muscles. Objectives: To investigate the muscle tone and stiffness of masseter muscles in patients with chronic stroke. Design: A case study. Methods: This study included 20 stroke patients. Muscle tone and stiffness of the paretic and non-paretic masseter muscles in stroke patients were measured using MyotonPRO. Measurements were performed under relaxation and masseter muscle contraction conditions. Results: There was no significant difference in the muscle tone and stiffness of the paretic and non-paretic side masseter muscles in the relaxation conditions (P>.05). In male, left hemiplegia, right hemiplegia and infatction, the nonparetic side masseter muscles showed significantly increased than musle tone and stiffness of the paretic side masseter muscles in the contraction (P<.05). In female and hemorrhage, the non-paretic side masseter muscles showed significantly increased than musle tone the paretic side masseter muscles in the contraction (P<.05). Conclusion: The muscle tone and stiffness were lower on the paretic side than on the non-paretic side during masseter muscle contraction in stroke patients; thus, muscle strengthening training was required.

        Purpose: This was a qualitative study using a phenomenological research method. It was conducted to investigate the essential meaning of nursing students’ experiences participating in simulation education with standardized patients. Methods: This study’s research participants included 16 senior employees in the nursing department of two universities. They had experiences participating in simulation education with standardized patients. This study was conducted from June 1 to December 31, 2021, and the data were analyzed using Colaizzi’s (1978) phenomenological method. Results: After analyzing the meaning of simulation experiences with the standardized patients, 101 meaning formations were deduced. From this, 13 theme, 4 theme clusters and 2 categories—effective lesson, tasks to solve. Conclusion: Simulation with standardized patients is a recent approach that has been actively applied in simulation-based learning in nursing colleges. This study conducted qualitative research on students’ experiences with standardized patients and analyzed essential meaning of the experiences. Therefore, it is recommended that simulation education content with standardized patients is developed. This can be achieved by optimizing the advantages of standardized patients with various clinical cases in other subjects besides psychiatry.

        목적 : 국내·외에서 보고된 뇌졸중 환자 대상의 운전 시뮬레이터 훈련 연구의 체계적 고찰을 통해 연구 경향과 대상자 특성, 사용 방법 및 시뮬레이터를 통한 중재의 효과성에 대해 알아보고자 한다. 연구방법 : RISS, PubMed 등의 데이터베이스를 이용하여 선정 기준에 부합하는 5편의 국내·외 연구를 검색하였 다. 분석 대상 연구들은 연구의 근거 기반 질적 수준 분석 모델에 따라 분류하였고, 임상적 분석 방법인 P.I.C.O 방법에 따라 분석하여 정리하였다. 결과 : 고찰한 연구는 총 5개로 근거 수준은 I, IV였다. 연구 대상자는 뇌졸중 환자로 운전 시뮬레이터 훈련 중재 를 통해 운전 기능 향상의 효과를 살펴보았으며, 중재 결과 다양한 영역에서의 향상이 보고되었음을 확인할 수 있었고, 1편의 논문을 제외한 4편의 논문에서 운전 기능의 유의미한 향상을 보고하였다. 결론 : 시뮬레이터를 이용한 중재는 많은 연구를 통해 효과가 입증되고 있었다. 추후 연구 대상자를 더욱 다양하 게 하여 도로 주행 능력 및 다양한 영역에서의 기능 향상을 위한 운전재활 연구가 지속적으로 이루어져야 할 것이다.

        Background: The muscles associated with tension-type headache (TTH) are connected to the scalp fascia, which is firmly attached to the superciliary ridge of the frontal bone. However, there is a paucity of data on treatments for TTH that directly target the scalp fasciae. Objectives: We aimed to validate a new manual therapy to treat TTH by applying myofascial relaxation techniques to the scalp and to examine the changes in quality of life and headache characteristics after treatment. Design: pretest-posttest control group design study. Methods: Total 24 participants with TTH (10 males, 14 females) participated in the study. Manual therapy group (MT) received standard manual therapy and scalp myofascial release technique group (SMT) received scalp myofascial release technique twice per week for 4 weeks. Brief Pain Inventory (BPI) and Headache Impact Test (HIT) for quality of life, headache frequency, duration, and Visual Analog Scale (VAS) for intensity were assessed at the pre- and post-treatment. Results: There was a significant improvement within groups in headache frequency (MT P<.05, SMT P<.001), duration (MT P<.05, SMT P<.01), VAS (MT P<.01, SMT P<.001), HIT (MT P<.001, SMT P<.001), and BPI (MT P<.001, SMT P<.001) between the pre- and post-treatment. Group differences were significant for headache duration (P=.027), but the others were not. MT and SMT reduced the impact of headaches on daily life. Also Headache frequency, duration, intensity, and BPI were improved. Conclusion: MT and SMT reduced the impact of headaches on daily life. Also Headache frequency, duration, intensity, and BPI were improved. It has been suggested that both MT and SMT can be used as a non-invasive treatment to treat the TTH, and to improve the quality of life.

        Background: Low back pain is a very common musculoskeletal disorder. Since low back pain can indicate physical and psychological problems, reducing the pain level of low back pain can be the primary goal of rehabilitation. Objectives: This study was performed to explain the personalized treatment protocol of Maitland orthopedic physiotherapy based on the brick wall concept for low back pain patients with hypomobility. Design: Randomized controlled study. Methods: A total of 14 chronic low back pain patients were divided into two groups. The experimental group received orthopedic manual physical therapy intervention. The control group received traditional physical therapy intervention. After the 3 days intervention, the joint range of motion and pain of the low back were measured. Results: The low back flexion, extension, lateral flexion, and rotation joint range of motion was significantly increased in the experimental group than in the control group (P<.05). Low back pain was significantly reduced in the experimental group than in the control group (P<.05). Conclusion: It was confirmed that Maitland orthopedic physical therapy was an effective method as an intervention method to improve the joint range of motion and pain of chronic low back pain patients.
