        The purpose of this study was to assess the effects of Ankle exercise on balance ability of stroke patients. Participants' balance ability were measured prior to the training and they conducted ankle exercise. As for ankle exercises, ankle stretching training and ankle muscle strengthening training were performed for 10 minutes respectively. They did warm up exercise for 5 minutes and then ankle exercise for 20 minutes. And then they did cooldown exercise for 5 minutes. Their balance ability was measured after 9 weeks. According to the results of analyzing the change of balance ability that timed up and go and Berg balance scale score significantly improved but in the control group it did not significantly decrease. In comparison between the groups, Berg balance scale score significantly improved in the ankle exercise group compared to the control group. Ankle exercise performed by stroke patients are considered to be effective in improving balance ability.

        Background: Increased foot pronation causes biomedchanical changes at the lower limbs, which may result in musculoskeletal injuries at the proximal joints. Pronation rear-foot leads to plantar fasciitis, Achilles tendonitis, and posterior tibial tendonitis pathologically. According to the recent meta-analysis, They showed that therapeutic adhesive taping is more effective than foot orthoses and motion control footwear, low-Dye (LD) taping has become the most popular method used by physiotherapists. Objects: The purpose of this study was to determine the immediate effects of LD taping results in different ankle motion and ground reaction force (GRF) as before and after applied LD taping on pronated rear-foot during gait. Methods: Twenty-four participants were recruited for this study. The gait data were recorded using an 8-camera motion capture system and two force platforms. At first, the experiments were carried out that participants walked barefoot without LD taping. And then they walked both feet was applied LD taping. Results: The ankle inversion minimum was significantly greater after LD taping than before LD taping (p=.04); however, in the GRF, there were no significant differences in the inversion maximum or total motion of the stance phase (p=.33, p=.07), or in the vertical (p=.33), posterior (p=.22), and lateral (p=.14) peak forces. Conclusion: The application of taping to pronation rear-foot assists in increased ankle inversion.

        Background: Total knee arthroplasty (TKA) recovers the alignment of the knee joint, but fails to automatically restore the alignment and function of the hip and ankle joints. It may affect the alignment and stability of the knee joint, therefore therapeutic intervention in hip and ankle joint is necessary for the rehabilitation process after TKA. Objects: The aim of this study was to comparison of the effects of the two exercise methods on the coronal plane alignment after TKA. This study conducted an experiment by dividing subjects into a lower extremity isometric co-contraction group (LEIC) and a quadriceps isolated isometric contraction (QIIC) group. Methods: A total of 37 subjects were randomly assigned to the LEIC (n1=19) or the QIIC (n2=18). Exercise was applied to five times per week for three weeks, starting on the eighth day after surgery. Range of motion exercises were performed as a common intervention and then each group performed quadriceps isometric contraction exercises with 10 sets of 5 repetitions. Radiological imaging was performed prior to surgery, one month and six months after surgery. In addition, the hip-knee-ankle angle (HKA) and tibiotalar angle (TTA) were measured. Results: The HKA was close to neutral in the LEIC rather than the QIIC (p<.05). The LEIC showed varus and the QIIC exhibited valgus TTA (p<.05). In a comparison of HKA and TTA over time, there was no significant change in either group (p>.05). According to the comparison of the TTA before surgery, the LEIC showed significant changes in the varus direction (p<.05), while there was no significant change in the QIIC (p>.05). Conclusion: The LEIC method triggered changes in the TTA and brought the HKA close to the neutral. Thus, LEIC is more effective than QIIC in creating stability in the coronal plane alignment of the knee and ankle joints after TKA.

        We investigated the effects of a Virtual reality(VR) program on the proprioception and instability of functional ankle instability(FAI) patients. Among the 20s old individuals who lived in Republic of Korea; 20 people were selected through a public participation process. The 20 participants were then randomly divided into two groups, one of strength exercise(n=10) and one of balance exercise(n=10). Of the patients who had experienced an ankle sprain in the past and currently felt ankle instability, those who recorded 24 points or less on the Cumberland ankle instability tool(CAIT) were determined to have FAI. We selected the strength exercise and balance exercise in the Nintendo Wii Fit Plus as a VR program. The subjects used the program on the Wii Balance Board three times per week for 20 minutes and total 10-minute warm-up and down for four weeks. Proprioception and CAIT of the balance exercise group were improved significantly after the exercise compared to before the exercise(p<.05). Authors recommend that the balance exercise in the VR program be used as an aid for physical therapeutic intervention.

        This study observed the effects of ankle strengthening exercise and whole body vibration on the balance ability of older adults, thereby intending to provide basic materials for intervention methods aimed at improving older adults’ balance ability. The subjects were 20 older adults who had experienced a fall. They were equally divided into two groups. Ankle strengthening training was applied to one group and ankle strengthening training and whole body vibration were applied to the other group, a timed up and go (TUG) test and Tinetti performance oriented mobility assessment (POMA) were performed, and changes in the subjects’ limits of stability were observed. The TUG and POMA results significantly differed between before and after the experiment in the angle strengthening training (AST) group and the angle strengthening training with whole body vibration (ASTWV) group. In addition, the interaction between timing and each group was statistically significant. The limits of stability significantly changed after the intervention in both groups. Differences in the posterior and right limits of stability were significant between the AST group and ASTWV group. Therefore, ankle strengthening exercise and whole body vibration improve older adults’ balance maintenance and reduce falls or the risk factors for falls in older adults.

        This study was conducted to investigate the effects of the removable ankle-foot orthosis (RAFO) which was developed to improve the gait of stroke patients. The subjects of this study were five stroke patients who agreed to participate in this study by signing a written consent form. To verify gait improvement after wearing the orthosis, a Timed Up and Go test and Functional Gait Assessment were performed, and spatiotemporal gait variables such as gait speed, cadence, stride length, double limb support, and the efficient gait test of body sway angle were performed. For every variable, the differences prior to and after wearing the RAFO were compared using the Wilcoxon signed-rank test. Every gait variable improved significantly after wearing the RAFO compared to prior to wearing it. The pilot study will enhance future efforts to evaluate orthotic function objectively during gait in stroke patients.

        Balance control deficits have been indicated to be a primary problem among cerebral palsy (CP) patients. Fabric ankle foot orthosis (AFO) can allow more efficient balance control by facilitating proprioception. The purpose of this study was to investigate the immediate effect of fabric AFO on balance, compared to a barefoot condition in children with unilateral CP. Twelve children with unilateral CP participated in this study. Their balance ability was evaluated using pediatric balance scale and bubble test. Both pediatric balance scale and bubble test showed significant improvement with the use of the fabirc AFO (p<.05). The fabric AFO could improve functional balance ability, and promote better balance among children with unilateral CP. We demonstrated that fabric AFO contributed to improving balance among children with unilateral CP, classified as Gross Motor Function Classification System level Ⅰ and Ⅱ. Consequently, fabric AFO might be an assistive device leading to the improvement of balance instead of the typical AFOs.

        The common features of walking in patients with stroke include decreased gait velocity and increased asymmetrical gait pattern. The purpose of this study was to identify important factors related to impairments in gait velocity and asymmetry in chronic stroke patients. The subjects were 30 independently ambulating subjects with chronic stroke. The subjects’ impairments were examined, including the isokinetic peak torque of knee extensors, knee flexors, ankle plantarflexors, and ankle dorsiflexors. Passive and active ranges of motion (ROM) of the ankle joint, ankle plantarflexor spasticity, joint position senses of the knee and ankle joint, and balance were examined together. In addition, gait velocity and temporal and spatial asymmetry were evaluated with subjects walking at their comfortable speed. Pearson correlations and multiple regressions were used to measure the relationships between impairments and gait speed and impairments and asymmetry. Regression analyses revealed that ankle passive ROM and peak torque of knee flexors were important factors for gait velocity (R2=.41), while ankle passive ROM was the most important determinant for temporal asymmetry (R2=.35). In addition, knee extensor peak torque was the most significant factor for gait spatial asymmetry (R2=.17). Limitation in ankle passive ROM and weakness of the knee flexor were major contributors to slow gait velocity. Moreover, limited passive ROM in the ankle influenced the level of temporal gait asymmetry in chronic stroke patients. Our findings suggest that stroke rehabilitation programs aiming to improve gait velocity and temporal asymmetry should include stretching exercise for the ankle joint.

        The purpose of this study was to examine the relationships between the ankle dorsiflexion passive range of motion (DF PROM) under a non-weight bearing condition and the normalized reach distance in three directions of the Y-Balance Test (YBT). Sixty-one healthy adults (32 males and 29 females, age: 23.0±3.0 years, height: 169.3±8.9 ㎝, weight: 61.9±5.4 ㎏) participated in this study. The ankle DF PROM was measured using a goniometer. To assess dynamic balance, all subjects performed three trials to determine the maximum lower extremity reach in the anterior, posteromedial, and posterolateral directions of the YBT. The relationship between the ankle DF PROM and both the normalized reach distance in each direction and the composite score of the YBT were analyzed using the Pearson correlation. Only the normalized reach distance in the anterior direction of the YBT was significantly related to the ankle DF PROM measured under a non-weight bearing condition (r=.50, p<.001). Neither the normalized reach distances in the posterior directions nor the composite score of the YBT were significantly correlated with the ankle DF PROM measured under a non-weight bearing condition. These findings suggest that ankle DF PROM does not affect the overall dynamic balance of the lower extremity, with only the anterior dynamic balance affected among the three directions.

        The purpose of this study was to compare the change in electromyography (EMG) activity in the gluteus maximus (G-max) and the gluteus medius (G-med) in subjects with and without chronic ankle instability (CAI) during three functional postures. Twenty four females were recruited for this study. Subjects were assigned into two groups: with CAI (n1=12) and without CAI (n2=12). The assessment postures were rotational squat, one leg stand above a gradient and crossed leg-sway. Electromyographic activities of the G-max and the G-med were recorded using surface EMG and was normalized using the maximal voluntary isometric contraction elicited using a manual muscle testing. Independent t-test was used to determine the statistical differences between two groups during the three functional postures. The comparisons of the three posture between two groups were performed using a one-way repeated analysis of variance. A Bonferroni adjustment used for post hoc analysis. The activation of EMG on G-max performing the one leg stand above a gradient and crossed leg-sway in subjects with CAI is significantly higher than normal group (p<.05). The activation of EMG on the G-max during the rotational squat was significantly increased, compared to those of the one leg stand above a gradient and crossed leg-sway (p<.05). The activation of EMG on G-med performing three exercise at CAI is significantly higher than normal group (p<.05). The activation of EMG on the G-med during the crossed leg-sway was significantly increased, compared to the rotational squat (p<.05). This study provides valuable information for clinician who research CAI.

        목 적 : 족관절 염좌 환자의 자기공명영상 검사 시 전하경비인대(AITFL)와 전거비인대(ATFL)를 관찰하기 위한 사방향 관상면(oblique coronal plane) 2D FSE 영상과 Isotropic 3D FSE Cube 영상의 비교를 통하여 그 유용성에 대하여 알아보고자 하였다. 대상 및 방법 : 2013년 10월부터 2013년 12월까지 족관절 염좌를 주 호소로 본 기관을 내원한 환자 15명(남자: 6명, 여자: 9명)을 대상으로, 사방향 관상면 2D FSE 기법과 시상면 3D FSE Cube 영상기법을 이용하여 AITFL과 ATFL의 영상을 획득하였다. 획득한 영상에서 AITFL과 ATFL, 골조직, 배경신호의 SI 및 CNR을 측정하고, Mann-Whitney U-검정으로 두 기법간의 통계적 유의성을 검증하여 정량적 평가를 하였다. 또한 영상의학의 2명이 각각의 영상에서 보여지는 AITFL과 ATFL의 형태 및 연속성, 부분용적에 의한 인공음영, 병변의 묘사정도의 3가지 항목을 4점 척도(0: 진단불가-Non diagnostic, 1: 부족함-Poor, 2: 적합함-Adequate, 3: 좋음-Good)로 평가하고, 평가결과 간의 일치도 검증을 위하여 Kappa-value(SPSS 19) 검정을 통해 통계적 유의성( p<0.05)을 검증하여 정성적 평가를 하였다. 영상획득에 사용된 장비는 3.0T MR 장비와 Extremity RF Coil을 사용하였다. 결 과 : 정량적 평가에서 CNR의 평균값은 AITFL의 경우 사방향 관상면 2D FSE영상에서는 10.72±0.01, 3D FSE Cube 영상에서는 18.66±0.52이었다. ATFL의 경우에는 사방향 관상면 2D FSE 영상에서는 5.62±0.98, 3D FSE Cube 영상에서는 8.15±0.65이었다. AITFL과 ATFL 모두 3D FSE Cube 영상에서 CNR 값이 높았으며, 두 영상기법 간의 CNR 값의 비교는 통계적으로 유의하였다( p<0.05). 정성적 평가에서 관찰자 1에 의한 AITFL의 형태 및 연속성, 부분용적에 의한 인공음영, 병변의 묘사정도의 평가결과는 사방향 관상면 2D FSE 영상의 경우 2.4±0.5, 2.5±0.5, 2.6±0.5의 순으로 값을 나타냈고, 3D FSE Cube 영상에서는 2.8±0.3, 2.9±0.2, 2.9±0.2의 순으로 값을 나타냈다. ATFL의 경우는 사방향 관상면 2D FSE 영상에서 2.5±0.5, 2.3±0.4, 2.5±0.5의 순으로 값을 나타냈고, 3D FSE Cube 영상에서는 2.8±0.4, 2.9±0.2, 2.9±0.2의 값을 나타냈다. 관찰자 2에 의한 AITFL의 형태 및 연속성, 부분용적에 의한 인공음영, 병변의 묘사정도의 평가결과는 사방향 관상면 2D FSE 영상의 경우 2.4±0.5, 2.3±0.8, 2.6±0.4의 순으로 값을 나타냈고, 3D FSE Cube 영상에서는 2.8±0.3, 2.8±0.3, 2.8±0.3의 값을 나타냈다. ATFL의 경우는 사방향 관상면 2D FSE 영상에서 2.8±0.4, 2.4±0.5, 2.8±0.5의 순으로 값을 나타냈고, 3D FSE Cube 영상에서는 2.9±0.2, 2.9±0.2, 2.9±0.2의 순으로 값을 나타냈다. AITFL과 ATFL 모두 3D FSE Cube 영상에서 평가 결과가 높게 나타났으며, 두 관찰자 간 평가결과의 일치도 검정은 통계적으로 유의하였다( p<0.05). 결 론 : 족관절 염좌 환자에서 3D FSE Cube 영상이 사방향 관상면 2D FSE 영상에 비해 AITFL과 ATFL의 손상 감별 및 기타 인대손상의 평가에 매우 유용하다는 것을 알 수 있었다

        The purpose of this study is to observe the effect of elasticity of taping on ankle muscles’activity and endurance after plyometric training that easily causes ankle injury, and provide baseline data for physical therapy intervention methods. The study subjects are 24 male students in their 20s who attend N University in Choongnam. They were divided into three groups; 8 subjects in the elastic taping group, 8 in the non-elastic taping group, and 8 in the non-taping group(control group). They had plyometric training for 6 weeks. After the training, this study measured their maximum voluntary isometric contraction(MVIC) and muscle endurance of the muscles around ankle joint. The experiment result is as follows. After the training, all three groups showed improvement in muscle strength and endurance. The elastic taping group showed insignificant improvement in muscle strength but significant increase in muscle endurance in plantarflexion. In dorsiflexion, both muscle strength and endurance increased significantly. The non-elastic taping group showed insignificant improvement in muscle strength but significant increase in muscle endurance in plantarflexion. Taping during plyometric training had a little or insignificant effect on muscle endurance and strength compared to the non-taping group.

        This study was conducted to investigate the effect of FES and ankle mobilization on the ankle motion and the quality of gait of chronic hemiplegic patients with limited ankle joint motions. As research subjects, 24 chronic hemiplegic patients who could walk independently, regardless of assistive aids, were selected. Then, 8 subjects received mobilization randomly and 8 subjects received FES and 8 subjects received mobilization and FES, at the same time. The dorsiflexion PROM significantly increased in the group of mobilization therapy, mobilization and FES all together(p<.01). There were statistically significant differences among the three groups(p<.01). The 10m walking test significantly decreased in the group of mobilization therapy, mobilization and FES all together(p<.05). There were statistically significant differences among the three groups(p<.01). The gait velocity significantly increased in the group of mobilization therapy, FES therapy, mobilization and FES all together(p<.05). There were statistically significant differences among the three groups(p<.01). The stride length significantly increased in the group of mobilization therapy, mobilization and FES all together(p<.05). There were statistically significant differences among the three groups(p<.05). In conclusion, these findings demonstrate that rather than only using one treatment technique, applying mobilization and FES together brings a more satisfactory result to hemiplegic patients with limited ankle joint motions.

        Background and Purpose: The Ankle-Brachial Index (ABI) is the ratio of blood pressure in the lower legs to that in the arms. The intima-media thickness (IMT) of extracranial carotid arteries determined by B-mode ultrasound is a measurable index of the presence of atherosclerosis. A low ABI and a high carotid IMT are independently related to increased risk of cardiovascular events. This study examined the association between carotid IMT and ABI in patients with ischemic stroke. Materials and Methods: Retrospectively, 116 hospitalized patients with ischemic stroke were recruited. Using a pulse wave velocity ABI device along with carotid duplex sonography, we measured carotid IMT and ABI and investigated the correlation between average values. Results: There was a significant difference in carotid IMT between the normal and abnormal ABI groups (P=0.0262). The group with an abnormal ABI was more than five times as likely to have increased carotid IMT as the group with a normal ABI (age, sex-adjusted OR 5.67 (95% CI 1.85~17.38)). The ABI and carotid IMT showed a weak inverse linear correlation in patients with ischemic stroke (correlation coefficient −0.378 after adjusting for age and sex). Conclusion: Our study suggests that an abnormal ABI is associated with a high carotid IMT in patients with ischemic stroke.

        For professional drivers, there is a possibility to have musculoskeletal disorders on ankle joint due to repetitive pedaling operation. Therefore, this study have focused to examine ankle active range of motion (AROM), dorsiflexor strength, and pressure pain threshold (PPT) of tibialis anterior muscle (TA) in taxi drivers compared to a age-matched control group. Thirty male taxi drivers with at least 10 years of driving experience and thirty male sedentary workers were evaluated for ankle AROM, dorsiflexor strength, and PPT of TA. Multiple independent t-tests were used to identify significant differences between two groups. For the results, taxi drivers had significantly less AROM in dorsiflexion and greater AROM in external tibial rotation compared to the control group. Also, dorsiflexor strength and PPT of TA in taxi drivers was significantly lower than in the control group. This study indicates that the repetitive ankle movements associated with driving have an effect on ankle AROM, dorsiflexor strength, and PPT of TA and may lead to work-related musculoskeletal disorders on ankle. Professional drivers may need to be educated to prevent a potential musculoskeletal disorders associated with repetitive movement.

        The purpose of this study was to investigate the immediate effect of fabric ankle-foot orthosis on spatiotemporal gait parameters, compared to a barefoot condition in children with spastic cerebral palsy. Eleven children with spastic cerebral palsy participated in this study. Spatiotemporal gait parameters were measured with the GAITRite system. Fabric ankle-foot orthosis significantly improved Timed Up and Go test time and gait velocity. There was no significant difference in cadence. The step time significantly improved in both the more and less affected foot compared to the barefoot condition. The step length of the affected foot also significantly improved, but there was no significant difference in the step length of the less affected foot. There was significant improvement in the stride length of both the affected and less affected foot, but no significant difference in single stance or double stance. The fabric ankle-foot orthosis could improve stability, and selective control of the joint and promote better walking in children with cerebral palsy. Consequently, the fabric ankle-foot orthosis might be an alternative assistive device for neurological populations as a primary role instead of the typical ankle-foot orthosis.

        The purpose of this study was to investigate the effects of active and passive postural perturbation on ankle dorsiflexor responses in stroke patients. The subjects consisted of 13 stroke patients. Using wireless electromyography, the patients’ ankle dorsiflexor muscle responses were measured under the following conditions: active dorsiflexion (AD), active perturbation (AP), and passive perturbation (PP). Tibialis anterior muscle activity increased most significantly during PP of the affected side (118.64±56.28). The most significant increase for the non-affected side was in AD (72.64±24.56). Tibialis anterior muscle activity was compared under each condition. The affected side showed significant differences between PP and AD and between PP and AP (p<.05). The non-affected side showed not significant differences between each condition. The ratios of tibialis anterior muscle activity under AP to that under AD were 1.00 on the affected side and .75 on the non-affected side and the difference was not significant (p>.05). The ratios of tibialis anterior muscle activity under PP to that under AD were 3.30 on the affected side and 1.14 on the non-affected side and the difference was significant (p<.05). Passive perturbation improved tibialis anterior muscle activity on the affected side, and training based on this approach may have the potential to improve the ankle dorsiflexion of people with stroke.

        The purpose of this study was to implement a program of combined muscle stregth and proprioceptive exercises and to examine the impacts of these exercises on functional ankle instability(FAI). Experiments were conducted with 30 adult males and females in their 20s, and the exercise programs were implemented three days per week for four weeks. FAI was defined as a feeling of giving way after an ankle sprain and having a Cumberland ankle instability tool score of 24 points or less. The study subjects were randomly assigned to either a control group, a muscle-strengthening exercise group, or a combined muscle-strengthening and proprioceptive exercise group consisting of 10 subjects each. A Biodex isokinetic dynamometer was used to assess the subjects’ankle strength at selected speeds of 60°/sec and 120°/sec. The peak torque % body weight showed significant differences in plantar flexion, dorsiflexion, inversion, and eversion. There were also significant differences in proprioception. The results suggest that applying combined muscle-strength and proprioceptive exercises to subjects with FAI is a more effective intervention than applying only muscle-strengthening exercises.

        The purpose of this study to identified the effect of muscle strengthening of ankle joints on postural sway. The subjects of this study were 29 healthy adults aged between 20 and 30 years(male 18, female 11). All subjects received ankle muscle strengthening exercise for 3 times, 3 sessions, 30 minutes per week over 4weeks period. The measured item of muscle strength, postural sway. Data collected from all subjects the result were as follows. The ankle strength showed significant increase(p<.05). One leg stand test with eye close increase in static balance(p<.05), left-right sway distance and anterior-posterior sway distance with eye open and close in static balance(p<.05). The result findings show that strength of the ankle joint muscles is a factor which affects postural sway and the ankle joints are important in static balance.
