Background: Cadmium (Cd) is toxic heavy metal that accumulates in organisms after passing through their respiratory and digestive tracts. Although several studies have reported the toxic effects of Cd exposure on human health, its role in embryonic development during preimplantation stage remains unclear. We investigated the effects of Cd on porcine embryonic development and elucidated the mechanism. Methods: We cultured parthenogenetic embryos in media treated with 0, 20, 40, or 60 μM Cd for 6 days and evaluated the rates of cleavage and blastocyst formation. To investigate the mechanism of Cd toxicity, we examined intracellular reactive oxygen species (ROS) and glutathione (GSH) levels. Moreover, we examined mitochondrial content, membrane potential, and ROS. Results: Cleavage and blastocyst formation rates began to decrease significantly in the 40 μM Cd group compared with the control. During post-blastulation, development was significantly delayed in the Cd group. Cd exposure significantly decreased cell number and increased apoptosis rate compared with the control. Embryos exposed to Cd had significantly higher ROS and lower GSH levels, as well as lower expression of antioxidant enzymes, compared with the control. Moreover, embryos exposed to Cd exhibited a significant decrease in mitochondrial content, mitochondrial membrane potential, and expression of mitochondrial genes and an increase in mitochondrial ROS compared to the control. Conclusions: We demonstrated that Cd exposure impairs porcine embryonic development by inducing oxidative stress and mitochondrial dysfunction. Our findings provide insights into the toxicity of Cd exposure on mammalian embryonic development and highlight the importance of preventing Cd pollution.
Background: Interventions for acute low back pain include exercise therapy such as stretching, aerobic exercise, and sling exercise. Another treatment method for back pain is soft tissue release. Soft tissue release is a relaxation method that improves balance while allowing tension tissues to relax as much as possible. Objectives: To investigated the effect of acute low back pain on the massage ball exercise. Design: Randomized controlled trial. Methods: The twenty-eight subjects were randomly allocated to the Massage ball exercise with TENS group (MBETG, n=14) and the transcutaneous electrical nerve stimulation group (TG, n=14). For MBETG, massage ball exercise (BALLance Dr. Tanja Kühne method) was applied for 25 minutes followed by TENS for 15 minutes. The TG group applied TENS for 40 minutes. Each group conducted the intervention three sessions. The results were analyzed by measuring the numeric pain rating scale (NPRS), surface electromyography (%MVIC), and Oswestry disability index (ODI) before and after the intervention in each group. Results: Significant reduction was observed for NPRS and ODI in the MBETG (P<.05). And Significant higher was observed for %MVIC of the Erector spinae in the MBETG (P<.05). The NPRS and ODI in the MBETG were decreaed than the TG (P<.05). Conclusion: Massage ball exercise to patients with acute back pain will be effective in reducing pain, increasing muscle activity, and improving functional disability.
목적 : 본 연구의 목적은 후천성 뇌손상 환자의 집행기능 장애에 대한 메타인지중재 효과를 연구한 무작위 임상대조군연구를 대상으로 체계적 문헌연구를 실시하여 중재 내용을 추출하고 결과를 분석하여 최신의 근거를 알아보기 위함이다.
연구방법 : 본 연구는 검색 엔진으로 PubMed, Excerpta Medica Database(EMBASE), Cumulated Index in Nursing and Allied Health Literature(CINAHL), 한국학술지인용색인(Korea Citation Index; KCI), 한국학술정보(Korean Studies Information Service System; KISS), 한국교육학술정보원(Research Information Sharing Service; RISS)를 사용하여 2015년 1월부터 2022년 11월까지 출판된 국내ㆍ외 문헌 을 검색하였다. PRISMA checklist에 따라 문헌을 선택하여 최종적으로 7편의 문헌을 분석하였고, PICOS 방식을 사용하여 결과를 제시하였다. 최종 선정된 문헌은 PEDro scale을 사용하여 연구의 질적 수준을 평가하 였다.
결과 : 검색된 611편의 문헌 중 7편(n = 347)이 최종적으로 선정되었고, 연구의 질적 수준은 7편 모두 ‘좋음’ 이었다. 집행기능과 일상생활활동 및 참여 평가에서 중재 이후 그룹간 유의미한 차이가 있었던 중재는 Cognitive Orientation to daily Occupational Performance, Error-Based Learning이었다.
결론 : 본 연구는 후천적 뇌손상 환자를 대상으로 한 메타인지중재 연구들 중, 잘 설계된 무작위임상대조 군연구를 선정하고 분석하여 중재에 대한 체계적 문헌연구의 근거수준을 높였다는 점에서 임상적인 의 미가 있다. 그러므로 후천성 뇌손상 환자의 집행기능 장애를 다루기 위한 메타인지중재를 제공하는 과정 에서 기초자료로 활용될 수 있을 것이다.
Brassica oleracea var. italica (broccoli) is a type of cabbage that contains vitamins, minerals, and phytochemicals. Consequently, it is used as a potential nutraceutical source for improving human health by reducing oxidative stress and inflammatory responses. Here, the effects of broccoli sprout extract (BSE) on the inflammatory response were investigated through lipopolysaccharide (LPS)- induced inflammatory mouse models. First, we found that the BSE obviously reduce NO production in RAW 264.7 cells in response to LPS stimulation in in vitro study. Pretreatment with BSE administration improved sperm motility and testicular cell survivability in LPS-induced endotoxemic mice. Additionally, BSE treatment decreased the levels of the pro-inflammatory cytokines TNF-a, IL-1β, and IL-6, and COX-2 in testis of LPS-induced endotoxemic mice models. In conclusion, BSE could be a potential nutraceutical for preventing the excessive immune related infertility.
Background: Low back and neck pain are two of the most common medical problems in the adult population. It is estimated that between 70% and 80% adults experience an episode of low back pain at least once during their lifetime.
Objects: This study aimed to compare the effects of various stretching exercises and muscle energy techniques used for treatment of iliopsoas, quadratus lumborum, and hamstrings of patients with low back pain.
Methods: A total of 52 subjects were randomly assigned to the control group (n = 17), the muscle energy technology group (METG, n = 19), and the stretching exercise group (SEG, n = 16). Interventions were performed twice a week for 4 weeks. Outcomes were measured before and after 4 weeks of treatment in the three groups, using the Korean version of the Oswestry Disability Index, the Visual Analogue Scale (VAS), the Fear Avoidance Belief Questionnaire (FABQ), the Finger to Floor test (FTF), the Modified-Modified Schober Test (MMS), and the Trunk Flexion Range of Motion test.
Results: This study showed significant reduction in the pre-test and post-test KODI, VAS, and FABQ scores in all the three groups (p < 0.05). The KODI, VAS, FABQ and FTF assessed in this study showed interactions between the groups and the measurement time point (p < 0.01). After intervention, the KODI, VAS, FABQ and FTF in the SEG and METG were significantly higher (p < 0.05), and there was no difference between the METG and SEG. MMS and HFROM test showed no difference between the three groups (p > 0.05).
Conclusion: KODI, VAS, FABQ and FTF showed significant improvement after basic physical therapy, muscle energy technique, and stretching exercise intervention. And there was no significant difference in the intervention effect between the muscle energy group and the stretching exercise group.
Alzheimer’s disease (AD) is an irreversible and progressive neurodegenerative disease accompanied by aging, followed by memory impairment and cognitive decline. Although numerous attempts have been made to develop treatments for AD, most clinical trials have failed to delay or stop the progression of AD. Electroacupuncture (EA) is a complementary alternative medicine technique widely used to treat pain, inflammation, and neurodegenerative diseases. Additionally, blood-brain barrier (BBB) disruption is a known pathophysiology of neurodegenerative diseases, including AD. Moreover, amyloid beta deposition increases BBB permeability and produces inflammatory cytokines induced by glial activation. However, our previous study revealed that EA treatment at the Taegye acupoints (KI3) improves memory impairment through anti-neuroinflammation and increases glucose metabolism in 5XFAD mice. Therefore, we evaluated whether EA treatment at KI3 regulates BBB dysfunction in the prefrontal cortex of 5XFAD mice. For this study, 6.5-month-old 5XFAD mice were treated with EA stimulation at KI3 three times a week for two weeks. Western blotting, immunohistochemistry, and flow cytometry were used to evaluate the effects of EA treatment on BBB dysfunction. We found that EA stimulation attenuates BBB integrity by protecting BBB tight junction proteins (CD31, aquaporin 4, occludin, and claudin 5) in the prefrontal cortex of 5XFAD mice. In addition, EA treatment regulated inflammatory cytokines (IL-1α, IL-1β, IL-17, IL-23, IFN-ɣ, monocyte chemoattractant protein 1 (MCP-1), granulocyte-macrophage colony stimulating factors [GM-CSF], and IL-10) in the peripheral circulation of 5XFAD mice. Therefore, our data suggest that EA treatment could be a therapeutic agent for enhancing BBB dysfunction in AD.
Nitric oxide (NO)-induced protein S-nitrosylation triggers mitochondrial dysfunction and was related to cell senescence. However, the exact mechanism of these damages is not clear. In the present study, to investigate the relationship between in vitro aging and NO-induced protein S-nitrosylation, oocytes were treated with sodium nitroprusside dihydrate (SNP), and the resultant S-nitrosylated proteins were detected through biotin-switch assay. The results showed that levels of protein S-nitroso thiols (SNO)s and expression of S-nitrosoglutathione reductase (GSNOR) increased, while activity and function of mitochondria were impaired during oocyte aging. Addition of SNP, a NO donor, to the oocyte culture led to accelerated oocyte aging, increased mitochondrial dysfunction and damage, apoptosis, ATP deficiency, and enhanced ROS production. These results suggested that the increased NO signal during oocyte aging in vitro, accelerated oocyte degradation due to increased protein S-nitrosylation, and ROS-related redox signaling.
목적 : 마이봄샘기능저하증(meibomian gland dysfunction, MGD)을 수반하는 염증성 건성안의 감별진단에 대한 ‘TNF-α, IL-6, MMP-9’ 검사키트의 유용성을 평가하였다.
방법 : 건성안 이외의 안질환이 없는 20~30대 중 OSDI 설문 검사에 따른 건성안 총 118안을 대상하였고, 결막낭 메니스커스로부터 소량의 눈물을 채취하여 TNF-α, IL-6 및 MMP-9 검사를 하였다. 각막염색과 결막충혈 이 모두 Grade 1 이상인 경우는 염증성 건성안으로, 마이봄샘폐쇄와 마이봄샘구멍막힘이 모두 grade 1 이상인 경우는 MGD 관련 건성안으로 평가하였다. 염증성 건성안 및 MGD와 TNF-α, IL-6, MMP-9과의 상관성은 카 이제곱검정(Chi-square test)으로 분석하였고, ‘TNF-α, IL-6, MMP-9’ 검사키트의 염증성 건성안과 MGD를 수반하는 염증성 건성안 감별능력은 ROC 커브를 이용하여 민감도, 특이도 및 AUC(Area under the curve)를 구하고 정확도를 평가하였다.
결과 : 염증성 건성안은 TNF-α와 IL-6와 유의한 상관성을 보였고(p<.050), ‘TNF-α, IL-6, MMP-9’ 검사 키트는 MMP-9 검사키트와 80.20%의 높은 일치도를 나타냈으나(p<.050), 염증성 건성안 감별에 대한 민감도, 특이도, 정확도는 MMP-9 검사키트보다 낮았다. MGD는 MMP-9 검사와 상관성을 보이지 않았고, TNF-α와 IL-6 검사와는 유의한 상관성을 보였으며, MGD 감별에 대한 민감도, 특이도, 정확도는 각각 85.50%, 34.70%, 0.601, 85.50%, 32.70%, 0.591로 나타났다. MGD 수반한 염증성 건성안 감별에 대한 ‘TNF-α, IL-6, MMP-9’ 검사키트의 민감도, 특이도 및 정확도는 100.00%, 34.10%, 0.670로 MMP-9 검사키트보다 더 높았다.
결론 : MGD 진단에는 TNF-α, IL-6 검사가 유용하며, ‘TNF-α, IL-6, MMP-9’ 검사키트는 MGD를 수반한 염증성 건성안 평가에 유용할 것으로 사료된다.
Background: Thoracic spine self-mobilization exercise is commonly used to manage patients with neck pain. However, no previous studies have investigated the effects of thoracic spine self-mobilization exercise alone in patients with chronic neck pain.
Objects: The purpose of this study was to investigate the effects of thoracic self-mobilization using a tool on cervical range of motion (ROM), disability level, upper body posture, pain and fear-avoidance beliefs questionnaire (FABQ) in patients with chronic neck pain.
Methods: The subjects were 49 patients (21 males, 28 females) with chronic neck pain. The subjects were randomly divided into an experimental group (EG, n = 23) and control group (CG, n = 26). For the EG, thoracic self-mobilization was applied. We placed a tool (made with 2 tennis balls) under 3 different vertebral levels (T1-4, T5-8, T9-12) of the thoracic spine and the subjects performed crunches, which included thoracic flexion and extension in supine position. Five times × 3 sets for each levels, twice a week, for 4 weeks. Cervical pain, disability, upper body posture, FABQ results, and ROM were evaluated at baseline, after 4 weeks of intervention, and at 8 weeks of follow-up. Assessments included the quadruple visual analogue scale (QVAS); Northwick Park neck pain questionnaire (NPQ); craniovertebral angles (CVA), forward shoulder angle (FSA) and kyphosis angle (KA) measurements for upper body posture; FABQ and cervical ROM testing.
Results: The EG showed a statistically significant improvement after intervention in the QVAS (–51.16%); NPQ (–53.46%); flexion (20.95%), extension (25.32%), left rotation (14.04%), and right rotation (25.32%) in the ROM of the cervical joint; KA (–7.14%); CVA (9.82%); and FSA (–4.12%).
Conclusion: These results suggest that, for patients with chronic neck pain, thoracic selfmobilization exercise using a tool (tennis balls) is effective to improve neck pain, disability level, the ROM, and upper body posture.
Background: The prevalence of neck pain and neck dysfunction is high in general population. However, there is little literature on the relationship and factors affecting neck pain and neck dysfunction. Objective: To investigate the correlation between neck and shoulder pain, headaches, neck and shoulder dysfunction, and sleep quality in adults with chronic neck pain, and factors affecting neck pain and neck dysfunction.
Design: Cross-sectional study
Methods : The sample included 114 subjects, who had complained of chronic neck pain for more 12 weeks. We conducted a Pearson’s correlation between neck and shoulder pain, headaches, neck and shoulder dysfunction and sleep quality and a regression analysis of the related variables, thereby analyzing factors affecting neck pain and neck dysfunction.
Results : In the present study, in adults with chronic neck pain, neck pain was positively correlated with the Neck Disability Index (NDI), Shoulder Pain and Disability Index (SPADI)-Pain score, and SPADI-Total score (p<.05). The NDI was positively correlated with neck pain, SPADI-Pain score, and SPADI-Total score, as well as with Pittsburgh Sleep Qulity Index(PSQI-K) (p<.05). Among the factors affecting neck pain, shoulder disability as assessed by the SPADI was a significantly associated with neck pain, while shoulder pain and shoulder disability determined by the SPADI were identified as significant variables among the factors affecting neck disability.
Conclusion : These results indicated that as neck pain worsened , shoulder pain and neck and shoulder dysfunction also increased, which suggested that shoulder disability affected neck pain. In addition, as the neck dysfunction increased, neck pain and shoulder pain and shoulder dysfunction increased, and sleep quality deteriorated, which suggested that shoulder pain and shoulder disability affected neck disability.
목적 : 본 연구는 대한작업치료학회지를 중심으로 국내 작업치료사들의 정신적 기능장애를 대상으로 한 작업치료 중재들을 체계적으로 알아보고자 한다.
연구방법 : 대한작업치료학회지를 중심으로 창간연도인 1993년부터 2017년까지 총 653편의 연구 중 정신적 기능장애를 대상으로 한 실험연구를 분석 대상 연구로 정하였으며 23편을 선정하였다. 분석 대상 연구는 Arbesman, Scheer와 Lieberman(2008)에 의해 개발된 연구의 질적 근거 수준에 의해 분류하고 PICO(Patient, Intervention, Comparison, Outcome)에 의해 정리하였다.
결과 : 질적 근거 수준은 Ⅲ(34.8%), Ⅳ(34.8%)으로 가장 많았다. 정신적 기능장애 대상자는 치매(74.0%)로 가장 많았고, 다음으로 지적장애(13.0%), 조현병(13.0%)으로 나타났다. 정신적 기능장애에 대한 중재로는 신경인지치료중재가 14 편(60.9%)으로 가장 많이 이루어졌으며, 환자중심중재는 4편(17.4%), 직업훈련과 교육이 3편(13.0%), 환경조정 2편 (8.7%)순으로 나타났다.
결론 : 본 연구를 통해 작업치료사의 업무범위 확대에 따라 정신적 기능장애에 대한 작업치료의 중재 방법과 그 효과에 대해 확인할 수 있었다. 따라서 작업치료사에게 임상적 근거를 제공하였으며, 법적인 근거 확립과 함께 다양한 연구진행 을 통해 작업치료사가 정신보건분야에 적극적으로 참여할 수 있도록 해야 한다.
This study aimed to identify specific psychological and brain activation responses relating to the processing of negative emotions in patients with alcohol dependency. The authors hypothesized that patients with alcohol dependency would demonstrate the abnormal functioning of brain regions involved in negative emotions. Eleven male patients diagnosed with alcohol dependence in an inpatient alcohol treatment facility and 13 social drinkers with similar demographics were scanned using functional magnetic resonance imaging (fMRI) as they viewed film clips that evoked negative emotions. During exposure to negative emotional stimuli, the control group evinced significantly greater activity in the right anterior cingulate cortex (ACC) in comparison to patients with alcohol dependency. Correlation analyses demonstrated a negative association in the relationship between beta values from the right ACC and amygdala in participants classified in the control group. No statistically significant relationship was observed for blood oxygenation level-dependent (BOLD) changes between the two regions in the patient group during the elicitation of negative emotions. On the other hand, patients exhibited a greater activation of the amygdala as negative emotions were induced. These results suggest that alcoholism presents pathophysiology of brain activation that is distinct from the responses of healthy individuals functioning as controls.
This study aimed to determine the effect of depression between the healthrelated quality of life and pain in elderly persons with physical disabilities. A total of 111 patients who were treated at hospital B (Seoul, Daejeon, Daegu, Gwangju, and Busan) participated in the Survey. The SF-36 Health Assessment was used to determine the quality of life of subjects, the Korean version of the Geriatric Depression Scale-Short form to assess the level of depression, and Numeric Rating Scale to determine the level of pain. To determine the moderating effect of depression on the relationship between quality of life and pain, simple regression analysis, and the Sobel test were performed. There was a significant negative correlation between health-related quality of life and pain (r<-.3, p<.05), and a significant positive correlation between depression and pain (r=.251, p<.05). Thus, health-related quality of life had a simple regression relationship with depression and pain. Depression also showed a mediating effect between health-related quality of life and pain. The results of this study suggest that depression mediates between pain and quality of life.
The purpose of this study was to examine the effects of Maca water and/or ethanol extract on the nitric oxide (NO) production in human umbilical vein endothelial cells HUVAC and on erectile dysfunction in rats. Maca was extracted due to both solutions, which are water and ethanol. Each Maca extract was applied to HUVAC, and NO production was checked. Additionally, three different dosages (250, 500 and 1,000 mg/kg) of Maca ethanol extract was administered to Sprague-Dawley rats for 4 weeks. All rats were sacrificed and each sample was collected for analysis. The control rats received only the saline vehicle. The NO production of HUVAC was significantly increased by domestic and homemade Maca water extracted at 60℃ group. Both NO generation and testosterone release were not influenced due to the oral administration of Maca. In the EtOH group rats, the number of sperm was reduced compared to that of the control group. All Maca groups had a high number of sperm and each sperm count had increased as a result of the Maca extract dose. The results of this research suggest that Maca has a positive effect on male erectile dysfunction, which need to be examined further in future studies.
Sperm adhesion molecule 1 (SPAM1) and Hyaluronidase 5 (HYAL5) has been well-known as assistants for sperm penetrate through the cumulus mass surrounding the ovulated eggs. However, so far their role in mammalian fertilization remain elusive, because mouse sperm lacking SPAM1 or HYAL5 were still capable of penetrating the cumulus mass despite a delayed dispersal of cumulus mass. Those data collectively demonstrated that SPAM1 or HYAL5 deficiency alone was not sufficient to cause male infertility in mice. In the present study, SPAM1 and HYAL5-simultaneous deficient male mice model was generated. Because of inhibition in sperm hyaluronidases, SPAM1 and HYAL5-deficient male mice produced significantly smaller numbers of offspring than hetero type and wild type mice.
Hyaluronic acid degradation assay and cumulus oocyte complex dispersal assay as well as sperm motility assay using double knock out sperm and extracts had severe adverse effects on the dispersal of cumulus oocyte complex, which was the main reason for the impaired fertility of double knock out male sperm. Moreover, hyaluronic acid degradation assay using human sperm extracts revealed that sperm hyaluronidase has a principal role in sperm penetration through the cumulus oocyte complex. In conclusion, our results suggest that sperm hyaluronidase deficiency may be sufficient to cause male sterility in mammal because SPAM1 and HYAL5 deficiency sperm not impaired the sperm motility in hyaluronic acid but also cumulus oocyte complex penetration.
The purpose of this study was to identify the effects of Pilates and Tai Chi as physical therapy interventions for pain and dysfunction in chronic lumbago patients. A total of 31 females who were at least 20 years old and diagnosed with chronic lumbago were divided into a Pilates group(PG) (n=10), Tai Chi group(TCG)(n=11), and conservative group(CG)(n=10), to measure their pain and dysfunction before and after a six week intervention. With regard to the changes in the lumbago consciousness scale before and after the six-week intervention, all three groups showed statistical significance(p<.05). The intergroup differences were significant between the PG and CG and between the TCG and CG. All three groups also showed statistical significance(p<.05) in the Oswestry Disability Index before and after the six-week intervention. Again, the intergroup differences were significant between the PG and CG and between the TCG and CG. Based on these results, the application of Pilates and Tai Chi with conservative treatment may be effective in reducing pain and improving dysfunction in chronic lumbago patients.
Background: Chronic back pain shows a high correlation with lumbar disability, physical disability for daily activities, and psychosocial factors, such as depression. Object: The purpose of this study was to examine the correlation of the level of pain and disability with psychosocial factors, which are potential disturbance variables, in patients with chronic lumbar pain. Method: The sample included 258 patients, who had complained of chronic lumbar pain for more than three months. The Quadruple Visual Analogue Scale was used to measure the level of pain, and a Korean version of Oswestry Disability Index was used to measure the level of disability. Psychosocial factors were measured using the Tampa scale for Kinesiophobia-11, Fear Avoidance Beliefs Questionnaire, and Pain Self-Efficacy Questionnaire. The collected data was analyzed using PASW 18.0, and an independent samples t-test was used to examine frequency, percentage, mean, and standard deviation of sociodemographic characteristics and major variables. Pearson’s correlation coefficient was used to investigate the correlation between the level of pain and disability and psychosocial factors. Stepwise multiple regression analysis was done to determine the level of pain and psychosocial factors of functional disorder. The significance level was set at α=.05. Result: There is a strong correlation between the level of pain and functional disorder and psychosocial factors in patients with chronic lumbar pain. The study also revealed that as the levels of pain and fear avoidance increase, pain self-efficacy decreases. Conclusion: The results suggest that negative perceptions towards pain, limitations of physical movement, and severe fear avoidance directly affect the decrease in pain self-efficacy. Therefore, it is recommended to test pain self-efficacy when measuring the level of pain and disability in patients with chronic low back pain.
