        When a fire breaks out in a distribution center, it causes a lot of damage. And the most casualties are caused by Fire accidents. Therefore, training for fire prevention should be mandatory at the distribution center. Also, the contents of education should be different in room temperature warehouses and low temperature warehouses. Fire education in low-temperature warehouses should be more emphasized. This is because many fires occur in low-temperature warehouses. In this study, a study was conducted to determine the important order of training hours and contents for fire prevention education according to the type of distribution center. The importance of time and content for safety education in all types of warehouses did not differ significantly. It was first decided that safety prevention training should be conducted periodically in all types of warehouses

        This study analyzed the factors affecting the effectiveness of the Chemical Hazard Risk Management (CHARM). A survey was conducted on 104 learners who participated in the Risk Assessment training course at Occupational Safety and Health Training Institute. Through a self-administered questionnaire, the effect of personal characteristics, corporate characteristics, and safety and health level of the company on the effectiveness of chemical risk assessment was investigated. As a result of statistical analysis, the safety and health level of the company had a positive (+) effect on the effectiveness of Chemical Hazard Risk Management(CHARM), but personal characteristics and corporate characteristics had no relation to it. This study can be used as basic data for further research related to chemical risk assessment in workplaces.

        Working during decommissioning of nuclear facilities can subject workers to a number of industrial health and safety risks. Such facilities can contain hazardous processes and materials such as hot steam, harsh chemicals, electricity, pressurized fluids and mechanical hazards. Workers can be exposed to these and other hazards during normal duties (including slips, trips and falls, driving accidents and drowning). Industrial safety accidents, along with their direct impacts on the individuals involved, can negatively affect the image of nuclear facilities and their general acceptance by the public. Industrial safety is the condition of being protected from physical danger as a result of workplace conditions. Industrial safety program in a nuclear context are the policies and protections put in place to ensure nuclear facility workers are protected from hazards that could cause injury or illness. Preventive actions are those that detect, preclude or mitigate the degradation of a situation. They can be conducted regularly or periodically, one time in a planned manner, or in a predictive manner based on an observed condition. Corrective actions are those that restore a failed or degraded condition to its desired state based on observation of the failure or degradation. In industrial safety, the situations or conditions of interest are those observed via the performance monitoring, investigations, audits and management reviews. Preventive and corrective actions are those designed to place or return the system to its desired state. Preventive actions where possible are preferred as they eliminate the adverse condition prior to it occurring. When an accident or incident occurs, the primary focus is on obtaining appropriate treatment for injured people and securing the scene to prevent additional hazards or injuries. Once the injured personnel have been cared for and the scene has been secured, it is necessary to initiate a formal investigation to determine the extent of the damage, causal factors and corrective actions to be implemented. Certain tools may be needed to investigate such incidents and accidents. Initial identification of evidence immediately following the incident includes a list of people, equipment and materials involved and a recording of environmental factors such as weather, illumination, temperature, noise, ventilation and physical factors such as fatigue and age of the workers. The five Ws (what, who, when, where and why) are useful to remember in investigation of incidents and accidents.

        Glass fiber, which was used as an insulation material in pipes near the steam generator system of nuclear power plants, is brittle and the size of crushed particles is small, so glass fiber radioactive waste (GFRW) can cause exposure of workers through skin and breathing during transport and handling accidents. In this study, Q-system which developed IAEA (International Atomic Energy Agency) for setting the limit of radioactivity in the package is used to confirm the risk of exposure due to an accident when transporting and handling GFRW. Also, the evaluated exposure dose was compared with the domestic legal effective dose limit to confirm safety. Q-system is an evaluation method that can derive doses according to exposure pathway (EP) and radioactivity. Exposure doses are calculated by dividing into five EP: QA, QB, QC, QD, and QE. Since the Q-system is used to set the limit of radioactivity that the dose limits is satisfied to nearby workers even in package handling accidents, the following conservative assumptions were applied to each EP. QA, QB are external EP of assuming complete loss of package shielding by accident and radiation are received for 30 minutes at 1 m, QC is an internal EP that considers the fraction of nuclides released into the air and breathing rate during accident, and QD is an external EP that skin contamination for 5 hours. Finally, QE is an internal and external EP by inert gases (He, Ne, Ar, Kr, Xe, Rn) among the released gaseous nuclides, but the QE pathway was excluded from the evaluation because the corresponding nuclide was not present in the GFRW products used for evaluation. In this study, the safety evaluation of GFRW was performed package shielding loss and radioactive material leakage due to single package accident according to assumption of four pathways, and the nuclide information used the average radioactivity for each nuclide of GFRW. As a result of the dose evaluation, QA was evaluated as 2.73×10−5 mSv, QB as 1.06×10−6 mSv, QC as 7.53×10−3 mSv, and QD as 2.10×10−6 mSv, respectively, and the total exposure dose was only 7.56×10−3 mSv, it was confirmed that when compared to the legal limits of the general public (1 mSv) and workers (20 mSv) 0.756% and 0.038%, respectively. In this study, it was confirmed that the legal limitations of the general public and workers were satisfied evens in the event of an accident as a result of evaluating the exposure dose of nearby targets for package shielding loss and radioactive material leakage while transporting GFRW. In the future, the types of accidents will be subdivided into falling, fire, and transportation, and detailed evaluation will be conducted by applying the resulting accident assumptions to the EP.

        예부선의 운항은 일반적으로 자항능력을 갖추고 있지 못한 부선과 예인선을 결합하여 운항하는 해상운송의 한 형태로 해양안 전심판원이 해마다 발행하는 재결사례집에 실린 재결을 분석하여보면 지난 5년간의 해양사고 6백여 건 중 예인선은 65척, 부선은 총 69척 에서 사고가 발생한 것으로 나타났다. 본 논문은 예부선 사고의 저감 대책을 제안하기 위하여, 예부선의 운항 형태와 운항현황을 알아보 고, 재결에 나타난 주요 사고 형태별 사고방지 교훈 내용을 분석하였다. 이를 통하여 인적요소가 해양사고의 원인으로서 차지하는 비율이 크다는 것을 확인할 수 있었다. 또한 본 연구는 사고방지를 위하여 예부선 운항자들에 대한 관련 내용의 효율적인 홍보와 교육을 위한 방 안으로서 예부선 협회를 통해 홍보하는 방안과 한국해양수산연수원의 예인선직무교육에 교육자료 형태로 제공하는 방안, 한국해운조합 을 통해 홍보물을 배포하는 방법 등을 제안하였다. 이러한 연구 결과는 예부선 사고에서 나타난 사고방지 교훈을 본 연구에서 제안한 홍 보방식을 통하여 효과적인 교육을 실현하여 예부선 사고의 저감에 활용될 것으로 기대된다.

        In the past, measures for the reduction of industrial accidents has led the safety and health policy of construction sites by the guidance, supervision and the regulation according to「Occupation Safety and Health Act」handled by Ministry of Employment and Labor (MOEL). Nowadays companies realize that due to the factors such as diversification of employment types, outsourcing of hazardous work, new construction method and complicated types of construction, etc., there is a limit on safety management for construction sites as far as it is passively led by the governmental safety management policy and regulation. In this study, in order to practice the autonomous safety management escaping from the government-led safety management, improvement plans are proposed and applied by analyzing the basic cause of the all kinds of fall accidents that could possibly occur in the apartment construction sites and by verifying the hazardous factors and risks by fields.

        In order to collect basic data for the improvement of fishing systems in coastal improved stow net fishery, a questionnaire survey and on-site hearing were conducted from May 10 to June 11, 2019 on the basic items of coastal improved stow net fishery and safety accidents that occurred during fishing operation. The questionnaire for the survey on the actual conditions of coastal improved stow net fishery consisted of a survey on basic matters (six questions) and a questionnaire (six questions) on safety accidents occurring during fishing operation. The results of the survey on basic items were analyzed by region (Incheon, Seocheon, Gunsan and Mokpo), by the captain’s age (less than 50 years of age, 50 to 60 years and more than 60 years of age), by the captain’s career (less than 20 years, 20 to 30 years, 30 to 40 years and more than 40 years) and by the age of fishing vessel (less than 10 years, 10 to 20 years and more than 20 years). According to the survey on basic items of coastal improved stow net fishery such as the captain’s age, the captain’s career, the age of fishing vessel, the fishing nets in use, the crews on board and the operation days per voyage by region, the average captain’s age was 55.7 years, the average captain’s career was 20.5 years, the average age of fishing vessels was 9.0 years, the average numbers of nets used by fishing boats was 14.0 sets, the average numbers of crew on board a fishing boat was 4.4 persons and the average numbers of operation days per voyage was 4.9 days (p < 0.05). As a result of the survey on safety factors during fishing operations, such as experience of ship accidents, major causes of ship accidents experienced, causes of ship accidents (first priority), experience of human accidents, major causes of human accidents, and causes of human accidents (first priority), more than 96% of the respondents experienced ship accidents including collisions with other vessels or fishing gear during fishing operations. The most significant cause of the accident was the other’s fishing gear installed in the fishing grounds. The first possible causes of ship accidents during fishing operations were found to be other fishing gear installed in fishing grounds, steering or engine failure, and inability to avoid accidents during casting and hauling nets. The survey of the experience of human accidents, such as injuries or sea falls, showed that more than 90% of the respondents experienced human accidents during fishing operations. The most important cause of accidents experienced during fishing operations was stucked in a fishing gear during casting and hauling nets. The first important causes of accidents during fishing operations were movement of the fishing gear during casting and hauling nets, damage of the fishing gear such as rope cutting. The results are expected to be provided as a basic data to prevent safety accidents occurring during fishing operation and improve the fishing system in the coastal improved stow net fishery.

        Purpose: The purpose of this study was to describe the nursing unit manager's experience with patient safety accidents. Methods: Data were collected from April, 2017 to December, 2017 through in-depth interviews with seven unit managers who worked in General wards, OPD or in the ICU of a general hospital. Qualitative content analysis method was used to analyze the data. Results: The following four categories were elicited; dimensions are different from each other, complex feelings about the person after the accident, ambivalence for the accident triggers, leadership learned from accident management. Conclusion: The findings provided valuable informations on the nursing unit managers' experience with patient safety accidents, which held many nursing implications. Based on the findings, it is possible to develop accident management guidelines and the support system for accident management personnels.

        The aim of this study was to investigate status of safety accidents of school foodservice cooks in Daegu and to analyze the factors affecting the occurrence of the accidents in order to seek effective ways for preventing safety accidents in school foodservice. The survey showed that the most frequent safety accidents were ‘bruises’, followed by ‘burns’, ‘contact with harmful substances such as disinfectants’, ‘fall-off ’ and ‘sprains’. The mental fatigue perception of the respondents was generally lower than the physical fatigue perception. The means of the perception levels of work intensity, cooking environment of the foodservice place, and safety-related behaviors, and consciousness were 3.15, 2.99, and 4.06 out of 5 points, respectively. In addition, the annual average of the number of participating in the accident prevention training per person was 17.34 times, that is, the respondents received the training at least once a month on average. A multiple regression analysis was conducted to investigate the variables affecting the occurrence of safety accidents that happened to foodservice cooks. It revealed that the work intensity perception and the cooking environment perception influenced the frequency of safety accidents.

        우리 조선업은 기술적인 분야에서 세계를 선도하는 역할을 수행하고 있으나 조선업에서는 매년 2,000여명의 재해자가 발생하 고 있으며, 이중 약 40명은 사망하고 있는 등 근로자의 안전측면에서도 과연 우리 조선업이 세계적인 수준인지는 의문이 들 수밖에 없다. 따라서 이 연구는 국내 조선업에서 2006년~2015년 동안 발생한 중대재해의 통계자료를 고찰하여 조선업 중대재해의 발생현황 및 원인을 분석하고, 조선업 근로자를 대상으로 설문조사를 실시하여 산업현장에서의 안전교육 형태를 분석하였으며, 이를 바탕으로 조선업 안전교 육의 문제점을 도출하여 개선방안을 마련하였다. 첫째, 조선소별로 수행하는 안전교육의 종류 및 수준이 상이하거나 교육품질이 미흡한 문제점을 해결하고 체계적으로 교육과정을 운영하기 위해서 조선업 안전훈련 표준체계의 마련이 필요하다. 둘째, 조선업 안전훈련 표준 체계를 바탕으로 조선업에 특화된 안전교육과정을 지속적으로 개발하고 표준화하여 조선업 중대재해를 감소시키고 근로자의 안전의식을 향상시킬 필요가 있다. 셋째, 조선업에서의 중대재해를 예방하여 근로자의 안전을 확보하기 위해서는 사업주와 근로자 모두가 적극적으 로 안전교육을 수행하고 참여하여 사업장의 안전문화를 형성할 필요가 있다.

        중·저준위 방사성폐기물 처분시설의 운영 중 사고로 인한 방사선적 영향을 평가하기 위해서는 운영 중 발생 가능한 사고 에 대한 타당성이 입증되어야 한다. 본 논문에서는 처분시설의 운영 중 사고분석 체계를 처분시설의 구성요소에 대한 안전 기능분석, 잠재위험요소분석, 위험도분석, 그리고 향후 조치대안으로 사고평가체계를 개선하였다. 이를 위하여 위험도분석 에 필요한 설계대안과 관리대안을 추가하여 설계-운영-평가가 연계되도록 하였다. 또한 운영 중 사고의 발생확률과 평가결 과의 심각성에 따라 운영중 사고에 대한 분류기준을 제안하여 처분시설 운영 중 대표 사고시나리오에 대한 정당성을 확보 하였다. 본 논문의 개선된 평가체계를 우리나라의 2단계 중·저준위 방사성폐기물 표층처분시설에 대한 처분시설 운영 중 사고분석의 사례에 대해 적용하였다.
