목적 : 본 연구는 노인들의 인지기능과 삶의 만족도간의 관계에서 노인구강건강요인의 매개효과를 분석하 기 위해 수행되었다. 연구방법 : 8차 고령화연구패널조사(2020년)에 수집된 60세 이상의 중고령자 6,169명을 대상으로 분석하 였다. 노인구강건강 요인은 노인구강건강평가지수(Geriatric Oral Health Assessment Index; GOHAI)의 기능적 차원 요인, 심리사회적 차원 요인, 통증 및 불편함 요인으로 세분화하였고 각 요인의 매개효과를 Mplus 8.4를 활용하여 경로분석을 수행하였다. 결과 : 6,169명의 평균 연령은 70.77세였다. 인지기능과 삶의 만족도 사이에서 노인구강건강 요인 중 구강건 강관련 기능적 차원 요인(β = 0.207, 95% Confidence Interval; CI = 0.169, 0.207)이 23%로 가장 큰 부분매개효과를 나타났다. 심리사회적 차원 요인은 22%(β = 0.202, 95% CI = 0.171, 0.236), 통증 및 불편함 요인은 16%(β = 0.146, 95% CI = 0.119, 0.175)로 기술한 순으로 부분매개효과를 나타냈다. 결론 : 인지기능과 삶의 만족도 사이에 구강건강 관련 기능적 차원 요인, 심리사회적 차원 요인, 통증 및 불편함 요인이 미치는 영향력을 확인하였다. 따라서 노인의 인지수준을 고려하고, 삶의 만족도에 영향을 미치는 구강건강 요인을 세분화하여 파악한 결과를 통해 개개인의 맞춤형 재활치료 접근 전략을 세우고 노인의 삶의 만족도를 높일 필요성을 제시하고자 한다.
This study investigated the potential role of dietary factors associated with obesity and metabolic syndrome (MetS) in Koreans. The scoping review method was used to evaluate the studies that utilized the secondary data sets comprising the Korean National Health and Nutrition Examination Survey (KNHANES) and the Korean Genome and Epidemiology Study (KoGES). Articles published between 2012 and 2022 were identified using RISS, KISS, DBpia, PubMed, and ScienceDirect databases. In all, there were 32 published articles on obesity and 119 on MetS. Obesity research included eight articles on nutrients, 12 on food items/food groups, two on dietary patterns, nine on dietary behavior/eating habits, and one on the dietary index. MetS studies comprised 34 articles on nutrients, 43 on food items/food groups, seven on dietary patterns, 25 on dietary behavior/eating habits, and 10 on the dietary index. Carbohydrates, alcohol, and coffee consumption were the most frequently studied dietary factors for obesity and MetS. The primary areas of study were largely focused on nutrients and food items/food groups. Thus, to overcome the paucity of information on the relationship of dietary patterns and dietary indexes with obesity and MetS, there is a need for further research using the KNHANES and KoGES data sets.
This study was undertaken to examine body image distortion among female adolescents and identify related factors. Raw data from the 14th Korean Youth Health Behavior Survey were used. The proportion of participants exhibiting body image distortion was 39.5%, with the ratio being higher among female students of normal weight as compared to underweight female students. Logistic regression revealed that the risk of having a distorted body image was higher among high school females than in middle school females having lower subjective academic performance and household economic status. In addition, the risk of having a distorted body image was higher for students who drank alcohol when compared to students who did not drink, for the group engaging vigorously in physical activity (exercise) more than three times per week as opposed to the group exercising less than three times per week, and for the group consuming less than one serving of fruit per day as compared to the group consuming more than one serving of fruit per day. Taken together, the results of this study indicate that continuous nutrition education needs to be provided so that adolescents can correctly perceive their body images and form desirable eating habits.
본 연구는 한국의 다문화 청소년 및 일반 청소년의 건강위험행위와 폭력피해경험을 비교하고 폭력피해 경험과 관련된 요인을 건강위험행위를 중심으로 규명하고자 하였다. 제15차 청소년 건강행태 온 라인 설문조사(2019)의 자료를 사용하여 35,574명의 청소년을 대상으로 하였다. 자료 분석은 SPSS/Win 22.0을 사용하여 복합 표본 분석을 하였다. 연구 결과, 다문화청소년은 일반청소년보다 흡연, 습관적 약물 사용, 폭력피해 경험이 유의하게 더 높았다. 다문화청소년과 일반청소년 모두 가족과 함께 거주하지 않을 수록, 흡연경험과 습관적 약물사용 경험이 많을수록 폭력피해경험이 더 많았다. 일반청소년의 폭력피해경 험은 성별, 학력, 경제적 상태, 음주경험과 유의한 관련이 있었다. 따라서 다문화 청소년과 일반 청소년의 폭력피해 경험을 예방하기 위해서는 가족과 함께 살지 않는 청소년에 대한 사회적 지원이 필요하며, 흡연 및 습관적 약물 사용 경험이 있는 청소년에 대한 세심한 주의가 필요하다. 또한 다문화 청소년은 일반 청소 년에 비해 건강위험 행위와 폭력피해에 더 취약하므로 다문화청소년의 올바른 건강행위 실천과 폭력피해 경험을 예방하기 위한 교육 프로그램이 필요하다.
In this study, data from the 7th (2016~2018) and 8th (2019) Korea National Health and Nutrition Examination Surveys were used, which included 5,325 subjects. Health behavior, dietary and nutrient intake status, physical measurement and biochemical characteristics, and risk factors for elderly related chronic diseases were classified and analyzed according to the changing composition of single households and other households in the current society. As a result, the ratio of current smokers and drinkers in young adult single households, walking less than 30 minutes per day, subjective health status was poor, breakfast rate less than three times per week, eating out frequency more than once a day, lipid intake ratio to total calories, saturation fatty acid intake were significantly higher. In addition, waist circumference, and diastolic blood pressure were significantly higher. However, dietary fiber intake level was significantly lower. The results for hypertension, which is the representative chronic disease that causes old age-related chronic diseases, were significantly higher in single households (ORs=1.400 (95% CI: 1.095, 1.791), p=0.007). Although young adults may not have showed particularly serious health problems yet, education is believed as important to recognize and prevent age-related disease risk factors.
Sodium intake has been associated with the health-related quality of life (HRQoL). This study examined the demographic factors related to HRQoL and whether there is an association between sodium intake and HRQoL in adults residing in rural areas. A total of 1,980 adults (aged ≥20 years) were selected from a cross-sectional study conducted between December 2013 and February 2014 in Gyeongju, South Korea. The sodium intake and HRQoL were measured using a validated food frequency questionnaire and the EuroQol Five-Dimension (EQ-5D) (mobility, self-care, usual activities, pain/discomfort, and anxiety/depression), respectively. The education levels (<5 years) were inversely associated with all five dimensions of HRQoL. Men had better EQ-5D scores for each of the five components examined. Adults living with spouses had better scores related to usual activities and anxiety/depression, whereas widowers had decreased scores of mobility and self-care. Unlike older adults aged 70-79 years, adults aged <60 years were more likely to have greater scores related to mobility, usual activities, and pain/discomfort. The income levels were also inversely related to the scores for self-care, usual activities, pain/discomfort, and anxiety/depression. Sodium intake was positively associated with the HRQoL related to self-care, usual activities, and pain/discomfort, but it was negatively associated with anxiety/depression. In conclusion, these findings suggest that the demographic factors and sodium intake were independently associated with HRQoL. The results highlight the need to provide tailored, targeted policies and interventions that consider personal characteristics and dietary intake to improve the overall HRQoL in adults residing in rural areas.
본 논문은 전후기 여성 독거노인의 건강관련 삶의 질 영향 요인을 알아보고자 하는 서술적 조사 연구로 2017년 <여성 독거노인의 통증, 영양위험, 외로움, 지각된 건강상태가 건강관련 삶의 질에 미치는 영향>인 일차연구 자료와 추가 수집한 자료 총 85부를 이용한 이차분석 연구이다. SPSS/WIN 18.0 program을 이용하여 분석한 결과, 전기 여성 독거노인의 건강관련 삶의 질 영향 요인은 통증, 규칙적인 운동, 사회적지지 순으로 설명력 70.6%이며 후기 여성 독거노인의 건강관련 삶의 질 영향 요인은 규칙적인 운동, 일상생활수행능력, 통증 순으로 건강관련 삶의 질에 영향을 주는 것으로 설명력은 69.7%로 나타났 다. 따라서 통증 감소를 위한 방안과 사회적지지 체계의 강화, 생활체육 활성화 및 그에 따른 일상생활 수행 유지를 위한 노력 등의 모색을 통해 건강관련 삶의 질을 향상 시킬 필요가 있다.
목적: 제 5기 국민건강영양조사(2011년, 2012년) 자료를 기반으로 우리나라 나이관련황반변성(age-related macular degeneration)과 비타민과 무기질의 섭취의 상관성을 분석하였다.
방법: 국민건강영양조사 제 5기 과정 중 2011, 2012년도 조사대상자 중 건강 검진, 안과 검진과 영양검진 조사를 받은 45세 이상 성인을 대상으로 나이관련황반변성과 비타민, 무기질섭취량의 상관성을 분석하였다. 나이관련황반변성의 연관성은 교차분석과 카이제곱 검정으로 분석하고, 위험비(odds ratio)는 로지스틱 회귀분석으로 구하고 유의수준은 0.05 이하로 하였다.
결과: 국민건강영양조사를 받은 45세 이상의 한국인 6,219명 중 나이관련황반변성 유병률은 9.6%로 나타 났다. 초기 나이관련황반변성은 비타민 A, B1, B2, B3 및 C와 칼슘, 인, 철, 나트륨, 칼륨의 섭취와 상관성이 있었으며, 비타민 B3 섭취량이 평균 미만인 경우는 평균이상인 경우보다 초기 나이관련황반변성의 발생 위험이 1.35배(95% 신뢰구간:1.03~1.78) 높았다. 나이관련황반변성 말기 환자 군에서는 칼륨과 비타민 B2 섭취량이 평균보다 적으면 유병률이 높았으며, 칼륨 섭취량이 평균 미만인 경우는 발병 위험이 4.3배(95% 신뢰구간:1.76~10.59) 높았다.
결론: 비타민과 무기질의 섭취는 나이관련황반변성의 발병 위험과 병의 진행을 억제하는 것으로 사료된다.
목적 : 본 연구는 지역사회에 거주하는 뇌졸중 환자의 건강 관련 요인을 Geyh 등(2004)이 개발한 포괄적 ICF(International Classification of Functioning, disability and health) 핵심 지표로 확인하여 웰니스에 영향을 미치는 요인을 파악하고자 하였다. 연구방법 : 지역사회에 거주하는 뇌졸중 환자 157명을 대상으로 총 21주간에 걸쳐 자료를 수집하였다. 대상자의 건강 관련 요인은 포괄적 ICF 핵심 지표(comprehensive ICF core set for stroke)를 사용하여 평가하였고, 웰니스는 한국형 웰니스 생활양식 측정(Korean Wellness Life Style Scale; KWLISS)을 통해 측정하였다. 수집된 자료 중 대상자 선정 기준에 부적합한 21명을 제외한 136명을 최종 분석하였다. 결과 : 웰니스에 영향을 미치는 변인들의 상관관계를 분석한 결과 환경 요인(p<.05)에서 정적인 상관관계가 나타났다. 최종적으로 지역사회 거주하는 뇌졸중 환자의 건강 관련 요인 중 환경(p<.01), 신체 구조(p<.05)가 웰니스를 52% 설명하는 것으로 분석되었다. 결론 : 지역사회에 거주하는 뇌졸중 환자의 건강 관련 요인 중 환경 요인이 웰니스에 영향을 미치는 주요변인으로 분석되었다. 향후 웰니스 지수를 높이기 위한 작업치료 중재 접근 시 환경 요인을 고려해야 할 것이다. 더 나아가 작업 수행을 지지하는 지역사회 자원을 확인하고, 이를 뇌졸중 환자들의 삶에 적용할 수 있는 작업치료 접근이 필요하겠다.
목적: 한국 폐경기 여성에서 호르몬 관련 요인과 건성안의 연관성에 대하여 알아보고자 하였다.
방법: 본 인구 기반 단면 연구는 제5기 국민건강영양조사(2010-2012)를 완료한 총 4,586명의 폐경기 여성을 대상으로 복합표본설계 프로시저를 구성하여 분석을 시행하였다. 폐경기 여성을 대상으로 건성안 유병 률을 산출하였고, 공변량을 보정한 다중 로지스틱 회귀분석을 시행하여 내생적 및 외생적 호르몬 관련요인 과 건성안의 연관성을 평가하였다. 결과: 폐경기 여성에서 건성안의 가중된 유병률은 14.2%(95% 신뢰구간: 13.0-15.8)이었다. 사회 인구학 적 요인, 건강행태 요인, 동반질환을 보정한 후에도 경구피임약을 23개월 이상 복용한 경우 건성안 발생 위 험이 1.47배(95% 신뢰구간: 1.03-2.09) 증가하였으며, 여성호르몬제를 36개월 이상 복용한 경우 건성안 발생 위험이 1.70배(95% 신뢰구간: 1.17-2.47) 증가하였다. 결론: 한국 폐경기 여성에서 장기간의 경구피임약과 여성호르몬제의 복용은 건성안의 위험을 유의하게 증가시켰다.
This study was performed to determine the prevalence and associated risk factors of osteoarthritis in the elderly using the Korea National Health and Nutrition Examination Survey (2013~2014 KNHANES VI). A total of 2,148 (936 males, 1212 females) elderly participants aged 65 years or older were selected. Prevalence of osteoarthritis was higher in 460 female (80.7%) and 167 rural (29.1%) groups, than in 110 male (19.3%) and 404 urban (70.9%) groups. While the mean values of LDL-cholesterol, triglyceride level, and waist circumference were significantly higher in the osteoarthritis group than the normal group, other parameters including height, HDL-cholesterol, fasting blood glucose, hemoglobin and hematocrit values were not. Quality of life (EQ-5D) was also significantly lower in the osteoarthritis group. The results of logistic regression analysis showed that smoking, alcohol intake, obesity, hypercholesterolemia and energy intakes were significantly different in regards to the prevalence of osteoarthritis. This study suggests that obesity and energy intakes were associated with osteoarthritis, whereas risk factors of chronic disease, such as hypertension, Hypo-high density lipoprotein, hypertriglyceridemia, anemia, diabetes, vigorous physical activity, moderate physical activity and walking were not. Long-term prospective study is necessary to investigate the effects of these factors on osteoarthritis.
Although adolescents’ obesity prevalence increases recently in Korea due to Westernized dietary life, according to social and economic development, and physical activity decline, distorted body image and improper dietary habits are prevalent, because of excessive obsession with weight control, deriving from psychological impacts including appearance supremacy. This study conducted a survey on health oriented awareness and attitude related with dietary habits targeting adolescents, such as high school students and college students, younger than 24, from May 11 to May 29, 2015. In this study, 280 questionnaire copies were collected out of 300 distributed questionnaire copies, and used 269 copies as analysis data, except 11 copies of which responses were inadequate. The results of this study are as follows: First, the high school students showed higher dietary regularity than the college students. Especially, 95.5% of the college students showed very high irregularity of lunch, compared with just 4.5% of the high school students. Second, the adolescents showed distorted perceived body image in comparison with body mass index (BMI). As a result of examining BMI relationship, according to one’s own perception on body type, 28.6% of the respondents perceived themselves fat, despite normal BMI, and 40.4% of the respondents perceived themselves normal, despite low BMI. Third, health behavior activities level was higher, as the regularity of breakfast (B=1.093), lunch, (B=0.650), and dinner (B=0.765) was higher. The variable affecting the most (β =0.372) was the regularity of breakfast. As interest in weight control was higher, health behavior activities was lower. Because over-interest in weight control may be linked with improper dietary habits or weight control, a caution is needed. The results of this study are expected to be utilized as the basic data for the policy and health-oriented program development to improve adolescents’ health behavior practice.
목 적: 2011, 2012년도 국민건강영양조사 내용을 기반으로 우리나라 나이관련황반변성(Age-related macular degeneration, AMD)의 위험요인을 분석하고자 하였다.
방 법: 국민건강영양조사 제5기 과정 중 2011년, 2012년도 조사대상자의 건강 검진과 안과 검진을 받은 45세 이상의 성인을 대상으로 AMD와 상관성이 있는 위험요인인 성별과 연령 통제 전과 후로 구분하여 로지 스틱 회귀분석으로 통계처리를 하였으며 유의수준은 0.05 이하로 하였다.
결 과: 인구·보건학적인 요인에서는 성별과 연령을 통제하기 전에는 여성, 연령이 많을수록, 교육정도 가 낮을수록, 읍/면 거주자에서 발병률이 높게 나타났으나, 성별과 연령을 통제한 경우에는 거주지는 상관 성이 없고 여성과 연령이 많을수록, 교육정도가 낮을수록 발병률이 높게 나타났다. 신체·질병학적인 요인 에서는 성별과 연령 통제하기 전에는 고혈압, 중성지방 및 가족력이 위험요인이었으나 성별과 연령 통제 후 에는 모든 요인들이 AMD와 상관성이 없었다. 성별과 연령을 통제하기 전에는 난시, 근시, 원시, 백내장 및 안질환 가족력이 AMD와 상관성이 있었으나, 성별과 연령을 통제 후에는 원시안과 백내장이 AMD 위험요인 으로 나타났고 근시안은 AMD 위험요인이 낮은 것으로 나타났다. 건강·행태학적인 요인에서는 성별과 연령 을 통제하기 전에는 햇볕 노출 5시간 이상만 상관성이 있었으나, 성별과 연령을 통제한 후에는 흡연, 햇볕 노출 5시간 이상이 AMD 발병을 높이는 위험요인으로 분석되었다.
결 론: AMD 발병은 나이와 성별에 연관이 있고, 성별과 연령을 통제한 경우에는 원시, 백내장, 흡연 및 햇볕 노출 시간이 상관성이 있음을 확인하였다.
Objective : The main purpose of this study was to analyze the structural relationship among factors that have an impact on Health-Related Quality of Life (HRQOL) in three groups: A stroke patient group (Exp 1), high-risk stroke patient group (Exp 2), and a normal control group.Methods : For this survey, we conducted a secondary analysis using ‘the fifth Korea national health and nutrition examination survey (2010-2011)’, and investigated 2,293 elderly people over 65 years in age.Results :The results were as follows: HRQOL had a significant difference among groups (control›Exp2›Exp1) (p<.05). In the total effect of the experimental 1 groups, their HRQOL was significantly affected by their mental health (β=.425), self-rated health (β=.391), and medical health (β=.290)(p<.05). In the total effect of the experimental 2 group, their HRQOL was significantly by their self-rated health (β=.425), medical health (β =.272), mental health (β=.249), and functional health (β=.234) (p<.05). In the total effect of the control group, their HRQOL was significantly affected by their self-rated health (β=.398) and mental health (β=.184)(p<.05).Conclusion : In conclusion, this study suggested two matters to improve the HRQOL in people with or without a stroke. First, to improve the HRQOL in people suffering from senility, a different approach is needed according to whether the person has had or not had a stoke. Particularly in stroke patients, we should treat both their physical disability and mental health. Second, regardless of whether they have had a stroke, their HRQOL is influenced strongly by their self-rated level of health, and therefore an improvement of HRQOL will be induced by promoting positive health behaviors.
This study was carried out to analyze the relationship between body mass index (BMI) and general characteristics, psychological factors, eating habits, dietary behaviors, and health related quality of life using survey of 335 women aged 20 to 29 years in the Seoul and Kyungin areas. The 335 study subjects were divided into 4 groups by BMI (㎏/㎡) levels; normal group(18.5≤BMI≤22.9), overweight group(23.0≤BMI≤24.9), mild obesity group(25.0≤BMI≤29.9), and heavy obesity group (BMI≥30) by Asian-Pacific obesity index criteria. The family income of the heavy obesity group was significantly (p<0.05) less than that of the normal weight group. Psychological factors, such as stress and depression of the normal weight group tended to be higher than those of overweight and obesity groups. The higher BMI level had the lower self-efficacy(p<0.05) among all subjects. As the BMI level increased, the preference for sweet, salty, and hot taste was significantly high. We found that normal weight women had healthier eating habits and dietary behaviors and a higher level of health-related quality of life than did those who were overweight, mildly obese, and heavily obese women. Overweight and mildly and heavily obese women were strongly associated with decreased physical and mental health related quality of life. Therefore, weight loss is desirable, and is likely to be beneficial for health-related quality of life in obese adult women. In conclusion, this study contains evidence to suggest that obesity management programs including different strategies according to obesity are required to determine the types of programs that are suitable for adult women, prior to their initiation of a program. The findings are helpful to inform researchers and practitioners who are seeking to implement appropriate strategies to create positive changes in the health behaviors of obese adult women.
The purpose of this study was to estimate the consumption prevalence of vitamin and mineral supplements as health functional foods (VM-HFF) and to examine the factors associated with VM-HFF consumption behaviors in adolescents. A total of 1,407 adolescents attending middle or high schools from various cities and rural communities in Korea participated in this study. The prevalence of VM-HFF consumption was 41.7%, with boys showing a higher consumption than girls (p<0.01). VM-HFF consumption was higher in families with higher socioeconomic status and for families with parents that exhibited higher concerns about their child's health, growth, and nutritional intake (p<0.001). Most consumers of VM-HFF consumed HFFs 'when healthy' (50.1%), and acquired nutritional information from 'their families & relatives' (50.9%). Most consumers responded that VM-HFF was 'a little effective' (54.3%), followed by 'no obvious effects' (37.1%), and 'very effective' (7.4%). The effectiveness of consuming VM-HFF was mainly for 'fatigue recovery' (39.0%) and 'health improvement' (28.2%). Most consumers purchased HFFs at 'pharmacies & oriental medicine clinics' (53.8%) and at 'health functional food stores' (18.8%). Most consumers 'occasionally' (51.1%) or 'seldom' (27.3%) checked nutrition facts when purchasing, with 58.9% of consumers understanding the nutritional label for 'the most part', but only 8.7% of them understanding it 'very well'. Among the VM-HFF, consumers preferred calcium- and vitamin C-supplements. Consumers' mini-dietary assessment scores were higher than those of non-consumers. The results above showed that VM-HFF consumption was widely spread among adolescents, but few consumers checked and understood the nutrition label when they purchased VM-HFF, and were highly dependent on the advice and information from non-professional nutritionists, such as families & relatives. Therefore, it is necessary to educate adolescents to help them read nutrition labels and select the proper VM-HFF.
We surveyed 848 university students, 21.4±2.5 years of age, attending university in Seoul, Kongju, Chongyang-Gun in the Chungnam province of Korea, for their use of health functional foods (HFF) and for significant variables in this use, namely demographic characteristics, health related variables, dietary variables and nutritional beliefs. The prevalence of the use of HFF was 33.6%. Among all types of HFF, nutritional supplements were taken most frequently, followed by (in order) red ginseng products, apricot extract products and ginseng products. HFF use was higher in females (p<.05), in subjects living in a dormitory (p<.05) and those from families with a relative high socioeconomic status (p<.001). Self-health concern (p<.05) and usage ratio of HFF by family (p<.001) was higher in users than in nonusers of HFF. The total score of dietary assessment was higher in users than in nonusers of HFF (p<.001), and users had a more positive point of view concerning the potential health benefits of HFF than did nonusers (p<.001). Most users took HFF when they were healthy (37.2%) or when they were sick (27.4%). They did not feel special effects through the HFF use (49.8%). Most users got the information about the specifics of HFF from family and/or relatives (55.8%). Most of users confirmed nutrition facts when they purchased the HFF (70.9%), but a considerable number of users could not understand these nutrition facts (48.1%). Users preferred vitamin C- and Ca-supplements most among vitamin · mineral supplements belonged to HFF. These results show that the use of HFF is common among university students. As well, the use of HFF by students is affected by various variables. Supplementary nutritional education should be undertaken among university students, in order to give them a reasonable guideline for the use of HFF, based upon influencing factors and usage behaviors that we learned from this survey.
