Objectives : To develop a basic ADL instrument for persons with stroke, reflecting our cultural values. Method : The ADL instruments that have been developed previously were gathered, excluding the ADL items, which lowered the validity of the instrument. We surveyed occupational therapists on the values of basic ADL with a total of thirty-four item cards. The value of ADL was measured by Q sorting technique. And ninety-eight patients with stroke were evaluated to gain final items selection and weighted items. Results : Ten items for inpatients were selected. For inpatients, functional ambulation at indoor was weighted to four times; the excluded items using proper utensils, and cleaning mouth were weighted to two times. Conclusion : We developed a basic ADL instrument for inpatients and the range of score of each instrument was 0∼80. The internal consistency for inpatients was high (Cronbach α=0.94). However, further study requires the concurrent validity, construct validity and the test-retest reliability of the present ADL items.

Objective : This study was conducted to assess the changes in the functional levels of affected upper extremities after treating patients with brain injury by intensive CIMT (Constraint-Induced Movement Therapy). Method : Design was single-subject experimental reversal design (A-B-A). Subjects was 6 persons with brain injury (F: 1, M: 5), mean age (57.0±9.6). Intensive CIMT was performed over 6 hours a day for 2 weeks. Box and block test and nine hole pegboard were used for assessment of affected upper extremity function before and after intervention. Results : Compared baseline 1, functional level in both intervention and baseline 2 revealed improved functional levels. In addition, carry over effect was shown in baseline 2 period. Conclusion : Intensive CIMT based on task-oriented activity is considered to be effective because it decrease the time to use the paretic limb and thereby overcomes the “learned nonuse”.

Purpose : This study investigated the consumer satisfaction of the rehabilitation counseling via internet for parents of children with developmental disabilities aged under 6. Methods : The web site for internet counseling (ddchild.com) consisted of 1,500-page information for educating children at risk, with mental retardation, autism, learning disabilities, and cerebral palsy. Internet counseling was conducted in the form of Q/A, real-time chatting, monthly interview, and search engine. The consumer survey form was dispatched to a total of 1,459 registered members and 91 (7%) of them replied to the survey. The survey included general information of the respondents, the types of disabilities they used mostly, their satisfaction level, criticisms and suggestions to this internet counseling. Results : The general characteristics of the respondents were: (1) The users were distributed across the nation from Seoul and its vicinity (61%) to Cheju (1%); (2) The respondents’ 68% was middle or lower middle class; (3) The types of disabilities were children at risk including learning disabilities and ADHD (37%), autism (21%), mental retardation (21%), cerebral palsy(18.5%). The 68% of the respondents were satisfied with the internet counseling, and 65% actively used the data base consisted of rehabilitation information for educating children with developmental disabilities. Regarding the usage of the data base, 49% replied that this program was very helpful, because the instructions were described with concrete examples, 47.7% “helpful in understanding how to teach these children,” and 27.7% “in better understanding the behaviors of these children.” As to the suggestions, the respondents demanded two things. One is that the government build more schools and centers for educating these children along with more economic support, and the other requests are related to the operation of this internet counseling: enforcing of speech therapy program, opening of demonstration program with regard to the usage of this system, and setting up a psychological counseling for the parents. Conclusion : Operation of the internet counseling system can be a viable approach for providing early intervention programs for parents with developmental disabilities with low-cost effectiveness in the country where early intervention has not been systematically established.

Objective : This paper presents an empirical study on the bias toward the disabled by the students studying occupational therapy, who will make an expert group to take care of them in the future. It was investigated how much sense of distance the students had according to the physical characteristics of the disabled. Method : Total 128 students majoring in occupational therapy were asked to fill out a self-administered questionnaire. The physical characteristics of the disabled and the sense of distance toward the disabled were measured as follows: The visible physical characteristics of the disabled were restricted to the disorders of external physical functions based on the Article 29 ‘Classification of the Disabled’ of the Disabled Welfare Act. The functional characteristics of the disabled were measured by using the FIM (Functional Independence Measure) method. Meanwhile, Westie (1952)’s inventory of the sense of distance was adopted for the inventory for the sense of distance toward the disabled in the study. Results : The subjects showed the biggest sense of distance toward the disabled in the order of the disabled with encephalopathy, physical handicap, language disorder, visual disturbances and acoustic disturbances. As for the functional characteristics, the sense of distance was the greatest in the order of the completely dependent disabled, partially dependent disabled, and completely independent disabled. The average sense of the distance was compared in terms of the subjects’ school and year, and the results revealed there were statistically significant differences in all the areas except for the sense of distance toward the completely dependent disabled. Conclusions : The sample needs to be expanded in number, and random sampling is required to secure diverse social and cultural backgrounds. In addition, the causality verification should be done in order to understand the nature of sense of distance in a study on it according to the physical characteristics of the disabled. Finally, an evaluation tool needs to be developed that can be used to measure the sense of distance toward the disabled.

The purpose of this study was to investigate whether training of writing a daily work log using cognitive therapy approach can help increase work performance in a patient with depression using a single subject experimental reversal design (ABA). Method : The patient is a 19-year male youth with a diagnosis of depression of which onset was 2 years. While being on a work-aid program for individuals with various disabilities, he has shown helplessness, inferiority with his appearance, insomnia, and suicidal impulse besides depression. His supervisor reports that he often fails to complete his own work, makes errors and mistakes, and cuts his work hours. His training of writing a daily work log consisted of the following steps; (a) analyzing and writing about the circumstances, his behaviors, thoughts, and feelings at the happiest and unhappiest work or time on a daily basis; (b) analyzing and writing his plans about how to improve his unhappy feelings or circumstances, implement them accordingly, and the results post implementation of these plans; (c) reviewing his daily work log and discussing about it with the main investigator daily; and (d) writing 3 times and reading aloud 2 times his resolutions regarding the unpleasant circumstances, methods to improve the circumstances or feelings, and the expected positive results from his resolutions. This training was conducted for 5 days. Results : The results were: on self-reporting depression scale, a mean of 80% in the baseline and 68% in the training; on self-reporting on the patient’s work performance satisfaction, a mean of 68% in the baseline, and 88% in the training; the supervisor’s reporting of the patient on the Work Personality Profile, a mean of 58% in the baseline and 70% in the training; and on the self-reporting on the Dysfunctional Attitude Scale, 215 out of 280 full points in the baseline and 164 in the training. Conclusions : The results indicate that the use of writing a daily log using cognitive approach helps improve not only depression state but also work performance in a patient with depression.

Objectives : To investigate recognition toward elderly’s lifestyle among university students, college students, specific department students of physical therapy, nursing, home economics educated elderly health and welfare program Method : The survey is performed by 1290 university students, 2399 college students, 192 students of specific departments. The instruments in this study consisted of a 13-items related to elderly’s lifestyle using nominal scale. We sent questionnaire by mail and received from chair of each department. Analysis of the data was done by using frequency statistics. Results : The opinion of specific three departments related to health care was higher than university and college in government’s policy, medical health for elderly. Conclusions : The recognition of students toward elderly’s lifestyle was impacted by characteristics of department.

Objectives : This study evaluated norms for fine motor dexterity skills on kindergarten and elementary school children. Method : Participants were recruited from five kindergartens and two elementary schools in Seosan area. The sample consisted of 160 healthy students ranging in age from five to eight years. The Grooved Pegboard Test (GPT) was utilized to investigate fine motor dexterity. Results : The GPT showed statistically significant differences in hand dominance based on the findings that the scores of nondominant hands were substantially lower than those of dominant hands (p<0.05). The GPT showed significant age group differences in which 8 year old group demonstrated substantially higher scores than those of all other younger age groups (p<0.05). The results indicated a positive relationship between age and GPT scores. Conclusion : The results of this study support the results of previous studies that compared hand function between healthy children and abnormal children. These data will provide standard value of GPT for occupational therapists.

Since the commencement of the government health insurance program in 1977, there has been criticism that the national insurance fee for health care service had been set too low by the Korean government. The existing insurance fee schedule for occupational therapy service was developed first in 1980 in Korea. But they are not reflecting the current health care environment. Even though the fee schedule has been revised almost every year, the fee has been estimated too low to warrant a quality service. To make the situation worse, Health Insurance Review Agency has recently proposed to substantially cut down the fee for occupational therapy service. Many medical institutions opening occupational therapy clinics are avoiding investment in this “financial deficit” department. This is threatening the survival of the occupational therapy profession that is essential for rehabilitation process. The objective of this study is to review and analyze the current Korean health insurance fee schedule and its changing process in occupational therapy service and suggest the better solutions. The researcher suggests that the health insurance fee schedule must cover computer-based occupational therapy, a Pediatric occupational therapy and evaluation, and treatment for the use of equipment such as Baltimore Therapeutic Equipment. Also, the government must increase the fee for occupational therapy service at least up to the level of consumer price index.

The purpose of this review article were to suggest ways for practitioners to determine the presence of sensory integrative dysfunction and something about its characteristics in the absence of the Sensory Integration and Praxis Tests (SIPT) and to suggest to need the use of a top-down assessment approach in the sensory integration approach. The review constructed in two major ways: sensory modulation disorders and disorders of praxis in sensory integration dysfunctions. The methods of test for sensory modulation disorders included a interview, take a sensory history in all sensory system and informal or/and formal observation of children in general and specific environment. Tests for sensory modulation were Sensory Profile (Dunn, 1999), Touch Inventory for Elementary School Children (TIE) and Evaluation of Sensory Processing etc.. The methods of test for disorders for praxis included direct clinical observation, performance of tasks, interview, sensory history, sequencing and balance in occupational performance. The test tools were Bruininks Oseretsky Test of Motor Proficiency (BOTMP), Motor Assessment Battery for Children (MABC), DeGangi-Berk Test of Sensory Integration, Clinical Observation of Motor and Postural Skills (COMPS), Clinical Test of Sensory Interaction with Balance (CTSIB) and Clinical Observations of Neuromotor Performance etc.. This article reviewed assessments of sensory modulation disorders and disorders of praxis within the context of a top-down approach to evaluation. It also presented formal and informal assessments and proposed a mean of assessing sensory integration in the absence of the SIPT.

Objective : The purpose of this study is to introduce Problem-Based Learning (PBL) to practicing occupational therapists and educators of occupational therapy. PBL is the educational method that has been applied to not only a number of universities abroad but also many domestic universities, especially medical colleges. Method : This study presents the definition and features of PBL, explores the current trends of its application and research, and investigate the example of PBL application to one of the programs in domestic universities. Result : PBL has been applied to the 2002 class of the program of occupational therapy for one year. Then the students of the program were asked to answer a questionnaire that surveyed the improvement of presentation, debate, and problem solving skills. Approximately 70% of the students responded affirmatively. Conclusion : In conclusion, the implication of this study is that when it is necessary to enhance the quality of educational service in the occupational therapy field, this study shows that the possible alternatives include PBL.
