It is well known that oral pathology is an essential bridge between basic and clinical science in dental field. Although oral pathology has been introduced to Korean dental science since 1945, there is not yet presented about oral pathologic history. The purpose of this study are to summarize and to introduce Korean oral pathologic history in serial form for Korean oral pathologists.

New therapeutic measure are needed to improve the outcome for patients with oral squamous cell carcinoma(OSCC) because OSCC continues to portend a relatively unfavorable prognosis. Recently RNA interference(RNAi) has emerged as an effective method to target specific genes for silencing. Although overexpression of urokinase-type plasminogen activator receptor(uPAR) has been implicated in progression and metastasis of OSCC, the transfection effect of RNAi- uPAR on OSCC has been rarely reported. The purpose of this study were to examine the efficient and specific inhibition of uPAR mRNA and protein expression by siRNA targeting of uPAR through RT-PCR and immunoslot blotting, and to study cell proliferation activity, adhesion, invasion and migration in vitro compared to the controls. In MTT assay, siRNA-uPAR transfected cells showed about 70-80% cell proliferation compared to OSCC cell lines after 2 days. In adhesion assay, siRNA-uPAR transfected cells showed about 20-30% adhesion activity compared to OSCC cell lines, but similar features to those of BSA coated wells. In migration assay, siRNA-uPAR transfected cells showed about 60% migration activity compared to OSCC cell lines, but higher 3.5 folds to those of BSA coated wells. In invasion assay, siRNA-uPAR transfected cells showed about 55% invasive activity compared to parental cell lines. mRNA expression of siRNA-uPAR transfected cells showed about 10-15 % compared to parental cell lines by RT-PCR. Protein expression of siRNA-uPAR transfected cells showed about 25% compared to parental cell lines by ELISA assay. It suggested that RNAi-uPAR tranfection might be used as a potent and specific therapeutic tool for the treatment of oral squamous cell carcinoma, especially in inhibiting invasion and metastasis.

Poor oral hygiene can cause severe periodontal disease. Therefore, it is necessary to educate individuals regarding the importance of maintaining good oral hygiene. To this end, in this study, we evaluated the efficacy of using a phase contrast microscope as an instructional tool for oral hygiene. Forty patients, randomly divided into test and control groups, were recruited 6 months after receiving oral prophylaxis. A phase control microscope was used as a demonstration tool for the test group, and a dentiform model was used to demonstrate the ideal tooth-brushing method for the control group. In the test group, awareness regarding the importance of proper oral hygiene was found to increase by 30%, and in the control group, awareness was increased by 20%. The overall satisfaction with the instructional program was 80% in the test and 70% in the control group. The level of understanding was 99% in the test and 80% in the control group. Instructions were easily followed by 80% of the test and 60% of the control group. Understanding of the program was significantly higher in the test group than in the control group (p < 0.05). The overall satisfaction and understanding of proper oral hygiene was over 70% for both the groups. The phase contrast microscope-based instructional program yielded higher actual practice rates. The phase contrast microscope is desirable tool for motivating patients and spreading awareness regarding the importance of maintaining good oral hygiene.

Oral erythroleukoplakia is characterized by severe dyskeratosis intermingled with multifocal erosive spots on the buccal mucosa, dorsal tongue, and lower lip, etc. A case of oral erythroleukoplakia was diagnosed among 83 cases of common oral leukoplakia since 1997. The pathological examination showed the typical features of leukoplakia with severe epithelial dysplasia, exhibiting dyskeratosis, acanthosis, and basal hyperplasia. The oral erythroleukoplakia was explored in comparison with a representative common oral leukoplakia by the immunohistochemical method using PCNA, β-catenin, EGFR, p53, TNFα, pAKT, and STAT3. Oral erythroleukoplakia showed strong positive reaction of PCNA, p53, EGFR, TNFα, pAKT1 and STAT3 in its spinous layer cells and these reactions were reduced in its basal layer cells, while common oral leukoplakia showed diffusely weak reaction of those proteins. Particularly, β-catenin was positive in the nuclei of some basal and spinous layer cells of oral erythroleukoplakia contrast to the common oral leukoplakia. These findings indicated that the present oral erythroleukoplakia was proliferative with the activation of β-catenin pathway, revealed the dysplastic changes of epithelium by the overexpression of EGFR, p53, and pAKT, and also produced inflammatory reaction through the activation of cytokine-dependent signalings of TNFα and STAT3. These data indicated that the present oral erythroleukoplakia might undergo the early stage of multi-step carcinogenesis via the overexpression of different oncoproteins, especially β-catenine, p53, pAKT, and STAT3.

A Mixed tumor is a tumor that derives from multiple tissue types. This lesion appears to be mostly Mixed Radiolucent and Radiopaque aspect and typical examples are odontoma, ameloblastic fibroodontoma, ameloblastic fibroma etc. A odontogenic tumor is a neoplasm of the cells or tissues that initiate Odontogenic processes. Typical example is a odontoma. The odontoma is a hamartoma of odontogenic origin. There are two types: compound and complex. A compound odontoma still has the three separate dental tissues (enamel, dentin and cementum), but may present a lobulated appearance where there is no definitive demarcation of separate tissues between the individual "toothlets". It usually appears in the anterior maxilla. The complex type is unrecognizable as dental tissues, usually presenting as a radioopaque area with varying densities. It usually appears in the posterior maxilla or in the mandible. We reported 20-years old male patient who had an atypical mixed radiolucent and radiopaque lesion on right mandibule body tentative diagnosed as complex odontoma and excised.

Osteoblastoma is a very rare disease and in case of the jaw, maxilla infrequently occurs more than mandible. On account of difficulty to discriminate from other benign tumor or sarcoma, we need to pay attention to make differential diagnosis based on clinical, radiographic, pathological findings. A tumor on the maxilla and maxillary sinus could lead to a bad result for aesthetic appreciation and function. Therefore, reconstruction after maxillectomy is one of the most important issues concerning a surgery. The aim of this study is to report a case of osteoblastoma on the left maxilla which was misdiagnosed as ossifying fibroma and was treated with hemimaxillectomy and reconstruction using deep iliac circumflex artery musculo-osseous flap which resulted in satisfactory result.
